ABSTRACT

OBJECTIVE: To validate the content of an evaluation matrix for the nursing process in the hospital context. METHOD: A methodological study conducted in four stages, carried out from March to December 2022. Including: bibliographic research and selection of structuring foundations; construction of the logical model and analysis and judgment matrix; content validation by a committee of judges (n=11) using the Delphi technique; analysis of content validation data by calculating the Concordance Rate and Content Validity Index. RESULTS: At the end of the two rounds, the matrix had a concordance rate of 90% regarding format and 100% regarding other structural aspects. It also had a 100% concordance rate regarding the scope and representativeness of the matrix domains. Regarding the average Content Validity Index, the values were 0.97 and 0.98, for clarity and representativeness, respectively. CONCLUSION: The evaluation matrix - consisting of four dimensions and fourteen evaluation items referring to the necessary stages for the implementation and operationalization of the nursing process - demonstrated excellent content validity, which can contribute to supporting institutional initiatives for its consolidation in hospital nursing services.

MÉTODO: estudo descritivo, quantitativo, de abordagem metodológica, realizado em etapas, onde, após elaboração do instrumento com 69 critérios de processo, esse foi avaliado, conforme escala tipo Likert, por 49 enfermeiros de hospitais acreditados em 2009, e validado por 16 juízes, por meio do ciclo de Delphi, em 2010. RESULTADO: o instrumento inicial, analisado pelos enfermeiros com 69 critérios de processo, foi julgado pelo grau de importância, e passou para 39 critérios. No 1º ciclo Delphi, os 39 critérios atingiram o consenso entre os 19 juízes, com média de confiabilidade pelo alfa Cronbach. No 2º ciclo, foram validados 40 critérios convergentes, por 16 juízes, com alta confiabilidade. Os critérios abordam a gestão, custo, ensino, educação, indicadores, protocolos, recursos humanos, comunicação, entre outros. CONCLUSÃO: os 40 critérios de processo validados formam um instrumento para avaliação do serviço de enfermagem hospitalar que, ao ser mensurado, pode melhor direcionar as intervenções pelo enfermeiro para o alcance e fortalecimento dos resultados. .

MÉTODO: estudio metodológico descriptivo, cuantitativo realizado en etapas. Un instrumento construido con 69 criterios del proceso fue evaluado por 49 enfermeras de los hospitales acreditados en 2009, utilizando una escala Likert, y validado por 16 jueces a través de rondas Delphi en 2010. RESULTADO: el instrumento original evaluado por enfermeras tenía 69 criterios del proceso que fueron juzgados por el grado de importancia, y se modificaron 39 criterios. En la primera ronda Delphi, los 19 jueces llegaron a un consenso acerca de los 39 criterios, con una confiabilidad media obtenida a través de alfa de Cronbach. En la segunda ronda, 40 criterios convergentes fueron validados por 16 jueces, con una alta confiabilidad. Los criterios abordados fueron gestión, costos, enseñanza, educación, indicadores, protocolos, recursos humanos, comunicación, entre otros. CONCLUSIÓN: los 40 criterios del proceso formaron un instrumento validado para evaluar el servicio de enfermería hospitalario, que, cuando se mide, puede dirigir mejor las intervenciones de enfermería para alcanzar y fortalecer los resultados. .

