ABSTRACT

INTRODUCTION: Inguinal hernia is defined as a projection of an organ through the inguinal canal. This can be incarcerated as a consequence of continuous inflammation of the hernial sac, which will prevent its return, causing damage to the venous and lymphatic return of the viscera. The neutrophil-to-lymphocyte ratio (NLR) is an easily accessible inflammatory biomarker obtained from blood cell counts. Therefore, the objective was to determine if the NLR is useful as a predictor of intestinal resection in incarcerated inguinal hernias. MATERIAL AND METHOD: An observational, analytical, diagnostic test and retrospective study was carried out in a hospital in northern Peru from January 2013 to August 2019 in the Department of General Surgery and Emergency Surgery and Critical Care. Patients diagnosed with unilateral inguinal hernia with intestinal obstruction were included. For the relationship between the event and the exposure, it was analyzed using Chi square (χ2) and T-Student. The sensitivity, specificity, positive predictive value, negative predictive value of the NLR as well as the area under the ROC curve were found to determine the predictive accuracy. RESULTS: 161 patients with incarcerated inguinal hernia were studied: group I (20 patients with intestinal resection) and group II (141 patients without intestinal resection). The mean age in groups I and II were 69±16 and 60±17 years (p<0.05); the frequency in males was 70% in group I and 76% in group II (p>0.05). Intestinal obstruction and duration of incarceration >24 hours and the platelet-to-lymphocyte ratio demonstrated significant differences. With respect to NLR taking a cut-off point ≥6.5, a sensitivity of 75%, a specificity of 93.62%, a positive predictive value of 62.5% and a negative predictive value of 96.35% were observed; In addition, when analyzing with the ROC curve, a value of 5.14 was obtained as a predictor of intestinal resection with a sensitivity of 90% and a specificity of 84.4% (p<0.001). Therefore, the NLR >5.14 predicts intestinal resection in patients with incarcerated inguinal hernias with an area under the curve of 0.92 at the Belen Hospital of Trujillo. CONCLUSIONS: The neutrophil-to-lymphocyte ratio is useful for predicting intestinal resection with a diagnostic accuracy of 92%.

INTRODUCTION: recurrence rates for primary hernia repair range from 0.5 to 15 percent depending upon the hernia site, type of repair, and clinical circumstances. Many risk factors are known and they must be considered before the procedure. In developing countries, follow up and maintenance of databases are critical to understand the real numbers. METHODS: a retrospective cohort study analyzed adult patients who have undergone inguinal hernia repair at Hospital de Clínicas de Porto Alegre, a tertiary care government public hospital, between 2013 and 2015. Medical records, telephone, and letter contact have been reviewed in order to complete the minimum period of 5 years of follow-up. The analyzed data focused on the surgeon's experience and the recurrence rate in 5 years of follow-up. RESULTS: a total of 1094 medical records were selected and a complete five years follow-up were possible in 454 patients - 538 inguinal hernia repairs due to bilateral approach in 84 patients. These 454 patients answered, in a validated questionnaire about symptoms of recurrence. The total recurrence rate was 9.29%. For the patients who had Nyhus IV, recurrence rate was 24.1% against 9.9% after primary hernia repair, with a 2.4 higher risk. There was no difference in recurrence between surgeons and training surgeons. CONCLUSION: our data reveal an acceptable recurrence rate in a tertiary care hospital with residents, and to our knowledge is the first Brazilian report with long term follow up. An increased re-recurrent hernia was found when compared with primary hernia repair.

ABSTRACT

Laparoscopy was introduced more than 100 years ago. However, in some fields its use still meets resistance. Technology such as laparoscopy may help to identify rare and complex disorders, even in very ordinary procedures, such as inguinal hernia repair. This report highlighted the importance of early diagnosis of a complex condition using commonly available technology. To the best of our knowledge, there has not been a similar reported case in such a young patient during laparoscopic inguinal hernia repair.

