ABSTRACT

Objetivo: Caracterizar as notificações dos incidentes e eventos adversos do Centro de Material e Esterilização (CME) de um hospital de ensino. Método: Estudo retrospectivo, documental, descritivo e quantitativo em um CME classe II, em que foram analisadas as notificações contidas no banco de dados do Núcleo do Sistema de Gestão da Qualidade e Segurança do paciente, entre 2016 e 2020. Como técnica de análise dos dados coletados, utilizou-se o diagrama de causa e efeito, e a descrição das notificações foram agrupadas conforme o diagrama de 6M: método; material; mão de obra; máquina; medida; e meio ambiente. Resultados: Entre as notificações realizadas, as variáveis matéria-prima e método foram as que mais geraram incidentes durante todo o período, representando 28,54 e 26,44%, respectivamente. Além disso, o Centro Cirúrgico foi o que mais notificou e foi notificado pelo CME. Conclusão: Foi possível conhecer os incidentes que mais predominam no CME da instuição, e as variáveis materiais e método, com o centro cirúrgico, foram as variáveis e setor que mais geraram notificações no período pesquisado (AU)

ABSTRACT

Introdução: Um grande desafio para a utilização de registros e bases de dados secundárias é a qualidade do registro e o percentual de perdas em variáveis estratégicas e necessárias à plena utilização do banco. Objetivo: Propor um método de correção para a variável de estadiamento no âmbito dos Registros Hospitalares de Câncer (RHC), a fim de aprimorar sua completude e qualidade. Método: Estudo descritivo, abrangendo as Unidades da Federação, utilizando-se como fonte de informação o RHC, de janeiro de 2013 a dezembro de 2019. O câncer de pulmão foi escolhido como caso para a correção do banco, em razão da sua alta taxa de mortalidade no Brasil e no mundo. As análises foram realizadas com o software de análises estatísticas SAS Studio e a base de dados organizada em Excel. Resultados: O total de casos registrados no RHC foi de 86.026, e a variável de interesse, o estadiamento, teve um total de 32,0% de perda. Ao final de todas as etapas de correção, a perda foi de 9,8%, correspondendo a 22,2% de recuperação. Conclusão: A metodologia proposta representa um avanço na correção do banco do RHC, possibilitando a utilização dos dados de base secundária, com melhor representatividade das diferentes Regiões do país, sobre o tratamento de câncer de pulmão, com possibilidade de expansão de seu uso para outras topografias


Introduction: A major challenge to utilize the registries and secondary databases is the quality of the data and the percentage of losses in strategic and necessary variables for better effectiveness of the database. Objective: To propose a correction method for the cancer staging variable of the HospitalBased Cancer Registry (HBCR), to improve its completeness and quality. Method: HBCR-based descriptive analysis covering Brazil's Federation Units from January 2013 to December 2019. Due to its high mortality in Brazil and worldwide, lung cancer was selected as case for database correction. The analyzes were performed with the software SAS Studio for statistical analyzes and the data were organized in Excel. Results: The total number of cases registered at the HBCR was 86,026, and 32% the variable of interest, staging, were missed. At the end of the correction process, the missed data reached 9.8%, corresponding to a recovery of 22.2%. Conclusion: The proposed methodology is an advance for the correction of the HBCR database on the treatment of lung cancer, allowing a more extensive use, with better representativeness of different country regions, and potential utilization in other topographies


Introducción: Un gran desafío para el uso de registros y bases de datos secundarias es la calidad del registro en sí, el porcentaje de pérdidas en variables estratégicas y necesarias para el pleno uso de la base de datos. Objetivo: Proponer un método de corrección de la variable estadificación en el ámbito de los Registros Hospitalarios de Cáncer (RHC), con el fin de mejorar su exhaustividad y calidad. Método: Análisis descriptivo, abarcando las Unidades de la Federación. Se utilizó el RHC como fuente de información, de enero de 2013 a diciembre de 2019. El cáncer de pulmón fue elegido como caso para la corrección de la base de datos, debido a su alta tasa de mortalidad en el Brasil y en el mundo. Los análisis se realizaron con el software de análisis estadístico SAS Studio y los datos se organizaron en Excel. Resultados: El total de casos registrados en el RHC fue de 86.026, y la variable de interés, la estadificación, tuvo una pérdida total del 32,0% Al final de todas las etapas esta fue de 9,8%, es decir el 22,2% de recuperación. Conclusión: La metodología propuesta representa un avance en la corrección del RHC, permitiendo una mejor utilización de la base de datos, con una mejor representatividad de las diferentes regiones del país, sobre el tratamiento del cáncer de pulmón, con la posibilidad de expandir su uso a otras topografías