ABSTRACT

INTRODUÇÃO: O objetivo deste estudo é descrever a evolução da cicatrização de úlcera crônica de perna, utilizando o instrumento Pressure Ulcer Scale for Healing (PUSH). MÉTODO: Os dados foram coletados no período de julho de 2010 a maio de 2011. A inclusão dos pacientes no estudo obedeceu à ordem de chegada. A lesão foi avaliada semanalmente, sendo aplicada a escala PUSH. RESULTADOS: Foram incluídos no estudo 15 (30%) pacientes diabéticos com pé ulcerado e 35 (70%) pacientes com úlcera venosa. No início da coleta dos dados, a média do comprimento e da largura foi de 9,26, caracterizando que a lesão mensurava de 12,1 cm² a 24 cm². Com 9 meses de tratamento, a úlcera apresentou média de comprimento e de largura de 2,04, caracterizando que a lesão mensurava de 0,3 cm² a 0,6 cm². Com relação à quantidade do exsudato, no início da coleta de dados a média foi de 1,71, caracterizando que a lesão apresentava quantidade moderada e, 9 meses após o início do tratamento, houve redução do exsudato, com média de 0,14, significando ausência de exsudato. Aos 9 meses de tratamento, 19 (38%) pacientes apresentavam úlcera fechada; 17 (34%), úlceras com tecido de granulação; e 14 (28%), tecido epitelizado. CONCLUSÕES: O instrumento PUSH possibilitou acompanhar o processo de cicatrização da lesão por meio da avaliação de comprimento versus largura, quantidade do exsudato e tipo de tecido existente na ferida, favorecendo, assim, a escolha da cobertura ideal para cada fase da cicatrização.


INTRODUCTION: In the present study, we aimed to describe the evolution of the healing of chronic leg ulcers using the Pressure Ulcer Scale for Healing (PUSH) tool. METHODS: The data were collected from July 2010 to May 2011. The inclusion of patients in the study followed the order of arrival. The lesion was evaluated weekly according to the PUSH tool. RESULTS: The study included 15 (30%) patients with diabetes and foot ulcers and 35 (70%) patients with venous ulcers. At the beginning of the data collection process, the average ulcer length and width were 9.26 cm² (range, 12.1-24.0 cm²). At 9 months of treatment, the average ulcer length and width was 2.04 cm² (range, 0.3-0.6 cm²). At the beginning of the study, the average amount of exudate was 1.71 (moderate amount), whereas at 9 months after the beginning of treatment, the average amount of exudate was 0.14 (absence of exudate). At 9 months of treatment, 19 (38%) patients had closed ulcers, 17 (34%) had ulcers with granulation tissue, and 14 (28%) had ulcers with epithelialized tissue. CONCLUSIONS: Use of the PUSH tool enabled monitoring of the ulcer healing process through the evaluation of length vs. width, exudate amount, and type of tissue present in the wound, thus favoring the selection of the correct dressing for each stage of wound healing.

The study aimed to identify performance indicators adopted by the Nursing Service of a public hospital and to analyze the opinions of the nurses regarding the use of these indicators to evaluate the quality of the nursing care. This is descriptive exploratory study with a quantitative approach, which used data from the management reports of the institution and applied a questionnaire in a sample of 25 nurses. It was found that the institution works with three databases of indicators, two being general and one specific for Nursing, which analyze 11 indicators. The indicators of pressure ulcer incidence and incidence of falls were the only ones considered highly relevant to qualify the nursing care for 100% of the nurses. It was concluded that the institution uses indicators for monitoring outcomes and tends to valorize the use of process indicators by the nurses to evaluate the Nursing performance, with it being necessary to expand the analysis to include multi-disciplinary indicators.

O estudo objetivou identificar indicadores de desempenho, adotados pelo Serviço de Enfermagem de hospital público, e analisar a opinião dos enfermeiros em relação à utilização desses indicadores para avaliar a qualidade da assistência de enfermagem. Trata-se de estudo descritivo exploratório, com abordagem quantitativa, que utilizou dados dos relatórios gerenciais da instituição e aplicou questionário numa amostra de 25 enfermeiros. Verificou-se que a instituição trabalha com três bancos de dados de indicadores, sendo dois gerais e um específico de enfermagem que analisa 11 indicadores. Os indicadores de incidência de úlcera por pressão e incidência de quedas foram os únicos considerados muito pertinentes para qualificar a assistência de enfermagem, por 100 por cento dos enfermeiros. Concluiu-se que a instituição utiliza indicadores para acompanhamento de resultados e há valorização da utilização de indicadores de processos pelos enfermeiros para avaliar desempenho da enfermagem, sendo necessário ampliar a análise para indicadores multidisciplinares.

