ABSTRACT

AIM: The purpose of this intervention was to treat the patient, a 51-year-old woman, who was wearing a maxillary denture and a mandibular bilateral distal-extension partial denture (both unfit) and was suffering from temporomandibular disorder (TMD). METHODOLOGY: The treatment suggested was to construct a superior repositioning splint (SRS), and after remission of the symptomatology, fabricate new dentures. RESULTS: The occlusal splint was effective and the symptomatology ceased after about five months of treatment. The new dental prostheses were made and followed up for 10 years with no return of the pain. CONCLUSIONS: SRS and subsequent construction of new dentures after remission of the symptomatology was an effective treatment for TMD in the case described.

ABSTRACT

OBJECTIVE: Photodynamic therapy (PDT) is an effective method for Candida spp. inactivation in vitro and in vivo, but as yet, no clinical trial has been conducted. This report describes 5 cases of denture stomatitis (DS) treated with PDT. STUDY DESIGN: Five subjects with clinical and microbiologic diagnosis of DS were submitted to 6 sessions of PDT 3 times a week for 15 days. In each session, patients' dentures and palates were sprayed with 500 mg/L Photogem, and, after 30 minutes of incubation, irradiated by light-emitting diode light source at 455 nm (37.5 and 122 J/cm(2), respectively). Cultures of Candida spp. from dentures and palates and standard photographs of the palates were taken at baseline (day 0), at the end of the treatment (day 15), and at follow-up time intervals (days 30 and 60). RESULTS: Four patients showed clinical resolution of DS (no inflammation) after PDT sessions, and only 1 subject demonstrated reduction in palatal inflammation. Recurrence of DS was observed in 2 patients during the follow-up period. CONCLUSIONS: PDT appears to be an alternative treatment for DS.

ABSTRACT

Inflammatory papillary hyperplasia of the palate (IPHP) is a tissue-reactive overgrowth characterized by hyperemic mucosa with nodular or papillary appearance in the palate. The exact pathogenesis is still unclear. In this study, the presence of Candida albicans in the epithelial lining was evaluated using the indirect immunofluorescence staining technique. Strongly stained C albicans was observed only in the lesions of the IPHP group. Therefore, the detection of C albicans in almost all samples from IPHP tissue enabled a suggestion as to the microbial etiology of the disease, since the use of dental prostheses was reported.

ABSTRACT

AIM: The purpose of this report is to present a case of surgical and prosthetic treatment of a woman with inflammatory fibrous hyperplasia (IFH) and her evaluation during a six month period. BACKGROUND: IFH is a benign pathology, prevalent in female patients, and principally associated with ill-fitting prosthetic devices in need of adjustment. It is common for patients to require surgical removal of the hyperplastic tissue and fabrication of a new prosthesis. CASE REPORT: A 55-year-old female with a history of smoking presented with a chief complaint of missing the scheduled adjustment of her maxillary complete denture and the presence of moveable tissue under the denture. Surgical excision of the hyperplastic tissue followed with fixation of the prosthesis for six months to guide the healing of the soft tissue and to reshape the contours of the maxillary supporting tissues. SUMMARY: Surgical removal of hyperplasic soft tissue is a routine procedure, and the fixation of the prosthesis for the support of tissue during healing improves intraoral conditions for the fabrication of a new prosthesis in the future.

ABSTRACT

Con el objetivo de precisar la posible relación entre la gravedad clínica de la estomatitis protética y el espesor epitelial característico de estas lesiones se realizó un estudio en biopsias de 34 pacientes portadores de prótesis totales que presentaban estas lesiones en la mucosa palatina. Las muestras fueron fijadas en formalina neutra al 10 por ciento, procesadas por la técnica de inclusión en parafina y coloreadas con hematoxilina-eosina y PAS. Con el empleo de un sistema computarizado analizador de imágenes Leitz ASM-68K se tomaron 4 medidas perpendiculares con los espesores epiteliales suprapapilares e infrapapilares desde el borde superior de la capa granulosa hasta el borde inferior de la capa basal en cada fragmento. Los datos obtenidos fueron resumidos y sometidos al análisis estadístico determinándose las medias aritméticas y las desviaciones standard comparándose los resultados con la prueba t de Student. Concluye el trabajo el plantearse que el espesor epitelial en la estomatitis protética no presenta diferencias significativas en relación con la gravedad clínica de la lesión (p>0.005)

