ABSTRACT

Surgical stapler allows surgeons to do simultaneous cut and suture quickly in excellent quality. Surgical stapler is currently used widely and has become indispensable tool for thoracic surgery, especially for endoscopic surgery such as thoracoscopic lung lobectomy, although there was some reluctance for use at the dawn of the era of mechanical stapling device. Devices should be used for right place in right manner. There are various kinds of stapling devices and surgeons should follow manufactures' instruction appropriately to avoid possible malfunctions of the devices. Surgeons must be aware of possible risks in use of stapling devices and must learn pitfalls. Stapling devices do and will continue to be evolved, and surgeons have to keep the know-how updated.

ABSTRACT

247 elective rectal operations were performed between 1st of Jan., 1996 till 31st of Dec., 2001 in our surgical department. The observation period was divided into one 4-year and one 2-year period. 148 operations were performed in the first and 99 in the second period. In the second period some personal, methodical and technical changes occurred. Age and sex distributions were similar, and the groups were comparable. The early postoperative mortality rate decreased from 9% to 4%. Due to the double stapling technique the rate of anterior rectal resections increased from 40% to 69% and the rate of abdomino-perineal rectal extirpation decreased from 43% to 23% in the same time. Large differences were found in the rate of palliative stoma performing operations, the rate decreased from 16% to 8% in the second period, so the quality of life of the patients improved. The operative time, the length of hospital stay and the rate of blood transfusions decreased as did the rate of postoperative infections. The new quality program improved the results significantly.

ABSTRACT

This study aims to provide an overview of the different suturing techniques used in surgery, paying special attention to cancer surgery which, compared to general surgery, has precise requirements correlated to the type of patient and form of exeresis used, which often tends to be extensive. In this panorama which manages to avoid any form of superficiality, the discussion ranges from mechanical automatic suturing devices to recent advances in so-called biological repair, without omitting an analysis of the gold standard of tissue repair, namely suture thread, which appears to have been overwhelmed by the enthusiasm for technological innovation, even in the literature.

ABSTRACT

For almost two centuries, surgeons have been using mechanical devices to join tissue. One of the most successful methods is that of stapling, which has become very common in more recent times. The range of staplers consists of five established stapler categories (circular, linear, linear cutting, ligating and skin staplers), along with recent variations which lend themselves to minimally invasive surgery. Within each category, several commercial models are available, many of which have their own unique features. The procedures which are enhanced by these instruments are many and varied. Applications have been further expanded and improved by the instrument design developments seen in recent years. This review attempts to present a rationalized overview of the array of stapling instruments, with relevant procedures. The authors believe that surgical stapling may be greatly enhanced by further research and development, taking the instrument designs and procedures further into the realm of minimally invasive surgery.
