ABSTRACT

Se describe el caso clínico de una paciente de 18 años de edad, quien acudió a la Clínica Estomatológica Provincial Docente Mártires del Moncada de Santiago de Cuba, por presentar persistencia de caninos temporales. Para el diagnóstico y tratamiento exitoso fue necesaria la participación de un equipo multidisciplinario integrado por especialistas en: ortodoncia, imagenología, prótesis y cirugía maxilofacial, quienes corroboraron la retención de caninos superiores. Mediante el tratamiento conjunto de ortodoncia y prótesis, se logró la rehabilitación estética y funcional de la paciente.


The case report of an 18 years patient is described. She went to Mártires del Moncada Teaching Provincial Stomatologic Clinic in Santiago de Cuba, due to persistence of temporary canine teeth. The participation of a multidisciplinary team integrated by specialists in: orthodontics, imaging, prosthesis and maxillofacial surgery was necessary for the successful diagnosis and treatment. They corroborated the retention of upper canine teeth. By means of the combined treatment of orthodontics and prosthesis, the patient cosmetic and functional rehabilitation was achieved.

Introdução:Aenorme dificuldade de deglutição e fala apresentada em pacientes que sofreram maxilectomiatotal ou parcial por consequência de patologias bucais, afeta grande parte da população que realizou tal procedimento. A alternativa de minimizar o impacto causado pela sequela deixada após cirurgia em maxila está na confecção de um obturador palatino, que tem como objetivo obstruir a comunicação oronasal.Objetivo:Este trabalho objetivou descrever um relato de caso evidenciando o passo a passo clínico e laboratorial de uma prótese total obturadorae a satisfação do paciente com a mesma.Método:Paciente do gênero masculino, maxilectomizado há seisanos devido a presença de um CarcinomaAdenóideCístico foi reabilitado com prótese total obturadora superior e prótese total convencional inferior. A reabilitação superior seguiu os mesmos passos de uma prótesetotal convencional, com exceção da moldagem funcional, na qual foi feita a moldagem da comunicação bucosinusal com silicona de condensação antes da moldagem do selado periférico e moldagem funcional propriamente dita. Na etapa laboratorial, foi necessáriaa realização de três prensagens a fim de deixar a prótese oca na porção obturadora.Resultados:O tratamento restabeleceu a dimensão vertical perdida, proporcionou uma oclusão adequada, se mostrou melhor esteticamentee o paciente mostrou-se extremamente satisfeito com a reabilitação.Conclusões:Como conclusão, obteve-se uma prótese total obturadora superior com qualidades funcionais e estéticas satisfatórias. Ademais, ao comparar com a antiga, a novamostrou-se satisfatória de acordo com a percepção do usuário epossui um menor peso, favorecendo uma melhor retenção (AU).


Introduction:The enormous difficulty in swallowing and speaking presented in patients who underwent total or partial maxillectomy as a result of oral pathologies affects a large part of the population that performed this procedure. The alternative of minimizing the impact caused by the sequelae left after maxillary surgery is by making a palatal obturator, which aims to obstruct oronasal communication.Objective:This study aimed to describe a case report highlighting the clinical and laboratory step by step of a complete obturator prosthesis and patient satisfaction with it. Methods:A male patient who had been maxillectomized for 6 years due to the presence of a cystic adenoid carcinoma was rehabilitated with superior obturator total denture and inferior conventional total denture. The superior rehabilitation followed the same steps of a conventional total prosthesis, except for the functional molding, in which the bucosinusal communication with condensation silicon was molded before the peripheral sealing molding and the functional molding itself. In the laboratory stage, it was necessary to perform three presses in order to leave the hollow prosthesis in the obturator portion.Results:The treatment restored the lost vertical dimension, provided an adequate occlusion,was better aesthetically and the patient was extremely satisfied with the rehabilitation.Conclusions:In conclusion, a superior total obturator prosthesis with satisfactory functional and aesthetic qualities was obtained. Moreover, when compared to the old one, the new one was satisfactory according to the user's perception and has a lower weight, favoring a better retention (AU).


