OBJECTIVES: During the Coronavirus disease (COVID-19) pandemic, clinicians recommended awake-prone positioning (APP) to avoid the worst outcomes. The objectives of this study were to investigate if APP reduces intubation, death rates, and hospital length of stay (HLOS) in acute COVID-19. METHODS: We performed a retrospective cohort with non-mechanically ventilated patients hospitalized in a reference center in Manaus, Brazil, 2020. Participants were stratified into APP and awake-not-prone positioning (ANPP) groups. Also, we conducted a systematic review and performed a meta-analysis to understand if this intervention had different outcomes in resource-limited settings (PROSPERO CRD42023422452). RESULTS: A total of 115 participants were allocated into the groups. There was no statistical difference between both groups regarding time to intubation (HR: 0.861; 95CI: 0.474-1.1562; p=0.622) and time to death (HR: 1.666; 95CI: 0.939-2.951; p=0.081). APP was not significantly associated with reduced HLOS. A total of 86 articles were included in the systematic review, of which 76 (88,3%) show similar findings after APP. Also, low/middle, and high-income countries were similar regarding such outcomes. CONCLUSION: APP in COVID-19 does not present clinical improvement that affects mortality, intubation rate and HLOS. The lack of a prone position protocol, obtained through a controlled study, is necessary. After 3 years, APP benefits are still inconclusive.

Background: The coronavirus disease-2019 (COVID-19) pulmonary rehabilitation (PR) seems to be a better choice to improve physical and functional capacity after acute infection. However, there is a lack of evidence regarding the effects of different strategies to optimize post-acute phase rehabilitation and reduce long COVID-19 physical deteriorations. Objective: To compare the use of a noninvasive ventilation (NIV) plus aerobic exercise strategy during PR program with to a standard PR (without NIV) on physical capacity and quality of life outcomes in post-COVID-19. Methods: Double-blinded randomized controlled clinical trial. A total of 100 individuals discharged from hospital in a post-acute phase of severe COVID-19 will be randomized into two groups: PR + NIV (Group 1) and PR (Group 2). Inclusion criteria include participants who present symptomatic dyspnea II and III by the modified Medical Research Council, aged 18 years or older. Both groups will receive aerobic and resistance exercise, and inspiratory muscle training. However, group 1 will perform aerobic training with bilevel NIV. Cardiopulmonary exercise test will assess the O2 peak uptake, 6-minute walk test will assess the walking distance and short-form 36 will assess the quality of life before and after 8 weeks (after 24 PR sessions). Moreover, patients will be contacted by telephone every 3 months for one year to record possible adverse events, hospitalizations, and death. All data will be registered in RedCap, and analyses will be performed in the STATA v13 software. Clinical Trial Registration: RBR-3t9pkzt.

The diagnosis of long COVID is troublesome, even when functional limitations are present. Dynapenia is the loss of muscle strength and power production that is not caused by neurologic or muscular diseases, being mostly associated with changes in neurologic function and/or the intrinsic force-generating properties of skeletal muscle, which altogether, may partially explain the limitations seen in long COVID. This study aimed to identify the distribution and possible associations of dynapenia with functional assessments in patients with long COVID. A total of 113 patients with COVID-19 were evaluated by functional assessment 120 days post-acute severe disease. Body composition, respiratory muscle strength, spirometry, six-minute walk test (6MWT, meters), and hand-grip strength (HGS, Kilogram-force) were assessed. Dynapenia was defined as HGS < 30 Kgf (men), and < 20 Kgf (women). Twenty-five (22%) participants were dynapenic, presenting lower muscle mass (p < 0.001), worse forced expiratory volume in the first second (FEV1) (p = 0.0001), lower forced vital capacity (p < 0.001), and inspiratory (p = 0.007) and expiratory (p = 0.002) peek pressures, as well as worse 6MWT performance (p < 0.001). Dynapenia, independently of age, was associated with worse FEV1, maximal expiratory pressure (MEP), and 6MWT, (p < 0.001) outcomes. Patients with dynapenia had higher intensive care unit (ICU) admission rates (p = 0.01) and need for invasive mechanical ventilation (p = 0.007) during hospitalization. The HGS is a simple, reliable, and low-cost measurement that can be performed in outpatient clinics in low- and middle-income countries. Thus, HGS may be used as a proxy indicator of functional impairment in this population.