Abstract Introduction: Head and neck cancer has an impact on the global burden of diseases, representing an important cause of morbidity and mortality in Brazil, as well as worldwide. Objective: To learn and describe the clinical, epidemiological and care configuration provided to patients with cancer of the oral cavity and oropharynx recorded in Brazil, diagnosed from 2007 to 2016. Methods: This is a cross-sectional study, carried out using secondary hospital-based data, using the indirect documentation technique. Results: There were 52,799 hospital records of oral cavity cancer and 34,516 cases of oropharyngeal cancer in the assessed period. There was a predominance of male patients, aged 50-59 years, mostly Caucasians, and with a low level of schooling. Throughout the period there was a significant reduction in the positive history of alcohol and tobacco consumption, except for alcoholic beverages in oral cavity cancer. Most patients were diagnosed at an advanced stage of the disease (III or IV). Most patients with oral cavity cancer had no evidence of the disease on follow-up, while most patients with oropharyngeal cancer died. The first most frequent treatment offered to patients with oral cavity cancer was surgery, while for patients with oropharyngeal cancer it chemoradiotherapy predominated. Conclusion: Despite the fact that, in general, there was a reduction in the records of patient alcohol and tobacco consumption, the increase in the number of medical consultations, the predominantly late diagnosis and the patients' low level of schooling indicate the need for health education, primary prevention and early diagnosis of cancer of the oral cavity and oropharynx.


Resumo Introdução: O câncer de cabeça e pescoço impacta a carga global de doenças, representa importante causa de morbimortalidade no Brasil e no mundo. Objetivo: Conhecer e descrever a compleição clínica, epidemiológica e assistencial dos atendimentos a pacientes com câncer de cavidade oral e orofaringe registrados no Brasil, diagnosticados de 2007 a 2016. Método: Estudo transversal, feito a partir de dados secundários de base hospitalar, por meio de técnica de documentação indireta. Resultados: Houve 52.799 registros hospitalares de câncer de cavidade oral e 34.516 casos de câncer de orofaringe no período considerado. Predominaram pacientes do sexo masculino, da faixa etária de 50-59 anos, predominantemente branca, e de baixo nível de escolaridade. Ao longo do período houve redução expressiva do histórico positivo de consumo de bebida alcoólica e tabaco, exceto para bebida alcoólica no câncer de cavidade oral. A maioria dos pacientes foi diagnosticada em estágio avançado da doença (III ou IV). A maior parte dos pacientes de câncer de cavidade oral apresentava-se sem evidência da doença, enquanto que grande parte dos pacientes com câncer de orofaringe evoluiu a óbito. O primeiro tratamento mais frequente oferecido aos pacientes com câncer de cavidade oral foi cirurgia, enquanto para os pacientes com câncer de orofaringe foi a quimioradioterapia. Conclusão: Apesar de se observar, de maneira geral, redução dos registros do consumo de bebida e tabaco, o aumento do número de atendimentos, o diagnóstico tardio predominante e o baixo nível de escolaridade dos pacientes apontam para a necessidade da educação em saúde, de prevenção primária e do diagnóstico precoce do câncer de cavidade oral e orofaringe.

ABSTRACT

Automatic geocoding methods have become popular in recent years, facilitating the study of the association between health outcomes and the place of living. However, rather few studies have evaluated geocoding quality, with most of them being performed in the US and Europe. This article aims to compare the quality of three automatic online geocoding tools against a reference method. A subsample of 300 handwritten addresses from hospital records was geocoded using Bing, Google Earth, and Google Maps. Match rates were higher (> 80%) for Google Maps and Google Earth compared with Bing. However, the accuracy of the addresses was better for Bing with a larger proportion (> 70%) of addresses with positional errors below 20m. Generally, performance did not vary for each method for different socioeconomic status. Overall, the methods showed an acceptable, but heterogeneous performance, which may be a warning against the use of automatic methods without assessing quality in other municipalities, particularly in Chile and Latin America.

Objetivo: caracterizar o câncer infantojuvenil no Brasil, a partir da investigação nacional e regional dos registros hospitalares de câncer, no período de 2000 a 2016. Método: estudo observacional, retrospectivo e de base secundária, com amostra de 71.925 registros contendo informações de crianças e adolescentes (0 a 19 anos). Resultados: o câncer infantojuvenil foi mais frequente no sexo masculino com 39.049 (54,3%) casos, existindo 22.391 (31,1%) registros de crianças de 0 e 4 anos de idade e 19.892 (40,4%) com cor de pele parda. As leucemias, doenças mieloproliferativas e doenças mielodisplásicas acometeram 20.744 (28,8%) indivíduos. Os tumores sólidos foram diagnosticados em 42.087 (58,5%) crianças e adolescentes, sendo 23.941 (48,7%) submetidas a quimioterapia no início do tratamento e 56.518 (78,6%) que iniciaram o tratamento até 60 dias após comprovação diagnóstica. Conclusão: foi possível conhecer o perfil demográfico, clínico e assistencial das crianças e adolescentes com câncer no Brasil, e por regiões

