ABSTRACT

A reconstrução do escroto, com adeqüada proteção dos testículos, representa um desafio após síndrome de Fournier. Os autores relatam o caso, de um paciente de 20 anos, apresentando lesão extensa acometendo escroto e pênis, devido a síndrome de Fournier. O tratamento do pênis consistiu de enxerto de pele, disposto de forma helicoidal, para evitar retrações. O escroto foi reconstruído utilizando-se um retalho fasciocutâneo de região supéro-medial da coxa, de pedículo inferior, 43 dias após o primeiro desbridamento. O resultado mostrou-se satisfatório, funcional e esteticamante, apesar do tratamento não estar finalizado.


Scrotal reconstruction with adequate protection of the testicles remains the challenge following Fournier’s gangrene. The author report on a 20-years-old man presented with an extensive defect of the scrotum and penile caused by Fournier’s gangrene. The penile coverage was gained by grafts helicoidally placed to avoid retractions. The scrotum was reconstructed using a fasciocutaneous supermedial thigh inferiored based flap, 43 days after the first debridement.The results were satisfactory both functionally and cosmetically, although the treatment is not complete.

O objetivo do trabalho é mostrar a experiência do serviço de penioscopia do Centro de Referência em DST-AIDS da Vila Prudente com os pacientes sob suspeita de doença sexualmente transmissível (DST) ou que tiveram contato com parceira com DST. Concluiu-se que a penioscopia, a citologia e a histologia são métodos importantes para o diagnóstico e tratamento das formas não-verrucosas (subclínicas) de papiloma vírus humano (HPV); que esses métodos diagnósticos, ao possibilitarem a detecção e tratamento dessas formas no homem, tornam-se essenciais na epidemiologia do HPV; que incluem fatores individuais no tipo de manifestação clínica do HPV e na proteção quanto à transmissão

ABSTRACT

The papaverine test has been widely used as a diagnostic procedure in erectile impotence. However, when patients do not achieve full erection on the test, it is necessary to differentiate between insufficient inflow and excessive outflow. Gravity cavernosometry is a method designed to evaluate the pressure responses in the corpora when they are subjected to a constant infusion pressure. The infusion flow is of minor significance. The intracavernous pressure (ICP) was measured in cadavers, in psychogenically impotent patients and in patients with arteriogenic impotence, following administration of papaverine and gravity perfusion. During perfusion in cadavers and in psychogenic patients, the ICP showed values above 110 cm H2O, while in arteriogenic patients the values ranged from 30 to 141 cm H2O. In the arteriogenic group, 11/20 patients had an ICP lower than 110 during perfusion. There was no correlation between the pre-perfusion pressure and the final pressure with perfusion. Gravity cavernosometry is a simple, cost-effective and reliable method for the assessment of corporeal competence.

ABSTRACT

Se realiza el análisis preliminar de los resultados de los primeros 15 pacientes intervenidos quirúrgicamente que padecen de impotencia sexual vasculogénica o mixta. Se realizaron tres procederes quirúrgicos, el de Michal II en dos pacientes; la técnica de Virag se hizo en diez pacientes y en tres, endarteriectomía iliohipogástrica. En el 87 % de los pacientes mejoró la flujometría peneana posquirúrgica, y en el 80 % la erección se restituyó o mejoró con respecto al preoperatorio. Se hace una revisión del tema, enumerando los medios diagnósticos utilizados. Se enfatiza en la necesidad de un equipo multidisciplinario para abordar este complejo capítulo de la medicina, que es la disfunción sexual eréctil del pene
