RESUMEN

Given their large surface area and versatile chemical reactivity, single-walled carbon nanotubes (SWCNTs) are regarded as the basis of new pharmacological complexes. In this study, SWCNTs are chemically functionalized with fluorescein, folic acid, and capecitabine, a drug that is commonly used against colorectal cancer. These functionalized SWCNTs are dispersed in water by taking advantage of their synergistic interaction with type-II nanocrystalline cellulose (II-NCC), and the resulting colloidal system is tested in vitro on both normal (differentiated) and cancerous (proliferative) human colon cells (Caco-2). The functionalized SWCNT/II-NCC hybrids show a higher activity than the reference (capecitabine) against the Caco-2 cancer cell line. However, this effect appears to be intrinsically associated with the SWCNT/II-NCC complex, particularly boosted by fluorescein, as the presence of capecitabine is not required. In addition, confocal microscopy fluorescence imaging using cell cultures highlights the enormous potential of this nanohybrid platform for colon cancer theranostics.

AIM: To describe a case series of thyrotoxicosis likely triggered by SARS-CoV-2 vaccination and to warn physicians about this potential correlation. To report clinical, laboratory and imaging findings and provide further information that goes in line with the underlying mechanisms. METHODS: Single-center case series based on all the information collected in the hospital medical records, as well as the temporal sequence between the onset of symptoms and COVID-19 vaccination. RESULTS: We report 8 cases with thyrotoxicosis after SARS-CoV-2 vaccination. 4 cases of Graves' disease (GD), 2 cases of subacute painful thyroiditis (SAT), 1 case of concurrent GD and SAT and 1 case of atypical subacute thyroiditis. Five patients received BNT162b2 mRNA vaccine, 3 patients 1273 mRNA vaccine. The onset of symptoms following vaccination ranged from 10 to 14 days in six of eight patients and from 7 to 8 weeks in two patients. CONCLUSIONS: Several hypotheses have been proposed to explain the potential correlation between SARS-CoV-2 vaccination and thyrotoxicosis, including immune system hyper-stimulation, molecular mimicry and Autoimmune/Autoinflammatory Syndrome Induced by Adjuvants (ASIA). We should pay greater attention to thyroid disorders in patients receiving vaccine against SARS-CoV-2.

RESUMEN

A new analytical method was optimized to determine 18 disinfection by-products (DBPs) in drinking water, including four different chemical groups. For this purpose, spiral-shaped hollow-fibre liquid phase microextraction with 1-octanol as the acceptor solvent assisted by effervescence was applied using a homemade supporting device that was specifically designed for this application. The device was printed in a 3D printer and allows for an increased fibre surface even with a low sample volume, which significantly facilitates the extraction. The samples were analysed by gas chromatography coupled to both an electron capture detector and a mass spectrometer for the quantification and unequivocal identification of the analytes, respectively. Effervescence was generated using citric acid and bicarbonate at a molar ratio 1:2, which significantly improves the extraction efficiency and reduces mechanical operations, since stirring and modifiers are not required. The results showed enrichment factors ranging from 13.1 to 140.1. Satisfactory recoveries (80-113 %) were obtained, with relative standard deviations from 3 to 15 % and good linearity. The detection limits (ng L-1) ranged from 10 to 35 (trihalomethanes), 12 to 220 (halonitromethanes), 17 to 79 (haloacetonitriles) and 10 to 16 (haloketones). The applicability of the method was assessed in 6 local water distribution systems.

The shift from rainfed to irrigated agriculture is associated with a change in the fertilization rates due to increases in expected production and the fact of growing more N demanding crops. In addition, the circulation of irrigation return flows (IRF) mobilizes soluble salts stored in soils or geological materials. As a consequence, it implies severe modifications in the dynamics and total amount of soluble salts and nitrogen exported, especially in semi-arid watersheds. In this study, long-term data collected by the regional authorities was used to assess the effects of irrigation implementation on salinity (using electrical conductivity, EC, as a proxy) and nitrate concentration (NO3-) after the transformation of ca. 77 km2 from rainfed to irrigated agriculture in the Cidacos River (CR) watershed. The results indicate that water quality in the lower reaches of the CR was significantly modified after the diffuse incorporation of IRF. In contrast, neither EC nor NO3- were different in those monitoring stations whose contributing watersheds did not include transformed area. In addition, the temporal dynamics in the analysed variables shifted from a rainfed land signal typical in the region to an irrigated land signal, and the hydrochemical type of the CR shifted from mixed-to-Na+-mixed-to-HCO3- to mostly Na+-mixed type, typical of waters affected by IRF in the region. Groundwater EC and NO3- also increased in those wells located within the irrigated area. Although there are great uncertainties in the actual amount of salt and N reaching the CR via IRF, the expected contribution of waste water spilled into the CR is minor in comparison to other sources, mostly agricultural sources in the case of N. The observed changes have promoted the designation of the lower reaches of the CR as "affected" by NO3- pollution, and the whole CR watershed as a Nitrate Vulnerable Zone, with the emergent question about whether irrigation implementation as carried out currently in Spain is against the environmental objectives of the Water Framework Directive.