Introducción: La hernia inguinal, es una de las enfermedades quirúrgicas que más polémicas ha provocado a través de la historia, existen disimiles clasificaciones e incontables técnicas para su reparación. Objetivo: Mostrar los resultados del tratamiento quirúrgico laparoscópico de la hernia inguinal en el Centro Nacional de Cirugía de Mínimo Acceso. Métodos: Se realizó un estudio retrospectivo y descriptivo de 737 pacientes intervenidos con el diagnóstico de hernia inguinal por cirugía laparoscópica en el Centro Nacional de Cirugía de Mínimo Acceso, en el período comprendido entre enero del 2011 hasta diciembre el 2018. Resultados: Se mostró la efectividad de las técnicas laparoscópicas, con un bajo índice de recidiva, de 1,4 por ciento, bajo índice de conversión, con 0,5 por ciento, la inguinodinia se presentó en 5 pacientes para un 0,5 por ciento y predominó el seroma entre las complicaciones postoperatorias. Conclusiones: La cirugía laparoscópica en la hernia inguinal es una opción segura y eficaz en el tratamiento de la enfermedad(AU)


Introduction: Inguinal hernia has been one of the most controversial surgical diseases throughout history. There are several classifications and countless techniques for inguinal hernia repair. Objective: To show the outcomes of laparoscopic surgical treatment of inguinal hernia at the National Center for Minimal Access Surgery. Methods: A retrospective and descriptive study was carried out of 737 patients with a diagnosis of inguinal hernia operated on by laparoscopic surgery, in the period from January 2011 to December 2018, at the National Center for Minimal Access Surgery. Results: The effectiveness of laparoscopic techniques was shown, with a low recurrence rate of 1.4 percent and a low conversion rate of 0.5 percent, while inguinodynia appeared in five patients, accounting for 0.5 percent, and postoperative seroma was a predominating complication. Conclusions: Laparoscopic surgery is a safe and effective treatment option for in inguinal hernia(AU)

ABSTRACT

Se describe el caso clínico de un paciente de 58 años de edad con antecedentes personales de insuficiencia cardiaca congestiva mal controlada, quien acudió a emergencias del Hospital Luis Bernaza de Guayaquil, Ecuador, por presentar dolor abdominal difuso, constipación y masa inguinoescrotal gigante. Se realizan los estudios pertinentes y se diagnostica una hernia inguinoescrotal gigante con pérdida de domicilio, sin datos de obstrucción intestinal. Se empleó la técnica de Goñi Moreno durante 12 días para aumentar el diámetro de la cavidad abdominal y regresar el contenido del saco a su lugar. Se efectuó laparotomía exploratoria y hernioplastia con resultado satisfactorio y seguimiento por consulta externa durante 2 meses, sin alteraciones.


The case report of a 58 years patient with personal history of a poor controlled congestive heart failure is described, who went to the emergency service of Luis Bernaza Hospital in Guayaquil, Ecuador, due to a diffuse abdominal pain, constipation and giant inguinoescrotal mass. The pertinent studies were carried out and a giant inguinoescrotal hernia is diagnosed with home loss, without data of intestinal obstruction. The Goñi Moreno technique was used during 12 days to increase the diameter of the abdominal cavity and return the content from the sack to its place. An exploratory laparotomy and hernioplastia were carried out with satisfactory result and follow up in the outpatient department during 2 months, without alterations.

Resumen Introducción: La hernia de pared abdominal es una patología habitual; la presentación más frecuente es de tipo inguinal, cercano a un 70% del total. La incidencia de un saco herniario inguinal conteniendo ovarios y trompas de Falopio, es un hecho reportado en 2,9% de los casos. Caso clínico: Se presenta una paciente, sexo femenino, de 42 años de edad, con historia de hernia inguinal derecha de larga data, ingresa por aumento de volumen doloroso e irreductible, en región inguinal derecha, no impresiona estrangulada; ingresa a pabellón de urgencia. Dentro de los hallazgos quirúrgicos destacan saco herniario que contiene útero y ambos ovarios, sin compromiso vascular. Paciente evoluciona de forma favorable egresando 2 días posterior a la cirugía.