Solid tumors were diagnosed in 42,087 (58.5%) children and adolescents,23,941 (48.7%) of which underwent chemotherapy at the beginning of treatment and 56,518 (78.6%) who started treatment within 60 days of the diagnosis. Las leucemias, enfermedades mieloproliferativas y enfermedades mielodisplásicas afectaron a 20.744 (28,8%) individuos. Introdução: A organização da rede de serviços de saúde é fundamental para realização de assistência de crianças, adolescentes e adultos jovens com câncer. As Regiões de Saúde precisam estar organizadas, contendo todos os serviços necessários para garantir adequado cuidado. Objetivo: Compreender a organização da rede de atenção à saúde da criança, adolescente e adulto jovem com câncer, na Região Oeste de Santa Catarina. Método: Pesquisa documental, exploratória, com análise de documentos e registros hospitalares de câncer, referente aos anos: 2008, 2017 e 2018. Utilizaram-se estatísticas descritivas, identificação dos fluxos e distribuição espacial. Resultados: Chapecó foi indicado como referência da rede assistencial oncológica para a Macrorregião de saúde do Grande Oeste e Meio-Oeste. Os atendimentos foram intensificados no período, porém não foi identificada a existência de uma unidade de alta complexidade de oncologia pediátrica após a análise dos documentos. Conclusão: Não há evidência de que a rede assistencial esteja estruturada e organizada para a faixa etária com base nos documentos analisados. O município de Chapecó é referência para a Região Oeste, tendo sido observado aumento do acesso à assistência oncológica e interiorização da assistência


Introduction: The organization of the health care services network is essential for children, adolescents and young adults with cancer. Health Regions need to be organized to ensure adequate care. Objective: To understand the organization of the health care network for children, adolescents and young adults with cancer in the West Region of Santa Catarina. Method: Documentary, exploratory study with analysis of documents and hospital records of cancer were carried out in the years 2008, 2017 and 2018. Descriptive statistics were utilized, in addition to identification of the flow and spatial distribution. Results: Chapecó is the reference of the oncology care network for the health Macro-region of the Greater West and Midwest. Consultations were intensified in the period, but it was not identified the existence of a highly complexity unit for pediatric oncology after reviewing the documents. Conclusion: There is no evidence that the healthcare network is structured and organized for the age range in the documents analyzed. The municipality of Chapecó's is the reference for the West Region with increase of access to oncological care and more services offered in the rural área


Introducción: La organización de la red de servicios de salud es fundamental para brindar asistencia a los niños, adolescentes y adultos jóvenes con cáncer. Es necesario organizar las regiones sanitarias, para garantizar una atención adecuada. Objetivo: Comprender la organización de la red de atención a la salud de niños, adolescentes y jóvenes con cáncer en la Región Occidental de Santa Catarina. Método: Investigación documental, exploratoria, con análisis de documentos y registros hospitalarios de cáncer, referidos a los años 2008, 2017 y 2018. Se utilizó estadística descriptiva, identificación de flujo y distribución espacial. Resultados: Chapecó es referencia de la red de atención oncológica para el Macrorregión de salud del Gran Oeste y Medio Oeste. La asistencia se intensificó en el período, pero no se identificó en los documentos la presencia de una unidad de alta complejidad para oncología pediátrica. Conclusión: La estructuración y organización de la red asistencial para el grupo de edad no fue presentada de manera específica en los documentos analizados. El municipio de Chapecó es el referente para la Región Oeste para accesibilidad a asistencia oncológica con más servicios en el área rural

Introdução: O câncer de cavidade oral e faringe é um problema de saúde pública, apresentando elevadas taxas de incidência e mortalidade. Objetivo: Caracterizar o perfil clínico-epidemiológico e os atrasos no diagnóstico e tratamento de casos atendidos nas unidades de referência da Região Norte do Brasil. Método: Estudo descritivo dos casos de câncer de cavidade oral e faringe registrados nas bases de dados dos Registros Hospitalares de Câncer (RHC) de 2012 a 2015. Foram analisados os intervalos entre a primeira consulta e o diagnóstico, e o diagnóstico e o tratamento. As variáveis contínuas foram avaliadas por meio de medidas de tendência central e de dispersão. As diferenças entre proporções nas variáveis categóricas foram avaliadas pelo teste qui-quadrado, considerando-se o nível de significância de 5%. Resultados: Entre os homens, os casos de orofaringe foram mais incidentes (19,2%) e nas mulheres os mais incidentes (16,3%) de outras partes e partes não especificadas da língua. A maior parte dos casos era de homens, cor parda, >60 anos, sem cônjuge, com escolaridade até o nível fundamental. O carcinoma de células escamosas foi o mais comum. Com relação ao tempo, 69,6% dos casos que chegaram ao hospital sem diagnóstico e sem tratamento foram diagnosticados até 30 dias após a primeira consulta, e 67,8% dos casos receberam o primeiro tratamento em até 60 dias. Conclusão: A maioria dos casos analisados apresentou tempo de diagnóstico dentro do prazo recomendado, porém foi observado que os casos com diagnóstico e sem tratamento demoraram mais de 60 dias


