ABSTRACT
Introducción. La gangrena de Fournier es un proceso infeccioso progresivo que compromete piel, tejido celular subcutáneo, grasa y fascia subyacente, con una incidencia de 1,6 pacientes por cada 100.000 personas/año. Se considera una urgencia quirúrgica, que requiere de manejo oportuno, ya que puede llegar a ser fatal, con una tasa de mortalidad del 20 al 35 %, que es más alta en hombres, en la tercera década de la vida y en pacientes inmunocomprometidos. Caso clínico. Se presenta el caso clínico de un paciente masculino de 44 años de edad, quien cursó con gangrena de Fournier secundaria a una espina de pescado de 5 cm de largo, incrustada en la unión anorrectal. Resultados. El paciente fue manejado por urología y cirugía general, requirió hospitalización en la Unidad de Cuidados Intensivos y curaciones por parte de terapia enterostomal, con resultados satisfactorios. Conclusiones. Sus posibles causas son múltiples y en ocasiones puede ser desencadenada por un factor externo, como un cuerpo extraño. Uno de los factores predisponentes es la obesidad. El diagnóstico oportuno y un tratamiento con intervención multidisciplinaria mejoran la sobrevida y la calidad de vida de los pacientes.
Introduction. Fournier's gangrene is a progressive infectious process that involves skin, subcutaneous tissue, fat and underlying fascia, with an incidence of 1.6 per 100,000 people/year. It is considered a surgical emergency, which requires timely management since it can be fatal, with a mortality rate of 20 to 35%, which is higher in men, in the third decade of life and in immunocompromised patients. Clinical case. Clinical case. A 44-year-old male patient is presented with Fournier's gangrene secondary to a 5 cm long fishbone embedded in the anorectal junction. Results. The patient was managed by urology and general surgery, requiring hospitalization in the ICU and treated by enterostomal therapy with satisfactory results. Conclusions. Its possible causes are multiple and sometimes it can be triggered by an external factor, such as a foreign body. One of the predisposing factors is obesity. Timely diagnosis and treatment with multidisciplinary intervention improve survival and quality of life of patients

OBJECTIVE: The purpose of this study was to research the neutrophil-lymphocyte ratio (NLR), lymphocyte-to-C-reactive protein ratio (LCR), and Fournier's Gangrene Severity Index (FGSI) for predicting prognosis and mortality in patients with Fournier's gangrene (FG). MATERIAL AND METHODS: Patients diagnosed with FG and treated in a tertiary referral hospital in the period from January 2013 to June 2020 were reviewed. LCR, FGSI, and NLR values were calculated. RESULTS: Our series included a total of 41 patients. Of the patients, 78% survived and 21.9% (n = 9) died. Survivors were significantly younger than non-survivors (p = 0.009). Hospital costs were higher in non-survivors and close to statistical significance (p = 0.08). The ROC analysis revealed that the FGSI, LCR, and NLR parameters were significant in identifying survivors and non-survivors (AUC = 0.941 [0.870-1.000], p < 0.001; AUC = 0.747 [0.593-0.900], p = 0.025; and AUC = 0.724 [0.548-0.900], p = 0.042). CONCLUSION: A low LCR value can be used as a marker to assess mortality and disease severity in patients with Fournier's gangrene.
OBJETIVO: Investigar el cociente neutrófilos-linfocitos (CNL), el cociente linfocitos-proteína C reactiva (CLP) y el índice de gravedad de la gangrena de Fournier (IGGF) para predecir el pronóstico y la mortalidad en pacientes con gangrena de Fournier (GF). MÉTODO: Se revisaron los pacientes diagnosticados de GF y atendidos en un hospital de tercer nivel de referencia en el período de enero de 2013 a junio de 2020. Se calcularon los valores de CLP, IGGF y CNL. RESULTADOS: Nuestra serie incluyó 41 pacientes, de los cuales el 78% sobrevivieron y el 21.9% (n = 9) fallecieron. Los supervivientes eran significativamente más jóvenes que los no supervivientes (p = 0.009). Los costes hospitalarios fueron mayores en los no supervivientes y cercanos a la significación estadística (p = 0.08). El análisis ROC reveló que los parámetros IGGF, CLP y CNL fueron significativos para identificar supervivientes y no supervivientes (AUC: 0.941 [0.870-1.000], p < 0.001; AUC: 0.747 [0.593-0.900], p = 0.025; AUC: 0.724 [0.548-0.900], p = 0.042). CONCLUSIONES: Un valor bajo de CLP se puede utilizar como marcador para evaluar la mortalidad y la gravedad de la enfermedad en pacientes con GF.