Introduction: Abdominal wall hernia is a frequent pathology, the most frequent hernia are the inguinal type, closed to 70% of all. Although the incidence of inguinal hernial sac containing ovary and Fallopian tubes are reported on a 2.9% of the cases. Case Report: Female patient 42 years old, with a long-term history of right inguinal hernia, with sudden pain and irreducible increase of volume in the correspondent inguinal zone that doesn't look strangled; she was admitted to the emergency operating room. Among surgical findings hernia's sac content was uterus and both ovaries with no signs of vascular compromise. The patient's favorable evolved let her to be discharged from the hospital after 2 days from post operative care

Las hernias inguinales son las anomalías más comunes en pediatría que requieren de tratamiento quirúrgico. El 70% desarrolla una hernia inguinal unilateral y el 30% restante desarrolla hernia inguinal contralateral. La finalidad de llevar a cabo una exploración contralateral es disminuir el riesgo de recurrencia de hernias metacrónicas en pacientes de riesgo que tienen persistencia del processus vaginalis, así como evitar el daño a estructuras subyacentes por llevar a cabo una exploración abierta1,2. El objetivo fue determinar la prevalencia de hernias inguinales contralaterales en niños y niñas menores de 5 años con factores de riesgo, identificadas a través de peritoneoscopía del 1 de marzo de 2015 al 31 de marzo del 2018 en el Hospital Roosevelt. El diseño de este estudio fue descriptivo y transversal. Se realizó en el departamento de Cirugía Pediátrica del Hospital Roosevelt en la ciudad de Guatemala. Los materiales y métodos empleados consistieron en un estudio realizado a través de la revisión de 128 expedientes de menores de 5 años del Departamento de Cirugía Pediátrica del Hospital Roosevelt que hayan presentado una hernia inguinal entre el período indicado y a quiénes se les practicó peritoneoscopía. Como resultado se comprobó que la prevalencia de hernia inguinal contralateral diagnosticada por peritoneoscopía fue del 59% IC(44,72), 26 casos, en cuyos casos se efectuó cirugía correctiva bilateral, sin recurrencia. Esto permitió concluir que la prevalencia de hernias inguinales se da con mayor frecuencia en niños y niñas menores. (AU)


Inguinal hernias are the most common anomalies in pediatrics that require surgical treatment. 70% develop a unilateral inguinal hernia and the remaining 30% develop a contralateral metachronous inguinal hernia. The purpose of performing a contralateral examination is to decrease the risk of recurrence of metachronoushernias in risk patients who have persisten tprocessus vaginalis, as well as to avoid damage to underlying structures by conducting an open exploration1,2. The objective was to determine the prevalence of contralateral inguinal hernias in boys and girls under 5 years of age with risk factors, identified through peritoneoscopy from March 1, 2015 to March 31, 2018 at Roosevelt Hospital. The design of this study was descriptive and transversal. It was performed in the Pediatric Surgery department of the Roosevelt Hospital in Guatemala City. The materials and methods used consisted of a study conducted through the review of 128 records of children under 5 years of the Department of Pediatric Surgery at Roosevelt Hospital who presented an inguinal between the indicated period and who underwent peritoneoscopy. As a result, it was found that the prevalence of contralateral inguinal hernia diagnosed by peritoneoscopy was 59% IC(44,72) 26 cases, in which cases bilateral corrective surgery was performed, without recurrence. This allowed us to conclude that the prevalence of inguinal hernias occurs more frequently in boys and girl. (AU)

INTRODUCCIÓN: La apendicitis aguda neonatal es una patología de muy baja frecuencia, con escasos reportes en los últimos 30 años. La clínica e imágenes son inespecíficas, diagnosticándose la mayoría de las veces durante el procedimiento quirúrgico. OBJETIVO: Presentar un caso de apendicitis aguda perfora da en paciente prematura, con diagnóstico prequirúrgico y resolución laparoscópica, asociada a aparición posterior de hernia inguinal. CASO CLÍNICO: Recién nacido femenino de 34 semanas que a los 17 días de vida presentó fiebre, irritabilidad y aumento del residuo lácteo. Al examen físico destacaba distensión y sensibilidad del abdomen y tanto la radiografía como ecografía abdominales eran compatibles con apendicitis aguda perforada, por lo cual ingresó a pabellón para laparoscopía. Confirmado el diagnóstico, se realizó apendicectomía. Dos semanas después en consulta ambulatoria, se evidenció y confirmó con ecografía una hernia inguinal bilateral que se corrigió quirúrgicamente. CONCLUSIÓN: La apendicitis aguda debe considerarse dentro de los diagnósticos diferenciales en neonatos con clínica de abdomen agudo, principalmente en prematuros. En este caso, la ecografía permitió realizar el diagnóstico pre-operatorio. La posible asociación con hernia inguinal, debiera motivar a examinar siempre conductos inguinales durante la evaluación con imá genes y en el procedimiento quirúrgico.

