ABSTRACT

BACKGROUND: Skin cancer is the most frequent cancer worldwide and the most frequent periocular tumor. Keratinocyte Carcinomas (KC) located in periorificial areas, such as periocular tumors, are considered high-risk tumors. Mohs Micrographic Surgery (MMS) is considered the first line for the treatment of high-risk KC, providing a lower recurrence rate than conventional wide excision. OBJECTIVE: To describe the clinical-pathological features of periocular KC treated with MMS in a tertiary university center in Chile. METHODS: A single-center, retrospective study of patients with KC located on the periocular area, that underwent MMS between 2017‒2022. MMS details were recorded. RESULTS: One hundred thirteen patients with periocular carcinomas were included. The mean age was 59 ± 13 years; 52% were women. The most frequent location was the medial canthus (53%), followed by the lower eyelid (30.1%). The most frequent BCC histology was the nodular variant (59.3%). Regarding MMS, the average number of stages was 1.5 ± 0.7, and 54% of the cases required only 1 stage to achieve clear margins. To date, no recurrence has been reported. Tumors larger than 8.5 mm in largest diameter or 43.5 mm2 were more likely to require complex reconstruction. STUDY LIMITATIONS: Retrospective design and a relatively low number of patients in the SCC group. Possible selection bias, as larger or more complex cases, may have been referred to oculoplastic surgeons directly. CONCLUSION: The present study confirms the role of MMS for the treatment of periocular KCs. Periocular KCs larger than 8.5 mm might require complex reconstruction. These results can be used to counsel patients during pre-surgical visits.

We report the case of a 73-year-old woman with a relapsing primary cutaneous anaplastic large cell lymphoma on her eyelid. A systemic extension study excluded extracutaneous involvement. Systemic chemotherapy resulted in an optimal response, with complete regression of the cutaneous lesions. There has been no recurrence during the 2 years of follow-up.


RESUMO O acometimento ocular adicional nos distúrbios linfoproliferativos CD30+ é raro. Relatamos o caso de uma mulher de 73 anos com linfoma de grandes células anaplásicas primárias recidivantes em sua pálpebra. A avaliação sistêmica excluiu envolvimento extracutâneo. A quimioterapia sistémica resultou em uma resposta ótima, com regressão completa das lesões cutáneas. Não houve recidiva durante 2 anos de acompanhamento.

They can develop on the eyelid and surrounding area and rarely appear at other sites. While most cutaneous cases are diagnosed incidentally, the lesions may be symptomatic based on their location. The etiology of fibroepithelial polyps is not fully understood but may involve trauma, chronic irritation, allergic factors, and developmental or congenital causes. Although fibroepithelial polyps are benign lesions, they can be malignant in extremely rare cases. Herein we discuss 2 cases of fibroepithelial polyp with an atypical presentation and their treatment.


RESUMO Pólipos fibroepiteliais são tumores benignos não-epiteliais que surgem do tecido mesodérmico e são comumente encontrados na pele. Eles podem se desenvolver na pálpebra e na área adjacente, e raramente aparecem em outros locais. Embora a maioria dos casos cutâneos seja diagnosticada incidentalmente, as lesões podem ser sintomáticas com base em sua localização. A etiologia dos pólipos fibroepiteliais não é completamente compreendida, mas pode envolver trauma, irritação crônica, fatores alérgicos e causas de desenvolvimento ou congênitas. Apesar dos pólipos fibroepiteliais serem lesões benignas, podem ser malignos em casos extremamente raros. Aqui discutimos 2 casos de pólipo fibroepitelial com uma apresentação atípica e seu tratamento.

ABSTRACT

The case is presented of a 57 year-old man with a one-year history of enlargement of the left upper eyelid. An excisional biopsy was performed, and the histological and immunohistochemical study confirmed the diagnosis of schwannoma. Schwannomas are benign peripheral nerve sheath tumours, derived from a Schwann cells proliferation. Eyelid involvement is extremely uncommon. To make the diagnosis, a detailed histopathological and immunohistochemical study is essential. This case suggests that schwannomas should be included within the differential diagnosis of any solid eyelid lesion.