Introducción. La perforación del recto por trauma cerrado es poco frecuente y se asocia a fracturas pélvicas. En pacientes con perforaciones de recto no traumáticas se ha reportado fascitis necrosante en miembros inferiores, en la mayoría de los casos asociada a alta mortalidad. Caso clínico. Hombre de 36 años con trauma cerrado abdomino-pélvico y perforación de recto inferior, quien recibió manejo quirúrgico mediante derivación intestinal y fijación pélvica. Evolucionó con hematoma escrotal sobreinfectado, inestabilidad hemodinámica, signos de fascitis necrosante y choque séptico 4 días posterior a su ingreso. Resultados. Se tomó muestra para cultivo del hematoma escrotal que reportó E. coli. La patología del desbridamiento escrotal informó necrosis de coagulación en toda la muestra. Conclusión. El tacto rectal debe realizarse siempre ante la presencia de enfisema subcutáneo al examen físico o en la tomografía, para un diagnóstico temprano y manejo quirúrgico multidisciplinario oportuno, según el caso. La presencia de enfisema subcutáneo debe aumentar la sospecha de perforación de recto. Hay pocos reportes de fascitis secundaria a perforación de recto por trauma cerrado, por lo que no se conoce con precisión la mortalidad asociada.
Introduction. Rectal perforation due to blunt trauma is rare and associated with pelvic fractures. Signs of necrotizing fasciitis in lower limbs have been reported in non-traumatic rectal perforations, in most cases associated with high mortality. Case report. A 36-year-old man presents blunt abdomino-pelvic trauma and perforation of the lower rectum. Surgical management by intestinal diversion and pelvic fixation is performed. 4 days after admission, evolves with over-infected scrotal hematoma, hemodynamic instability, signs of necrotizing fasciitis and septic shock. Results. A sample for culture was taken from a scrotal hematoma that reported E. coli. Pathology of scrotal debridement reported coagulation necrosis in the entire specimen. Discussion. Digital rectal examination should always be performed in the presence of subcutaneous emphysema on physical examination or CT scan for early and multidisciplinary diagnosis and surgical management as appropriate. Conclusion. The presence of subcutaneous emphysema should raise the suspicion of rectal perforation. There are few reports of rectal perforation due to blunt trauma and fasciitis, so the associated mortality is not precisely known.

OBJECTIVE: To assess the prognostic value of mortality of a new system for quantifying the lesion length circumscribed to the genitals and adjacent anatomical areas in patients with Fournier's gangrene. METHODS: An observational, analytical and prospective study in the Urology service of the Arnaldo Milián Castro University Hospital, between January 2007 and December 2019. The sample consisted of 60 patients diagnosed with Fournier's gangrene. To determine the lesion length, a new quantification system was designed by the authors. Its predictive capacity was determined by binary logistic regression analysis and its discriminatory capacity by the area under the ROC curve. RESULTS: In the studied sample, Fournier's gangrene mortality was 26.7%. The logistic regression analysis demonstrated the prognostic value of mortality of the quantification system evaluated, for each point that the extension of the lesion increased, the risk of death increased by approximately five times, thus being superior to the methods previously used to quantify the injury. Results of the calibration of the quantification system by the Hosmer-Lemeshhow test showed that the data were adjusted to the regression equation (p = 0.278). An area under the curve of 0.957 was observed, which reflects a good discriminatory capacity, showing a sensitivity of 93.75% and a specificity of 93.18%. CONCLUSIONS: The system for quantifying the lesion length in patients with Fournier's gangrene proposed in the present study showed an excellent prognostic value for mortality. The integration of this new method into others prognostic scores could substantially increase sensibility and specificity on diagnostic, by means of offering a more precise approach of the lesion dimensions.