OBJECTIVE: To describe a neonatal case of acute perforated appendicitis associated with later appearance of inguinal hernia, with pre-surgical diagnosis and treated through laparoscopy. CLINICAL CASE: A 17-day-old preterm female newborn presented with fever, irritability, and increased milk intolerance. Physical examination showed abdominal distention, tenderness and both, abdominal X- ray and ultrasound showed compatible images with acute perforated appendicitis. Once the diagnosis was confirmed, we performed an appendicectomy through laparoscopy. Two weeks later, during an outpatient visit, we observed a bilateral inguinal hernia which was confirmed by ultrasound, and then it was surgically corrected. CONCLUSION: Acute appendicitis should be considered within the differential diagnosis in neonates with acute abdominal symptoms, mainly in premature infants. In this case, ultrasound scan allowed us to make the pre-operative diagnosis. The possible association with inguinal hernias should motivate to examine inguinal ducts during imaging assessment and surgical procedure.

RESUMEN Introducción: En la provincia de Mayabeque se emplea la técnica de Lichtenstein en la reparación de la hernia inguinal, con una recidiva inferior al 3 por ciento. Objetivo: Caracterizar a los pacientes con diagnóstico de hernia inguinal operados por la técnica de hernioplastia de Lichtenstein. Métodos: Se realizó un estudio longitudinal y retrospectivo en pacientes operados de hernia inguinal en el Hospital "Leopoldito Martínez" de enero-2013 a diciembre-2017. El universo estuvo constituido por 218 y una muestra probabilística sistemática de 128 casos. Los datos se obtuvieron de las historias clínicas. Las variables cuantitativas se resumieron mediante media aritmética y desviación estándar y las variables cualitativas mediante los por cientos. La comparación de proporciones se realizó a través del chi-cuadrado y la dócima de Duncan, considerándose un nivel de significación para p < 0,05. Resultados: Predominó el sexo masculino (94,54 por ciento), la edad entre 41 - 60 años (42,96 por ciento), tipo de hernia III A (60,15 por ciento) y evolución media de la misma 42,3 meses; el tiempo quirúrgico fue de 1-2 horas (65,62 por ciento) con media de 1 h, 26 minutos. Prevalecieron las complicaciones dolor agudo (13,28 por ciento) e infección del sitio quirúrgico (3,12 por ciento) y estadía hospitalaria < 24 horas (79,68 por ciento) con una recidiva de 2,34 por ciento. Conclusiones: La técnica de Lichtenstein es efectiva en la reparación de la hernia inguinal en la provincia de Mayabeque. La media del tiempo quirúrgico se estima en 1 hora y 26 minutos como máximo y la estadía hospitalaria a expensas de las complicaciones de 24-72 horas. El por ciento de recidivas es alto y debe mejorar depurando la técnica(AU)