Introduccion: La enorme dificultad para tragar y hablarpresentada en pacientes que han sufrido una maxillectomía total o parcial como resultado de patologías orales afecta a gran parte de la población que realizó este procedimiento. La alternativa de minimizar el impacto causado por las secuelas que quedan después de la cirugía maxilar es hacer un obturador palatino, que tiene como objetivo obstruir la comunicación oronasal. Objetivo: Este estudio tuvo como objetivo describir un informe de caso que muestra la clínica y el laboratorio paso a paso de una prótesis obturadora completa y la satisfacción del paciente con ella. Método:Un paciente masculino que había sido maxillectomizado durante seis años debido a la presencia de un carcinoma adenoide quístico fue rehabilitado con una prótesis obturadora total superior y una prótesis total inferior convencional. La rehabilitación superior siguió los mismos pasos de una prótesis total convencional, excepto el moldeado funcional, en el cual la comunicación bucosinusal con silicio de condensación se moldeó antes del moldeado de sellado periférico y el moldeado funcional en sí. En la etapa de laboratorio, fue necesario realizar tres prensas para dejar la prótesis hueca en la porción del obturador. Resultados:El tratamiento restableció la dimensión vertical perdida, proporcionó una oclusión adecuada, fue mejor estéticamente y el paciente quedó extremadamente satisfecho con la rehabilitación. Conclusiones:Como conclusión, se obtuvo una prótesis obturadora superior con cualidades funcionales y estéticas satisfactorias. Además, en comparación con el anterior, el nuevo fue satisfactorio de acuerdo con la percepción del usuario y tiene un peso menor, lo que favorece una mejor retención (AU).

ABSTRACT

O cisto dentígero é o mais comum dos cistos odontogênicos, com sua gênese a partir da separação do folículo da coroa de um dente incluso, sendo comumente encontrado nos terceiros molares e caninos superiores. Apresentam-se principalmente em jovens, de forma assintomática, podendo gerar assimetria facial. Seu diagnóstico é radiográfico e histopatológico, e seu tratamento clássico é feito através da enucleação, com o possível auxílio da marsupialização através da descompressão. O presente relato descreve paciente do sexo masculino, 14 anos, com queixas de aumento de volume e deformidade facial. A hipótese diagnóstica de cisto dentígero confirmou-se após a biópsia-marsupialização, com melhora radiográfica após nove meses, quando foi feita a enucleação total. Após 5 anos da segunda intervenção, o paciente apresentou total neoformação ossea (AU).


Dentigerous cyst is the most common type of odontogenic cyst. It is originated from the separation of crown follicles of an unerupted tooth and is usually found in third molars or superior canines. They are present mainly in young people, asymptomatically, however may cause facial asymmetry. Its diagnosis can be found through radiographic examination as well as oral histopathological diagnosis. Its classic treatment is performed through enucleation, with possible use of marsupialization through decompression. The present report describes the clinical case of a male patient, 14 years old, with complaints of increased volume and facial deformity. The diagnostic hypothesis of dentigerous cyst was confirmed after the biopsy- marsupialization with evidence of radiographic improvement after nine months, when total enucleation was performed. After five years following the second intervention, patient presented total bone neoformation (AU).

ABSTRACT

PURPOSE: Patients with maxillofacial defects secondary to the removal of benign and malignant pathologies of midface have a profound impact on quality of life. The aim of this study was to collect and analyze the data pertaining to 4 designs of obturator by assessment of the patient with obturator functioning scale (OFS) MATERIALS AND METHODS:: This retrospective analysis included all the patients who underwent prosthetic rehabilitation of maxillary defects using 4 different types (conventional, cast partial, hollow bulb, and magnet retained) of obturators from 2009 to 2016 with minimum 1 year of follow up. Demographics, number, size, location, type of pathology, postmorbid dentition, and the treatment rendered were recorded. Obturator functioning scale was used to subjectively assess the patient satisfaction. RESULTS: The mean score on OFS was 5.67 (standard deviation: 1.8). There was a fair and statistical improvement in chewing/eating, speech clarity in public/on phone, swallowing of foods and liquids, pronunciation of words, and social interaction (P < 0.05). The outcome was best in magnet retained followed by cast partial group. The least outcome was in conventional obturator group. CONCLUSION: Obturators of different designs do play an important role in rehabilitation of maxillary defects with a satisfactory outcome.