Introducción. La gangrena de Fournier es una fasciitis necrosante fulminante y progresiva, de origen infeccioso sinérgico polimicrobiano, que afecta las regiones perianal, perineal, genital y abdominal. Su incidencia es mayor en hombres, la edad promedio de presentación reportada está en los 54,7 ± 15,6 años, su mortalidad es del 3-67 %, aunque hay estudios que informan una mortalidad en hombres del 7,5 % y en las mujeres del 12,8 %. Métodos. Se hizo una revisión de la literatura en las bases de datos y fuentes de información PubMed, Scielo y Google Scholar, publicados entre 1950 y 2018, utilizando términos como "fournier gangrene", "fasciitis, necrotizing", "wound infection", "therapy"[subheading] y "GRADE approach". Se realizó una segunda revisión para artículos latinoamericanos en español hasta 2020 usando las mismas fuentes y palabras claves. Resultados. Se seleccionaron artículos que reportaron definiciones, datos históricos, actualizaciones en diagnóstico y terapéutica para hacer una revisión actualizada. Para el uso de las imágenes se solicitó consentimiento informado. Conclusión. La gangrena de Fournier continúa siendo una emergencia quirúrgica potencialmente letal. Gracias a las investigaciones realizadas se ha avanzado en su tratamiento, mejorando los resultados. Es importante analizar los factores de riesgo en cada paciente y su etiología para establecer el tratamiento más adecuado.
Introduction. Fournier's gangrene is a fulminant and progressive necrotizing fasciitis of synergistic polymicrobial infectious origin that affects the perianal, perineal, genital and abdominal regions. Its incidence is greater in men, the average age of reported presentation is 54.7 ± 15.6 years; its mortality is 3-67%, although there are studies that report a mortality of 7.5% in men and 12.8% in women. Methodology. A review of the literature was carried out in the databases and information sources: PubMed, Scielo, and Google Scholar, published between 1950 and 2018 using terms such as "Fournier Gangrene", "Fasciitis, Necrotizing", Wound Infection, "therapy"[Subheading] and "GRADE Approach". A second review was performed for articles in Latin American Spanish up to 2020 using the same sources and keywords. Results. Articles that reported definitions, historical data, diagnostic and therapeutic updates were selected, performing an updated review. Informed consent was requested for the use of images.Conclusion. Fournier's gangrene continues to be a potentially lethal surgical emergency, thanks to the research carried out, progress has been made in its treatment, improving results. It is important to analyze the risk factors for each patient and its etiology to establish the most appropriate treatment

Objetivo: Descrever a experiência de discentes do curso de graduação em Enfermagem da Universidade Federal de Alagoas durante a implementação do Processo de Enfermagem a um indivíduo acometido pela Síndrome de Fournier. Métodos: Estudo descritivo, do tipo relato de experiência, baseado na vivência dos estudantes de enfermagem, durante a atividades curriculares de Estágio Supervisionado em uma Unidade de Terapia Intensiva de um hospital público alagoano. A experiência ocorreu ao longo do mês de outubro de 2019. A implementação do Processo de Enfermagem foi guiada à luz da Teoria das Necessidades Humanas Básicas, proposta por Wanda Aguiar Horta. Resultados: Foi implementado o Processo de Enfermagem, com base em suas seis fases, que identificou os problemas do indivíduo e possibilitou o levantamento das intervenções necessárias para efetivar o cuidado, conforme a teoria sugere. A implementação dessa ferramenta científica, de maneira dinâmica e inter-relacionada, proporcionou uma assistência hospitalar mais segura e humanizada ao indivíduo. Conclusão: O cumprimento das seis fases do Processo de Enfermagem permitiu aos enfermeirandos vivenciarem a experiência hospitalar de forma enriquecedora, pelo fato de ter contribuído não apenas com a evolução satisfatória do paciente assistido, mas também com o fortalecimento do trabalho em equipe e da Enfermagem enquanto ciência aplicada. Objective: To describe the experience of undergraduate Nursing students at the Federal University of Alagoas during the implementation of the Nursing Process to an individual affected by Fournier's Syndrome. Methods: Descriptive study, of the experience report type, based on the experience of Nursing students, during the curricular activities of the Supervised Internship in an Intensive Care Unit of a public hospital in Alagoas. The experience occurred throughout the month of October, 2019. The implementation of the Nursing Process was guided according to the Theory of Basic Human Needs, proposed by Wanda Aguiar Horta. Results: The Nursing Process was implemented, based on its six phases, which identified the individual's problems and enabled the survey of the necessary interventions to carry out care, as the theory suggests. The implementation of this scientific tool, in a dynamic and interrelated way, provided safer and more humanized hospital care to the individual. Conclusion: The fulfillment of the six phases of the Nursing Process allowed the Nursing students to undergo the hospital experience in an enriching way, because it contributed not only to the satisfactory evolution of the assisted patient, but also to the strengthening of the teamwork and of Nursing as an applied science. Metodos: Estudio descriptivo, tipo de informe de experiencia, basado en la experiencia de estudiantes de Enfermería durante las actividades curriculares de pasantías supervisadas, en una Unidad de Cuidados Intensivos de un hospital público en Alagoas. La experiencia ocurrió durante todo el mes de Octubre de 2019. La implementación del Proceso de Enfermería se guió a la luz de la Teoría de las Necesidades Humanas Básicas, propuesta por Wanda Aguiar Horta. Resultados: El proceso de enfermería se implementó, en base a sus seis fases, que identificaron los problemas del individuo y permitieron la encuesta de las intervenciones necesarias para llevar a cabo la atención, como sugiere la teoría. La implementación de esta herramienta científica, de manera dinámica e interrelacionada, brindó una atención hospitalaria más segura y humanizada al individuo. Conclusión: El cumplimiento de las seis fases del Proceso de Enfermería permitió a los estudiantes de Enfermería vivir la experiencia hospitalaria de manera enriquecedora, debido a que contribuyó no solo a la evolución satisfactoria del paciente asistido, sino también al fortalecimiento del trabajo en equipo y de la Enfermería como ciencia aplicada. Resumen Objetivo: El objetivo de este manuscrito es presentar el caso de un varón de 41 años que debuta con shock séptico y fascitis necrotizante abdominal en el posoperatorio del desbridamiento de un absceso perianal para focalizar la atención del lector en la posible evolución clínica hacia gangrena de Fournier. Materiales y Método: Tras la intervención, el paciente refiere aumento de temperatura y sensación de crepitación subcutánea a nivel abdominal, junto con empeoramiento clínico y hemodinámico, evidenciándose evolución tórpida hacia gangrena de Fournier extendida a región abdominal. Resultados: Tras la reintervención, el paciente presentó una evolución favorable aunque requirió sucesivas curas y desbridamientos quirúrgicos. Conclusiones y Discusión: Cabe destacar la importancia de una exploración clínica completa y detallada previa a cualquier intervención quirúrgica, así como el diagnóstico temprano en situaciones de shock séptico que permitan inicio de antibioterapia precoz y control del foco eficaz.
Aim: The objective of this manuscript is to present the case of a 41-year-old man with septic shock and abdominal necrotizing fasciitis after drainage of an interesphinteric perianal abscess to focus the reader's attention on the possible clinical evolution towards Fournier's gangrene. Materials and Method: After the intervention, the patient reported an increase in temperature and a sensation of subcutaneous crepitus at the abdominal level, with clinical and hemodynamic worsening, showing a torpid evolution towards Fournier's gangrene extended to the abdominal area. Results: After the reoperation, the patient presented a favorable evolution, although he required successive cures and surgical debridements. Conclusions and Discussion: It is worth highlighting the importance of a complete and detailed clinical examination prior to any surgical intervention, as well as the early diagnosis in situations of septic shock that allow early initiation of antibiotic therapy and effective control of the focus.

PURPOSE: Management of Fournier's Gangrene (FG) includes broad-spectrum antibiotics with adequate surgical debridement, which should be performed within the first 24 hours of onset. However, this treatment may cause significant loss of tissue and may delay healing with the presence of ischemia. Hyperbaric oxygen therapy (HBOT) has been proposed as adjunctive therapy to assist the healing process. However, its benefit is still debatable. Therefore, this systematic review and meta-analysis aimed to evaluate the effect of HBOT as an adjunct therapy for FG. MATERIALS AND METHODS: This study complied with the Preferred Reporting Items for Systematic Reviews and Meta-analyses protocol to obtain studies investigating the effect of HBOT on patients with FG. The search is systematically carried out on different databases such as MEDLINE, Embase, and Scopus based on population, intervention, control, and outcomes criteria. A total of 10 articles were retrieved for qualitative and quantitative analysis. RESULTS: There was a significant difference in mortality as patients with FG who received HBOT had a lower number of deaths compared to patients who received conventional therapy (Odds Ratio 0.29; 95% CI 0.12 - 0.69; p = 0.005). However, the mean length of stay with Mean Difference (MD) of -0.18 (95% CI: -7.68 - 7.33; p=0.96) and the number of debridement procedures (MD 1.33; 95% CI: -0.58 - 3.23; p=0.17) were not significantly different. CONCLUSION: HBOT can be used as an adjunct therapy to prevent an increased risk of mortality in patients with FG.