ABSTRACT Introduction: In Mayabeque Province, the Lichtenstein technique is used to repair inguinal hernia, with a recurrence below 3 percent. Objective: To characterize patients with a diagnosis of inguinal hernia operated on by the Lichtenstein hernioplasty technique. Methods: A longitudinal and retrospective study was carried out in patients operated on for inguinal hernia at Leopoldito Martínez Hospital, from January-2013 to December-2017. The universe consisted of 218, with a systematic probabilistic sample of 128 cases. The data were obtained from medical records. Quantitative variables were summarized by arithmetic mean and standard deviation, while qualitative variables were summarized by percentages. The comparison of proportions was carried out through the chi-square and Duncan's test, considering a level of significance of P< 0.05. Results: There was a predominance of the male sex predominated (94.54 percent), the age between 41 and 60 years (42.96 percent), and the hernia type III A (60.15 percent), with mean evolution of 42.3 month. Surgical time was 1-2 hours (65.62 percent), with a mean of one hour and 26 minutes. There was predominance of acute pain complications (13.28 percent), surgical-site infection (3.12 percent) and hospital stay below 24 hours (79.68 percent), with a recurrence of 2.34 percent. Conclusions: The Lichtenstein technique is effective for inguinal hernia repair in the Mayabeque Province. Mean surgical time is estimated to be a maximum of one hour and 26 minutes, while hospital stay, at the expense of complications, was 24-72 hours. The percentage of recurrences is high and should improve by refining the technique(AU)

PURPOSE: Relying solely on in-person encounters to assess long-term outcomes of hernia repair leads to substantial loss of information and patients lost-to-follow-up, hindering research and quality improvement initiatives. We aimed to determine if inguinal hernia recurrences could be assessed using the Ventral Hernia Recurrence Inventory (VHRI), a previously existing patient-reported outcome (PRO) tool that can be administered through the telephone and has already been validated for diagnosing ventral hernia recurrence. METHODS: A prospective, multicentric comparative study was conducted. Adult patients from two centers (United States and Brazil) at least 1 year after open or minimally invasive inguinal hernia repair were asked to answer the questions of the VHRI in relation to their prior repair. A physical exam was then performed by a blinded surgeon. Testing characteristics and diagnostic performance of the PRO were calculated. Patients with suspected recurrences were preferentially recruited. RESULTS: 128 patients were enrolled after 175 repairs. All patients answered the VHRI and were further examined, where a recurrence was present in 32% of the repairs. Self-reported bulge and patient perception of a recurrence were highly sensitive (84-94%) and specific (93-94%) for the diagnosis of an inguinal hernia recurrence. Test performance was similar in the American and Brazilian populations despite several baseline differences in demographic and clinical characteristics. CONCLUSION: The VHRI can be used to assess long-term inguinal hernia recurrence and should be reestablished as the Hernia Recurrence Inventory (HRI). Its implementation in registries, quality improvement efforts, and research could contribute to improving long-term follow-up rates in hernia patients.

Abstract: Background: hernia is one of the most frequent pathologies in General Surgery, a common reason for surgical consultation, the resolution of this pathology currently has extensive debates about the surgical approach related to open or laparoscopic hernioplasty, with advantages and disadvantages exposed to each. Objective: to determine the prevalence and factors associated with complications of inguinal posthernioplasty at the Vicente Corral Moscoso and José Carrasco Arteaga Hospitals, 2018-2019.Material and methods: It's analytical, cross-sectional study. 240 clinical records that met the inclusion criteria were reviewed. Data were collected using a form and were tabulated in the SPSS version 15 program. The qualitative variables were analyzed with frequency and percentage and the quantitative variables with arithmetic mean and standard deviation, the associated factors were analyzed with Odds Ratio, 95% CI, Chi square and binary logistic regression (p <0.05).Results: 240 patients who had the inclusion criteria were analyzed, the majority were men (81.7%), the mean age was 53.4 years SD ± 16.57. The prevalence of postoperative complications was 27.5%, with those occurring within the first 24 hours being more frequent with 15.8%. Among the factors associated with statistical significance were: male sex (OR 4.6; 95% CI 1.5-13.5; p <0.00) and surgical technique (OR 3.4; 95% CI 1.2-9; p <0.01).Conclusions: prevalence of postoperative complications was similar to the literature consulted and was associated with factors such as: male sex and surgical technique.

