RESUMEN

Stand-off detection of latent traces avoids the scene alteration that might occur during close inspection by handheld forensic lights. Here, we describe a novel sensor, named Crime Light Imaging (CLI), designed to perform high-resolution photography of targets at a distance of 2-10 m and to visualize some common latent traces. CLI is based on four high-power illumination LEDs and one color CMOS camera with a motorized objective plus frontal filters; the LEDs and camera could be synchronized to obtain short-exposure images weakly dependent on the ambient light. The sensor is integrated into a motorized platform, providing the target scanning and necessary information for 3D scene reconstruction. The whole system is portable and equipped with a user-friendly interface. The preliminary tests of CLI on fingerprints at distance of 7 m showed an excellent image resolution and drastic contrast enhancement under green LED light. At the same distance, a small (1 µL) blood droplet on black tissue was captured by CLI under NIR LED, while a trace from 15 µL semen on white cotton became visible under UV LED illumination. These results represent the first demonstration of true stand-off photography of latent traces, thus opening the way for a completely new approach in crime scene forensic examination.

OBJECTIVES: Osteoarthritis (OA) patient stratification is an important challenge to design tailored treatments and drive drug development. Biochemical markers reflecting joint tissue turnover were measured in the IMI-APPROACH cohort at baseline and analysed using a machine learning approach in order to study OA-dominant phenotypes driven by the endotype-related clusters and discover the driving features and their disease-context meaning. METHOD: Data quality assessment was performed to design appropriate data preprocessing techniques. The k-means clustering algorithm was used to find dominant subgroups of patients based on the biochemical markers data. Classification models were trained to predict cluster membership, and Explainable AI techniques were used to interpret these to reveal the driving factors behind each cluster and identify phenotypes. Statistical analysis was performed to compare differences between clusters with respect to other markers in the IMI-APPROACH cohort and the longitudinal disease progression. RESULTS: Three dominant endotypes were found, associated with three phenotypes: C1) low tissue turnover (low repair and articular cartilage/subchondral bone turnover), C2) structural damage (high bone formation/resorption, cartilage degradation) and C3) systemic inflammation (joint tissue degradation, inflammation, cartilage degradation). The method achieved consistent results in the FNIH/OAI cohort. C1 had the highest proportion of non-progressors. C2 was mostly linked to longitudinal structural progression, and C3 was linked to sustained or progressive pain. CONCLUSIONS: This work supports the existence of differential phenotypes in OA. The biomarker approach could potentially drive stratification for OA clinical trials and contribute to precision medicine strategies for OA progression in the future. Fast gating in Raman spectroscopy is used to reject the fluorescence contribution from the sample and/or the substrate. Several techniques have been set up in the last few decades aiming either to enhance the Raman signal (CARS, SERS or Resonant Raman scattering) or to cancel out the fluorescence contribution (SERDS), and a number of reviews have already been published on these sub-topics. However, for many reasons it is sometimes necessary to reject fluorescence in traditional Raman spectroscopy, and in the last few decades a variety of papers dealt with this issue, which is still challenging due to the time scales at stake (down to picoseconds). Fast gating (<1 ns) in the time domain allows one to cut off part of the fluorescence signal and retrieve the best Raman signal, depending on the fluorescence lifetime of the sample and laser pulse duration. In particular, three different techniques have been developed to accomplish this task: optical Kerr cells, intensified Charge Coupling Devices and systems based on Single Photon Avalanche Photodiodes. The utility of time domain fast gating will be discussed, and In this work, the utility of time domain fast gating is discussed, as well as the performances of the mentioned techniques as reported in literature.

An innovative hyperspectral LIDAR instrument has been developed for applications in marine environment monitoring research activities, remotely detecting the fluorescence spectra produced in the spectral interval between 400 nm and 720 nm. The detection system is composed by a custom made photomultiplier charge integrating and measuring (CIM) unit, which makes automatic background signal subtraction, and a liquid crystal tunable filter (LCTF). The new instrument therefore has hyperspectral resolution and allows automatic background subtraction; it is compact and automated by custom software that permit to adapt the instrument properties depending on the environmental conditions. Laboratory tests to characterize the instrument performance have been carried out, concluding that this sensor can be employed in remote sites for Chl-a detection.