Introduction: Oral cavity and pharyngeal cancer is a public health problem with high incidence and mortality rates. Objective: To characterize the clinical-epidemiological profile and delays in diagnosis and treatment of cases of oral cavity and pharyngeal cancer treated in reference units in Brazil's North Region. Method: Descriptive study of oral cavity and pharyngeal cancer registered in the Hospital-based Cancer Registry (HBCR) in the North Region from 2012 to 2015. The intervals between the 1st consultation and diagnosis and diagnosis and treatment were analyzed. Continuous variables were evaluated using measures of central tendency and dispersion. Differences between proportions in categorical variables were assessed using the chi square test, considering a significance level of 5%. Results: Among men, cases of oropharynx were more incidents (19.2%) and in women, the most incidents (16.3%) were from other parts and unspecified parts of the tongue. Most of the cases were males, brown, >60 years old, without spouse and elementary education. Squamous cell carcinoma was the most common. 69.6% of cases that arrived at the hospital without diagnosis and without treatment were diagnosed within 30 days after the 1st consultation and 67.8% of cases received the 1st treatment within 60 days. Conclusion: Most of the cases analyzed were diagnosed within the recommended timeframe. However, cases with diagnosis and without treatment took more than 60 days


Introducción: El cáncer de cavidad oral y faringe es un problema de salud pública, con altas tasas de incidencia y mortalidad. Objetivo: Caracterizar el perfil clínico-epidemiológico y las demoras en el diagnóstico y tratamiento de los casos de cáncer de cavidad oral y faringe atendidos en unidades de referencia del Norte de Brasil. Método: Se realizó un estudio descriptivo de los casos analíticos de cáncer de cavidad oral y faringe registrados en las bases de datos del Registros Hospitalarios de Cáncer (RHC) en la Región Norte de 2012 a 2015. Se analizaron los intervalos entre la primera consulta y el diagnóstico y diagnóstico y tratamiento. Las variables continuas se evaluaron mediante medidas de tendencia central y dispersión. Las diferencias entre proporciones en variables categóricas se evaluaron mediante la prueba chi cuadrado, considerando un nivel de significancia del 5%. Resultados: Entre los hombres, los casos de orofaringe fueron más incidentes (19,2%) y entre las mujeres los de otras partes y partes no especificadas del idioma fueron los más incidentes (16,3%). La mayoría de los casos fueron del sexo masculino, moreno, >60 años, sin pareja, con educación hasta el nivel elemental. El carcinoma de células escamosas fue el más común. En cuanto al tiempo, el 69,6% de los casos que llegaron al hospital sin diagnóstico y sin tratamiento fueron dentro de los 30 días posteriores a la primera consulta y el 67,8% de los casos recibieron el 1er tratamiento dentro de los 60 días. Conclusión: La mayoría de los casos analizados presentaron tiempo de diagnóstico dentro del período recomendado. Sin embargo, se observó que los casos con diagnóstico y sin tratamiento demoran más de 60 días

Resumen: Los métodos automáticos de geocodificación se han convertido en algo popular durante los últimos años para facilitar el estudio de la asociación entre resultados de salud y lugar para vivir. No obstante, más bien pocos estudios han evaluado la calidad de la geocodificación, siendo realizados la mayoría de ellos en EE.UU. y Europa. El objetivo de este artículo es comparar la calidad de tres herramientas automáticas de geocodificación en línea frente a un método de referencia. La submuestra de 300 direcciones escritas a mano, procedentes del registro hospitalario, se geocodificaron usando Bing, Google Earth y Google Maps. Los porcentajes de coincidencia fueron mayores (> 80%) en el caso de Google Maps y Google Earth comparados con Bing. Sin embargo, la precisión de las direcciones fue mejor con Bing, en una proporción más grande (> 70%) de direcciones que tenían errores de posición por debajo de 20m. En general, el rendimiento no varió en cada método para diferentes niveles estatus socioeconómico. En general, los métodos mostraron un rendimiento aceptable, pero heterogéneo. Esto previene contra el uso de métodos automáticos sin evaluar la calidad en otras ciudades, particularmente en Chile y Latinoamérica.


Resumo: Os métodos de geocodificação automática se tornaram populares nos últimos anos para facilitar o estudo da associação entre desfechos de saúde e lugar de residência. Entretanto, poucos estudos avaliaram a qualidade da geocodificação, e a maioria dos estudos existentes foi realizada nos Estados Unidos e Europa. O estudo teve como objetivo comparar a qualidade de três ferramentas de geocodificação eletrônica automática em relação a um método de referência. Foi geocodificada uma subamostra de 300 endereços anotados à mão em prontuários hospitalares, usando Bing, Google Earth e Google Maps. As taxas de correspondência dos registros foram mais altas (> 80%) com Google Maps e Google Earth, comparado com Bing. Entretanto, a acurácia dos endereços foi melhor com Bing, com uma proporção maior (> 70%) de endereços com erros de localização menores que 20 metros. Em geral, o desempeno não variou para cada método de acordo com condição socioeconômica. Os métodos apresentaram desempenho geral aceitável, porém heterogêneo. Os resultados servem de alerta contra o uso de métodos automáticos sem avaliar a qualidade em outras cidades, particularmente no Chile e no resto da América Latina.