BACKGROUND Fournier's gangrene (FG) is a rapidly progressive necrotizing infection of the perineum. Risk factors include male sex and immunosuppression. Inflammatory bowel disease and pregnancy may alter immune response by complex mechanisms but have rarely been associated with necrotizing infections of the perineum. To the best of our knowledge, only 5 cases of FG in association with IBD have been reported in the literature, and none of them occurred during pregnancy. CASE REPORT We report the case of a young woman with long-standing Crohn's disease in clinical remission with Infliximab monotherapy who developed FG in the third trimester of pregnancy. A cesarean section was undertaken at 35 weeks due to fetal distress, followed by debridement, diverting stoma, and vacuum-assisted therapy. The perineal defect was closed following 4 debridements and vacuum-therapy exchanges with a unilateral medial thigh advancement flap, and a draining seton was placed in the suprasphincteric fistula. The patient was discharged after 28 days and her recovery was unremarkable. The neonate also recovered well. CONCLUSIONS The treatment of FG is multidisciplinary and includes early debridement and intestinal diversion. Perianal pain should not be disregarded, as it may be the initial symptom of severe perianal sepsis in the immunosuppressed. To the best of our knowledge, this is the first case report of FG during pregnancy in a patient with Crohn's disease.

Lesões perineais são produtos de um vasto grupo de patologias e atingem o ser humano como um todo, uma vez que afetam não só a saúde do paciente na sua forma propriamente dita, como também a sua saúde mental. Dentre tais etiologias podemos citar câncer anal, gangrena de Fournier e irradiação local. Ao longo dos anos diversas técnicas cirúrgicas foram desenvolvidas com o intuito de trazer resolubilidade para essa questão tão delicada, dentre algumas podemos citar enxertos de pele, retalhos de avanço local, retalhos múltiplos fasciocutâneos e miocutâneos. Tais técnicas trazem consigo indicações e possíveis complicações, portanto vários fatores devem ser levados em consideração para a escolha final, como bons resultados finais e menores taxas de complicação. O estudo analisou dez pacientes submetidos às mais diversas técnicas de cobertura das áreas cruentas, devido gangrena de Fournier e lesões perianais, com evolução e resultado satisfatórios. Em conclusão fica claro que a comparação das possíveis intervenções permanece desafiadora. Todavia, os resultados aqui apresentados reforçaram fortemente o uso de retalhos fasciocutâneos para redução da morbidade perineal. Palavras-chave: Reconstrução Perineal. Cirurgia Plástica. RESUMEN La ingestión de cuerpos extraños (en su mayoría espinas de pescado y huesos de pollo) durante la alimentación es frecuente. En gran parte de los casos ocurre accidentalmente, de manera inadvertida, habitualmente silente, en personas alcohólicas, con enfermedad mental o que usan prótesis dental. Generalmente pasan a través del tubo digestivo sin causar daño, y son expulsadas cerca de los siete días después de la ingestión. En cambio, en algunos pacientes puede ocasionar oclusión o perforación del tubo digestivo. Entre las zonas más afectadas están: la región ileocecal y la rectosigmoidea, por su angulación. Se presenta un paciente de 54 años de edad, bebedor habitual y fumador, con perforación del recto por espina de pescado, la cual fue ingerida de forma inadvertida días anteriores. Se le realizó una transversostomía para descompresión; posteriormente desarrolló una gangrena de Fornier en la región genital, y se le realizó una necrectomía y orquiectomía derecha.
ABSTRACT Foreign body ingestion (mostly fish bones and chicken bones) during feeding is common. In most cases, it occurs accidentally, inadvertently, usually silently, in people who are alcoholics, mentally ill, or who wear dental prostheses. They generally pass through the digestive tract without causing damage, and are expelled about seven days after ingestion. On the other hand, in some patients it can cause occlusion or perforation of the digestive tract. The ileocecal and rectosigmoid regions are the most affected areas, due to their angulation. We present a 54-year-old male patient, a habitual drinker and smoker, with a rectal perforation caused by a fish bone, which was inadvertently ingested days before. A decompressing transversostomy was performed; later he developed Fornier's gangrene in the genital region, and a necrosectomy and right orchiectomy were performed.

