O estudo objetiva avaliar a influência das alterações fonoaudiológicas nos aspectos sociais e psicológicos da pessoa idosa. Trata-se de um estudo analítico, descritivo, observacional do tipo transversal, quantitativo. Foi realizada na Clínica Escola da Universidade CEUMA (Clínica Ana Lúcia Chaves Fecury), na cidade de São Luís, no estado do Maranhão. A amostra teve 20 idosos acima 60 anos de idade, sendo 10 do sexo masculino e 10 do sexo feminino. A coleta aconteceu entre os meses de fevereiro a março de 2020. Utilizou-se um questionário semiestruturado que investigou os seguintes aspectos: comprometimento auditivo, vocal, deglutição, aspectos sociais e comprometimento da linguagem. Como resultados: 60% (13) sente falta de memória, 65% (13) esquecimento, 35% (7) repetição de palavras, 45% (9) esquece o nome de objetos e 10% (2) tem constrangimento por conta de sua fala; 38% (6) sensação de comida presa na garganta, 34% (5) tosse durante as refeições e 28% (4) vergonha de se alimentar em locais públicos. As condições estudadas afetam a qualidade de vida das pessoas idosas, principalmente em relação às condições psicológicas e sociais.

Social Anxiety Disorder (SAD) has its usual onset during adolescence when it is a highly prevalent and debilitating condition. Evidence regarding the processes that underline social anxiety and SAD is not compelling, especially in adolescents. Within an Acceptance and Commitment Therapy (ACT) framework, the causal role of ACT processes on adolescents' social anxiety and how these processes contribute to sustain social anxiety over time is still unknown. Hence, this study explored the role of psychological inflexibility (PI) and acceptance and committed action (as psychological flexibility processes) on social anxiety over time, in a clinical sample of adolescents. Twenty-one adolescents (Mage = 16.19, SD = 0.750) with a primary diagnosis of SAD completed a set of self-report measures assessing PI, acceptance (i.e., willingness to experience social anxiety symptoms), action (i.e., moving towards valued life directions despite social anxiety symptoms) and social anxiety. Path analysis was used to investigate a mediation model linking acceptance, committed action, and PI to social anxiety, directly and indirectly. Findings revealed that acceptance and action were negatively and directly associated with PI after 10-weeks. In turn, PI yielded a positive and direct effect on social anxiety after another 12-weeks. PI totally mediated the relation between acceptance and action and social anxiety, with significant indirect effects. Overall, findings offer evidence for the applicability of the ACT model to adolescent SAD and support the use of clinical interventions targeting PI to understand and alleviate adolescents' social anxiety.

Cashew tree gum is a polysaccharide material highly available in the Northeast region of Brazil. It has been explored for biocompatibility with human tissues. This research aimed to describe the synthesis and characterization of cashew gum/hydroxyapatite scaffold and evaluate the possible cytotoxicity in murine adipose-derived stem cells (ADSCs) cultures. ADSCs of the subcutaneous fat tissue of Wistar rats were collected, isolated, expanded, differentiated into three strains, and characterized immunophenotypically. The scaffolds were synthesized through chemical precipitation, lyophilized and characterized through scanning electron microscopy (SEM), infrared spectroscopy (FTIR), X-ray diffraction (XRD), thermal analysis (TG and DTG), and mechanical testing. The scaffold presented a crystalline structure and pores with an average diameter of 94.45 ± 50.57 µm. By mechanical tests, the compressive force and modulus of elasticity were like the cancellous bone. The isolated adipose-derived stem cells (ADSCs) presented fibroblast morphology, adhesion capacity to plastic, differentiation in osteogenic, adipogenic and chondrogenic lineages, positive expression for the CD105 and CD90 markers and negative expression for the CD45 and CD14 markers. The MTT test showed increased cell viability, and the biomaterial showed a high level of hemocompatibility (<5 %). This study allowed the development of a new scaffold for future surgical applicability in tissue regeneration.

