BACKGROUND: Urinary tract infections (UTIs) are one of the most common health problems worldwide and mainly affect women. This study aimed to evaluate the prevalence of UTIs in pregnant women and determine the antimicrobial resistance patterns of bacterial pathogens isolated from pregnant and nonpregnant women in Riyadh, Saudi Arabia. METHODS: This retrospective cohort study was conducted at an academic medical center in Riyadh, Saudi Arabia, from January to June 2022. The study included all urine cultures performed for adult women during the study period. We excluded urine culture performed for women on antibiotics prescribed for any infection, children, and men. Using the SPSS (version 27) package, descriptive statistics and chi-square tests were used to analyze the data, and p < 0.05 was considered to indicate statistical significance. RESULTS: A total of 2,418 urine cultures performed during the study period were included (985 and 1,433 for pregnant and nonpregnant women, respectively). The overall prevalence of UTIs in pregnant women was 5% (95% CI 3.6-6.4); 10 (1%) women were symptomatic, and 40 (4%) women were asymptomatic. Of the entire cohort, 244 (10.1%) women were diagnosed with UTIs based on bacterial cultures. The predominant bacteria in both pregnant and nonpregnant women were Escherichia coli (134, 54.9%), followed by Klebsiella pneumoniae (48, 19.6%). The antibiotic susceptibility criteria for Escherichia coli and Klebsiella pneumoniae were as follows: nitrofurantoin (94% and 18.8%, respectively), amoxicillin-clavulanic acid (82.8% and 70.8%, respectively), ciprofloxacin (65.7% and 83.3%, respectively), trimethoprim-sulfamethoxazole (65.7% and 79.2%, respectively) and cephalothin (47% and 68.8%, respectively). CONCLUSION: Compared to the findings of other similar studies, the prevalence of UTIs was lower in pregnant women. This may be because the patient population was composed of healthy and educated women who received prenatal education and underwent prenatal assessment as per institutional guidelines. Nitrofurantoin and amoxicillin-clavulanic acid are recommended for use as an empirical therapy for UTIs in pregnant and nonpregnant women because bacteria have the least amount of resistance to these drugs.

Blood transfusion-requiring diseases such as sickle cell anemia and thalassemia are characterized by an imbalance between iron intake and excretion, resulting in an iron overload (IOL) disorder. Hepatotoxicity is prevalent under the IOL disorder because of the associated hepatocellular redox and inflammatory perturbation. The current work was devoted to investigate the potential protection against the IOL-associated hepatotoxicity using chrysin, a naturally-occurring flavone. IOL model was created in male Wistar rats by intraperitoneal injection of 100 mg/kg elemental iron subdivided on five equal injections; one injection was applied every other day over ten days. Chrysin was administered in a daily dose of 50 mg/kg over the ten-day iron treatment period. On day eleven, blood and liver samples were collected and subjected to histopathological, biochemical, and molecular investigations. Chrysin suppressed the IOL-induced hepatocellular damage as revealed by decreased serum activity of the intracellular liver enzymes and improved liver histological picture. Oxidative damage biomarkers, and pro-inflammatory cytokines were significantly suppressed. Mechanistically, the levels of the redox and inflammation-controlling proteins SIRT1 and PPARγ were efficiently up-regulated. The liver iron load, NLRP3 inflammasome activation, and NF-κB acetylation and nuclear shift were significantly suppressed in the iron-intoxicated rats. Equally important, the level of the antioxidant protein Nrf2 and its target HO-1 were up-regulated. In addition, chrysin significantly ameliorated the IOL-induced apoptosis as indicated by reduction in caspase-3 activity and modulation of BAX and Bcl2 protein abundance. Together, these findings highlight the alleviating activity of chrysin against the IOL-associated hepatotoxicity and shed light on the role of SIRT1, NLRP3 inflammasome, and Nrf2 signaling as potential contributing molecular mechanisms.