ABSTRACT

Purpose: To describe and evaluate the eyelash transplantation with strip composite eyebrow graft to improve eyelid anatomy in tarsoconjunctival flap eyelid reconstruction procedure. Methods: Twenty two patients with eyelid tumors managed by local excision and reconstruction using tarsoconjunctival flap reconstruction procedure were included. Eyelash graft was performed during flap division by harvesting a graft from the eyebrow and creating a recipient pocket in the receptor eyelid. Patients had a 1-year follow up. Eyelash orientation and follicle survival rate were recorded. Demographic variables were analyzed with descriptive statistics. For eyelash follicles survival rate assessment, we evaluated the number of follicles found in the first postoperative day with the follicles found at the end of the study using the Wilcoxon signed rank test. P < 0.05 was considered statistically significant. Results: All eyelash grafts showed a good follicle orientation and no cases of eyelashes misdirection toward the eyeball were reported. No statistical differences (p > 0.05) were found in Wilcoxon signed rank test when comparing the number of follicles present at the first postoperative day and the number of follicles present at the 12th postoperative month, meaning that the vast majority of follicles survived at the end of the study. Graft ischemia, necrosis or infection were not observed. Conclusions: Tarsoconjunctival flap is an outstanding option for eyelid reconstruction and eyelash transplantation with an eyebrow composite graft is an excellent procedure to enhance and recreate a normal eyelid.

ABSTRACT

Ocular adnexal lymphoma (OAL) constitutes 8% of all extranodal lymphomas, being rare in the eyelids (5-20%). The objective was to report a case of eyelid follicular lymphoma in a young adult woman. A 35-year-old female with swelling in left lower eyelid with development for 2 months. The visual acuity was 20/20 in both eyes, the tumor mass was 3 cm wide and 1.5 cm long. The contrast-enhanced computed tomography showed a soft-tissue injury in the left lower eyelid but had no signs of local spread according to description. Grade 2 follicular lymphoma was diagnosed by biopsy. Oncological evaluation assessed positron emission tomography scan and bone marrow aspiration with negative results for systemic spread of disease. Patient started radiotherapy. This case highlights that young patients may present painless swelling lesions that may be largely underdiagnosed and should take into consideration some oncological pathologies.

ABSTRACT

INTRODUCTION: Primary Apocrine adenocarcinomas (PAA) are very infrequent tumors that are often confused initially with benign lesions. Little is known about this disease and there is still much to be clarified. We present a case of PAA on the eyelid successfully treated with surgery alone and a literature review regarding what is currently described about this disease. METHODS: Noncomparative, retrospective case report of a patient with PAA on the eyelid succesfully treated with surgery alone and a literautre review. RESULTS: A 91-year-old man with a 2 months lesion on the upper left eyelid was treated with surgery alone with oncological margins of 5mm. The Hystopathology diagnosis was a PAA of the eyelid and free margins were obtained. After 12 months of follow-up, the patient does not show any signs of local recurrence or distant metastasis. A review of the literature suggests these tumors are located more frequently in the axilla (50%) and secondly in the head and neck (35%), with similar distribution in the upper (41%) and lower eyelid (45%). The most commonly used treatment is surgical excision, but radiotherapy and chemotherapy have also been used with variable results. CONCLUSIONS: PAA is a very rare and aggressive tumor. Because it is so infrequent, treatments are based on the sporadic cases encountered in the literature. As more cases are reported, more can be elucidated about the characteristics of this tumor, its behavior and best treatment choice and this may allow progress in the understanding and management of this disease.