ABSTRACT

BACKGROUND: There is an increasing use of cesarean delivery (CD) based on preference rather than on medical indication. However, the extent to which nonmedically indicated CD benefits or harms child survival remains unclear. Our hypothesis was that in groups with a low indication for CD, this procedure would be associated with higher child mortality and in groups with a clear medical indication CD would be associated with improved child survival chances. METHODS AND FINDINGS: We conducted a population-based cohort study in Brazil by linking routine data on live births between January 1, 2012 and December 31, 2018 and assessing mortality up to 5 years of age. Women with a live birth who contributed records during this period were classified into one of 10 Robson groups based on their pregnancy and delivery characteristics. We used propensity scores to match CD with vaginal deliveries (1:1) and prelabor CD with unscheduled CD (1:1) and estimated associations with child mortality using Cox regressions. A total of 17,838,115 live births were analyzed. After propensity score matching (PSM), we found that live births to women in groups with low expected frequencies of CD (Robson groups 1 to 4) had a higher death rate up to age 5 years if they were born via CD compared with vaginal deliveries (HR = 1.25, 95% CI: 1.22 to 1.28; p < 0.001). The relative rate was greatest in the neonatal period (HR = 1.39, 95% CI: 1.34 to 1.45; p < 0.001). There was no difference in mortality rate when comparing offspring born by a prelabor CD to those born by unscheduled CD. For the live births to women with a CD in a prior pregnancy (Robson group 5), the relative rates for child mortality were similar for those born by CD compared with vaginal deliveries (HR = 1.05, 95% CI: 1.00 to 1.10; p = 0.024). In contrast, for live births to women in groups with high expected rates of CD (Robson groups 6 to 10), the child mortality rate was lower for CD than for vaginal deliveries (HR = 0.90, 95% CI: 0.89 to 0.91; p < 0.001), particularly in the neonatal period (HR = 0.84, 95% CI: 0.83 to 0.85; p < 0.001). Our results should be interpreted with caution in clinical practice, since relevant clinical data on CD indication were not available. CONCLUSIONS: In this study, we observed that in Robson groups with low expected frequencies of CD, this procedure was associated with a 25% increase in child mortality. However, in groups with high expected frequencies of CD, the findings suggest that clinically indicated CD is associated with a reduction in child mortality.

Este trabalho visa identificar os fatores associados a óbitos de adultos hospitalizadas com aids. Para isso, foi realizado estudo do tipo transversal, desenvolvido mediante dados secundários obtidos de prontuários hospitalar de 263 adultos com diagnóstico de aids em Fortaleza, Ceará. As variáveis sociodemográficas e clínicas foram consideradas preditoras e o óbito foi a variável desfecho. Foram expressas as frequências absoluta e relativa de cada variável e aplicados os testes exato de Fisher ou qui-quadrado de Pearson. Os fatores relacionados significativamente ao risco de óbito entre adultos foram: ensino fundamental incompleto (p = 0,001, OR = 1,85; IC = 1,02-3,37), estar afastado do trabalho (p = 0,039, OR = 1,59; IC = 0,81-3,23), possuir doença renal (92%; OR = 3,05; IC = 1,49-6,59), sífilis (85%; OR = 3,05; IC = 1,49-6,58), herpes simples (95%; OR = 3,98; IC = 1,16-19,01), histoplasmose (82%; OR = 0,22; I.C = 0,09-0,46), última contagem de LTCD4 < 200 (p = 0,002; OR = 3,35; IC = 1,37-9,20), queixas de dispneia e febre na admissão hospitalar (p < 0,001; OR = 0,74; IC = 0,20-2,67), tempo de internamento até sete dias (p < 0,001; OR = 0,12; IC = 0,03-0,33), primeira admissão no hospital (p = 0,002; OR = 2,15; IC = 1,31-3,55), acima de três internações no hospital (p = 0,003; OR = 3,09; IC = 1,43-6,88), ter sido internado em UTI (p < 0,001; OR = 0,12; IC = 0,04-0,28), ter realizado hemodiálise (p < 0,001; OR = 0,09; IC = 0,02-0,24), ventilação mecânica (p < 0,001; OR = 0,06; IC = 0,02-0,14) e recebido hemocomponentes e/ou hemoderivados (p < 0,001; OR = 0,18; IC = 0,10-0,32). Com esses dados, concluímos que os óbitos de adultos com aids aconteceram majoritariamente entre pessoas com condições sociodemográficas vulneráveis e com incidência considerável de doenças oportunistas, com demandas de cuidados intensivos.