BACKGROUND/OBJECTIVES: Low-dose X-ray radiotherapy to treat tinea capitis during childhood is a well-known risk factor for scalp basal cell carcinomas (BCCs). Post-radiotherapy BCCs are often multiple, and it has been suggested that they display more aggressive features. Our main objective was to study the clinicopathological aspects of post-radiotherapy BCCs to evaluate their biological behaviour and identify features that may differ from other BCCs. METHODS: We performed an observational, retrospective study assessing multiple clinical and pathological characteristics of patients with post-radiotherapy BCCs. RESULTS: We studied 96 patients with 427 post-radiotherapy scalp BCCs. Post-radiotherapy BCCs were often multiple (median of 4 lesions/patient, ranging from 1 to 54). Significant comorbidities included a high incidence of thyroid disease and meningiomas. Recurrences were observed in 23% of patients, but there may be confounding factors, such as referral bias, heterogenous treatment modalities and occurrence of new tumours due to field effect. We found a high incidence of infundibulocystic BCCs (in 14.6% of patients and corresponding to 5.4% of the total number of tumours), trichoblastomas (5.2%) and neurofibromas of the scalp (10%). CONCLUSIONS: This study is consistent with the occurrence of multiple lesions (sometimes numerous) and a relatively high tendency for recurrence in post-radiotherapy BCCs, as suggested by previous studies. We also found a high incidence of the infundibulocystic variant and a higher risk of follicular tumours and neurofibromas, which suggests that radiotherapy may influence the type of differentiation of BCCs and contribute to induce neoplasms of different cell lines.

ABSTRACT: Patients submitted to radiotherapy for tinea capitis in childhood have an increased incidence of scalp basal cell carcinomas (BCCs) but also of other neoplasms, namely, follicular tumors. In a cohort of such patients, we also found a high incidence of infundibulocystic BCCs, an otherwise rare variant. We thus hypothesized that postradiotherapy BCCs could be more prone to display follicular differentiation. We compared the histological and immunohistochemical features of postradiotherapy BCCs [both conventional (16 cases) and infundibulocystic (16 cases)] with those of BCCs arising in sun-exposed areas (16 cases), using markers of follicular differentiation (PHLDA-1, CK15, CD34, ß-catenin, and calretinin). Postradiotherapy BCCs showed slightly higher tendency for infundibular and/or trichilemmal differentiation than BCCs from sun-exposed areas (37.5% vs. 18.8%), but this difference was not statistically significant. Nevertheless, infundibulocystic BCCs showed more frequent expression of PHLDA-1 and stronger cytoplasmic expression of CK15 compared with the other lesions. In addition, CD34 highlighted a characteristic meshwork of stromal cells surrounding the epithelial component in all infundibulocystic BCCs, in contrast to the other postradiotherapy BCCs and UV-related BCCs, in which 78.1% were negative or only focally positive. In conclusion, our study suggests a tendency for more frequent follicular differentiation in postradiotherapy BCCs compared with BCCs from sun-exposed areas. In addition, the immunohistochemical study confirms previous data from the literature regarding infundibulocystic BCCs (higher CK15 and PHLDA-1 expression) and shows a distinctive stromal positivity for CD34 that has not been previously acknowledged in these tumors.

Social fears arise when fearing to be judged in social events. When these fears are intense, persistent, and debilitating, the individual may suffer from social anxiety disorder (SAD), which has its most frequent onset during adolescence and tends to be chronic. Still, evidence on the prevalence of social fears and SAD in adolescence is scarce. This study analyzed the prevalence of social fears and of SAD in Portuguese adolescents. Of the initial sample (n = 1495), 26% presented with intense self-reported social fears. Of those, 53.9% accepted to be further assessed for diagnosis, resulting in a point-estimate prevalence of adolescent SAD of 9.4%; this is slightly higher than previously found. Social performance was the most feared social event. Of the adolescents with SAD, 12.9% were receiving psychological intervention, 12.1% refused intervention, and 92 (65.7%) accepted intervention. Findings confirm SAD as a highly prevalent mental disorder among adolescents, particularly girls, and additionally, that most of these adolescents did not seek treatment but are willing to receive help if made available. Hence, schools should be invested not only in identifying vulnerable adolescents but also in providing diverse intervention options, tailored to their needs, and directing them to successful developmental trajectories.