ABSTRACT

La hernia de Littré es la presencia de un divertículo de Meckel en cualquier hernia de la pared abdominal. Se encuentran pocos reportes en la literatura médica por su baja incidencia. Su sospecha clínica es infrecuente, por lo que su diagnóstico es intraoperatorio. Se presenta un caso de un paciente de 39 años de edad que acude al servicio de urgencias del Hospital Misionario de Vouga. Cunhinga, Bié. República de Angola, con dolor abdominal agudo, y no expulsión de gases. Al examen físico encontramos dolor abdominal, ligera defensa abdominal y tumor doloroso e irreductible en región inguinal derecha. Se ingresa con el diagnóstico de hernia inguinal derecha estrangulada. Es intervenido quirúrgicamente de urgencia con hallazgo transoperatorio de hernia de Littré estrangulada. Se realiza quelotomia más resección intestinal del segmento de íleo que contiene el divertículo y anastomosis término terminal, por los signos de isquemia y necrosis presentes más herniorrafia inguinal derecha. El paciente fue dado de alta a los 10 días con evolución favorable(AU)


The Littré hernia is the presence of a Meckel diverticulum in any hernia of the abdominal wall. There are few reports in the medical literature due to its low incidence. His clinical suspicion is infrequent, so his diagnosis is intraoperative. We present a case of a 39-year-old patient who goes to the emergency service of the Hospital Misionariode Vouga. Cunhinga, Bié. Republic of Angola, with acute abdominal pain, and no expulsion of gases. Upon physical examination we found abdominal pain, slight abdominal defense and a painful and irreducible tumor in the right inguinal region. The patient isadmitted with the diagnosis of strangulated right inguinal hernia. He underwent emergency surgery with a strangulated Littr hernia. We performed kelootomy plus intestinal resection of the ileus segment that contains the end-stage diverticulum and anastomosis, due to signs of ischemia and necrosis plus right inguinal herniorrhaphy. The patient was discharged after 10 days with favorable evolution(EU)

ABSTRACT

Inguinal hernias are a frequent problem and their repair is the most commonly performed procedure by general surgeons. In the last years, new principles, products and techniques have changed the routine of surgeons, who need to recycle knowledge and perfect new skills. In addition, old concepts regarding surgical indication and risk of complications have been reevaluated. In order to create a guideline for the management of inguinal hernias in adult patients, the Brazilian Hernia Society assembled a group of experts to review various topics, such as surgical indication, perioperative management, surgical techniques, complications and postoperative guidance.


As hérnias inguinais são um problema frequente e o seu reparo representa a cirurgia mais comumente realizada por cirurgiões gerais. Nos últimos anos, novos princípios, produtos e técnicas têm mudado a rotina dos cirurgiões que precisam reciclar conhecimentos e aperfeiçoar novas habilidades. Além disso, antigos conceitos sobre indicação cirúrgica e riscos de complicações vêm sendo reavaliados. Visando criar um guia de orientações sobre o manejo das hérnias inguinais em pacientes adultos, a Sociedade Brasileira de Hérnias reuniu um grupo de experts com objetivo de revisar diversos tópicos, como indicação cirúrgica, manejo perioperatório, técnicas cirúrgicas, complicações e orientações pós-operatórias.

RESUMEN Introducción: Es necesario conocer la historia de la cirugía, en general, y de la herniología, en particular, para poder comprender mejor los enormes avances experimentados en esas disciplinas. Muchas de las técnicas quirúrgicas modernas están basadas en los conocimientos y experiencias aportados por los primeros cirujanos, pero no fue hasta la divulgación de la obra del francés Eduardo Bassini, en 1889, que comenzó el impetuoso desarrollo de la cirugía de la hernia inguinal a escala mundial. Entre las más notables reparaciones de hernia inguinal, además de la de Bassini, se citan las de McVay, Halsted, Shouldice y la hernioplastia libre de tensión, popularizada por Lichtenstein. Objetivo: Profundizar en los aspectos cognoscitivos relacionados con los fundamentos históricos del diagnóstico y tratamiento de la hernia inguinal, su evolución y aplicación en la cirugía actual. Método: Revisión bibliográfica digital de publicaciones actualizadas en las bases de datos SciELO, Latindex, Elsevier, PubMed, Medline y Web of Science y Google Académico. Conclusiones: La cirugía de las hernias en general, y de las inguinales, en particular, nunca es rutina; debe ser individualizada a las características de cada enfermo y a los recursos disponibles. El futuro de la cirugía herniaria solo puede entenderse ligado al conocimiento de sus fundamentos históricos, del profundo dominio de la anatomía topográfica y de una esmerada técnica quirúrgica(AU)


