RESUMEN

Glycated fibrinogen (GP) and glycated hemoglobin of both types (HbA1c) were measured in 146 diabetics aged 16-66 years with disease duration of 2 weeks to 35 years. Sixty-six of these patients suffered from insulin-dependent and 80 from non-insulin-dependent disease. Control group consisted of normal subjects (33 male and 32 female) aged 2-67 years. GP was found to be an indicator of glycaemia level during 3-4 days before measurements (r = 0.6, p < 0.005). GP level in the patients is essentially higher than in normal controls (p < 0.001). GP level reflects the degree of carbohydrate metabolism compensation and is in good correlation with the content of HbA1c (r = -0.9). In contrast to HbA1c, measurements of GP can be used for express control of carbohydrate metabolism in diabetics after starting a new protocol of insulin therapy or other treatment.

The levels of glycosylated hemoglobin (HbA1c) and glycosylated keratin were studied along the hair length in 17 patients with newly detected insulin dependent diabetes mellitus (IDDM) aged 7 to 33, in 14 patients with newly detected noninsulin dependent diabetes mellitus (NIDDM) aged 46 to 73, and in 41 healthy subjects. The level of glycosylated keratin in healthy hairs along the entire length varied within 0.094-0.124 mumol of fructosamine per 100 mg of hair. In 10 of 17 patients of the IDDM group measurements of glycosylated keratin levels made it possible to determine the time of appearance of diabetes mellitus, in 13 patients of the NIDDM group these levels along the entire hair length were elevated. The determination of these levels permitted the estimation of the time of appearance of IDDM. Its manifestation followed a long period (1-2 yrs.) of latent derangements of carbohydrate metabolism. In NIDDM patients the time of appearance of disease is difficult to determine because of the limited hair length and a long latent period of disease (over 2-3 yrs.).

RESUMEN

Alterations in content of neutral lipids, phospholipids and of their lysoforms were studied in erythrocyte membranes of patients with various severity of vascular impairments considering clinical manifestation of diabetes mellitus. Impairments of lipid composition in erythrocyte membranes observed correlated with the steps of diabetic angiopathies, thus considering an importance of these alterations in pathogenesis of vascular diseases of patients with diabetes mellitus.

RESUMEN

The efficacy of a prolonged (an average of 24.2 days) subcutaneous insulin administration using a Soviet apparatus NDL (a portable drug dosing apparatus) of paracorporal type was studied in 30 patients with type I diabetes mellitus. As a result of therapy a stable compensation of carbohydrate metabolism was achieved in 26 patients, subcompensation in 4 patients; glycemia circadian variations and requirements in insulin in most cases decreased. Simultaneously a clinical improvement of the patients' general status was observed. The use of a prolonged subcutaneous administration of insulin using the NDL was recommended for patients with insulin-dependent diabetes mellitus with a grave form, a labile course and insulin resistance that was difficult to control.