ABSTRACT

Tooth impaction results from a mechanical blocking that prevents the tooth from erupt. This situation is frequently associated to permanent tooth as wisdom teeth and canines. Impaction of a primary tooth can be associated to systemic or local etiologic factors. Treatment options could include dental extraction, removal of lesion associated without removing the primary tooth and others. The aim of this article is to present a rare case report of an impacted primary tooth in a 42 yearold man, with no complain of swelling. Extra oral examination revealed no alteration of normality. The panoramic radiograph showed a primary tooth impacted in the left body of the mandible. Most of the time the cause for non-eruption of primary tooth is the presence of a mechanical obstacle, not anatomical, in its trajectory. The surgical intervention is a possibility, but it can stay unaltered with no problems related to the impacted tooth. Many times the professional experience and the activity together in various areas of knowledge are the most important and wise conduct to be taken in health care.(AU)


A impactação dental resulta de um bloqueio mecânico que impede o dente de erupcionar. Esta situação está frequentemente associada à dentes permanentes como os terceiros molares e os caninos. Impactação de um dente decíduo pode estar associada a fatores sistêmicos ou locais. Opções de tratamento podem incluir extração dental, remoção da lesão associada sem remoção do dente decíduo, entre outros. O objetivo deste artigo é apresentar um raro relato de caso de um dente decíduo impactado em um paciente do gênero masculino, 42 anos de idade, sem queixas de inchaço. A avaliação extra-bucal não mostrou alterações de normalidade. A radiografia panorâmica evidenciou um dente decíduo impactado em corpo de mandíbula, lado esquerdo. Na maioria dos casos a causa para não erupção de um dente decíduo é a presença de um obstáculo mecânico, não anatômico, em sua trajetória de erupção. A intervenção cirúrgica é uma possibilidade, mas pode permanecer inalterada sem problemas relacionados ao dente impactado. Em muitos casos a experiência profissional e a atividade em conjunto com várias áreas do conhecimento são as condutas mais importantes e sábias a serem tomadas no cuidado em saúde.(AU)

ABSTRACT

Os implantes de zircônia estão sendo usados com maior frequência na atualidade. Considerando suas vantagens e desvantagens, necessita-se de uma curva de aprendizado para que sejam instalados, uma vez que existem limitações em relação à versatilidade da questão protética, devido ao fato de seu corpo carregar junto o munhão protético. Dessa forma, o presente estudo tem como objetivo relatar um caso clínico em que foram realizadas exodontia e instalação imediata do implante em zircônia, assim como ativação de função estética imediata e, posteriormente, a reabilitação protética em cerâmica. Para isso se utilizou implante de zircônia da marca Straumann (Suiça) e reabilitação protética final com dissilicato de lítio. Os resultados apontam a eficácia na reabilitação e resultados estéticos favoráveis, especialmente, no que tange à cicatrização óssea e adaptação de tecidos moles, assim como na função mastigatória. Ressalta-se ainda a importância da curva de aprendizado do cirurgião, desenvolvimento de um desenho cônico dos implantes para estabilidade primária em tecidos ósseos com densidade III e IV, além de outros estudos na área (AU).


Zirconia implants are currently being more frequently used. Considering their advantages and disadvantages, a learning curve is necessary for them to be installed, as there are limitations related to the versatility of the prosthetic matter, due to the fact that its body carries together the prosthetic abutment. So the present study aims to report a clinical case in which was performed exodontia and immediate implantation of the zirconia implant, as well as activation of immediate aesthetic function, and after that prosthetic ceramics rehabilitation. To perform this, a zirconia implant of the brand Straumann (Switzerland) and final prosthetic rehabilitation with lithium disilicate were used. The results point to rehabilitation efficacy and favorable aesthetic results, particularly in regard to bone healing and soft tissue adaptation, as well as in masticatory function. It is also worth noting the importance of the surgeon learning curve, the development of implants conical design for primary stability in bone tissues with density III and IV, besides other studies in the area (AU).