ABSTRACT

Resumo O lentigo maligno é um melanoma in situ, de crescimento radial e lento, que acomete áreas fotoexpostas principalmente em idosos. Quando acomete a pálpebra, devido à proximidade a um órgão nobre, a conduta é controversa, porém a cirurgia é o método mais usado, com margens que variam de acordo com a referência utilizada. Terapias conservadoras são descritas, como o imiquimode 5% e a radioterapia. O presente relato tem como objetivo demonstrar a escassez de estudos sobre a margem cirúrgica e citar opções de tratamentos não cirúrgicos para o lentigo maligno da face.


Abstract Lentigo maligna is a melanoma in situ, of slow radial growth, which affects sun-exposed areas, especially in the elderly. When it affects the eyelid, due to the proximity to a noble organ, the conduct is controversial, but surgery is the method most commonly used, with with margins varying according to the reference used. Conservative treatments are described, such as imiquimod 5% and radiotherapy. This report aims to demonstrate the lack of studies on the surgical margin, and to name nonsurgical treatment options for lentigo maligna of the face.

ABSTRACT

Fibropapillomatosis (FP) is a multifactorial disease that affects all species of marine turtles, including green turtles Chelonia mydas (Linnaeus, 1758). It is characterised by the development of internal or external tumours that, depending on their locations and sizes, may intensely impact the health condition of sea turtles. The goal of this study was to characterise the disease in C. mydas found in a foraging area in southeastern Brazil, evaluate the prevalence in this region, and correlate presence and absence, size, body distribution, number of tumours, and disease severity with biometric variables of the captured green turtles. Between 2008 and 2014, the prevalence rate of FP was 43.09%, out of 246 green turtles. The size of the animals with FP was relatively greater than animals without tumours, and the prevalence of FP increased with animal size, peaking in the 60-80 cm size class. From 2013 to 2014, gross evaluation of fibropapillomas was performed. The number of tumours per turtle ranged from 1 to 158. The size of tumours ranged from <1 cm (Size A) to >10 cm (Size D); Size A tumours and turtles slightly affected by the disease (Score 1) predominated. Tumour progression (72.1%) and regression (32.8%) were seen in some recaptured individuals (n = 61). Moreover, 24.6% of these turtles showed both progressions and regressions of tumours.

OBJECTIVE: : to compare the accuracy of preoperative 2-mm punch biopsy at one site and at two sites in the diagnosis of aggressive subtypes of eyelid basal cell carcinoma (BCC). METHODS: : we randomly assigned patients to Group 1 (biopsy at one site) and Group 2 (biopsy at two sites). We compared the biopsy results to the gold standard (pathology of the surgical specimen). We calculated the sensitivity, specificity, positive predictive value, negative predictive value, accuracy and Kappa coefficient to determine the level of agreement in both groups. RESULTS: : we analyzed 105 lesions (Group 1: n = 44; Group 2: n = 61). The agreement was 54.5% in Group 1 and 73.8% in Group 2 (p = 0.041). There was no significant difference between the groups regarding the distribution of quantitative and qualitative variables (gender, age, disease duration, tumor larger diameter, area and commitment of margins). Biopsy at two sites was two times more likely to agree with the gold standard than the biopsy of a single site. CONCLUSIONS: : the accuracy and the performance indicators were better for 2-mm punch biopsy in two sites than in one site for the diagnosis of aggressive subtypes of eyelid BCC. ABSTRACT