Introducción: La comorbilidad psiquiátrica es común en epilepsia, de ahí la importancia de considerar en qué medida los fármacos antiepilépticos pueden influir en el estado de ánimo. El objetivo de este trabajo es analizar el efecto de lacosamida en la calidad de vida y en la conducta del paciente epiléptico en la práctica clínica. Métodos: Estudio multicéntrico, observacional y prospectivo en pacientes diagnosticados de epilepsia, mal controlados que recibieron tratamiento adyuvante con lacosamida. Mediante 4 visitas durante 12 meses se valoró el impacto del fármaco en la calidad de vida y el estado de ánimo utilizando el cuestionario de calidad de vida QOLIE-10, la escala hospitalaria de ansiedad y depresión (HADS) y la escala de impulsividad de Barratt (BIS-11), además se determinó su eficacia y seguridad. Resultados: Se incluyeron 55 pacientes, edad media 47,1 ± 18,4 años; porcentaje inicial de comorbilidad psiquiátrica 34,5% y número medio de crisis/mes previo 3,6 ± 4,3. Las escalas QOLIE-10 y HADS reflejaron mejoras estadísticamente significativas en pacientes que partían de una situación basal desfavorable (ansiedad, depresión y/o baja calidad de vida). La escala BIS-11 no detectó la aparición de conductas impulsivas durante el seguimiento. Tras 12 meses de tratamiento el 51,9% de los pacientes estuvo sin crisis, y un 77,8% presentó una educción ≥ 50%. La mayoría de efectos adversos fueron leves, obligando a retirar el fármaco en 10 casos (18,2%). Conclusiones: Lacosamida ofrece un perfil de eficacia y seguridad favorable, y podría constituir una opción terapéutica útil en pacientes con epilepsia y comorbilidad psiquiátrica

Adverse effects were mild in most cases; lacosamide was discontinued in 10 patients (18.2%)

The fracture dislocation of the hip is a rare lesion described up to 4 to 17% of the cases and usually secondary to automobile accidents. Generally of poor prognosis and with already well documented complications such as avascular necrosis of the femoral head, osteoarthritis and heterotopic ossification. The iatrogenic fracture of the neck or head of the proximal femur is a complication even more rare and is not documented its incidence in the world literature. Having as probable causes the irreducibility of a dislocation of hip, more than one attempt of closed reduction, inadequate technique and some problems with the anesthesia and relaxation of the patient. We present the case of a young patient attended in the first eight hours after his accident with an iatrogenic fracture of the femoral head with follow up to 18 months and emphasizing the importance of making an anatomic open reduction and stable fixation in order to get favorable results.


La fractura luxación de la articulación coxofemoral es una lesión rara descrita de 4 a 17% de los casos por lo regular secundaria a accidentes automovilísticos. Generalmente de mal pronóstico y con complicaciones ya bien documentadas como la necrosis avascular de la cabeza femoral, la osteoartritis y la osificación heterotópica. La fractura iatrogénica del cuello o de la cabeza femoral es una complicación aún más rara cuya incidencia no está documentada en la literatura mundial. Como probables causas se tienen la irreductibilidad de una luxación de cadera al hacer más de un intento de manipulaciones cerradas, la ausencia de una adecuada anestesia y la falta de relajación del paciente. Se presenta el caso de una paciente joven atendida en las primeras ocho horas posteriores al accidente con fractura iatrogénica de la cabeza femoral con seguimiento a 18 meses, haciendo énfasis en la importancia de realizar una reducción anatómica y fijación estable para obtener resultados favorables.

