ABSTRACT

We report the case of two young brothers with Epstein-Barr virus (EBV) otomastoiditis complicated by a facial nerve paralysis. The boys, aged 7 months (patient A) and 2 years and 8 months (patient B), were diagnosed with a facial nerve paralysis House-Brackmann (HB) grade IV (A) and V (B). After unsuccessful pharmacological treatment, patient A underwent mastoidectomy and atticoantrotomy and patient B underwent a transmastoidal surgical decompression of the facial nerve. They recovered to HB grades I and II facial nerve palsy (FNP), respectively. Although rare and relatively unknown, EBV should be considered in the differential diagnosis of children with FNP of unknown cause. Surgical intervention may be a viable therapy with good recovery.

ABSTRACT

AIM: analysis of the nature and the severity of medical complications of the hospitalized cases associated to an measles outbreak which appeared after a period of consistent interruption of measles virus circulation in the Bucharest municipality population. METHODS: analytical study on the cohort of measles cases hospitalized during the main period of the 2005-2006 outbreak from Bucharest municipality: data source--medical records of the cases enrolled in the study. Enrolled cases were allocated to 5 classes, based on the first two discharging diagnosis. Epi Info software has been used in order to: (a) analyze cases' prevalence by diagnosis classes and demographic characteristics and (b) analysis the prevalence of severe cases defined as cases with duration of hospitalization higher the value calculated at the 75 percentile of the series. RESULTS: 63% of laboratory confirmed measles cases have been hospitalized, the prevalence of measles cases with specific complications decreased with age. Longer hospitalization was strictly associated (p < .05) with the need to care for cases with measles specifically complications. CONCLUSIONS: when cases severity was expressed in terms of resources allocate (days of hospital stay) it was found that severity is not depending on age group (OR: 1.57: IC 95%: 0.93-2.63; p = 0.09), the study produced solid arguments regarding the good medical strategy (rational resources use) practiced in the infectious diseases clinics of the Bucharest municipality, demonstrated through.: (a) hospitalization authorized with priority for complicated cases and (b) longer hospitalization associated in special with clinically severe cases.

ABSTRACT

Effects of viruses and bacterial associations are suggested to promote secondary purulent intracranial complications in ENT patients. Fundamental aspects of pathogenesis of viral neuroinfections and pathogenetic mechanisms of different conditions provoked by viral factors promoting bacterial complications are presented. Hemoculture, and cerebrospinal fluid in 24 patients with secondary purulent rhinogenic and otogenic meningitis were studied with polymerase chain reaction for enteroviruses, cytomegaloviruses, Epstein-Barr, herpes, zoster viruses. The viruses were present in the studied media in 29.4% samples.

ABSTRACT

Administration of hot water extracts of a herbal formula containing Ganoderma lucidum, WTMCGEPP (Wisteria floribunda 0.38, Trapa natans 0.38, Miristica agrans 0.38, Coix lachryma-jobi 0.75, cultivated Ganoderma lucidum 0.75, Elfuinga applanata 0.38, tissue cultured Panax ginseng 0.3, and Punica granatum 0.38: numerals designate dry weight gram/dose), decreased herpes zoster pain for five Japanese patients suffering from shingles. Pain relief started within a few days of intake and was almost complete within 10 days. Two acute herpes zoster with manifestations including trigeminal nerve ophthalmia (both 74 years old), lower body zoster (70 years old), herpes zoster oticus (17 years old), and leg herpes (28 years old), responded quickly to treatment and no patient developed post-herpetic neuralgia (PHN) after more than one year of follow-up.