Objetivos: caracterizar la aplicación de las técnicas reconstructivas utilizadas en el manejo de los pacientes con tumores malignos de párpado inferior según edad, sexo, color de la piel, tipo de neoplasia, localización de la neoplasia, ojo afectado, clasificación quirúrgica y complicaciones posquirúrgicas. Métodos: estudio descriptivo y longitudinal en el Instituto de Oncología y Radiobiología de La Habana, entre septiembre de 2013 y agosto de 2014, de pacientes que presentaron tumores malignos del párpado inferior. Se constituyó una muestra de 38 pacientes. Resultados: el sexo más afectado fue el masculino (73,6 por ciento); las edades más frecuente de presentación fueron entre 60 a 69 años (42,3 por ciento), en pacientes de piel blanca (86,8 por ciento). El carcinoma basocelular fue el de mayor incidencia histológica (68,5 por ciento); el ojo derecho fue el más vulnerado (65,8 por cietno), en la localización del tercio medio del párpado inferior; el mayor grupo se clasificó como T2N0M0 (39,5 por ciento). La complicación aguda más frecuente fue el hematoma del sitio operatorio (10,5 por ciento) y dentro de las tardías el lagoftalmo (7,8 por ciento). La técnica quirúrgica más empleada fue el colgajo rotado de mejilla (34,2 por ciento). Conclusiones: los resultados logrados en la reconstrucción integral del párpado inferior fueron satisfactorios en todos los casos, estructural y funcionalmente, con una mejoría de la apariencia física y de la calidad de vida de los pacientes(AU)


Objectives: to characterize the use of reconstructive techniques to manage patients with malignant lower eyelid tumor as per age, sex, race, type and location of neoplasia, affected eye, surgical classification and postsurgery complications. Methods: descriptive and longitudinal study carried out in the National Oncology and Radiobiology Institute in Havana city from September 2013 through August 2014. The study sample was made up of 38 patients who suffered malignant lower eyelid tumors. Results: males (73,6 percent); 60 to 69 years old patients (42,3 percent) and Caucasians (86,8 percent) were the most affected people. Basal cell carcinoma showed the most histological incidence (68,5 percent); the right eye was the most vulnerable (65,8 percent) and tumor location in the middle third of the eyelid (31,5 percent) prevailed. Most part of the patients was included in the T2 N0M0 staging group; (39,5 percent). The main severe complication was hematoma in the surgical site (10,5 percent) whereas lagophtalmo (7,8 percent) was predominant in the postoperative complications. Rotate cheek flap technique was the most used for the reconstruction of the eyelid (34,2 percent). Conclusions: the results of the comprehensive reconstruction of the lower eyelid were operationally and structurally satisfactory in all cases, with better physical appearance and higher quality of life in all patients(AU)

ABSTRACT

Primary cutaneous adenoid cystic carcinoma (PCACC) is a rare malignant epithelial tumor most commonly observed in the scalp and skin of the chest and originating from the palpebral portion of the lacrimal gland in the orbit. Here we describe the diagnosis and treatment of a rare case of PCACC in an eye of a 52-year-old male. The patient presented with a mass lesion of the right lower eyelid. During incisional biopsy, lack of encapsulation and a secretion pattern different to that of chalazion was observed, which differentiated the lesion from chalazion. Pathological analysis revealed the diagnosis of PCACC. This case highlights the importance of careful inspection for macroscopic differentiation of PCACC from chalazion after initial surgery and pathological evaluation of all surgically removed mass lesions for accurate diagnosis and treatment.

RESUMOO carcinoma adenóide cístico cutâneo primário (PCACC) é um tumor maligno epitelial raro, mais comumente observado no couro cabeludo e na pele do peito sendo originário da porção palpebral da glândula lacrimal na órbita. Apresentamos o diagnóstico e tratamento de um caso raro de carcinoma adenóide cístico cutâneo primário do olho em um paciente do sexo masculino de 52 anos de idade, com uma lesão maciça da pálpebra inferior direita. A observação da falta de encapsulamento e um padrão de secreção diferente do calázio durante biópsia incisional permitiu a diferenciação da lesão de um calázio. A avaliação dos resultados do exame patológico resultou no diagnóstico de carcinoma adenóide cístico cutâneo primário. A análise do caso indica a importância de uma inspeção cuidadosa para a diferenciação macroscópica da carcinoma adenóide cístico cutâneo primário do calázio após a cirurgia inicial e avaliação patológica de todas as lesões de massa removidas cirurgicamente para o diagnóstico e tratamento corretos.