La ingesta insuficiente de frutas y verduras es una causa importante deenfermedades no transmisibles. La Organización Mundial de la Salud estableciócomo objetivo incrementar su consumo. En Argentina se han implementadodiversas iniciativas en el ámbito escolar, como comedores y otras modalidadesde distribución de frutas y verduras, kioscos saludables y huertas escolares.ObjetivosIdentificar la asociación entre las experiencias escolares de promoción delconsumo de frutas y verduras respecto de los factores personales y ambientales,y las prácticas de consumo en los escolares y sus familias.MétodosSe realizaron 84 entrevistas, 16 grupos focales y 2099 cuestionariosestructurados. La muestra quedó conformada por 41 escuelas de 5jurisdicciones: Córdoba, Jujuy, La Plata, Mar del Plata y Rosario.ResultadosEl 5,2% de los niños se aproximan a las cinco porciones por día. Las intervencionesescolares relevadas en pos del consumo de frutas y verduras fueron en generalde baja intensidad. La presencia de dichas intervenciones no ha evidenciadoasociación con la frecuencia de consumo de frutas y verduras, y sólo se encontrórelación de algunos tipos de intervenciones sobre variedad de consumo y factoresintermedios. En el kiosco saludable, la variedad de frutas consumidas superó ala de las demás intervenciones (media: 4,11 frente a 3,82; p<0,03). Los niños querecibieron distribución de frutas como colación tuvieron más intención de consumo,y probaron y prefirieron mayor variedad de frutas. Cuando la participación familiaren la intervención fue protagónica, se registró un mejor resultado.ConclusionesSe observaron asociaciones significativas de los factores familiares respectoa frecuencia de consumo de frutas y verduras e intención de consumo defruta por los niños, pero no de las intervenciones escolares con los factoresfamiliares. Conviene incrementar la intensidad de las intervenciones y mejorarla oferta escolar y la participación de las familias.

Calophyllum brasiliense (Calophyllaceae) is a tropical rain forest tree distributed in Central and South America. It is an important source of tetracyclic dipyrano coumarins (Soulatrolide) and Mammea type coumarins. Soulatrolide is a potent inhibitor of HIV-1 reverse transcriptase and displays activity against Mycobacterium tuberculosis. Meanwhile, Mammea A/BA and A/BB, pure or as a mixture, are highly active against several human leukemia cell lines, Trypanosoma cruzi and Leishmania amazonensis. Nevertheless, there are few studies evaluating their safety profile. In the present work we performed toxicogenomic and toxicological analysis for both type of compounds. Soulatrolide, and the Mammea A/BA + A/BB mixture (2.1) were slightly toxic accordingly to Lorke assay classification (DL50 > 3000 mg/kg). After a short-term administration (100 mg/kg/daily, orally, 1 week) liver toxicogenomic analysis revealed 46 up and 72 downregulated genes for Mammea coumarins, and 665 up and 1077 downregulated genes for Soulatrolide. Gene enrichment analysis identified transcripts involved in drug metabolism for both compounds. In addition, network analysis through protein-protein interactions, tissue evaluation by TUNEL assay, and histological examination revealed no tissue damage on liver, kidney and spleen after treatments. Our results indicate that both type of coumarins displayed a safety profile, supporting their use in further preclinical studies to determine its therapeutic potential.

INTRODUCTION: Renal transplant nephrectomy is a complex process carried out in various situations of renal graft failure. The objective of the study was to describe the experience in our center and to evaluate the safety and efficacy in performing transplantectomies. MATERIAL AND METHODS: This is a retrospective study of 70 transplantectomies performed at our center from 1995 to 2013. Multiple variables related to complications (according to Clavien classification) and mortality as the cause of nephrectomy, surgical technique, or urgent or elective surgery were analyzed. RESULTS: Seventy transplantectomies from a total of 996 transplantations were performed at our center. The mean age was 46.2 years, with 47 men. The most frequent cause of nephrectomy was chronic rejection graft intolerance of 58.5%; 75.7% of the surgical technique was subcapsular. The indication was scheduled at 78.57% (n = 55). Serious complications, defined as Clavien >III (excluded deaths, Clavien V), occurred in 21.43% (n = 15). There were 8 deaths, 6 of which were associated with the clinical severity of patients with urgent indication. The analysis unvariate transplantectomies scheduled group revealed no factor associated with increased morbidity and mortality. CONCLUSIONS: In referral centers, nephrectomy is safe and effective in the treatment of complications of renal graft surgical technique. The increased morbidity and mortality of the procedure depends mainly on the indication and clinical condition of the patient, severity increasing in situations where urgent nephrectomy is performed.