Severe traumatic brain injury (sTBI) is an important cause of disability and mortality and affects people of all ages. Current scientific evidence indicates that motor dysfunction and cognitive impairment are the main limiting factors in patients with sTBI. Transcranial direct current stimulation (tDCS) seems to be a good therapeutic option, but when it comes to patients with sTBI, the results are inconclusive, and some protocols have not yet been tested. In addition, there is still a lack of information on tDCS-related physiological mechanisms, especially during the acute phase. In the present study, based on current evidence on tDCS mechanisms of action, we hypothesized that performing tDCS sessions in individuals with sTBI, especially in the acute and subacute phases, together with conventional therapy sessions, could improve cognition and motor function in this population. This hypothesis presents a new possibility for treating sTBI, seeking to elucidate the extent to which early tDCS may affect long-term clinical outcomes.

Rheumatoid arthritis is up to three times more prevalent in women. It is often associated with anxiety and depression, comorbidities causing psychic suffering and potentiating pain perception. It is also related to a higher risk of suicide among diagnosed patients. The high rates of discontinuation of conventional pharmacological treatments are the predominant factor in the search for new therapeutic approaches for the treatment of anxiety and depression. Transcranial direct-current stimulation (tDCS) is a promising, safe and low-cost technique that is very associative with other therapies. When applied to the primary motor cortex (M1) it can induce long-term changes in the synaptic level leading to the improvement of neuroplasticity. The primary aim of this study is to evaluate the effect of tDCS on the symptoms of anxiety and depression. The secondary aim is to evaluate the interference of tDCS on the inflammatory profile, cardiac autonomic behavior and quality of life of patients with rheumatoid arthritis. This is a randomized, double-blind, placebo-controlled clinical trial. The intervention consists of 10 consecutive sessions (once a day) applying tDCS with a 2mA current for 20 minutes. The electrode assembly on the scalp is in accordance with the International Electroencephalogram System 10-20 (EEG) and the anodal electrode is placed over the area of the primary motor cortex (M1 - C3 or C4) and the cathodal electrode on the supraorbital contralateral area (SO - Fp1 or Fp2). The analysis of continuous variables will be described by mean and standard deviation for parametric data and median and interquartile interval for nonparametric data. The evaluation of the effect of tDCS on the inflammatory profile, heart rate variability and quality of life will be obtained by the ANOVA two-way test. tDCS is expected to have a greater effect on reducing anxiety and depression symptoms compared to the placebo, being able to decrease inflammation and improve the quality of life of volunteers.

RESUMO O objetivo foi investigar o impacto de um Programa de Intervenção Motora Domiciliar (PIMD), com a abordagem centrada na família, na funcionalidade de indivíduos com Distrofia Muscular de Duchenne (DMD). Foi realizado uma série de casos, entre novembro de 2020 a junho de 2021 e aplicado a função motora grossa dos membros superiores e inferiores antes e após o PIMD, durante 16 sessões. Permaneceram seis crianças entre 12-13 (±2,90) anos de idade; 9,14 (±0,90) anos para perda de deambulação e 6,38 (±1,06) anos para idade de diagnóstico. A Medida da Função Motora inicial foi 47,8 (±20,13) e final, 56 (±20,53); na Escala de Vignos, inicial foi 7 (±1,73) e final, 6,4 (±1,95); na Escala de Brooke, inicial foi 2,0 (±1,30) e final, 2,2 (±1,22); na Performance of the Upper Limb, inicial foi 28,29 (±11,94) e final, 35 (±13,28). Na criança deambuladora, a média do escore de North Star Ambulatory Assessment (NSAA) total inicial foi 25 e final, 27. Portanto, o PIMD pode ser uma alternativa para prolongar a funcionalidade do curso clínico da DMD, em períodos sem intervenção presencial. A telerreabilitação é uma estratégia promissora, entretanto, é necessário treinamento da equipe de cuidados à saúde e o envolvimento dos pais.