RESUMEN Introducción: La gangrena de Fournier se define como una forma específica de fascitis necrotizante Tipo I, que constituye una emergencia urológica, poco frecuente y de elevada mortalidad. Objetivo: Caracterizar la gangrena de Fournier en los pacientes atendidos en el servicio de Urología del Hospital General Docente "Dr. Agostinho Neto" en el periodo enero 2008 - diciembre 2018. Método: Se realizó un estudio descriptivo de variables demográficas y clínicas (edad, sexo, mortalidad, comorbilidad, causas desencadenantes, sitio primario de la infección, sitio de extensión de la infección, estadía hospitalaria) en 20 pacientes. La historia clínica fue la fuente de información primaria. Los datos fueron procesados mediante el programa estadístico SPSS versión 20.0, y se utilizó la estadística descriptiva para determinar las frecuencias absolutas y relativas (porcentajes) y media. Resultados: El 100 % correspondió a varones con un promedio de edad de 64,0 años (27-92); la mortalidad fue del 40 %. La diabetes mellitus fue la comorbilidad más frecuente en 10 pacientes (50,0 %), 12 (60,0 %) tenían más de un proceso comórbido. Las lesiones cutáneas constituyeron la causa primaria en 11 (55,0 %), y los escrotos fueron el principal sitio de origen de la infección en el 55,0 %. El promedio de estadía hospitalaria fue de 37,7 días (7-75 días). Conclusiones: La correcta evaluación de las variables demográficas y clínicas es necesaria para establecer un correcto diagnóstico y plan de tratamiento.
ABSTRACT Introduction: Fournier's gangrene is defined as a specific form of Type I necrotizing fasciitis, which constitutes a rare urological emergency with high mortality. Objective: To characterize Fournier's gangrene in patients treated in the Urology service of the Hospital General Docente "Dr. Agostinho Neto" in the period from January 2008 to December 2018. Method: A descriptive study of demographic and clinical variables (age, gender, mortality, comorbidity, triggering causes, primary site of infection, site of extension of the infection, hospital stay) was carried out in 20 patients. The medical history was the primary source of information. The data were processed using the statistical program SPSS, version 20.0, and descriptive statistics were used to determine the absolute and relative frequencies (percentages) and mean. Results: 100% corresponded to men with an average age of 64.0 years (27-92); mortality made a 40%. Diabetes mellitus was the most frequent comorbidity, in 10 of the patients (50.0%); 12 patients (60.0%) had more than one comorbid process. Skin lesions were the primary cause in 11 (55.0%), and the scrotum was the main site of origin of infection in 55.0%. The mean hospital stay was 37.7 days (7-75 days). Conclusions: The correct evaluation of the demographic and clinical variables is necessary to establish a correct diagnosis and treatment plan.
RESUMO Introdução: A gangrena de Fournier é definida como uma forma específica de fasceíte necrosante tipo I, que se constitui em uma rara emergência urológica com alta mortalidade. Objetivo: Caracterizar a gangrena de Fournier em pacientes atendidos no serviço de Urologia do Hospital Geral Universitário "Dr. Agostinho Neto" no período de janeiro de 2008 a dezembro de 2018. Método: Estudo descritivo de variáveis demográficas e clínicas (idade, sexo, mortalidade, comorbidade, causas desencadeantes, sítio primário de infecção, extensão do sítio de infecção, internação) em 20 pacientes. O histórico médico foi a principal fonte de informação. Os dados foram processados no programa estatístico SPSS versão 20.0, e a estatística descritiva foi utilizada para determinar as frequências absolutas e relativas (percentuais) e a média. Resultados: 100% corresponderam a homens com idade média de 64,0 anos (27-92); a mortalidade foi de 40%. Resumen Objetivo: Describir el manejo quirúrgico realizado para la reconstrucción genitoperineal (RGP) en pacientes con secuelas de Gangrena de Fournier (GF). Materiales y Método: Corresponde a una serie de casos retrospectiva de pacientes con secuelas de GF a los que se les realizó RGP entre los años 2011 y 2019. Se realizó un análisis descriptivo con las variables de técnica quirúrgica, edad, sexo, comorbilidades, subunidades anatómicas afectadas, origen anatómico de la gangrena de Fournier, número de procedimientos quirúrgicos, procedimiento de colostomía, terapia de presión negativa, Flexi-Seal®, bacterias aisladas, duración de estancia hospitalaria, tipo de procedimientos reconstructivos y complicaciones. Resultados: Se realizó RGP a 43 pacientes (81,1% hombres), con un promedio de edad de 59,1 (17-86 años), 72,7% eran diabéticos. El número de subunidades involucradas se asocia directamente y significativamente en relación al número de intervenciones quirúrgicas. Las técnicas utilizadas para la reconstrucción en orden de frecuencia fueron: colgajos (23%), cierre parcial más injerto dermoepidérmico de grosor parcial (IPP) (20%), cierre parcial (16%) e IPP (16%), cierre por segunda intención (10%), colgajo más IPP (7%) y cierre parcial para cierre por segunda intención de zona restante (5%). Discusión: La elección de reconstrucción se basa en las características del defecto, es decir, el tamaño, la ubicación y profundidad, así como la disponibilidad de tejido local. De preferencia optar por cierres primarios sin tensión, seguido de colgajos y de IPP. Conclusión: La RGP es un desafío para el cirujano plástico. Las técnicas descritas han demostrado ser seguras y reproducibles para el tratamiento quirúrgico de la gangrena de Fournier.