Objetivos: El objetivo de esta publicación es describir de forma retrospectiva los resultados de la colocación de la cinta AMS MiniArc a 4 años de seguimiento como tratamiento de la incontinencia urinaria (IU). Material y método: Presentamos un estudio de cohortes retrospectivo de 135 pacientes, 110 (81,5%) tenían incontinencia urinaria de esfuerzo y 25 (18,5%) incontinencia urinaria mixta. Todos los procedimientos se realizaron con anestesia local y en régimen de cirugía mayor ambulatoria. Las pacientes fueron controladas en consultas externas a los 6 meses (control 1), al año (control 2) y una vez al año (control 3, 4 y 5). Se les realizó una historia clínica y el cuestionario ICIQ-SF, al que le añadimos una pregunta para cuantificar el grado de satisfacción, así como la exploración física. Consideramos curación objetiva que la paciente en la exploración física con vejiga llena presentara un test de esfuerzo negativo. Para valorar la curación subjetiva evaluamos las respuestas al cuestionario ICIQ-SF y la pregunta de satisfacción de los resultados de la cirugía. Para el estudio estadístico de los datos descriptivos y de los resultados se aplicó la comparación de media «t» de Student utilizando el programa SPSS (V 19.0). Resultados: La mediana de seguimiento fue de 59 meses (rango entre 33-72 meses). Observamos que el 86,7% de las pacientes presentaban curación objetiva a los 4 años. Si analizamos los resultados según el tipo de incontinencia que presentaban las pacientes, con incontinencia urinaria mixta estuvieron curadas en el 80,8% y las pacientes con incontinencia urinaria de esfuerzo en el 89,2%. Con el cuestionario ICIQ-SF y la pregunta de satisfacción observamos un descenso medio en la puntuación de 12,7 puntos, con un 85,7% de las pacientes muy satisfechas. Conclusión: La colocación de AMS MiniArcs supone un dispositivo eficaz para el tratamiento quirúrgico de la incontinencia urinaria femenina a medio plazo, pero los resultados deben interpretarse con precaución dadas las limitaciones del estudio

Material and methods: We present a retrospective cohort study of 135 patients, 110 (81.5%) had urinary incontinence and 25 (18.5%) mixed urinary incontinence. All these procedures were performed with local anesthesia and in «out patient surgery». Patients were monitored in the outpatient clinic at 6 months (control 1), one year (control 2) and annually (control 3, 4, and 5). During the following up, clinical history was made in every woman with ICIQ-SF questionnaire, that included a fourth question to evaluate the degree of satisfaction after surgery, as well as physical examination. We considered objective cure when negative stress with full bladder. We use the SPSS program (V19.0) for statiscal analysis of the results. Results: The mean follow-up was of 59 months (range from 33-72 months). When evaluating the success rate of anti-incontinence surgery,86.7% of patients showed objective cured (80.8% with MUI and 89.2% with SUI). The ICIQ-SF decreased average of 12.7 points, 85.7% patients were very or fairly satisfied. Conclusion: The AMS MiniArc is an optim anti-incontinence procedure a medium term. But the results should be interpreted with caution given the limitations of the study

A number of recent reports emphasize the risk of zoonotic diseases and the high degree of prevalence of asymptomatic animals infected with Leptospira interrogans. This report sought to assess the prevalence of antibodies to certain serovars of L. interrogans, and to describe the association between seropositivity and risk factors associated with within-flock transmission in a mountainous region of Mexico. Overall seroprevalence to L. interrogans was 54·5% (95% confidence interval 48·3-60·7); the most frequent serovar was Icterohaemorrhagiae. The accumulation of placentas and fetuses at a site close to lambing paddocks can play a significant role as a risk factor for within-flock transmission of L. interrogans in transhumant farming systems in the municipality of Xalatlaco. The high prevalence of L. interrogans antibodies supports the hypothesis that natural foci of this zoonosis are present in sheep flocks in this area. These findings emphasize the need for planning and implementation of control programmes for ovine leptospirosis in Mexico and elsewhere.

