RESUMEN

Breast milk is a source of all the essential nutritional components necessary for the full growth and development of the child, therefore, it is necessary to study its composition and physical and chemical properties in order to adapt human milk substitutes. Adapted infant milk formulas are produced mainly from cow's milk, bringing formula nutrient composition closer to the composition of women's milk, adapting it in accordance with the requirements of the infant body. However, technological processes for the production of dairy products contribute to the activation of oxidative reactions, the violation of protein conformation. The purpose of the study was to compare the intensity of formation of carbonyl derivatives of human and cow's milk proteins during spontaneous and metal-catalyzed oxidation. Material and methods. The object of the study were samples of mature milk of healthy nursing mothers (n=12), and samples of drinking ultra-pasteurized milk for baby nutrition (n=8) which were used as a comparison material. The intensity of oxidative modification of milk proteins was determined spectrophotometrically by the interaction of carbonyl derivatives of amino acid residues with 2.4-dinitrophenylhydrazine to form 2.4-dinitrophenylhydrazone derivatives in a native sample of biological material and under induction of protein oxidation in vitro by the Fenton reaction by adding FeSO4 and hydrogen peroxide solutions. The content of nonprotein sulfhydryl groups was determined after protein precipitation spectrophotometrically with 5.5'-dithio-bis-2-nitrobenzoic acid. Results. The intensity of spontaneous (basic) oxidation doesn't have significant differences between the indicators of breast and cow's milk. Significant differences were established in the content of carbonyl derivatives of amino acid residues of human and cow's milk proteins during metal-catalyzed oxidation. Incubation with iron ions caused 1.5-2.5 fold more formation of both aldehyde and ketone derivatives of cow's milk proteins, recorded in the visible and ultraviolet spectrum. In cow's milk during spontaneous oxidation and induction of oxidation by a metal, the percentage of aldehyde-dinitrophenylhydrazones was lower than in breast milk and, conversely, the proportion of ketone-dinitrophenylhydrazones, late markers of oxidative degradation of proteins, was significantly higher. The content of non-protein sulfhydryl groups in cow's milk was 2 times less than in fresh human milk. A significant excessive content of aldehyde-dinitrophenylhydrazones (2 times) and ketone-dinitrophenylhydrazones (2.6 times) undet metal-catalyzed protein oxidation of cow's milk in comparison with breast milk indicates a lower level of antioxidant reserves of cow's milk. This is confirmed by the reduced level of non-protein sulfhydryl groups. The results obtained indicate the need to improve the antioxidant status of dairy products for infant nutrition.

The purpose of the study was to identify the pathogenesis features of prepiloric and pyloric ulcers and to establish the cause of resistance to conservative therapy and the tendency to complicated course of type III ulcers according to H.D. Johnson. A comparative analysis of acid production was carried out in 150 patents with duodenal ulcer (n-80) and peptic ulcer of type III according to H.D. Johnson classification (n-70). We revealed a definite individual rhythm of daily intragastric acidity with a maximum rise at night in patients with duodenal ulcer. The paper explores the peculiarities of digestive juice secretion depending on ulcerative substratum localization. The pathogenetic validity of the use of antisecretory drugs for duodenal ulcer treatment and the need to further improve the algorithm for pyloric and prepyloric ulcer therapy are shown. For pyloric and prepiloric ulcers characteristic specific morphological changes in the muscle layer of the wall of the antrum, leading to the motor-evacuation disorders and promoting duodenogastric reflux - dystrophy myocytes circular muscle layer vacuolation of the cytoplasm, edema and hypertrophy ganglia intermuscular plexus, hemorrhages in the muscle sheath, violation of muscle bundles.

RESUMEN

Recently, a new high energy proton microscopy facility PRIOR (Proton Microscope for FAIR Facility for Anti-proton and Ion Research) has been designed, constructed, and successfully commissioned at GSI Helmholtzzentrum für Schwerionenforschung (Darmstadt, Germany). As a result of the experiments with 3.5-4.5 GeV proton beams delivered by the heavy ion synchrotron SIS-18 of GSI, 30 µm spatial and 10 ns temporal resolutions of the proton microscope have been demonstrated. A new pulsed power setup for studying properties of matter under extremes has been developed for the dynamic commissioning of the PRIOR facility. This paper describes the PRIOR setup as well as the results of the first static and dynamic proton radiography experiments performed at GSI.

