RESUMEN

Purpose: In this study an evaluation of the imaging performance of an electronic portal imaging device (EPID) is presented. The evaluation performed employing the QC-3V image quality phantom. Methods: An EPID system of a 6 MV LINAC, was used to obtain images of a QC-3V EPID phantom. The X-ray source to phantom distance was 100 cm and the field size was 15x15 cm2. The irradiation conditions comprised Dose Rates (DR) of 200, 400 and 600 for a 2 MU-100 MU range. The Contrast Transfer Function (CTF), the Noise Power Spectrum (NPS), the Normalized Noise Power Spectrum (NNPS) and the Contrast-to-Noise Ratio (CNR) were studied. In addition, an alternative factor showing a frequency related output signal-to-noise ratio (SNR), the Signal-to-Noise-Frequency Response (SNFR), has been introduced. SNFR is a comprehensive quality index, easily determined in clinical environment. Results: The CTF curves were found comparable to each other. The lowest values were measured at 2 MU and 200 MU/min. Concerning the NPS and NNPS graphs it was found that the values decrease up to approximately 0.3 lp/mm and demonstrate a white noise shape afterwards. SNFR values were found reducing with spatial frequency. Highest CNR were found between the region 7 and 11 of the phantom. Conclusions: The influence of MU and DR on EPID performance were investigated. Image quality was assessed using the QC-3V phantom. The presented results can lead to image quality amelioration and act supportively to current image quality control routine protocols.

OBJECTIVES: To evaluate and compare the efficiency of orthodontic treatment and surgical uprighting of first and second mandibular molars. MATERIALS AND METHODS: An electronic literature search in PubMed, Science Direct, Embase, Scopus, Web of Science, Cochrane Library, LILACS, and Google Scholar, as well as a hand search was conducted by two independent researchers to identify relevant articles up to January 2022. In addition, a manual search was done that included article reference lists, grey literature, and dissertations. The risk of bias of the included prospective and retrospective studies was assessed with the Risk Of Bias Tool In Non-randomized Studies of Interventions (ROBINS-I) assessment tool. RESULTS: A total of six nonrandomized clinical trials (non-RCT) evaluating the efficiency of mandibular molar orthodontic and/or surgical uprighting were included. The quality analysis showed certain defects of the Non-RCTs included and, according to the criteria used, the majority of the articles were judged to be of moderate quality. CONCLUSIONS: Based on the evidence, orthodontic and surgical uprighting appear to be effective treatment methods for mandibular molars. Surgical uprighting may be associated with more complications than orthodontic uprighting. However, the existing literature on the subject is limited, heterogeneous, and methodologically limited. Therefore, the outcomes should be interpreted carefully.

BACKGROUND: The present study aims to define through questionnaires the frequency and the variety of accidental incidents occurring in orthodontic clinical practice among Greek practicing orthodontists. METHODS: A questionnaire survey was conducted among orthodontists from the registry of orthodontists in Greece. The questionnaire was divided into two parts. The first part involved three questions relating to the socio-demographic status and the background of the orthodontist, and the second part concerned exclusively the frequency of accidental incidents that have occurred during clinical practice with three possible answers: never, once, more than once. RESULTS: From the 200 initially distributed questionnaires, 124 were finally completed and sent back (response rate: 62%). The results showed that orthodontists with more years of clinical practice had faced more accidental incidents. Among the ingestion incidents caused by foreign objects, the most frequently occurring was the ingestion of elastic separators, followed by the ingestion of elastic ligatures and ingestion of hooks. The most commonly reported traumatic incidents were the trauma-lesion of the mucosa by the orthodontic wire or part of it, followed by trauma-lesion by hooks and wire ligatures. The reported number of incidents with further complications and with patients referred to an emergency room was very low. CONCLUSIONS: The results of the present study determined a high frequency of accidental incidents among Greek orthodontists. The longer clinical experience was accompanied by more accidental incidents. Orthodontists, like other health professionals, must learn and continuously update their knowledge regarding the management protocols of medical emergencies.

Skeletal Class II malocclusion is the most common skeletal anomaly in orthodontics. Growth in the body of the deficient mandible is induced by periosteal apposition and endochondral ossification in the condyle. Functional appliances have been used in the correction of Class II malocclusions by inducing mandibular growth. Despite their utilization though, their effect still remains controversial. The aim of the present study is to review the existing literature regarding the effects of mandibular protrusion in mandibular growth of growing rats. A protocol was followed according to the guidelines of the Cohrane Handbook for Systematic Reviews. Databases were searched using a specific algorithm. From the ten studies finally analyzed, we conclude that the use of a functional appliance in growing rats induces cell proliferation and bone formation in their condyles, resulting in mandibular growth.