The aim of this study was to determine the influence of long-term docosahexaenoic acid (DHA) dietary supplementation on the erythrocyte fatty acid profile and oxidative balance in soccer players after training and acute exercise. Fifteen volunteer male athletes (age 20.0 ± 0.5 years) were randomly assigned to a placebo group that consumed an almond-based beverage (n = 6), or to an experimental group that consumed the same beverage enriched with DHA (n = 9) for 8 weeks. Blood samples were taken in resting conditions at the beginning and after 8 weeks of nutritional intervention and training in resting and in post-exercise conditions. Oxidative damage markers (malonyldialdehyde, carbonyl and nitrotyrosine indexes) and the activity and protein level of antioxidant enzymes (catalase, superoxide dismutase, glutathione reductase and peroxidase) were assessed. The results showed that training increased antioxidant enzyme activities in erythrocytes. The experimental beverage increased DHA from 34.0 ± 3.6 to 43.0 ± 3.6 nmol/10(9) erythrocytes. DHA supplementation increased the catalytic activity of superoxide dismutase from 1.48 ± 0.40 to 10.5 ± 0.35 pkat/10(9) erythrocytes, and brought about a reduction in peroxidative damage induced by training or exercise. In conclusion, dietary supplementation with DHA changed the erythrocyte membrane composition, provided antioxidant defense and reduced protein peroxidative damage in the red blood cells of professional athletes after an 8-week training season and acute exercise.

INTRODUCCIÓN Y OBJETIVOS: La dermatomiositis del adulto es paraneoplásica hasta en una cuarta parte de los casos. Hasta la fecha no existe ningún rasgo clínico, histológico o analítico absolutamente específico de paraneoplasia en dermatomiositis. Además, los estudios sobre dermatomiositis del adulto no suelen hacer referencia a la frecuencia de aparición de los distintos signos cutáneos propios de la dermatomiositis en los casos asociados a cáncer. Por todo ello decidimos revisar nuestros casos de dermatomiositis paraneoplásicas. MATERIAL Y MÉTODOS: Estudiamos 12 casos de dermatomiositis paraneoplásicas en los cuales se recogió la edad, el sexo, el cáncer asociado, el tiempo entre el inicio de la dermatomiositis y el cáncer, 9 signos cutáneos, afectación muscular, disfagia, enfermedad pulmonar, niveles de creatinfosfocinasa y de autoanticuerpos circulantes. RESULTADOS: La media de edad fue de 61 años y los 2 cánceres asociados más frecuentes fueron el de ovario y el de vejiga. El tiempo medio transcurrido entre el diagnóstico del cáncer y el de la dermatomiositis fue de 7 meses, y en la mayoría el diagnóstico de cáncer precedió al de dermatomiositis. Siete pacientes tuvieron dermatomiositis amiopática. De los signos cutáneos, los más frecuentes fueron una erupción violácea fotodistribuida que respetaba la región interescapular y el rash heliotropo, seguidos de las pápulas de Gottron y la afectación de cutículas. Se encontró necrosis cutánea superficial en 3 casos. Los autoanticuerpos específicos de miositis resultaron negativos en los 6 casos estudiados. CONCLUSIONES: La dermatomiositis paraneoplásica es muchas veces amiopática. No existe ningún rasgo cutáneo específico de paraneoplasia en la dermatomiositis. Los anticuerpos específicos de miositis no se asocian a la dermatomiositis paraneoplásica


INTRODUCTION AND OBJECTIVES: Adult dermatomyositis presents as a paraneoplastic syndrome in up to 25% of cases, but no clinical, histologic, or laboratory markers completely specific for paraneoplastic disease in dermatomyositis have been identified to date. Furthermore, studies on adult dermatomyositis do not usually report the frequency of cutaneous features of dermatomyositis in patients with associated cancer. Our aim was to review the characteristics of paraneoplastic dermatomyositis in patients seen at our hospital. MATERIAL AND METHODS: We studied 12 cases of paraneoplastic dermatomyositis and recorded patient age and sex, associated cancer, time between onset of dermatomyositis and cancer, emergent cutaneous manifestations, muscle involvement, dysphagia, lung disease, and levels of creatine phosphokinase and circulating autoantibodies. RESULTS: The mean age of the patients was 61 years and the 2 most common malignancies were ovarian cancer and bladder cancer. The mean time between the diagnosis of cancer and dermatomyositis was 7 months and in most cases, the cancer was diagnosed first. Seven patients had amyopathic dermatomyositis. The most common cutaneous signs were a violaceous photodistributed rash sparing the interscapular area and a heliotrope rash, followed by Gottron papules and cuticle involvement. Superficial cutaneous necrosis was observed in 3 cases. Myositis-specific autoantibodies were not detected in any of the 6 patients who underwent this test. CONCLUSIONS: Paraneoplastic dermatomyositis is often amyopathic. There are no specific cutaneous markers for malignancy in dermatomyositis. Myositis-specific antibodies are not associated with paraneoplastic dermatomyositis