BACKGROUND: Smoking is a significant cause of mortality and is strongly associated with the development of morbid diseases, such as obesity. There is a considerable interest in understanding the impact of smoking on body weight. The exact nature remains unclear due to the mixed results observed in the literature review. The aim of this study is to evaluate the association between smoking and body weight concerning demographic characteristics in the western region of Saudi Arabia. METHODS: This cross-sectional study was carried out in the western region of Saudi Arabia from November 15, 2022 to July 15, 2023. To collect data, an online survey was utilized, and the survey link was shared across various social media platforms. The survey was completed by a total of 744 individuals who were at least 18 years old and answered a self-reported questionnaire on the web. RESULTS: Analysis showed that (39%) of participants lead a sedentary lifestyle, and (58.8%) have an abnormal BMI. Among them approximately 25% are overweight, 12.4% are obese, and 7.4% are extremely obese. Moreover, a greater number of them were traditional smokers. Our study reported that individuals who had quit smoking were more likely to be overweight. Regarding self-perception of their weight, almost half of the participants consider themselves overweight and they're more likely to smoke, whether it be traditional or electronic cigarettes. CONCLUSION: This study revealed a strong link between smoking habits and increased weight status, as measured by body mass indexes. These findings have important implications for public health interventions aimed at reducing smoking rates and obesity levels. By recognizing the potential connection between these two risk factors, experts in public health can develop more effective strategies to promote healthy behaviors and prevent chronic diseases among young individuals.

PURPOSE: To evaluate the effect of printing orientation combined with different postcuring times on the flexural strength of 3D-printed resins. MATERIALS AND METHODS: A total of 480 rectangular specimens with the dimensions of 64×10×3.3 mm were designed and fabricated from two 3D-printed acrylic resins and one heat-polymerized resin (HP). 3D-printed groups were divided into 3 groups according to printing orientations (0-, 45-, 90-degree); each group was subdivided into 4 groups according to postcuring time (30, 60, 90, 120 min.). All specimens were subjected to thermal cycling (10,000 cycles) before testing flexural strength. Fractured surfaces were examined under a scanning electron microscope (SEM). ANOVA and Tukey's post hoc tests were used for data analysis (α = 0.05). RESULTS: The result of this study showed that the highest flexural strength values of 3D-printed resin (NextDent, and ASIGA) were in 0-degree groups. Also, the flexural strength values increased when postcuring time was increased, regardless of the printing orientation; the highest flexural strength was recorded at 120 minutes postcuring time in all orientations. SEM analysis showed a rougher surface with irregular lamellae which represented a ductile fracture confirming that high energy is required for crack propagation and these features markedly increased as postcuring time increased. CONCLUSION: The results showed that the 0-degree orientation groups showed higher flexural strength compared to other groups. Similarly, with increased postcuring time, the flexural strength increased.

The effect of printing parameters on the surface characteristics of three-dimensional (3D)-printed denture base resins (DBRs) is neglected. Therefore, this study investigated the effect of printing orientation and post-curing time on the surface roughness and hardness. One conventional heat-polymerized (HP) resin and two 3D-printing resins (NextDent (ND) and ASIGA (AS)) were used to fabricate a total of 250-disc (10 × 2.5 mm) specimens. ND and AS specimens were printed with different orientations (0-, 45-, and 90-degree) and each orientation group was subjected to four post-curing times (30, 60, 90, 120 min). Printed specimens were thermo-cycled (10,000 cycles) followed by the measuring of surface roughness (Profilometer (Ra)) and hardness (a Vickers hardness (VH)). ANOVA and post hoc tests were used for data analysis (α = 0.05) at significant levels. AS and ND showed no significant changes in Ra when compared with HP (p ˃ 0.05), except the 45-degree orientation (AS/90 min and AS/120 min) significantly increased surface roughness (p ˂ 0.001). There was no significant difference in Ra with different orientations and post-curing time for both materials AS and ND (p ˃ 0.05). Compared with HP, 3D-printed DBRs showed low VH values (p ˂ 0.001). For AS, 90-degree orientation showed a significant decrease in VH at 60, 90, and 120 min when compared with 0- and 45-degree orientation (p ˂ 0.001), while ND showed no significant difference in VH with different printing orientations (p ˃ 0.05). The VH of AS and ND improved when increasing post-curing time to 120 min (p ˂ 0.001), and the printing orientations and post-curing time did not affect the Ra of 3D-printed DBRs.