ABSTRACT

Morphological and functional disturbances induced by postsurgical defects and loss of tissues in the stomatognathic system due to the treatment of tumors in the maxillofacial region determine the therapeutic needs of patients. The study aimed at clinical and epidemiological evaluation of patients under prosthetic treatment in order to establish the algorithm for rehabilitation. The study group was composed of the patients after midface surgery (45.74%); surgery in a lower part of the face (47.38%); mixed postoperative losses (3.44%); loss of face tissues and surgery in other locations in the head and neck region (3.44%). The supplementary treatment was applied in 69.63% of patients. Clinical and additional examinations were performed to obtain the picture of postoperative loss, its magnitude, and location to plan the strategy of prosthetic rehabilitation. The management algorithm for prosthetic rehabilitation in patients after surgical treatment of maxillofacial neoplasms was based on its division in stages. The location and magnitude of postoperative losses, as well as the implementation of supplementary treatment of the patients after treatment of maxillofacial tumors, influence the planning of prosthetic rehabilitation that plays a key role and facilitates the patients' return to their prior living situation, occupational and family lives.

ABSTRACT

O presente artigo teve como objetivo esclarecer as diferentes alternativas utilizadas para o reposicionamento labial e melhora da estética do sorriso, propondo uma nova classificação para o diagnóstico e tratamento do sorriso gengival. A técnica cirúrgica descrita no relato de caso associa o recontorno gengival e o reposicionamento labial com tratamento estético restaurador. A cirurgia realizada proporcionou a correção da erupção passiva alterada dos dentes anterossuperiores, combinada com o alongamento do feixe lateral do músculo levantador do lábio superior e da asa do nariz, e a contenção do mesmo com sutura mononylon, diminuindo o sorriso gengival. Notou-se, após a cirurgia, que a linha alta do sorriso foi corrigida sem comprometer a harmonia labial. A paciente manifestou alto grau de satisfação com o tratamento. O diagnóstico e a técnica cirúrgica proposta neste estudo podem ser uma opção terapêutica para o reposicionamento labial e harmonia do sorriso.


The present article aims to address the different alternatives used for lip repositioning and improved smile aesthetics, proposing a new classification for the diagnosis and treatment of the gummy smile. The surgical technique described this a case report associates gingival recontouring and lip repositioning with restorative aesthetic treatment. The surgery provided the correction of altered passive eruption of the anterior superior teeth combined with lengthening of the lateral bundle levator muscle of the upper lip and the ala of the nose, containing them with mononylon sutures, decreasing the gummy smile. It was noted that after surgery, the high smile line was corrected without compromising the labial harmony. The patient expressed high satisfaction with the treatment. The diagnosis and surgical technique proposed in the present study may be a therapeutic option for lip repositioning and smile harmony.

ABSTRACT

A disponibilidade óssea é importante na Implantodontia para proporcionar estabilidade primária e a manutenção dos tecidos peri-implantares necessários para a estética e osseointe- gração. Em algumas situações, os defeitos ósseos devem ser corrigidos antes da instalação dos implantes usando enxertos. Os clínicos buscam um material ideal de enxerto ósseo e o objetivo desta revisão de literatura foi avaliar os resultados obtidos após o uso da hidroxiapatita. A literatura consultada mostrou que o uso deste material favorece a neoformação óssea em defeitos críticos, o que justifica sua utilização em alvéolos após exodontia com paredes que não apresentem integridade; em defeitos ósseos periodontais, peri-implantares ou periapicais e em aumentos ósseos verticais na região de seio maxilar, de forma isolada ou em conjunto com outros biomateriais. Diante da diversidade de tipos de hidroxiapatita, o profissional deve selecionar aquele com propriedades que favoreçam sua reabsorção para diminuir a presença de partículas residuais.


In Implantology, bone availability is important to provide primary stability and maintenance of peri-implant soft and hard tissues necessary for aesthetics and osseointegration. In some situations, osseous defects must be corrected before implant placement using bone grafts. Professional search for an ideal bone grafting material and the aim of this literature review was to evaluate the results obtained by the use of hydroxyapatite. It has been shown that this material favors neo bone formation in critical defects, supporting its use in extraction sockets with non-intact bone structure, periapical, peri-implantar or periodontal osseous de¬fects and maxillary sinus grafting, alone or combined with other biomaterials. Because of the many types of hydroxyapatite, professionals shall select the one with proprieties that support its reabsorption, in order to have fewer residual particles.