This cross-sectional study identifies the factors associated with deaths of hospitalized adults with AIDS. In conclusion, deaths of adults with AIDS occurred mostly among those with vulnerable sociodemographic conditions and with considerable incidence of opportunistic diseases, demanding intensive care.

ABSTRACT

Prólogo. La obra recopiló los hitos históricos, las acciones en gestión y la visión futura de las Unidades Médicas, Administrativas y de Enfermería, en homenaje a los 50 años de función del Hospital de Especialidades Carlos Andrade Marín como ícono de la Seguridad Social del Ecuador, entre el periodo 1970 a diciembre 2020. Cada autor registró el título entrelazado un pensamiento, previo a la cronología de los hechos suscitados en los servicios brindados en beneficio de los pacientes que se atendieron en esta casa de salud. La obra se clasificó en capítulos por Unidades Médicas: Críticas, Clínicas, Quirúrgicas, Administrativas y de Enfermería, se incluyeron tablas, figuras, producción médica, patologías, cambios de técnicas o procedimientos y sustentó su accionar con citas textuales y referen-cias bibliográficas. Además, se narraron mejoras continuas transmitidas de los ilustres y legados maestros, especialistas médicos por varias generaciones, en conjunto con el personal de salud con vocación de servicio, en una reflexión profunda con mensajes vivos de su accionar. En las modalidades de publicación se realizaron aportes de visión nacional e internacional en los ámbitos asistenciales, administrativos, docente, de investigación y producción científica de alto nivel del contexto y naturaleza del emblemático hospital. Finalmente, se agradece a los funcionarios y ex trabajadores que colaboraron durante el proceso de construcción de la obra, así como a ex colaboradores que motivaron aportes de experiencias y la apreciación personal.


Prologue. The work compiled the historical milestones, management actions and future vision of the Medical, Administrative and Nursing Units, in tribute to the 50 years of function of the Carlos Andrade Marin Specialty Hospital as an icon of the Social Security of Ecuador, between 1970 and December 2020. Each author recorded the title intertwined with a thought, prior to the chronology of the events that took place in the services provided for the benefit of patients who were treated in this health center. The work was classified in chapters by Medical Units: Critical, Clinical, Surgical, Administrative and Nursing, and included tables, figures, medical production, pathologies, changes in techniques or procedures, and supported his actions with textual quotations and bibliographical references. In addition, continuous improvements were narrated and transmitted from the illustrious and legacy masters, medical specialists for several generations, together with the health personnel with vocation of service, in a deep reflection with living messages of their actions. In the publication modalities, contributions of national and international vision were made in the care, administrative, teaching, research and scientific production of high level of the context and nature of the emblematic hospital. Finally, we would like to thank the employees and former workers who collaborated during the construction process, as well as former collaborators who contributed with their experiences and personal appreciation.

This study investigated the underreporting of deaths due to Acquired Immunodeficiency Syndrome (AIDS) in Brazil, using the product of the linkage between the Mortality Information System (SIM) and the Hospital Information System (SIH) for the years 2008 to 2012. The hypothesis was that there were deaths recorded in the SIM that should have AIDS as the underlying cause of death, but that had been poorly classified. Many of the decedents had previous hospitalizations due to the illness. Underreported AIDS deaths were defined taking into account the coding rules of the International Classification of Diseases 10th Revision, in order to discriminate AIDS deaths from those due to other causes in people living with HIV / AIDS. In this period, 60,362 deceased had AIDS as the underlying cause, and we found another 2,671 (4.2%) as underreported causes. Underreported deaths increased the average AIDS mortality rate from 6.3/100,000 to 6.6/100,000. In conclusion, this study demonstrated that, through linkage in between SIH and SIM, it is possible to find underreporting of AIDS deaths in Brazil. These results serve as a warning for the need for actions that would promote a better certification of the causes of death among AIDS patients.

O estudo investigou a configuração da rede social significativa de uma mulher com anorexia nervosa, buscando articular a percepção do apoio social recebido e o curso do transtorno. Utilizou-se como estratégia metodológica o estudo de caso e o enfoque das redes sociais para análise dos dados. Participou do estudo uma jovem de 24 anos com diagnóstico de anorexia nervosa do tipo compulsivo-purgativo. Os instrumentos utilizados foram: roteiro de entrevista semiestruturada, Genograma, Mapa de Rede e consulta ao prontuário hospitalar. O conteúdo da entrevista foi transcrito na íntegra e literalmente, e o material coligido constituiu o corpus da pesquisa. O Genograma e o Mapa de Rede foram analisados de acordo com as recomendações da literatura e o referencial teórico utilizado. Os resultados apontaram dificuldades no estabelecimento de vínculos e sofrimento decorrente da percepção de falta de suporte da rede familiar. Certos aspectos da dinâmica de personalidade da participante resultavam em afastamento das pessoas e consequente isolamento social, o que restringe o potencial de apoio da rede e suscita vivências de solidão e desamparo. Enfatiza-se a necessidade de aproveitar o potencial das redes de apoio na assistência em saúde para fortalecer os laços sociais de jovens com diagnóstico de anorexia....(AU)