ABSTRACT Introduction: It is necessary to know the history of surgery, in general, and herniology, in particular, in order to understand better the enormous advances experienced in these disciplines. Many of the modern surgical techniques are based on the knowledge and experiences contributed by the first surgeons, but it was not until the disclosure of the work of the French Eduardo Bassini, in 1889, that the impetuous development of inguinal hernia surgery began to scale worldwide. Among the most notable inguinal hernia repairs, in addition to that of Bassini, are those of McVay, Halsted, Shouldice and tension-free hernioplasty, popularized by Lichtenstein. Objective: To deepen the cognitive aspects related to the historical foundations of the diagnosis and treatment of inguinal hernia, its evolution and application in current surgery. Method: Digital bibliographic review of publications updated in the databases SciELO, Latindex, Elsevier, PubMed, Medline and Web of Science and Google Scholar. Conclusions: Surgery for hernias in general, and particularly for inguinal hernias is never routine. They must be individualized to the characteristics of each patient and the resources available. The future of hernia surgery can only be understood if it is associated with the knowledge of its historical foundations, the deep mastery of topographic anatomy and a careful surgical technique(AU)

ABSTRACT

PURPOSE: Bladder hernias are asymptomatic in most cases and are found incidentally during exploration for inguinal hernia repair. The treatment of inguinal bladder hernia is either reduction or resection of the herniated bladder, followed by herniorrhaphy. We present a case series with preoperative diagnoses, along with their surgical outcomes. METHODS: We retrospectively reviewed the medical records from a single institution over a 5-year period (2012-2017) of five patients with diagnosis of large bladder inguinal hernia. Demographics, clinical status, medical history, anatomical structure of the hernia, and surgical outcomes were all analyzed. RESULTS: Patients' median age was 51 years (range 45-81 years). The median size of the hernial sac was 13 cm (range 8-20 cm). The diagnosis was made with computed tomography in three patients and with ultrasonography and cystography in two patients. Median length of hospital stay was 2 days (range 1-6 days), and median length of follow-up was 28 months (range 18-72 months). All patients continue to be alive and well, without hernia recurrence. The five cases are described separately along with their surgical managements. CONCLUSION: The main objectives in treatment of inguinal bladder hernia are to preserve the voiding function and to avoid bladder injuries in a tension-free hernia repair. To our knowledge, this is the first series of cases in which all inguinal bladder hernias were diagnosed preoperatively.

ABSTRACT

INTRODUCCIÓN: Los tumores paratesticulares representan del 7%-10% de las masas intraescrotales. Los sarcomas abarcan el 90% de las lesiones malignas del cordón espermático y de éstas, aproximadamente, el 3%-7% son liposarcomas. CASO CLÍNICO: Presentamos el caso de un varón de 45 años, que consultó en urgencias por una masa inguinoescrotal derecha no reductible, diagnosticándose de hernia inguinal incarcerada. Se realizó cirugía urgente evidenciando una gran tumoración de aspecto lipomatoso, dependiente de cordón espermático. Se realizó orquiectomía y hernioplastía inguinal. La anatomía patológica, reveló un liposarcoma bien diferenciado de cordón espermático. Posteriormente, se realizó estudio de extensión, sin afectación a distancia y no precisó tratamiento adyuvante. Actualmente, tras dos años de seguimiento no ha presentado recidiva. DISCUSIÓN: Sólo alrededor de 200 casos han sido comunicados previamente en la literatura y sólo 61 de éstos se presentaron simulando una hernia inguinal incarcerada. Debido a la baja incidencia de esta patología es difícil de conocer la historia natural y llegar a conclusiones sobre los resultados del tratamiento, el cual hasta el momento sigue siendo la orquiectomía radical, con escisión amplia de los tejidos locales. El papel de la radio y quimioterapia aun es controvertido. CONCLUSIONES: Los sarcomas del cordón espermático son neoplasias raras con alta tasa de recurrencia local. Su manejo inicial es quirúrgico. Se requiere de un alto índice de sospecha clínica para el diagnóstico ya que las implicaciones oncológicas varían en función del tratamiento que, en ocasiones, es llevado a cabo por cirujanos generales al simular una hernia inguinal.