ABSTRACT The objective was to investigate the impact of a Home Motor Intervention Program (PIMD), with a family-centered approach, on the functionality of individuals with Duchenne Muscular Dystrophy (DMD). A series of cases was carried out between November 2020 and June 2021 and applied to the gross motor function of the upper and lower limbs before and after PIMD, during 16 sessions. Six children between 12-13 (±2.90) years of age remained; 9.14 (±0.90) years for loss of ambulation and 6.38 (±1.06) years for age at diagnosis. The initial Motor Function Measure was 47.8 (±20.13) and final, 56 (±20.53); on the Vignos Scale, initial was 7 (±1.73) and final, 6.4 (±1.95); on the Brooke Scale, initial was 2.0 (±1.30) and final, 2.2 (±1.22); in the Performance of the Upper Limb, initial was 28.29 (±11.94) and final, 35 (±13.28). In the ambulatory child, the initial total North Star Ambulatory Assessment (NSAA) mean score was 25 and the final score was 27. Therefore, PIMD can be an alternative to prolong the functionality of the clinical course of DMD, in periods without face-to-face intervention. Telerehabilitation is a promising strategy, however, training of the health care team and parental involvement is required.

OBJECTIVE: The aim of this review is to synthesize the available evidence on the effectiveness of pulmonary rehabilitation on the functional capacity of people who have finished drug treatment for pulmonary tuberculosis. INTRODUCTION: Pulmonary rehabilitation is a treatment option for physical functional problems that persist after pharmacological treatment for tuberculosis; however, the results of primary studies on the subject are inconclusive. INCLUSION CRITERIA: This review will consider for inclusion studies carried out with adults (≥18 years old) who have completed pharmacological treatment for tuberculosis. Outcomes of interest will include functional capacity, quality of life, lung function, respiratory and skeletal muscle strength, subjective perception exertion, and dyspnea. METHODS: Searches for published and unpublished studies will be carried out on MEDLINE (via PubMed), LILACS, PEDro, CINAHL, Web of Science, Embase, Science Direct, Scopus, Cochrane Library (CENTRAL), Sport Medicine and Education Index, Rehabilitation and Sports Medicine Source, and SPORTDiscus. The results of the review will be reported according to PRISMA guidelines. There will be no language or time limitations. Two independent reviewers will select and critically appraise the selected studies and extract the data using standardized instruments. The certainty of evidence will be reported according to GRADE. Where appropriate, studies will be subjected to meta-analysis. Effect sizes will be expressed in odds ratios (for dichotomous data) or in standardized mean difference (for continuous data) and its 95% CI. Our aim was to evaluate: (1) the prevalence of coexistence of heart failure (HF) and chronic obstructive pulmonary disease (COPD) in the studied patients; (2) the impact of HF + COPD on exercise performance and contrasting exercise responses in patients with only a diagnosis of HF or COPD; and (3) the relationship between clinical characteristics and measures of cardiorespiratory fitness; (4) verify the occurrence of cardiopulmonary events in the follow-up period of up to 24 months years. The current study included 124 patients (HF: 46, COPD: 53 and HF + COPD: 25) that performed advanced pulmonary function tests, echocardiography, analysis of body composition by bioimpedance and symptom-limited incremental cardiopulmonary exercise testing (CPET) on a cycle ergometer. Key CPET variables were calculated for all patients as previously described. The [Formula: see text]E/[Formula: see text]CO2 slope was obtained through linear regression analysis. Additionally, the linear relationship between oxygen uptake and the log transformation of [Formula: see text]E (OUES) was calculated using the following equation: [Formula: see text]O2 = a log [Formula: see text]E + b, with the constant 'a' referring to the rate of increase of [Formula: see text]O2. Circulatory power (CP) was obtained through the product of peak [Formula: see text]O2 and peak systolic blood pressure and Ventilatory Power (VP) was calculated by dividing peak systolic blood pressure by the [Formula: see text]E/[Formula: see text]CO2 slope. After the CPET, all patients were contacted by telephone every 6 months (6, 12, 18, 24) and questioned about exacerbations, hospitalizations for cardiopulmonary causes and death. We found a 20% prevalence of HF + COPD overlap in the studied patients. The COPD and HF + COPD groups were older (HF: 60 ± 8, COPD: 65 ± 7, HF + COPD: 68 ± 7). In relation to cardiac function, as expected, patients with COPD presented preserved ejection fraction (HF: 40 ± 7, COPD: 70 ± 8, HF + COPD: 38 ± 8) while in the HF and HF + COPD demonstrated similar levels of systolic dysfunction. The COPD and HF + COPD patients showed evidence of an obstructive ventilatory disorder confirmed by the value of %FEV1 (HF: 84 ± 20, COPD: 54 ± 21, HF + COPD: 65 ± 25). Patients with HF + COPD demonstrated a lower work rate (WR), peak oxygen uptake ([Formula: see text]O2), rate pressure product (RPP), CP and VP compared to those only diagnosed with HF and COPD. In addition, significant correlations were observed between lean mass and peak [Formula: see text]O2 (r: 0.56 p < 0.001), OUES (r: 0.42 p < 0.001), and O2 pulse (r: 0.58 p < 0.001), lung diffusing factor for carbon monoxide (DLCO) and WR (r: 0.51 p < 0.001), DLCO and VP (r: 0.40 p: 0.002), forced expiratory volume in first second (FEV1) and peak [Formula: see text]O2 (r: 0.52; p < 0.001), and FEV1 and WR (r: 0.62; p < 0.001). There were no significant differences in the occurrence of events and deaths contrasting both groups. The coexistence of HF + COPD induces greater impairment on exercise performance when compared to patients without overlapping diseases, however the overlap of the two diseases did not increase the probability of the occurrence of cardiopulmonary events and deaths when compared to groups with isolated diseases in the period studied. CPET provides important information to guide effective strategies for these patients with the goal of improving exercise performance and functional capacity. Moreover, given our findings related to pulmonary function, body composition and exercise responses, evidenced that the lean mass, FEV1 and DLCO influence important responses to exercise.