BACKGROUND: The Coronavirus Disease of 2019 (COVID-19) pandemic is a worldwide global public health threat. Although acceptance of COVID-19 vaccination will be a critical step in combating the pandemic, achieving high uptake will be difficult, and potentially made more difficult by social media misinformation. This study aimed to examine the association between social media use and acceptance of receiving COVID-19 vaccine among the general population in Saudi Arabia. METHODOLOGY: A cross-sectional study was conducted from June 17 to June 19, 2021 among 504 participants of the general population in Saudi Arabia. The data were collected using a three-part online questionnaire (sociodemographic characteristics, medical and vaccination history, pattern of social media use). RESULTS: Among 504 participants who completed the survey, 477 participants were acceptant of the vaccine and 27 were non-accepting. A total of 335 individuals had already received the vaccine, 142 were willing to receive the vaccine and 27 were unwilling. One hundred and thirty participants denied using social media for COVID-19 news. Four factors were found to be significant in influencing vaccine acceptance in univariate analysis: having a chronic condition (odds ratio (OR) = 0.367, P = 0.019), believing that infertility is a side effect of the COVID-19 vaccine (OR = 0.298, P = 0.009), being concerned about a serious side effect from the vaccine (somewhat concerned: OR = 0.294, P = 0.022, very concerned: OR = 0.017, P < 0.0001), and basing the decision to be vaccinated on social media information (OR = 0.260, P = 0.006). Two of these factors fell away on multivariate analysis: basing the decision on social media information (OR = 0.356, P = 0.071), and a belief that vaccination causes infertility (OR = 0.0333, P = 0.054), suggesting that the associations are dependent on other factors. CONCLUSION: In conclusion, there was no significant independent relationship between social media usage and people's willingness to receive a COVID-19 vaccination. Further studies to explore the association between social media use and vaccine decisions are required to generalize this observation to the Saudi population.

BACKGROUND: Wearing face masks has been an essential part of healthcare workers' lives since the coronavirus disease 2019 (COVID-19) pandemic. This study aims to determine the association between prolonged face mask-wearing and dry eye disorder (DED) among female nurses. METHODS: An online questionnaire-based cross-sectional study was conducted at King Abdulaziz University Hospital, Jeddah, Saudi Arabia, between May 2021 and February 2022. It covered sociodemographic data, conditions associated with ocular irritation, and questions related to mask-wearing duration. The Ocular Surface Disease Index (OSDI) survey was used to measure DED severity. Binary logistic regression analysis was done and Odd's ratios (OR) with 95% confidence intervals (CI) were reported. RESULTS: A total of 266 female nurses responded to this study. The majority of the sample (71.1%) fell in the normal-mild DED category (OSDI 0-22), while (28.9%) were categorized as the moderate-severe DED category (OSDI >22). We found a significant independent association of dry eye disorder with wearing a mask for >6 hours/day (OR 2.066, 95% CI: 1.083-3.944). Other significant predictors of DED in this study were wearing corrective eyeglasses (OR 2.382, 95% CI: 1.296-4.376) and having rheumatoid arthritis (OR 17.289, 95% CI: 1.794-166.7). CONCLUSION: Wearing a face mask for > 6 hours/day was significantly associated with moderate to severe DED among female nursing staff. Ophthalmologists should be aware of this adverse effect in order to promote ways to relieve this condition.