ABSTRACT

Pessoas que apresentam grandes deformidades dentofaciais podem ter prejuízos sociais, pessoais e emocionais. A cirurgia ortognática é utilizada para correções dessas deformidades, que conseqüentemente afeta as aparências das faces dos pacientes. Este estudo teve como objetivos: 1) Avaliar amostras de faces da população de Ribeirão Preto, SP e as faces prototípicas provenientes delas, consideradas sem deformidades; 2) Analisar a atratividade facial de dez pacientes submetidos a cirurgias ortognáticas nas fases pré e pós-cirúrgicas; ao relacionar suas atratividades aos parâmetros físicos alterados pela cirurgia; e também avaliar as conseqüências na qualidade de vida, ansiedade e auto-estima destes pacientes. As análises faciais foram frontais e obedeceram aos critérios preconizados na literatura de meios, terços e quintos faciais. A coleta de dados com os pacientes foi feita no CEDEFACE (Centro de Pesquisa e Tratamento das Deformidades Buco-Faciais). Foram obtidas fotografias nas fases pré e pós-cirúrgicas; momentos em que também foram aplicadas escalas de qualidade de vida (WHOQOL bref), de ansiedade (IDATE e Escala Analógica de Humor) e autoestima (Escala de Autoestima de Rosenberg). Dois experimentos foram realizados. Um com as fotos das faces pré e pós-cirúrgicas a fim de avaliar a atratividade facial dos pacientes antes e depois das cirurgias e outro para comparar a atratividade facial dos pacientes com a atratividade de faces prototípicas relativas ao sexo e etnia correspondentes. Nesses experimentos os julgamentos de atratividade foram provenientes de uma amostra de 40 participantes leigos. Os resultados apontaram que: 1) As análises do banco de faces e protótipos apresentaram parâmetros físicos faciais diferentes daqueles apontados na literatura, principalmente na análise dos terços e quintos; 2) A cirurgia ortognática contribuiu para o aumento da atratividade dos pacientes, embora estes ainda...(AU)

The orthognathic surgery is used to correct these deformities, and consequently, it affects the racial appearances of the patients. The general aims of this study were: 1) Evaluate samples of faces from a population of Ribeirão Preto, SP and their prototypical faces considered as without deformities; 2) Analyze the facial attractiveness of ten patients before and after being submitted to orthognathic surgery; searching the relationship between the attractiveness and physical parameters modified on the faces by the surgery; and also assessing the effects of this surgery on the quality of life, the anxiety and the self-esteem of these patients. The facial analyses were frontal and from the halves, the thirds and the fifths of the faces. The pictures of faces before and after the orthognathic surgery were from voluntary patients from CEDEFACE (Centro de Pesquisa e Tratamento das Deformidades Buco-Faciais) at Araraquara, SP. Before and after the surgery, these patients also had their quality of life (by WHOQOL bref scale), the anxiety (by IDATE and Escala Analógica de Humor) and the self- esteem (by Escala de Auto-Estima de Rosenberg) evaluated. There were two experiments. From an experiment, forty volunteers judged the attractiveness of the faces pictured before and after the surgery. From the other experiment, the volunteers compared the attractiveness of the patient faces with the attractiveness of the prototypical faces, respectively to their sex and race. Introducción. La cirugía de la base craneal es en la actualidad unarealidad que se ha ido consolidando en las últimas décadas. El vertiginosoavance tecnológico desarrollado ha actuado como motor en la evolución delas técnicas quirúrgicas que abordan dicha región anatómica. Su impulsodefinitivo se ha cimentado en el concepto básico de equipo multidisciplinario.Material y métodos. Se describe el abordaje subcraneal como alternativa alos tradicionales abordajes a la base craneal anterior. Discusión y revisión históricade los distintos pasos claves en la realización del mismo.Discusión. La elección del abordaje más adecuado a la base craneal en cadacaso es esencial para la obtención de resultados quirúrgicos globales satisfactorios.Inicialmente dependerá de la localización anatómica exacta de lalesión dentro de la base craneal y de su extensión tridimensional, así comode la naturaleza de la misma. El abordaje subcraneal representa un pasoimportante en el desarrollo de las técnicas quirúrgicas de la base craneal.Conclusiones. Introduction. Skull base surgery is now a reality that hasbecome consolidated over the last decades. The dizzyingly rapidtechnological advances that have taken place have served as amotor for the development of surgical techniques to approachthis anatomic region. The foundation for the definitive push forwardof this technique was multidisciplinary teamwork.Material and methods. The subcranial approach is described asan alternative to traditional approaches to the skull base. The historyof different key steps in the subcranial approach is described anddiscussed.Discussion. The selection of the most suitable approach to the skullbase in each case is essential for obtaining satisfactory surgicalresults overall. Initially, the selection of an approach depends onthe exact anatomic location of the lesion in the skull base, its threedimensionalextension, as well as the nature of the lesion. Thesubcranial approach is an important step in the development ofsurgical techniques for the skull base.Conclusions. The anterior subcranial approach provides ample andoptimal exposure of all the planes of the anterior cranial fossa, fromthe anterior ethmoidal roof to the clivus-sphenoid plate, includingthe orbital roofs. This objective is achieved without having to retractthe frontal lobes, thus avoiding the associated morbidity andmortality that this maneuver entails. The anterior subcranialapproach makes precise earlier management of oncologic pathologywith intradural and extradural involvement possible, as well as thetreatment of fractures produced by high-energy impact withinvolvement of the anterior cranial fossa(AU)