Mandibular protrusion and its treatment is challenging for the orthodontist. The aim of the present research was to identify macroscopic changes in the mandible, based on three-dimensional Cone Beam Computed Tomography analysis. Seventy-two male Wistar rats were divided into two equal groups, experimental (group A) and control (group B). Each consisted of three equal subgroups of 12 rats (A1, A2, A3, B1, B2, B3). Full-cast orthodontic intraoral devices were attached to the maxillary incisors of the experimental animals, and effected functional posterior mandibular displacement. Throughout the experimental period, all animals were fed with mashed food. Animals were sacrificed at 30 days (A1, B1), 60 days (A2, B2) and 90 days (A3, B3). At the 60th day of the experiment, the orthodontic devices were removed from the remaining experimental subgroup A3. Measurements revealed significant differences in the anteroposterior dimensions between experimental and control subgroups. However, the observed changes in the vertical dimensions, Condylion/Go'-Menton and the Intercondylar distance proved insignificant. Posterior mandibular displacement of the mandible in growing rats affects the morphology of the mandible and culminates in the development of a smaller mandible at a grown age.

OBJECTIVE: This systematic review aims to investigate and summarize the mid-term effects and stability of the treatment with facemask appliance. METHODS: Search without restrictions in five databases (Pubmed, Medline, Embase, Scopus, Cochrane Central Register of Controlled Trials) since inception and hand searching until March 2021 was conducted. Detailed search strategies were based on the PubMed strategy and adapted accordingly. Studies assessing the mid-term effects of facemask appliance, with a follow-up post-treatment period of more than 6 years or after the peak of pubertal growth, were to be included. The risk of bias in individual studies was assessed using the Cochrane guidelines for the RCT and the ROBINS-I tool for the non-RCT studies. RESULTS: Five studies were finally considered eligible for inclusion (one RCT and four CCTs). According to the reported evidence, the mid-term success rate ranged from 62.7% to 100%. Mid-term success was identified with positive overjet as well as acceptable aesthetic and functional characteristics. CONCLUSIONS: Class III malocclusion treated with facemask presents a high percentage of success in the mid-term follow-up period. The mid-term effects of facemask were stable for the maxilla, in terms of sagittal and anteroposterior dentoskeletal dimensions, and any kind of relapse is attributed to uncontainable residual growth of the mandible. However, considering the high risk of bias and the follow-up period limitations, more high-quality studies are necessary in order to achieve further clarification. Future research should be based also on identifying biomarkers to indicate likely treatment responses. REGISTRATION NUMBER: PROSPERO: CRD42020179402.

While rapid technological advances in radiotherapy techniques have led to a more precise delivery of radiation dose and to a decreased risk of side effects, there is still a need to evaluate the efficacy of the new techniques estimating the biological dose and to investigate the radiobiological impact of the protracted radiotherapy treatment duration. The aim of this study is to compare, at a cytogenetic level, advanced radiotherapy techniques VMAT and IMRT with the conventional 3D-CRT, using biological dosimetry. A dicentric biodosimetry assay based on the frequency of dicentrics chromosomes scored in peripheral blood lymphocytes from prostate cancer patients and PC3 human prostate cancer cell line was used. For each patient blood sample and each subpopulation of the cultured cell line, three different irradiations were performed using the 3D-CRT, IMRT, and VMAT technique. The absorbed dose was estimated with the biodosimetry method based on the induced dicentric chromosomes. The results showed a statistically significant underestimation of the biological absorbed dose of ~6% for the IMRT and VMAT compared to 3D-CRT irradiations for peripheral blood lymphocytes, whereas IMRT and VMAT results were comparable without a statistically significant difference, although slightly lower values were observed for VMAT compared to IMRT irradiation. Similar results were obtained using the PC3 cell line. The observed biological dose underestimation could be associated with the relative decreased dose rate and increase irradiation time met in modulated techniques compared to the conventional 3D-CRT irradiations.

INTRODUCTION: The purpose of this study was to evaluate radiation dose reduction in coronary computerised tomography angiography (CCTA), using a commercially available iterative reconstruction (IR) algorithm as well as the behaviour of the image noise. METHODS: A total cohort of 47 patients underwent CCTA examination on a 64-slice multi-detector CT. They were divided into four groups according to the time when the examination was performed (before or after the installation of iDose) and the acquisition protocol followed (prospective or retrospective electrocardiography-ECG gated). The images acquired with reduced dose settings were reconstructed using two levels (L4 and L6) of the iDose4 algorithm. Image noise was measured in all cases. RESULTS: In retrospective acquisition, images acquired with a 46% lower radiation dose and reconstructed with iDose4 L6 provided noise comparable to that in the full-dose filtered back-projection images. For the prospective acquisition mode, a slight decrease (26%) in radiation dose resulted in noise improvement in low-dose images reconstructed with iDose4 L4 (16% noise removal) and L6 (30% noise removal). CONCLUSIONS: The fact that image quality is improved while radiation exposure is reduced indicates that there is room for a further reduction in exposure settings. Additionally, the combination of iDose4 with prospective acquisition is able to significantly reduce the radiation dose associated with CCTA at values of about 2 mSv and even lower.