BACKGROUND: Transcranial direct current stimulation (tDCS) has the potential to modulate cortical excitability and enhance the effects of walking training in people with Parkinson's disease. This study will examine the efficacy of the addition of tDCS to a task-specific walking training to improve walking and mobility and to reduce falls in people with Parkinson's disease. METHODS: This is a two-arm, prospectively registered, randomized trial with concealed allocation, blinded assessors, participants and therapists, and intention-to-treat analysis. Twenty-four individuals with Parkinson's disease, categorized as slow or intermediate walkers (walking speeds ≤ 1.0 m/s), will be recruited. The experimental group will undertake a 30-min walking training associated with tDCS, for 4 weeks. The control group will undertake the same walking training, but with sham-tDCS. The primary outcome will be comfortable walking speed. Secondary outcomes will include walking step length, walking cadence, walking confidence, mobility, freezing of gait, fear of falling, and falls. Outcomes will be collected by a researcher blinded to group allocation at baseline (week 0), after intervention (week 4), and 1 month beyond intervention (week 8). DISCUSSION: tDCS associated with walking training may help improve walking of slow and intermediate walkers with Parkinson's disease. If walking is enhanced, the benefits may be accompanied by better mobility and reduced fear of falling, and individuals may experience greater free-living physical activity at home and in the community. OBJECTIVE: To investigate cerebral oxygenation (Cox) responses as well as respiratory (Res) and active peripheral muscle (Pm) O2 delivery during high-intensity cycling exercise and contrast responses between patients with coexistent chronic obstructive pulmonary disease (COPD)-heart failure (HF) and HF alone. METHODS: Cross-sectional study involving 11 COPD-HF and 11 HF patients. On two different days, patients performed maximal incremental cardiopulmonary exercise testing (CPET) and constant load exercise on a cycle ergometer until the limit of tolerance (Tlim). The high-intensity exercise session was 80% of the peak CPET work rate. Relative blood concentrations of oxyhemoglobin ([O2Hb]), deoxyhemoglobin ([HHb]) of Res, Pm (right vastus lateralis) and Cox (pre-frontal) were measured using near infrared spectroscopy. RESULTS: We observed a greater decrease in [O2Hb] at a lower Tlim in COPD-HF when compared to HF (P < 0.05). [HHb] of Res was higher (P < 0.05) and Tlim was lower in COPD-HF vs. HF. Pm and Cox were lower and Tlim was higher in (P < 0.05) HF vs. COPD-HF. In HF, there was a lower ∆[O2Hb] and higher ∆ [HHb] of Pm when contrasted to Cox observed during exercise, as well as a lower ∆ [O2Hb] and higher ∆ [HHb] of Res when contrasted with Cox (P < 0.05). However, COPD-HF patients presented with a higher ∆ [HHb] of Res and Pm when contrasted with Cox (P < 0.05). CONCLUSION: The coexistence of COPD in patients with HF produces negative effects on Cox, greater deoxygenation of the respiratory and peripheral muscles and higher exertional dyspnea, which may help to explain an even lower exercise tolerance in this multimorbidity phenotype.