Aim: To describe the surgical management performed for genital-perineal reconstruction (GPR) in patients with sequelae of Fournier gangrene (FG). Materials and Method: It is based on a retroactive series of cases of patients with effects of FG who were given GPR between 2011 and 2019. We performed a descriptive analysis using the variables surgical technique, age, sex, comorbidities, anatomical subunits affected, anatomic origin of the Fournier's gangrene, number of surgical procedures, colostomy procedure, negative pressure therapy, Flexi-Seal®, bacteria isolated, hospital stay, type of reconstructive procedures and complications. We performed GPR on 43 patients (81.1% male), with a mean age of 59.1 (17-86 years); 72% were diabetic. The number of subunits involved was directly and significantly associated with the number of surgical interventions. Results: The reconstruction techniques most used were, in descending order: flaps (23.2%), wound closure and split-thickness skin graft (STSG) (23.2%), primary closure (16.2%), STSG (16.2%), secondary closure (9.3%), flap and STSG (6.9%) and partial wound closure for healing of the remaining area for a second intention (4.6%). Discussion: The reconstruction choice is based on the characteristics of the defect, that is, the size, location and depth, as well as the availability of local tissue. Preferably opt for primary closures without tension, followed by flaps and IPP. Conclusion: The RGP is a challenge for the plastic surgeon. The techniques described have proven safe and reproducible for the surgical treatment of Fournier Gangrene.

There is no consensus about the role of adjunctive hyperbaric oxygen therapy (HBOT) in the management of Fournier's gangrene. The aim of this study was to compare the evolution of patients with Fournier's gangrene treated with all classical measures with and without adjuvant HBOT. A retrospective comparative study regarding the evolution of patients treated for Fournier's gangrene was conducted in two periods. In period I, from 1990 to 2002, patients received standard treatments for Fournier's gangrene, which consisted of surgical debridement, antibiotic therapy and intensive care. In period II, from 2012 to 2019, adjunctive HBOT was added to the classical management strategy. All patients were assigned into four groups according to the anatomical severity classification and the area affected after the first debridement. This classification ensured that the groups could be comparable. The total number of patients in this study was 197, and these patients were divided into control group (118/59.9%) and HBOT group (79/40.1%). The mean age, comorbidities, and anatomical severity classification were similar between the two groups. In period I, 34 out of 118 (28.8%) patients died, while in the HBOT group, 3 out of 77 (3.7%) patients died (P < 0.001). The use of adjuvant HBOT in combination with classical treatment was associated with reduced mortality. This study was approved by the Institutional Review Board and the Ethics Committee of Ribeirão Preto Medical School, University of São Paulo, Brazil (No. 08/2018) on May 2, 2018.

PURPOSE: To determine the effectiveness of Vacuum-Assisted Closure Device in the postoperative wound care for Fournier's gangrene METHODS: We performed a systematic review in the following databases: Medline (Ovid), EMBASE, and The Cochrane Central Register of Controlled Trials (CENTRAL), from inception to nowadays. We included RCTs and analytical observational studies. Meta-analysis was not possible given the clinical and methodological heterogeneity of the studies. RESULTS: We included six studies that compared VAC treated patients and a control group. The length of stay of the VAC treated vs. the conventional dressing treated patients was higher for the VAC treated patients in all but one of the included studies. The VAC group had the highest number of surgical debridements requiring anesthesia. The conventionally treated patients had a higher number of daily dressings, and the need for additional dressing changes, without anesthesia. Two studies found significantly higher mean scores for VAS, requiring a higher need for daily analgesics for the control group patients. CONCLUSIONS: VAC therapy is an effective method, but it is not better than conventional dressing treatment. VAC carries fewer dressing changes, less pain, and less need for analgesics, but it comes with a higher need for surgical interventions requiring anesthesia.