ABSTRACT

Con el objetivo de precisar la posible relación entre la gravedad clínica de la estomatitis subprótesis y el grosor epitelial característico de estas lesiones se realizó un estudio en biopsias de 34 pacientes portadores de prótesis totales que presentaban estas lesiones en la mucosa palatina. Se efectuó un examen citológico previo a la toma de la biopsia, siguiendo los criterios de Papanicolaou, realizándose esta en los casos correspondientes a los grupos II y III. Las muestras fueron fijadas en formalina neutra al 10 por ciento y procesadas por la técnica de inclusión en parafina cortándose entre 4 y 6 micras de espesor y coloreadas con hematoxilina y eosina. En cada corte microscópico se realizaron 4 medidas perpendiculares con una escala ocular para procedimientos histométricos previamente calculando el valor de la escala según el método que establece el fabricante del microscopio MBB-1A. Para determinar las medidas se tomaron las distancias de la superficie libre epitelial a la membrana basal en los espesores suprapapilares y en los infrapapilares, obteniéndose así las medidas de ambas zonas realizándose en cada caso dos mediciones en las zonas referidas para un total de 4 por fragmento. Los datos obtenidos fueron resumidos para someterlos al análisis estadístico determinándose las medias aritméticas y las desviaciones standard comparándose las medias de los grupos con la prueba t de Student. Concluye el trabajo al plantearse que el espesor epitelial en la estomatitis subprótesis no presenta diferencias significativas en relación con la gravedad clínica de la lesión (p<0.05)

ABSTRACT

The presence of yeasts and bacteria was studied in 26 patients with denture stomatites, and the results compared with the data of the normal mucosa foi edentoulous patients, who used or not upper dentures. The use of dentures caused an increase in the amount of yeasts, and there was a correlation with the severity of the stomatitis. Gram positives cocci and bacilus predominated in all studied groups, but in cases of stomatitis there was an increase in the amount of Gram negative cocci and filamentous. These results suggest that besides yeasts, modifications of the bacteria flora can be relevant for the development of denture stomatitis.

ABSTRACT

The authors verified histologically in 20 no dentates patients, the influence of complete dentures on the hard palate mucosa. The patients were divided in two groups: I. Patients that never used complete dentures; II. Patients that were using complete dentures when this research was realized. New prosthesis were made for each one of the 20 patients. Immediately before they received the new prosthesis and ninety days after this, biopsies were realized. The circunferencial pieces of the hard palate mucosa were fixed in formol 10% and coloured with Hematoxylin/Eosin and Mallory's Tricomic for histomorphologic analysis and measuring of the stratum corneum of the epithelium. It was made the statistical analysis and the results showed that: 1. In normal conditions, the utilization of the full dentures produces the reduce of the thickness of the stratum corneum. This was more observed in Group I. 2. The range difference of the thickness of the stratum corneum between the first and the second biopsies in the groups I and II (4.86 microns and 2.18 microns) were not statistically significant. 3. With the use of complete dentures the stratum corneum of the epithelium showed tendence to parakeratinization.

ABSTRACT

Estudo histopatológico com coloraçöes pela hematoxilina-eosina (HE) e pelo ácido periódico de Schiff (PAS), paralelamente à imunofluorescência direta (ID) antiimunoglobulinas humanas totais e contra Candida albicans, foram efetuados em 25 indivíduos com próteses dentárias totais muco-suportadas superiores. Em 5 indivíduos sem sinais clínicos compatíveis com candidíase atrófica crônica (CAC) ao HE tivemos discreta inflamaçäo no conjuntivo e o PAS marcou lâmina basal contínua, os grânulos intracelulares da camada granulosa e paraqueratina uniforme na superfície do epitélio. Nos 20 demais, com sinais clínicos compatíveis com CAC, ao HE foram freqüentes achados comuns às infecçöes por Candida e o PAS revelou, além da descontinuidade da lâmina basal e superfície epitelial revestida por paraquetina frouxa, ou descontínua, a presença de corpúsculos arredondados maiores que os intracelulares da camada granulosa, por vezes isolados na porçäo mediana das papilas e cones interpapilares. Nos 5 primeiros indivíduos a ID antiimunoglobulinas humanas totais näo revelou sinais da vigência de fenômenos imunológicos humorais, bem como nos demais 20 pacientes com sinais de CAC o método näo mostrou qualquer vestígio de reaçäo imunológica mediada por anticorpos. Porém, a ID com conjugado contra C. albicans nos casos com aspectos clínicos compatíveis com CAC detectou aspectos condizentes com estruturas que podem ser assumidas pela Candida nos tecidos. Culturas do material de indivíduos com sinais de CAC positivaram-se em 100 por cento para Candida, 70 por cento presuntivamente albicans; porquanto em indivíduos sem sinais de CAC, a Candida foi isolada em 80 por cento das amostras, 67 por cento presuntivamente albicans