With the aim of the substantiation of hygienic measures for the improvement of the prevention of nutrition-related diseases among adolescents there was performed an analysis of indices of the health-related quality of life, lifestyle features, the character of nutrition and health of adolescents aged of 13-17 years in the territory of the Omsk region (n = 427). The study of the health-related quality of life was performed with the use of the validated Russian-language version ofthe general questionnaire PedsQL-4.0. According to the results of the study there were identified the priority factors influencing the formation of the quality of life of adolescents. The nutrition of adolescents has a significant influence on the formation of quality of life. Among the many parameters that characterize the nutrition, the greatest contribution to the quality of life is made by the factor of "the amount of consumed meat products". The decline in quality of life was noted in adolescents with chronic diseases or disorders of the nutritional status as overweight. There were marked forward-looking possibilities of prevention of nutrition-related diseases on the base of indices of the quality of life, including measures for correction of adolescent nutrition, in-time detection of disturbances of the nutritional status, early diagnosis and treatment of chronic diseases.

It was done the analysis of the results of 625 open and endoscopic prosthetic and autoplastic hernia repairs made in a planned and emergency basis during the period from 2005 to 2012. The autoplastic group was divided into three subgroups: the plastic by Bassini, Shouldays and Postempski. The prosthesis group was divided into open and endoscopic subgroups. The open prosthesis group was also divided into subgroups in accordance with the options of hernia sac preparation (excised, reseted, untouched) and the reconstruction of the posterior wall of the inguinal canal (without reconstruction, with a single-layer and double-layer plastic). It was noted hematocele scrotum - 19 (3.04%), hematoma of the spermatic cord - 16 (2.56%), wound infiltration with a hemorrhagic component - 13 (2.08%). It was not noted the hemorrhagic complications in the subgroups of open prosthesis without hernia sac preparation. Reducing surgical aggression against inguinal hernias during the operation can reduce the frequency of hematomas. The absence of a clear classification of hematomas brings to difficulties in assessment of the role of specific surgical techniques in the development of postoperative complications after surgery for inguinal hernias.

RESUMEN

Treatment of ulcerative gastroduodenal bleedings remains one of the most pressing problems in urgent surgery. Available methods of endoscopic hemostasis are not fully eligible to safety requirements, effectiveness and reliability. In this connection persist high levels of general and postoperative mortality in our country and around the world. The aim of this study is to identify the advantages and disadvantages of endoscopic radiofrequency effects in comparison with standard techniques of hemostasis, used in the treatment of ulcer bleeding in the world practice.

RESUMEN

The frequency of multiple congenital malformations, central nervous system and genital malformations were found to be associated to the magnitude and duration of ecological load. Xenobiotics identified in the ambient air of a large industrial center pose an increased risk for antenatal malformations. The largest number of statistically significant correlation coefficients was established between the study variables at a delay lag of 2 years. It is assumed that high ambient air pollution adversely affects the mechanisms of inheritance in future parents long before birth of a child.

RESUMEN

In various countries all over the world, the congenital malformations are among the leading causes of infant mortality and children disability. The historical approach is applied to consider epidemiological monitoring as a practice preceding the comprehensive analysis of causes and conditions of congenital malformations formation. The primary tasks of monitoring study are established and means of their implementation as well. The actual technique of registration and recording of various forms of the congenital malformations in children and its alternative are presented. The need of the coordinated activities of pediatric, medical genetic and epidemiological services in the organization and the execution of the monitoring in different regions of the country is emphasized.

RESUMEN

The aim of this survey was to study the ecological state in the Kurgan Region in the period of 1993 to 2003. The survey showed the west and northwest part of the Kurgan Region to be areas at risk for environment-dependent diseases. The major risk factors included a high pollution of water sources with limited water resources due to the radioactive pollution occurring in the 1940s to the 1950s in some areas of the region; transformation of natural landscapes caused by spring floods and soil erosions among other things.