La gangrena de Fournier es una fascitis necrosante de etiología polimicrobiana. Se ha descrito como la extensión de un foco infeccioso primario de estructuras adyacentes, en las regiones perineal, perianal y genital. Esta patología tiene una rápida progresión y alta tasa de mortalidad, por lo que su diagnóstico precoz es fundamental. Se presenta el caso de un paciente con gangrena de Fournier que fue diagnosticado a partir de hallazgos ecográficos y abordado quirúrgicamente con un desenlace favorable.
Fournier's gangrene represents a necrotizing fasciitis of polymicrobial etiology. It has been described as an extension of a primary infectious focus of adjacent structures in the perineal, perianal and genital region. This pathology presents a rapid progression and a high mortality rate, so its early diagnosis is essential in the approach. We present the case of a patient with Fournier's gangrene who was diagnosed based on ultrasound findings and treated surgically with a favorable outcome.

ABSTRACT Objective: To describe and analyze the cases of Fournier's Gangrene caused by perianal abscess treated in a tertiary hospital in western Paraná, correlating possible factors that influence mortality, with emphasis on late diagnosis and therapy. Methods: A retrospective and descriptive case series was carried out based on the analysis of medical records of patients with Fournier's Gangrene due to perianal abscess from January 2012 to December 2017. Results: Thirty-one patients with Fournier's Gangrene due to perianal abscess were treated in the period: 26 men and 5 women. Mean age was 53.51 ± 14.5 years. The most prevalent comorbidity in this group was type 2 diabetes mellitus, showing a strong correlation with mortality. The mean time from disease progression, from the initial symptom to the admission at the service, was 9.6 ± 6.81 days. All patients were submitted to antibiotic therapy and surgical treatment, with a mean of 3.25 ± 2.89 procedures/patient. Seven (22.58%) patients died and all of them showed signs of sepsis on admission; only 2 patients with sepsis did not die. Conclusion: The presence of sepsis on admission and type 2 diabetes mellitus were strongly correlated with mortality.
RESUMO Objetivo: Descrever e analisar os casos de gangrena de Fournier por abscesso perianal atendidos em hospital terciário do oeste do Paraná, correlacionando possíveis fatores que influenciem a mortalidade, com ênfase ao diagnóstico e terapêuticas tardias. Métodos: Realizou-se um estudo de série de casos, retrospectivo e descritivo baseado na análise de prontuários de pacientes portadores de gangrena de Fournier devido a abscesso perianal no período de Janeiro de 2012 à Dezembro de 2017. Resultados: Foram tratados 31 pacientes com gangrena de Fournier por abscesso perianal no período, sendo 26 homens e 5 mulheres. A média de idade foi de 53,51 ± 14,5 anos. A comorbidade de maior prevalência neste grupo foi diabete melitus tipo 2, demonstrando forte correlação com mortalidade. A média do tempo de evolução da doença, do sintoma inicial até entrada no serviço, foi de 9,6 ± 6,81 dias. Todos os pacientes foram submetidos à antibioticoterapia e tratamento cirúrgico com média de 3,25 ± 2,89 procedimentos/paciente. Sete (22,58%) pacientes evoluíram para óbito e todos estes apresentavam sinais de sepse na admissão; apenas 2 pacientes com sepse não evoluíram a óbito. Conclusão: Presença de sepse a admissão e diabete melitus tipo 2 foram fortemente correlacionadas com mortalidade.

Fournier's Gangrene is a fulminating necrotizing fasciitis of the perineum and genitalia. Standard treatment involves immediate excision of all necrotic tissue, aggressive antibiotic coverage, and supportive medical care. Still, the infection is commonly fatal or disfiguring. Wound treatment with disinfected blowfly larvae (maggot debridement therapy or MDT) has been shown to be highly effective, with multiple studies demonstrating effective debridement, disinfection, and promotion of granulation tissue. MDT also has been associated with preservation of viable tissue and minimised blood loss. This report describes a prospective clinical study of MDT for Fournier's gangrene aimed to test the hypothesis that early use of maggots could decrease the number of surgical treatments required to treat Fournier's gangrene. Subjects were provided with one initial surgical excision, followed by debridement using only medical grade Lucilia sericata larvae. Only two subjects were enrolled, both diabetic men. Intensive care and culture-directed antimicrobial coverage were administered as usual. Maggot debridement was associated with the disappearance of necrotic tissue, control of infection and granulation tissue growth. In both subjects, wounds healed without requiring further surgical resection or anatomical reconstruction. Maggot therapy decreased the number of surgical procedures that otherwise would have been necessary, and led to favourable outcomes.