Se trata de estudio descriptivo exploratorio con abordaje cuantitativo, que utilizó datos de los informes de gestión de la institución y aplicó un cuestionario en una muestra de 25 enfermeros. Se verificó que la institución trabaja con tres bancos de datos de indicadores, siendo dos generales y uno específico de enfermería que analiza 11 indicadores. Los indicadores de incidencia de úlcera por presión y de caídas fueron los únicos considerados muy pertinentes para calificar la asistencia de enfermería por 100 por ciento de los enfermeros. Se concluye que la institución utiliza indicadores para acompañamiento de resultados y existe una valorización de la utilización de indicadores de procesos por los enfermeros para evaluar desempeño de la enfermería, siendo necesario ampliar el análisis para indicadores multidisciplinares.

ABSTRACT

Estudo com abordagem qualitativa, desenvolvido em universidade pública do interior do Estado de São Paulo com o objetivo de identificar o conceito de qualidade na assistência de enfermagem hospitalar na perspectiva de alunos do último ano do curso de Bacharelado em Enfermagem. Foram realizadas entrevistas semi-estruturadas com 24 alunos, sendo as falas analisadas segundo o referencial de Bardin. Os núcleos de sentido das falas foram divididos em seis categorias: holismo, humanização, equipe capacitada técnica e cientificamente, promoção do autocuidado, gerenciamento adequado das unidades e respeito aos princípios de segurança na assistência. Os resultados nos permitiram concluir que a visão dos alunos acerca do tema é ampla, sendo necessário, no entanto, incorporar pontos importantes ao conceito de qualidade na assistência de enfermagem, tais como valorização do trabalho em equipe, sistematização da assistência e questões relacionadas à satisfação dos diversos atores envolvidos na assistência em saúde no ambiente hospitalar.


Estudio cualitativo desarrollado en una universidad pública en el Estado de Sao Paulo, Brasil, con el objetivo de identificar el concepto de calidad en la atención de enfermería hospitalaria en la perspectiva de estudiantes del último año del curso de bachillerato en enfermería. Para la recolección de los datos, fueron realizadas entrevistas semi-estructuradas con 24 alumnos y las declaraciones fueron analizadas según el referencial de Bardin. Las ideas principales se dividían en seis categorías: holismo, humanización, equipo capacitado técnica y científicamente, promoción del auto cuidado, gerenciamiento adecuado de las unidades y respecto a los principios de seguridad en la atención. La visión de los alumnos acerca del tema es amplia pero aún es necesario que identifiquen temas importantes como la valoración del trabajo en equipo, sistematización de la atención y la satisfacción de los atores involucrados en la atención de enfermería hospitalaria.

ABSTRACT

Qualitative study developed in a public university of São Paulo State, Brazil, to identify the conception of quality of hospital nursing care under the perspective of last year undergraduate nursing students. Semi-structured interviews with 24 students were performed to collect the data and the speeches were analyzed according to Bardin's conceptual framework. The central ideas of the speeches were divided into six categories: holism, humanization, technically and scientifically trained team, promotion of self-care, appropriate management of the units and respecting the safety principles in care. It is concluded that students' view regarding the theme is wide; however it is necessary to incorporate important issues to the concept of quality of hospital care, such as the valorization of team work, the systematization of care and the satisfaction of all actors involved in nursing care.

The study aimed to know the opinion of nurses in relation to the Quality Management implemented in a hospital service. This is an exploratory and descriptive study carried out with a sample of 17 individuals. The main results, among the 14 principles of quality, reveal that the seventh principle 'adopt and institute leadership' received the highest score. On the other hand, the lowest scored principle was the third: 'cease the dependency of mass inspection'. The obtained results, coupled with theoretical knowledge on the subject and professional experience on the management of nursing services, lead us to the conclusion that nurses consider Total Quality a practical philosophy to be implemented in the services under their responsibility and accept the challenge of overcoming barriers related to tradition, going from discourse to practice.