INTRODUCTION: Paratesticular tumors represent 7%-10% of intraescrotal masses. Sarcomas account for 90% of malignant lesions of the spermatic cord and of these approximately 3%-7% are liposarcomas. CLINICAL CASE: This is the case of a 45 year old male who consulted in the emergency department for a non-reducible right inguino-scrotal mass and was diagnosed with an incarcerated inguinal hernia. Emergency surgery was performed which revealed a large lipomatous tumor, originating from the spermatic cord. Orchiectomy and hernioplasty were performed. Histopathology revealed a well-differentiated liposarcoma of the spermatic cord. Later extension study was conducted, without distant affectation, and did not require adjuvant treatment. Today, after two years of monitoring has been no recurrence. DISCUSSION: Only about 200 cases have been previously reported in the literature and only 61 of these were presented mimicking an incarcerated inguinal hernia. Due to the low incidence of this disease it is difficult to know the natural history and draw conclusions on the results of treatment, which so far remains the radical orchiectomy with wide local excision of the tissue. The role of radiotherapy and chemotherapy is still controversial

ABSTRACT

La anestesia local es una opción válida para realizar procedimientos quirúrgicos de pequeña o mediana consideración. Permite la deambulación y el alta precoz. Objetivo: Evaluar los resultados del uso de anestesia local y bloqueo de los nervios ilioinguinal e iliohipogástrico, en pacientes a quienes se les realizó cura operatoria de hernia inguinal, en el Servicio de Cirugía del Hospital "Dr. Francisco Antonio Rísquez", en el periodo octubre 2017 - octubre 2018. Métodos: Estudio prospectivo, transversal que incluyó 23 pacientes que aceptaron se le realizara la hernioplastia con anestesia local. Resultados: Se realizaron 46 curas operatorias de hernia inguinal, y en 23 se realizó con anestesia local (50 %). Predominaron pacientes masculinos (87 %), con una distribución bimodal en relación a la edad (26,1 % entre 30 y 39 años y entre 70 a 79 años). La hernia inguinal derecha fue la más frecuente con el 40 % (10 casos). Todos los pacientes reportaron una puntuación de 0-1 en la escala visual análoga, tanto en el transoperatorio como en el posoperatorio. El 96% de los pacientes logró una deambulación precoz (antes de 4 horas) y todos toleraron la vía oral antes de las dos horas en el posoperatorio. No se observó ninguna complicación en el transoperatorio; hubo 1 recidiva. Conclusiones: La cura operatoria de hernia inguinal con anestesia local, resultó ser efectiva y bien tolerada por los pacientes, brindando un reintegro casi inmediato a sus actividades, lo que representa una disminución en los costos intrahospitalarios(AU)


Local anesthesia is an option to perform surgical procedures of small or medium account. It allows the ambulation, and early discharge. Objective: To evaluate the results of the use of local anesthesia and nerve block ilioinguinal and iliohypogastric, in patients who underwent surgical cure of inguinal hernia, surgery of the Hospital "Dr. Francisco Antonio Rísquez" service, in the October 2017 period - October 2018. Methods: Prospective, cross-sectional study involving 23 patients who accepted it will be the hernioplasty under local anesthesia. Results: 46 surgical cure of inguinal hernia were performed, and in 23 was held with local anesthesia (50%). Dominated male patients (87%), with a bimodal distribution in relation to the age (26.1% between 30 and 39 years and between 70 to 79 years). Right inguinal hernia was the most frequent with 40% (10 cases). All patients reported a score of 0-1 in the Visual analog escale, both in the intraoperative and postoperative. 96% of patients achieved early ambulation (within 4 hours) and all tolerated the mouth before the two hours in the postoperative. There were no complications in the intraoperative; there was 1 recurrence. Conclusions: e surgical cure of inguinal hernia under local anesthesia, proved to be effective and well tolerated by the patients, providing a return almost immediately to their activities, which represents a decrease in hospital costs(AU)