RESUMEN

Diffuse dermal angiomatosis is a rare, benign, reactive cutaneous vascular proliferation that has been reported in the context of end-stage renal failure and can rarely be associated with arteriovenous fistulas. We report a striking clinical resolution following prompt diagnosis and subsequent arteriovenous fistula reversal. This case further demonstrates that accurate diagnosis is particularly rewarding since correct therapeutic approach can be curative.

Tumoral melanosis is a rare histopathological finding characterized by aggregates of melanophages, in the absence of melanocytes, usually observed in sites of regressed melanocytic lesions, including melanoma. A 72-year-old woman with a history of a completely excised melanoma on her right arm (T3bN0M0, Stage IIb) presented with clinically-evident regional lymph node metastasis. This was treated with right axillary lymphadenectomy. Subsequently, a 2-centimeter blue-colored patch over the excision scar was identified, along with a blue nodule within the posterior aspect of the same arm, consistent with in-transit metastases. Additional metastases on the right hilar region of the lungs were detected by PET/CT. Hence, the patient began immunotherapy with pembrolizumab. After three months, a second PET/CT revealed a complete response, but the patient maintained the blue-colored patch previously observed. Given the discrepancy between the clinical and metabolic response she underwent a skin biopsy; histological examination showed findings compatible with tumoral melanosis resulting from complete regression of a metastatic lesion. In cases of metastatic melanoma under immunotherapy with anti-PD1 agents, especially pembrolizumab, tumoral melanosis has been anecdotally associated with tumor regression and favorable treatment response. The patient has been maintained on pembrolizumab, accomplishing 15 cycles, and has had a complete response to date.

AIMS: To analyze the association between demographic, socioeconomic, clinical, epidemiological, and primary healthcare factors with the severity of lower limb amputations (LLAs) in individuals with type II diabetes mellitus (DM-II) at a reference hospital in Fortaleza, Ceará, in Northeast Brazil. METHODS: A cross-sectional study was performed with a representative sample of individuals hospitalized with DM-II and the degree of LLA severity: (1) toes; (2) transmetatarsal or infrapatellar; (3) suprapatellar; (4) disarticulation or bilateral. Potentially associated factors with the outcome degree of amputation severity were identified in a semi-structured evaluation during hospitalization. The prevalence ratios of the degree of amputation severity as a function of associated factors were calculated with robust variance Poisson regression models. RESULTS: The prevalence of high degree of severity in amputations (suprapatellar, with disarticulation or bilateral) was high in the total sample of 385 patients, revealing to be 49% (187/385). Prevalence ratios (PR) indicated a higher prevalence of DM-II amputation severity in patients who lacked of specific guidance on DM-II amputation in primary care (PR = 1.52, 95% CI: 1.05-2.21). CONCLUSIONS: LLAs in DM-II were associated with age above 67 years, male gender, cardiovascular disease, and low support for guidance at the primary healthcare level.

Endometriosis is a chronic disorder characterized by the presence of ectopic functional endometrial tissue outside the uterine cavity and which affects about 10% of all women of reproductive age. Cutaneous endometriosis, usually localized in the abdominal wall, is a rare entity. We report the case of a nulliparous woman with an acute painful umbilical nodule that slowly increased in size during the days prior to observation. The lesion was surrounded by a painful periumbilical subcutaneous plaque. The patient denied associated constitutional symptoms and reported two similar episodes in the previous two years. A skin biopsy was performed, and histopathologic examination was consistent with the diagnosis of endometriosis. This article reviews this disease particularly rare in nulliparous and highlights the importance of recognizing this clinical presentation of endometriosis due to its frequent association with pelvic disease that can lead to reduced fertility.