OBJECTIVE: Among other regulatory functions, vitamin D has a role in modulating the inflammatory process of periodontal disease. Therefore, this retrospective study aimed to assess the relationship between vitamin D levels and periodontal health in dental patients from the Eastern Province of Saudi Arabia. METHODS: Radiographs and serum vitamin D levels of patients seeking dental treatment were collected. Exclusion criteria were systemic disease, smoking, recent vitamin D supplementation, and previous periodontal surgery. Gender, age, and alveolar crest height (ACH) were recorded. A total of 67 patients were categorized into three groups according to their serum vitamin D level (<10, <20, and > 20 ng/mL) and their bone loss compared. RESULTS: Differences in means were compared by t-test. ANOVA was used to compare vitamin D groups and the corresponding ACH, as well as the correlation (p < .05). Patients with vitamin D levels > 20 ng/mL demonstrated a mean ACH of 1.6 mm. The mean ACH was 3.1 mm for those with vitamin D levels < 20 ng/mL, and 4.6 mm for vitamin D levels < 10 ng/mL. A weak negative correlation was found between vitamin D and ACH in all groups (r = -0.055, p = .7). CONCLUSION: Serum vitamin D level seems to be an important factor that influences oral health, especially the periodontal condition, of both male and female patients.

OBJECTIVE: To evaluate empathy and its related factors among undergraduate dental students and interns enrolled in a public dental college in Dammam, Saudi Arabia. MATERIALS AND METHODS: This cross-sectional study used the Jefferson Scale of Empathy-Health Profession Students (JSE-HPS) version to determine empathy in 362 dental students and interns in Dammam, Saudi Arabia. The JSE-HPS is a 20-item 7-point Likert scale questionnaire, and its score ranges from 20 to 140 with high values indicating increased empathy. Influences of age, gender, class year, previous year's grade point average (GPA), educational attainment of parents, and monthly family income on empathy were evaluated. RESULTS: Of 501 enrolled students and interns, 362 returned completed questionnaires, and the response rate of the study was 72%. The sample's empathy score (JSPE-HPS scale) ranged from 70 to 129 with a mean of 96.75 (±13.76). Most participants believed that empathy is important for effective communication with patients (96.1%) and can improve the provider-patient relationship (95.6%). Females demonstrated a significantly higher mean empathy score (99.98 ± 14.01) than males (92.72 ± 12.35) (P < 0.001). Similarly, the participants with high GPA (98.06 ± 13.69) had significantly greater mean empathy scores than those with low GPA (94.84 ± 13.68) (P=0.029). The mean empathy score increased significantly from junior students (3rd and 4th year students) to senior students (5th and 6th year students) and interns (P=0.008). Multiple linear regression analysis showed that class year (B = 2.03, P=0.006) and GPA (B = 8.67, P=0.003) were significant factors associated with empathy. CONCLUSIONS: Empathy is important for effective patient communication and improved provider-patient relationship. Female gender, high GPA, and class years were associated with empathy. Empathy should be integrated into dental curricula for effective student learning and positive patient care outcomes.

RESUMEN

Surgical clip migration into the common bile duct with subsequent stone formation is a rare complication following laparoscopic cholecystectomy. Very few cases have been reported in the literature. We report a case of bile duct stone formation around a migrated surgical clip 16 years after laparoscopic cholecystectomy. The patient presented with right upper quadrant pain, fever, and chills for one week. Investigation with abdominal ultrasound showed dilatation of the common bile duct and moderate dilatation of the intrahepatic bile ducts. The diagnosis was confirmed by endoscopic retrograde cholangiopancreatography and the patient was managed successfully with sphincterotomy and stone extraction. The exact mechanism of clip migration is not fully understood. Presenting symptoms are similar to non-clip-induced choledocholithiasis. Time of presentation can vary significantly with an average of 26 months. Most cases reported in the literature required surgical intervention. Clip migration should be considered in the differential diagnosis of postcholecystectomy biliary colic and cholangitis. Management with endoscopic retrograde cholangiopancreatography is the treatment of choice.