El objetivo de este trabajo fue conocer la opinión del enfermero en lo que se refiere a la Administración de la Calidad implantada en un servicio hospitalario. Se trata de un estudio de carácter exploratorio y descriptivo, cuya muestra se constituyó de 17 sujetos y, como principales resultados, se puede citar que, entre los 14 Principios de la Calidad, el que recibió más puntuación de los enfermeros fue el séptimo, que se refiere a "adoptar e instituir el liderazgo". Por otro lado, el principio menos puntuado fue el tercero: "terminar con la dependencia de la inspección en masa". Los resultados obtenidos, aliados al conocimiento teórico sobre el tema y a la vivencia profesional en administración de servicios de enfermería, lleva a sugerir que los enfermeros consideran la filosofía de la Calidad Total como siendo viable en los servicios bajo su responsabilidad y aceptan el desafío de romper las barreras de la tradición, pasando del discurso a la práctica.


O objetivo deste trabalho foi conhecer a opinião do enfermeiro a respeito do Gerenciamento da Qualidade implantado em um serviço hospitalar. Trata-se de estudo de caráter exploratório e descritivo, cuja amostra constituiu-se de 17 sujeitos e, como principais resultados, pode-se citar que, dentre os 14 Princípios da Qualidade, o mais bem pontuado pelos enfermeiros foi o sétimo, que diz respeito a "adotar e instituir a liderança". Por outro lado, o princípio menos pontuado foi o terceiro: "cesse a dependência da inspeção em massa". Os resultados obtidos, aliados ao conhecimento teórico sobre o tema e à vivência profissional em gerência de serviços de enfermagem, leva a sugerir que os enfermeiros consideram a filosofia da Qualidade Total como viável aos serviços sob sua responsabilidade e aceitam o desafio para romper as barreiras da tradição, passando do discurso à prática.

ABSTRACT

A infecção hospitalar representa um dos principais problemas enfrentados pelas organizações hospitalares devido àalta prevalência, letalidade significativa, morbidade associada e altos custos econômicos e sociais gerados com seutratamento. Como a avaliar a qualidade das ações desenvolvidas com a finalidade de diminuir a incidência dessetipo de infecção tem sido desafio constante na área saúde. O objetivo deste estudo é avaliar a qualidade doPrograma de Controle de Infecção Hospitalar (PCIH) nos hospitais públicos estaduais de Salvador, quanto àestrutura, processo e resultado; identificar a interferência desses componentes na qualidade do Programa e avaliara qualidade dos níveis de desempenho desse Programa. Estudo avaliativo quantitativo. A coleta de dados foirealizada por meio de entrevistas semiestruturadas, com auxílio de um formulário. Como resultado verificou-se quea qualidade média do PCIH no nível 1 foi 81,71%, determinando um nível de desempenho bom, no nível 2 foi57,68%, valor considerado regular e no nível 3 foi 23,12%, considerado insuficiente. As variáveis de estruturainterferiram na qualidade do PCIH no nível 3, já as variáveis de processo nos níveis 2 e 3 e as variáveis de resultadointerferiram nos níveis 1 e 2. Observou-se que quanto maior é o nível de desempenho, menor é o atendimento aosaspectos de estrutura, processo e resultado.


Hospital infections represent one of the main problems faced by hospital organizations due to their highprevalence, significant lethality, associated morbidity, and high economic and social costs generated by theirtreatment. The quality evaluation of initiatives developed to minimize the incidence of these types of infections hasbeen a constant challenge in the area of health. The objectives of this study are to evaluate the quality of theHospital Infection Control Program (PCIH) in public state hospitals in the city of Salvador, regarding theirstructure, process, and results; to identify the interference of those components in the quality of the program; andto evaluate the quality of the levels of performance of the program. This is an evaluative, quantitative study. Thedata gathering was carried out through semi-structured interviews and the use of a questionnaire. The resultsillustrated that the average quality rating for the PCIH for level 1 was 81.71%, determining a good performancelevel; for level 2, 57.68%, considered an average rating; and for level 3, 23.12%, considered insufficient.Structure variables interfered with the quality of the PCIH in level 3. Process variables interfered with levels 2 and3, and result variables interfered with levels 1 and 2. The study revealed that the higher the level of performance,the lesser the attention to structure, process, and result aspects.