This study investigated the configuration of the significant social network of a 24-yearold girl diagnosed with Compulsive-Purgative AN (AN) to articulate the participant´s perception of social support and the symptoms of her eating disorder. The methodological strategy involved a case study and the social networks approach for data analysis. Semi-structured interview, Genogram, Network Mapping and consultations to hospital records were used as instruments. The content of the interviews was transcribed in full and the collected material constituted the research corpus. The genogram and network mapping were analyzed according to recommendations from the scientific literature and to the theoretical framework used. The results showed issues in establishing emotional bonds and suffering due to perceived lack of social support from the family network. In addition, aspects of the participant's personality dynamics resulted in emotional withdrawal and consequent social isolation, restricting the potential support of the network and leading to experiences of loneliness and helplessness. We emphasize the need for integration of social networks in healthcare in order to strengthen the social bonds of young women diagnosed with anorexia....(AU)


El estudio investigó la configuración de la red social significativa de una mujer con anorexia nerviosa buscando articular la percepción del apoyo social recibido con el curso del trastorno. La estrategia metodológica utilizada fue el estudio de caso y el enfoque en las redes sociales aplicado en el análisis de datos. Una joven de 24 años con diagnóstico de anorexia nerviosa del tipo compulsivo-purgativo participó en el estudio. Los instrumentos utilizados fueron: entrevista semiestructurada, genograma, mapa de red y consultas a los registros hospitalarios. El contenido de la entrevista fue transcrito completo y literalmente, y el material recopilado compuso el corpus de la investigación. El genograma y el mapa de red se analizaron de acuerdo con las recomendaciones de la literatura científica y el marco teórico utilizado. Se percibieron dificultades en el establecimiento de vínculos y sufrimientos que resultan de la percepción de falta de apoyo de la red familiar. Además, determinados aspectos de la dinámica de la personalidad de la participante llevaron a la retirada de las personas y el consiguiente aislamiento social, lo que restringe el potencial de apoyo de la red y conduce a experiencias de soledad e impotencia. OBJECTIVES: To compare the impact of Baby-Friendly designation vs rates of in-hospital breastfeeding initiation on breastfeeding outcomes at 3, 6, and 12 months postdischarge. STUDY DESIGN: Breastfeeding outcome data from the 2018 Centers for Disease Control (CDC) Breastfeeding Report Card were used as a basis for determining outcomes from the corresponding 2015 birth cohort. Linear regression models were used to determine the strength of association of breastfeeding initiation and Baby-Friendly hospital penetrance and attainment of postdischarge breastfeeding rates. All hospital births from all 50 states, 3 territories, and the District of Columbia were included in the study. RESULTS: Statewide breastfeeding initiation rates were positively associated with targeted breastfeeding outcomes. Similar associations were not found for Baby-Friendly hospital designation penetrance. CONCLUSIONS: To attain the Healthy People 2020 breastfeeding objectives, future public policy initiatives should consider the interaction of population demographics, individual hospital programs, and public health strategies used to support breastfeeding in states reporting high breastfeeding initiation rates.

ABSTRACT

OBJECTIVE: To describe hospital admissions of adolescents living in Sergipe, Northeast Brazil, from 2002 to 2012. METHODS: Descriptive study, based on data collected from the Hospital Information System of the Unified Health System. Hospital admissions were divided into four groups of causes: by pregnancy, childbirth and puerperium; by external causes; by primary care conditions; and other causes. Numbers, percentages and coefficients were used in the analysis and compared by year, sex, age (from 10 to 14 and from 15 to 19 years), and the average annual cost of hospitalizations for each group of causes. RESULTS: In the period studied, there were 149,850 hospital admissions of adolescents, 58.4% for pregnancy, childbirth and puerperium, 9.3% for primary care conditions, 8.3% for external causes and 24.0% for other causes. All coefficients decreased from 2002 to 2012 by 39.7%. Primary care conditions had the most significant reduction (143.1%), followed by external causes (60.1%). As for age groups, the coefficients for external causes in the age group of 15 to -19 years, and for pregnancy, childbirth and puerperium, in the age range of 10 to 14 years, are noteworthy because they remained stable in the period. There was an increase in the average cost of all admissions (234.7%), especially for external causes. CONCLUSIONS: Health actions to reduce hospital admission due to conditions sensitive to primary care should be given more attention, as well as those related to external causes and pregnancy, among adolescents living in Sergipe, Northeastern Brazil.

Compilación del trabajo de investigación desarrollado sobre un fondo documental de historias clínicas del período 1895-1987 del Archivo Intermedio del Archivo General de la Nación, provenientes del Hospital Neuropsiquiátrico de Mujeres José Esteves, de la Provincia de Buenos Aires (Argentina). La investigación tuvo el aval de la Universidad Nacional de Lanús (UNL) y el CONICET, y se centró en la trayectoria del hospital en ese período, y la atención a la salud/enfermedad mental, partiendo de una presentación socio-epidemiológica de las personas internadas, y situándolas en su contexto socio-histórico y cultural.