A endometriose é uma doença crónica caracterizada pela presença de endométrio funcional fora da cavidade uterina que afeta aproximadamente 10% das mulheres em idade reprodutiva. A endometriose cutânea com localização habitual na parede abdominal é uma manifestação rara da doença. Apresenta-se o caso de uma mulher jovem nulípara com um nódulo umbilical doloroso e de crescimento recente com placa dolorosa subcutânea periumbilical, sem outra sintomatologia associada. A anamnese revelou dois episódios semelhantes nos últimos dois anos. Foi realizada biópsia cutânea da lesão e o exame anatomopatológico foi compatível com endometriose. Os autores fazem uma revisão desta entidade clínica e realçam a importância do reconhecimento desta forma de apresentação, rara em nulíparas, dada a associação frequente a focos de endometriose noutras localizações e que podem condicionar um baixo nível de fertilidade.

Hereditary angioedema (HAE) is a rare group of genetic disease characterized by non-itchy swelling of subcutaneous and submucosal tissues of the extremities, genitalia, gastrointestinal tract, and upper airways, which can be life threatening. Moreover, unpredictability and recurrence of HAE attacks significantly affect patients' quality of life. Short- and long-term prophylaxis is used to decrease the severity and frequency of attacks, but during severe or potentially severe acute episodes, treatment with C1-INH replacement or icatibant is mandatory. Icatibant is a selective bradykinin B2 receptor antagonist that has been licensed for self-administration at home, resulting in earlier treatment of the attack and quicker recovery, less emergency admittance with a significant improvement of patients' quality of life, and decrease of health care costs. The authors present a case of a young woman, affected by Type I HAE, who has been successfully treated with icatibant on demand at home, resulting in reduction of emergency admissions and improvement of quality of life. The authors also review the different types HAE, their clinical aspects, diagnosis, and management.

Skin lesions occur, often at very early stages, in many of the most frequent inflammatory rheumatic diseases such as in systemic lupus erythematosus (SLE), dermatomyositis (DM), systemic sclerosis (SSc), Sjögren's syndrome, rheumatoid arthritis (RA), and psoriatic arthritis. It is important to recognize the different specific cutaneous lesions in SLE (e.g., "butterfly" rash in acute, annular or psoriasiform photosensitive lesions in the subacute form, and discoid lesions in the chronic form) for an early diagnosis and to estimate the associated risks of internal disease, whereas nonspecific lesions (exanthema, vasculitis, and alopecia) can be part of SLE flares. Cutaneous lesions in DM (Gottron's papules and sign, heliotrope rash, dystrophic cuticles, and nailfold capillary abnormalities) may occur before any clinically evident muscular or systemic organ involvement and are of utmost importance for early diagnosis. The pattern of cutaneous lesions and associated autoantibodies also allow the distinction of different phenotypes, either more prone to life-threatening interstitial lung disease (MDA-5) or with higher risk for neoplasia (TIF1-γ). Many other skin lesions, although not specific, require further investigation to look for a possible underlying inflammatory rheumatic disease: non-pruritic urticarial lesions in anti-C1q-associated urticarial vasculitis, Still's disease or hereditary auto-inflammatory syndromes, transient macular purpura of vasculitis in Sjögren's syndrome, Behçet's disease, or RA, Raynaud's phenomenon in SSc and mixed connective tissue disease, erythema nodosum or other panniculitis in RA, Behçet's disease and SLE, pustular eruptions in Behçet's disease, psoriasis, and hereditary auto-inflammatory syndromes. After reviewing in detail the cutaneous manifestations of the most frequent inflammatory rheumatic diseases, we describe a topographic and morphological approach to skin rashes, calling attention to facial rashes, hand involvement, scalp, nail, or leg lesions or to some morphological aspects of skin lesions (annular, pustular, urticarial, or exanthematous) that may be the initial manifestations of inflammatory rheumatic diseases. The importance of skin lesions is confirmed by their presence as part of the classification criteria of many inflammatory rheumatic diseases. They also contribute to early diagnosis, to characterize disease phenotypes, to aid in effective patient management, and, ultimately, to impact on disease prognosis.