La infección hospitalaria representa uno de los principales problemas enfrentados por las organizacioneshospitalarias debido a la alta prevalencia, letalidad significativa, morbidad asociada y altos costes económicosy sociales generados por su tratamiento. Evualar la calidad de las acciones desarrolladas con la finalidad dedisminuir la incidencia de ese tipo de infección ha sido un desafio constante en el área de la salud. El objetivo deeste estudio es evaluar la calidad del Programa de Control de Infección Hospitalaria (PCIH) en los hospitalespúblicos estaduales de Salvador, cuanto a la estructura, proceso y resultado; identificar la interferencia de esoscomponentes en la calidad del Programa y evaluar la calidad de los niveles de desempeño de ese programa.Estudio evaluativo cuantitativo. La colecta de datos fue realizada por medio de entrevistas semiestructuradas,con auxílio de un formulario. Como resultado se verificó que la calidad media del PCIH en el nível 1 fue 81,71%,determinando un buen nivel de desempeño, en el nível 2 fue 57,68%, valor considerado regular y en el nível 3 fue23,12%, considerado insuficiente. Las variables de estructura interfirieron en la calidad del PCIH en el nível 3,ya las variables de proceso en los niveles 2 y 3 y las variables de resultado interfirieron en los niveles 1 y 2. Seobservó que cuanto mayor es el nivel de desempeño, menor es el atendimiento a los aspectos de estructura, procesoy resultado.

ABSTRACT

This is a descriptive and quantitative study, carried out in three hospitals in the Northwest of São Paulo, Brazil. It aimed to verify the existence and use of Standard Operating Procedures (SOP) by nursing teams. SOPs describe each critical and sequential step of a task to ensure its expected result. A total of 261 nursing professionals participated in the study, randomly selected from different units and shifts between August and September 2006. Results indicate that 56.7 percent use SOPs only when they have doubts; 54.02 percent of the nursing technicians and auxiliaries and 62.86 percent of nurses do not believe the procedures are being complied with. These findings indicate the need for continuing training on SOP use and compliance by all professionals, with a view to improving nursing care.


Se trata de un estudio descriptivo con abordaje cuantitativa, realizado en 3 instituciones hospitalarias en el noroeste del estado de San Pablo, con los objetivos principales de verificar la existencia y la utilización, por el equipo de enfermería, de los Procedimientos Operacionales Estándar (POEs), desarrollados a fin de describir cada paso crítico y secuencial para garantizar el resultado esperado de una tarea. Participaron, de forma aleatoria, 261 profesionales de enfermería de diversas unidades de internación en los distintos turnos, entre los meses de agosto y septiembre de 2006. Se verificó que 56,7 por ciento acceden a los POE solamente cuando tienen dudas; 54,02 por ciento de los técnicos y auxiliares y 62,86 por ciento de los enfermeros no creen en el seguimiento de los POE. Esos valores llevan a reflexionar sobre la necesidad de ofrecer entrenamientos continuos en relación a la utilización y seguimiento de los POE por todos los profesionales, a fin de mejorar la asistencia de enfermería.


Trata-se de estudo descritivo com abordagem quantitativa, realizado em 3 instituições hospitalares do noroeste paulista, com os objetivos principais de verificar a existência e a utilização pela equipe de enfermagem dos Procedimentos Operacionais Padrão (POPs), desenvolvido a fim de descrever cada passo crítico e seqüencial para garantir o resultado esperado de uma tarefa. Participaram, aleatoriamente, 261 profissionais de enfermagem em diversas unidades de internação nos distintos turnos, entre os meses agosto e setembro de 2006. Verificou-se que 56,7 por cento acessam o POP somente quando têm dúvidas; 54,02 por cento dos técnicos e auxiliares e 62,86 por cento dos enfermeiros não acreditam no seguimento do POP. Esses valores levam à reflexão sobre a necessidade de treinamentos contínuos em relação à utilização e seguimento do POP por todos os profissionais, a fim de melhorar a assistência de enfermagem.