ABSTRACT

Objetivo: determinar la prevalencia de la fisura labio alveolo palatina (FLAP) en las prin-cipales ciudades del Ecuador: Quito, Guayaquil y Cuenca, en el período comprendido desde el año 2010 al 2018. Metodos: se realizó un estudio descriptivo, observacional de corte transversal y retrospectivo mediante la revisión de la base de datos de estudios realizados en medios hospitalarios de las ciudades de Quito, Guayaquil y Cuenca en pacientes con diagnóstico de FLAP nacidos en el periodo comprendido desde el año 2010 al 2018. La información mediante un formulario para el nacido vivo con FLAP sindrómico, no sindrómico y de la madre. Resultados: se encontró que la frecuencia de FLAP es 14.97 por cada 10,000 nacidos vivos, dato concordante con el de los países de la región. Según registros hospitalarios del sistema nacional de salud, 1132 pacientes con diagnostico FLAP fueron atendidos en el periodo 2010 a 2018. Con respecto al de edad de los niños que requirieron atención por FLAP, este fue de 3 años (DS: ±3.2). Su proporción por sexo fue 38.19% en mujeres vs. 61.68% en hombres. Conclusión: hasta ahora no se han encontrado estudios en el Ecuador que cuenten con criterios unifi-cados sobre diagnóstico y clasificación de las FLAP, para así aportar a la formulación políticas de salud pública y al mejoramiento de la calidad de vida del paciente fisurado.


Objective: To determine the prevalence of the cleft lip and palate (CLAP) in the main cities of Ecuador: Quito, Guayaquil and Cuenca, in the period from 2010 to 2018. Methods: A descriptive, observational, cross-sectional and retrospective study was carried out by reviewing the database of studies conducted in hospital settings in the cities of Quito, Guayaquil and Cuenca of patients diagnosed with FLAP born in the period from 2010 to 2018. The information was collected using a form for the live birth with syndromic, non-syndromic and mother CLAP. Results: The frequency of CLAP was found to be 14.97 per 10,000 live births, consistent with that of the countries in the region. 1132 patients diagnosed with CLAP were treated in the national health system according to hospital records, in the period 2010 to 2018; Regarding the average age of children who required CLAP care, it was 3 years (SD: ± 3.2). Their sex ratio was 38.19% women vs. 61.68% men Conclusion: No studies have been found in Ecuador that have unified criteria on diagnosis and classification of CLAPs that help formulate public health policies that improve the quality of life of the cracked patient.

Objetivo: Descrever o perfil clínico-epidemiológico de casos hospitalares de câncer primário de tireoide no Brasil. Métodos: Estudo descritivo dos casos informados pelos registros hospitalares de câncer que tiveram primeira consulta para tratamento no período 2000-2016 e cujo acompanhamento foi realizado pela instituição do registro informante. Resultados: Dos 52.912 casos, 83,4% eram femininos e 96,9% eram carcinomas diferenciados. Apresentaram menor tempo mediano para diagnóstico os casos anaplásicos (11 dias) e os residentes da região Sul do país (5 dias). O tratamento foi iniciado em até 60 dias em 88,8% dos casos que chegaram à instituição do registro sem diagnóstico e em 34,9% dos que chegaram com diagnóstico. Conclusão: Os achados são consistentes com a epidemiologia do câncer de tireoide, com predominância do sexo feminino e do carcinoma diferenciado. A análise do tempo para início do tratamento sugere dificuldades de acesso para aqueles que chegaram às instituições dos registros com diagnóstico.

Resultados: De los 52.912 casos, 83,4% eran mujeres y 96,9% era de carcinomas diferenciados. El tiempo promedio hasta el diagnóstico fue menor en los anaplásicos (11 días) y en la región Sur (5 días). El tratamiento se inició dentro de los 60 días en 88.8% de los casos que llegaron a la institución de registro sin diagnóstico y en 34.9% de los que llegaron con diagnóstico. Conclusión: Los resultados son consistentes con la epidemiología del cáncer de tiroides, con predominio del sexo femenino y carcinomas diferenciados. El análisis del tiempo de tratamiento sugiere dificultades de acceso para casos que llegaron con diagnóstico.

Results: Of the 52,912 cases, 83.4% were female and 96.9% were differentiated carcinoma cases. The median time to diagnosis was shorter for anaplastic cases (11 days) and for those living in Brazil's Southern region (5 days). Treatment was initiated within 60 days in 88.8% of cases that arrived at the hospitals without diagnosis and in 34.9% of those who arrived with diagnosis. Conclusion: The findings are consistent with thyroid cancer epidemiology, with a predominance of female cases and differentiated carcinomas. Analysis of time-to-treatment suggests access difficulties for those who already had diagnosis when they arrived at the hospitals.