This study aimed to apply the recently proposed Oslo hand osteoarthritis magnetic resonance imaging (MRI) scoring system to evaluate MRI findings in a cohort of patients affected by long-standing erosive hand osteoarthritis (EHOA). Eleven female EHOA patients (median 59 [interquartile range 62-52] years, disease duration 9.5 [interquartile range 13-3.75] years) underwent MRI (1.5 T) of the dominant hand, and synovitis, bone marrow lesions (BMLs), joint space narrowing, osteophytes, cysts, malalignment, and erosions were scored using the Oslo scoring system. Intra- and inter-reader reliability were assessed. The patients also underwent X-ray examination, and bone features were evaluated using the same scoring system. Pain and tenderness were assessed during a physical examination. Spearman's non-parametric test was used to analyze the correlations between variables. MRI intra- and inter-reader reliability were found between good and moderate for many features. No statistical differences were found between the radiographs and MRI with regard to detection of JSN, malalignment, and bone erosions. Synovitis was detected in 39.8 % of the 80 joints examined (in a mild form in 80 %), erosions were found in 51.1 %, and BMLs were identified in 20.5 and 23.9 % at the distal and the proximal side, respectively. BMLs at both the proximal and distal ends were correlated with tender joints (BML distal p = 0.0013, BML proximal p = 0.012). The presence of synovitis was correlated with tenderness (p = 0.004) and erosions at both the distal and proximal joints (p = 0.004). The presence of erosions correlated with tender joints (p < 0.01) and the mean visual analog scale (VAS) score (distal p = 0.03, proximal p = 0.01). Synovitis and BMLs were correlated with clinical symptoms in our patients affected with long-standing EHOA.

RESUMEN

A new Raman-based apparatus for proximal detection of energetic materials on people, was developed and tested for the first time. All the optical and optoelectronics components of the apparatus, as well as their optical matching, were carefully chosen and designed to respect international eye-safety regulations. In this way, the apparatus is suitable for civil applications on people in public areas such as airports and metro or railway stations. The acquisition software performs the data analysis in real-time to provide a fast response to the operator. Moreover, it allows for deployment of the apparatus either as a stand alone device or as part of a more sophisticated warning system architecture made up of several sensors. Using polyamide as substrate, the apparatus was able to detect surface densities of ammonium nitrate (AN), 2-methyl-1,3,5-trinitrobenzene (TNT), 3-nitrooxy-2,2-bis(nitrooxymethyl)propyl] nitrate (PETN) and urea nitrate (UN) in the range of 100-1000 µg/cm² at a distance of 6.4 m using each time a single laser pulse of 3 mJ/cm². The limit of detection calculated for AN is 289 µg/cm². AN and UN provided the highest percentages of true positives (>82% for surface densities of 100-400 µg/cm² and fingerprints) followed by TNT and PETN (17%-70% for surface densities of 400-1000 µg/cm² and fingerprints).

OBJECTIVES: to assess air pollution spatial and temporal variability in the urban area nearby the Ciampino International Airport (Rome) and to investigate the airport-related emissions contribute. DESIGN AND SETTING: the study domain was a 64 km2 area around the airport. Two fifteen-day monitoring campaigns (late spring, winter) were carried out. Results were evaluated using several runs outputs of an airport-related sources Lagrangian particle model and a photochemical model (the Flexible Air quality Regional Model, FARM). MAIN OUTCOME MEASURES: both standard and high time resolution air pollutant concentrations measurements: CO, NO, NO2, C6H6, mass and number concentration of several PM fractions. 46 fixed points (spread over the study area) of NO2 and volatile organic compounds concentrations (fifteen days averages); deterministic models outputs. RESULTS: standard time resolution measurements, as well as model outputs, showed the airport contribution to air pollution levels being little compared to the main source in the area (i.e. vehicular traffic). However, using high time resolution measurements, peaks of particles associated with aircraft takeoff (total number concentration and soot mass concentration), and landing (coarse mass concentration) were observed, when the site measurement was downwind to the runway. CONCLUSIONS: the frequently observed transient spikes associated with aircraft movements could lead to a not negligible contribute to ultrafine, soot and coarse particles exposure of people living around the airport. Such contribute and its spatial and temporal variability should be investigated when assessing the airports air quality impact.