Based on literature, one tries rescuing what is approached in the issues regarding the quality of the nursing care and rendering visible some of the spaces that have long been occupied by the nursing professionals as well as other spaces which are currently feasible to be occupied. The discussion focuses a case experienced by subjects who look for health care before the presuppositions that comprise the issue of health right. From this situation, one goes through Florence and reaches the 3-level requirements from the Credentials National Organization, followed by a questioning about the continuity or the rupture of a scenario of regulating informal actions of the alleged quality, distinguished by the presuppositions elected for this reflection. There is an opening for new questionings in view of the re/meaning of the referred presuppositions that echoes on the quality of the nursing service.

A consultoria em enfermagem psiquiátrica se configura como um dispositivo voltado para a qualificação do atendimento a pacientes com transtornos psiquiátricos ou outras formas de sofrimento mental internados em um hospital geral. Psychiatric nursing consultation is a tool for qualifying the treatment of patients hospitalized at a general hospital with psychiatric disorders or other forms of mental suffering. The main objective is to support and provide operational guidance to nursing teams concerning full health care of these patients when hospitalized in non-psychiatric units. Busca-se, com base na literatura, resgatar o que é abordado nas questões relativas à qualidade da assistência de enfermagem e visibilizar alguns dos espaços há muito ocupados pelos profissionais da enfermagem e outros ora preponderantes de serem ocupados. Discussão tecida sobre um caso vivenciado por sujeitos que buscam atendimento de saúde diante de pressupostos que englobam a questão do direito à saúde. Desta situação perpassa-se por Florence e chega-se às exigências do nível 3 pela Organização Nacional de Acreditação, seguido de um questionamento sobre a continuidade ou ruptura de um cenário de ações informais reguladoras da pretensa qualidade balizados pelos pressupostos elegidos para essa reflexão. Abre-se para novos questionamentos diante da re/significação dos referidos pressupostos ressoando sobre a qualidade no serviço de enfermagem.

Con base en la literatura, uno busca rescatar lo que es abordado en las cuestiones relativas a la calidad de la asistencia de enfermería y tornar visibles algunos de los espacios, hace mucho ocupado por los profesionales de la enfermería, y otros que, por el momento, son preponderantes de ocupación. Tratase de discusión tejida acerca de un caso vivenciado por sujetos que buscan atención de salud delante de presupuestos que engloban la cuestión del derecho a la salud. De esta situación, uno pasa por Florence y se llega a las exigencias del nivel 3 por la Organización Nacional de Acreditación, seguido de un cuestionamiento acerca de la continuidad o ruptura de un escenario de acciones informales reguladoras de la pretendida calidad, distinguida por los presupuestos elegidos para esa reflexión. Se abre para nuevos cuestionamientos delante de la re/significación de los referidos presupuestos resonando sobre la calidad en el servicio de enfermería.

ABSTRACT

The quality is already a part of daily reality in hospital organizations. Care is assessed through the organizational structures, work processes and obtained results. The goal was to identify the result evaluation criteria applied to the nursing service, used in accreditation programs, and to classify them in administrative, care and teaching/research activities. We carried out an exploratory, descriptive and quantitative study. Data were collected from 7 companies accredited by the National Accrediting Organization. These sent us 8 models, divided into groups A, B and C. Group A: models that use the manual; group B: models to which criteria were added; and group C: proper models. We identified 24 criteria: 13 administrative, 06 for care processes and 05 for teaching/research. Nursing services are evaluated with prominence to the administrative area, indicating that the other areas need to be reevaluated in terms of qualitative and quantitative aspects, in order to obtain more precise measurements.