Chromosomes were morphologically studied in the chorionic fibroblasts of induced abortions. Chorionic villi biopsy specimens taken during 103 abortions at gestational weeks 6-12 served as a material for this investigation. They were taken from women with pregnancies interrupted due to that there were social indications. Metaphase and prometaphase chromosomal specimens were obtained by the test modified by V. Baranov 4-5 hours after chorionic villi biopsy, without preliminarily tissue cultivation. A cytogenetic analysis revealed an abnormal karyotype in 2.91%. The frequency of chromosomal aberrations may serve as a control for prospective studies of spontaneous mutagenesis in a region and the chorion of induced abortion may be regarded as a test unit of mutagenic and embryotoxic effects on the fetus.

RESUMEN

Experimental and clinical results of use of fibrin glue for strengthening "high risk" sutures of anastomoses in elective surgery of the esophagus, stomach and urgent abdominal surgery are presented. In experimental studies this glue permitted to increase mechanical strength and containment of anastomosis, to accelerate microbial elimination, activated reparative processes. In clinical practice fibrin glue permitted to avoid suture insufficiency of gastrointestinal anastomoses in the most unfavorable conditions (peritonitis, acute intestinal obstruction, oncological process, etc). It is concluded that fibrin glue is the high effective protective agent for gastrointestinal anastomoses.

RESUMEN

The paper describes a system able to combine conventional techniques of diagnostic x-ray imaging with direct digital imaging on the original receiver with a resolution of 1.2 lin/mm. Digital roentgenography may be introduced into medical practice basing on the system of computers at the working places of roentgenologist and his assistant. Data base of long-term storage of x-ray images and input software for each x-ray picture are outlined. The authors have some experience of utilizing the system in the random sequence of patients (20,000) in general and specialized hospitals of Novosibirsk.

RESUMEN

Electromagnetic signals of special form produced by an ultra-high frequency generator were used to destroy various microorganisms: baker's yeast; blue-green alga Nostoc muscorum; mold fungus; and two flagellates, plant flagellate Euglena gracilis and an animal flagellate parasitizing on humans. The control samples before irradiation and experimental samples damaged and destroyed by irradiation were examined on a microscope with a computer system of image analysis. The results are presented as computer graph images.

With the purpose of studying the condition of haemodynamics and function of the right ventricle in 28 patients with unstable stenocardia paired transesophageal electrocardiostimulation and bicycle ergometry [correction of veloergometry] were done with recording of echocardiographic indicators at the height of stress-tests. Important changes were identified in the right ventricular ejection fraction peculiar to changes of end-diastolic and end-systolic volumes in conditions of simulating myocardial infarction.

RESUMEN

The paper deals with the system that allows the conventional procedures for obtaining diagnostic X-ray images to be integrated with direct digital ones. It presents patterns of introducing digital roentgenography into practical medicine and describes the system how diagnostic X-ray images are obtained, processed, stored, and transmitted, which consists of automatic working places for X-ray laboratory assistant and an X-ray physician. The requirements for long-term diagnostic archives data base and for the software support of input of the data accompanying each X-ray film are discussed in the paper. The information presented reflects the authors' experience in using the system in a multidisciplinary hospital having an unselected stream of patients (n = 6,000).

RESUMEN

Echocardiographic examination revealed in 184 (11%) of the 1680 persons an accessory chorda in the left cardiac ventricle (ACLV). It was found ACLV was often associated with paroxysms of cardiac of unclear etiology and ventricular fibrillation in myocardial infarction. The clinico-, phono-, electrocardiographic symptomatology and inheritability of ACLV are discussed. As defined by the authors the syndrome in ACLV includes: 1) cardialgia, 2) systolic murmur, 3) electrocardiographic syndrome of early ventricular repolarization, 4) paroxysms of cardiac fibrillation, 5) Predisposition to ventricular fibrillation in myocardial infarction.