OBJECTIVE: The objective was to compare the diagnostic accuracy of conventional radiography and ultrasonography (US) for the diagnosis of suspected bone fractures. METHOD: Eighty-six patients were assessed using conventional radiography and US on the affected bone district. RESULTS: Radiographic and sonographic findings were concordant in 93% of cases. In one case, US suggested a fracture not seen on radiographic assessment. Ultrasonography showed a sensitivity of 0.94 and a specificity of 0.92. CONCLUSION: In clinical practice, US could become the first diagnostic approach.

BACKGROUND: Cow's milk allergy (CMA) is the most frequent food allergy in childhood; the trend of CMA is often characterized by a progressive improvement to achieve tolerance in the first 4 to 5 years of life.It has been observed that specific IgE (sIgE) towards cow's milk proteins decrease when the age increases.Although food allergy can be easily diagnosed, it is difficult to predict the outcome of the oral food challenge (OFC), that remains the gold standard in the diagnosis of food allergy, by allergometric tests. METHODS: We considered 44 children with CMA diagnosed through OFC who returned to our Allergy and Immunology Pediatric Department between January to December 2010 to evaluate the persistence of allergy or the achievement of tolerance.On the basis of the history, we performed both allergometric skin tests and OFC in children that were still following a milk-free diet, whereas only allergometric skin tests those that had already undergone spontaneous introduction of milk protein at home without presenting symptoms. OBJECTIVE: The aim of this study was to investigate the relationship between the persistence of CMA or the acquisition of tolerance and the results of the end point prick test (EPT). RESULTS AND DISCUSSION: The OFC with cow's milk was performed on 30 children, 4 children were excluded because of a history of severe reactions to cow's milk, and 10 because they had spontaneously already taken milk food derivates at home without problems. 16/30 (53%) children showed clinical reactions and the challenge was stopped, 14/30 (47%) did not have any reaction.Comparing the mean wheal diameter of every EPT's dilution between the group of allergic children and the tolerant ones, we obtained a significant difference (p < 0.05) for the first 4 dilutions.We have also calculated sensitivity (SE), specificity (SP), the positive predictive value (PPV) and the negative predictive value (NPV) for each EPT dilution. CONCLUSIONS: EPT is a safe and cheap test, easy to be executed and that could provide good prediction of the outcome of OFC; so it might be used to avoid OFC-induced anaphylaxis in children affected by CMA. It can also help avoiding dietetic restrictions in tolerant children who show sensitization towards cow's milk proteins.

The LIDAR (laser radar) is an active remote sensing technique, which allows for the altitude-resolved observation of several atmospheric constituents. A typical application is the measurement of the vertically resolved aerosol optical properties. By using aerosol particles as a marker, continuous determination of the mixing layer height (MLH) can also be obtained by LIDAR. Some examples of aerosol extinction coefficient profiles and MLH extracted from a 1-year LIDAR data set collected in Milan (Italy) are discussed and validated against in situ data (from a balloon-borne optical particle counter). Finally a comparison of the observation-based MLH with relevant numerical simulations (mesoscale model MM5) is provided.

Although spiral computed tomography (CT) is being used increasingly as the first-line imaging procedure in the diagnostic workup of patients with clinically suspected pulmonary embolism (PE), the diagnostic value of negative findings, at least when using the four-detector row scanners, is still controversial. A total of 702 consecutive patients with clinical symptoms suggestive of PE underwent four-slice CT. Patients with negative findings received the determination of D-dimer. Those with positive D-dimer underwent further diagnostic workup to confirm or rule out the diagnosis of PE. Those with negative D-dimer were followed-up to 6 months to detect the development of symptomatic venous thromboembolism (VTE). The CT test was interpreted as negative in 536 patients (76.3%). These patients had the D-dimer determination, which was positive in 279 and negative in the remaining 257 patients. Of the former, PE subsequently was documented in 55 patients (19.7%). Of the latter, symptomatic VTE in the follow-up period developed in three patients (1.17%; 95% confidence interval, 0.24 to 3.38%). In conclusion, when using the four-detector row, the negative predictive value of CT findings in patients with clinically suspected PE and positive D-dimer is low. In contrast, it is safe to withhold anticoagulation from patients with negative findings and negative D-dimer.