ABSTRACT

El presente estudio se evaluó la eficacia clínica de férulas oclusales duras (tratamiento conservador) versus eminectomía(tratamiento quirúrgico) en sujetos con disfunción temporomandibuylar. Es un ensayo clínico exsperimental con un universo de 250 sujetos, el tamaño de la muestra fue de 120 sujetos. Los sujetos fueron valorados en etapas pre tratamiento y pos tratamiento a los 10 días, 45 días y 6 meses a través de Escala Analoga del dolor. palpación y auscultación para constatar la presencia de ruidos articulares, la maniobra de Kroug- Paulsen para valorar los movimientos anormales mandibulares en apertura y cierre. Resultados: ambas técnicas fueron efectivas para desaperecer los ruidos articulares, los movimientos anormales en las excursiones máxima apertura bucal y excursón laterotrusiva, el dolor dismnuyó en un 55 por ciento con tratamiento quirúrgico y un 70 por ciento con tratamiento conservador a los 6 meses: mandibular, con el tratamiento quirúrgico se obtuvo en deflexión un 90 por ciento y en excursión protusiva un 97 por ciento...

ABSTRACT

Se presentan las complicaciones de la cirugía ortognática ocurridas en los pacientes intervenidos en el Hospital de San José de Bogotá, del 1º de enero de 2004 al 31 de mayo de 2007. Como no existía una evaluación estadística que diera una visión real de la situación de estos pacientes en el hospital, se realizó una revisión de las bases de datos de cada servicio y de las historias clínicas con el fin de describir las complicaciones más frecuentes y su desenlace. Se encontró que en dicho período fueron intervenidos 31 pacientes de los cuales 10 (32%) presentaron alguna complicación. La más frecuente fue la hemorragia (13%) que en la mayoría de los casos se manifestó como epistaxis, le siguieron en frecuencia el déficit neurológico y las alteraciones anatómicas (6.5% cada una). Solo hubo necesidad de reintervenir a un paciente (3.2%). La cirugía que presentó mayor número de complicaciones fue la bimaxilar. Se concluye que durante el período estudiado la cirugía ortognática realizada en el Hospital de San José de Bogotá presentó una alto porcentaje de complicaciones (32%) en comparación con lo reportado en la literatura que es de 6.4%, que se puede explicar por el bajo número de casos. Ninguna de las complicaciones revistió gravedad.


The aim of this study is to present complications following orthognathic surgery performed at the San José Hospital of Bogotá, between January 1, 2004 and May 31, 2007. As a statistic assessment on the real outlook on these patients did not exist in the hospital we reviewed the data bases and clinical records of various departments in order to describe the most common complications and their outcomes. Thirty-one patients underwent this type of surgical procedure during this period of time, of which 10 (32%) had complications. The most frequent was hemorrhage (13%) and presented as epistaxis in most cases, followed by neurological deficits and anatomic alterations (6.5% respectively). Only one patient (3.2%) required a second surgical procedure. Bimaxillary surgery had the highest number of complications. We conclude that during the studied period, orthognathic surgery performed at the San José Hospital of Bogotá had a high percentage of complications (32%) compared to those reported by published literature (6.4%) and may be explained by the low number of cases. No complication was potentially serious.