OBJECTIVE: To understand the importance of group strategy in the process of nursing care to high-risk pregnant women. METHODS: Qualitative and descriptive study conducted at Assis Chateaubriand Teaching Maternity Hospital in February-March 2017, through focus groups with 24 hospitalized pregnant women. Data was submitted to content analysis and the relevant ideas of the discourses were extracted, forming categories. RESULTS: Three thematic categories were constructed: Interactive groups as a space of bond and coexistence, Educational strategy as an approach to good delivery and birthcare practices, Impact of group experience in strengthening the connection between the health team and the pregnant woman. The group of pregnant women is a space of coexistence and bonding that stimulates them to share knowledge and experiences, re-signifying links. The groups promote sharing of experiences, learning and reflection on the possibilities and limitations of the health-disease process, reducing anxiety and contributing to empowerment in decision making. CONCLUSION: The considerations and suggestions of the pregnant women guided the nurses in the development of the groups, which became strategic spaces of care, acquisition of knowledge and establishment of trust relationships with the nurses, with a consequent impact on the quality of care.

Resumo OBJETIVO Compreender a importância do grupo no processo de cuidado de enfermagem as gestantes de risco. MÉTODOS Estudo qualitativo, descritivo realizado na Maternidade Escola Assis Chateaubriand, de fevereiro a março/2017, através de grupos focais com 24 gestantes internadas. Dados submetidos à análise de conteúdo e as ideias relevantes dos discursos foram extraídas, formando categorias. RESULTADOS Construíram três categorias: Grupos interativos como espaço de vínculo e convivência, Estratégia educativa como abordagem das boas práticas do parto/nascimento, Repercussão da experiência do grupo no fortalecimento do vínculo entre equipe de saúde e gestante. Os grupos promovem partilha de experiência, aprendizado e reflexão sobre as possibilidades e limitações do processo saúde-doença, reduzindo a ansiedade e contribuindo para o empoderamento na tomada de decisões. CONCLUSÃO As considerações das gestantes nortearam a equipe no desenvolvimento do grupo e efetivá-lo como espaço estratégico de cuidados, ampliação do conhecimento e relação de confiança, impactando na qualidade da assistência.

Los datos sometidos al análisis de contenido y las ideas relevantes de los discursos fueron extraídas, formando categorías. RESULTADOS Se construyeron tres categorías temáticas: Grupos interactivos como espacio de vínculo y convivencia, Estrategia educativa como abordaje de las buenas prácticas del parto/nacimiento, Repercusión de la experiencia del grupo en el fortalecimiento del vínculo entre equipo de salud y gestante. El grupo de gestantes es un espacio de convivencia y vínculo que las estimulan a compartir conocimientos y experiencias, resignificando vínculos. Se comprendió que los grupos promueven compartir experiencia, aprendizaje y reflexión sobre las posibilidades y limitaciones del proceso salud-enfermedad, reduciendo la ansiedad y contribuyendo al empoderamiento en la toma de decisiones. CONCLUSIÓN Las consideraciones y sugerencias de las gestantes contribuyeron a orientar a la enfermera en el desarrollo del grupo y realizarlo como espacio estratégico de cuidados y ampliación del conocimiento y relación de confianza con el enfermero, impactando en la calidad de la asistencia.


Abstract OBJECTIVE To understand the importance of group strategy in the process of nursing care to high-risk pregnant women. METHODS Qualitative and descriptive study conducted at Assis Chateaubriand Teaching Maternity Hospital in February-March 2017, through focus groups with 24 hospitalized pregnant women. Data was submitted to content analysis and the relevant ideas of the discourses were extracted, forming categories. RESULTS Three thematic categories were constructed: Interactive groups as a space of bond and coexistence, Educational strategy as an approach to good delivery and birthcare practices, Impact of group experience in strengthening the connection between the health team and the pregnant woman. The group of pregnant women is a space of coexistence and bonding that stimulates them to share knowledge and experiences, re-signifying links. The groups promote sharing of experiences, learning and reflection on the possibilities and limitations of the health-disease process, reducing anxiety and contributing to empowerment in decision making. CONCLUSION The considerations and suggestions of the pregnant women guided the nurses in the development of the groups, which became strategic spaces of care, acquisition of knowledge and establishment of trust relationships with the nurses, with a consequent impact on the quality of care.