Giovanni Aldini was an Italian physicist interested in propagating Galvanism. With his uncle, Luigi Galvani, he learned techniques of electrical stimulation in humans and helped him to develop them. He later developed his own ideas and created transcranial electric stimulation. This paper presents some of Aldini's personal and professional traits, showing his trajectory and how his performance was important for the development of non-invasive brain stimulation techniques as a whole. Through research on animals and, later, on cadavers, his fundamental discoveries at the beginning of the treatment proposals are used so far.

RESUMO Giovanni Aldini era um físico italiano interessado em propagar o Galvanismo. Com seu tio, Luigi Galvani, aprendeu técnicas de estimulação elétrica em humanos e pode ajudá-lo a desenvolvê-las. Mais tarde, desenvolveu suas próprias ideias e criou a estimulação elétrica transcraniana. Este artigo apresenta alguns traços pessoais e profissionais de Aldini, mostrando sua trajetória e como seu desempenho foi importante para o desenvolvimento de técnicas não invasivas de estimulação cerebral como um todo. Através de pesquisas em animais e, posteriormente, em cadáveres, suas descobertas fundamentais no início das propostas de tratamento utilizadas até o momento.

Parkinson's disease (PD) is one of the most prevalent neurodegenerative diseases in the world, with a high degree of disability. Among the various therapeutic possibilities, brain stimulation appears in a promising approach, with deep brain stimulation (DBS) being the best described and successful, yet it has the limitation of being invasive. In this context we present transcranial direct current stimulation (tDCS), a non-invasive treatment that brings a new perspective when thinking about treatment of neurological diseases. It is easy to handle, low cost, few side effects and good adherence to patients. TDCS presents good evidence for clinical practice, but when it comes to PD the results obtained are inconclusive and some protocols have not yet been tested. In this hypothesis we propose that the use of tDCS applied in the supplemental motor areas, together with a gait training, can facilitate the motor learning and modulate the neurons for better potentiation of the exercises together with patients with walking difficulties due to PD.

Our aim was to identify optimal cardiopulmonary exercise testing (CPET) threshold values that distinguish disease severity progression in patients with co-existing systolic heart failure (HF) and chronic obstructive pulmonary disease (COPD), and to evaluate the impact of the cut-off determined on the prognosis of hospitalizations. We evaluated 40 patients (30 men and 10 woman) with HF and COPD through pulmonary function testing, doppler echocardiography and maximal incremental CPET on a cycle ergometer. Several significant CPET threshold values were identified in detecting a forced expiratory volume in 1 second (FEV1) < 1.6 L: 1) oxygen uptake efficiency slope (OUES) < 1.3; and 2) circulatory power (CP) < 2383 mmHg.mlO2.kg-1. CPET significant threshold values in identifying a left ventricular ejection fraction (LVEF) < 39% were: 1) OUES: < 1.3; 2) CP < 2116 mmHg.mlO2.kg-1.min-1 and minute ventilation/carbon dioxide production (V̇E/V̇CO2) slope>38. The 15 (38%) patients hospitalized during follow-up (8 ± 2 months). In the hospitalizations analysis, LVEF < 39% and FEV1 < 1.6, OUES < 1.3, CP < 2116 mmHg.mlO2.kg-1.min-1 and V̇E/V̇CO2 > 38 were a strong risk predictor for hospitalization (P ≤ 0.050). The CPET response effectively identified worsening disease severity in patients with a HF-COPD phenotype. LVEF, FEV1, CP, OUES, and the V̇E/V̇CO2 slope may be particularly useful in the clinical assessment and strong risk predictor for hospitalization.

BACKGROUND: Cardiorespiratory fitness (CRF) is an important prognostic marker in chronic obstructive pulmonary disease (COPD). Obstructive sleep apnea (OSA) also negatively affects exercise tolerance. However, the impact of their association on CRF has not been evaluated. We hypothesized that patients with overlap syndrome would demonstrate a greater impairment in CRF, particularly those with severe COPD. METHODS: Individuals with COPD were recruited. First, subjects underwent clinical and spirometry evaluation. Next, home-based sleep evaluation was performed. Subjects with an apnea-hypopnea index (AHI) < 15 episodes/h were allocated to the COPD group and those with an AHI ≥ 15 episodes/h to the overlap group. On the second visit, subjects underwent a cardiopulmonary exercise test. Subsequently, they were divided into four groups according to the severity of COPD and coexistence of OSA: COPDI/II; overlap I/II; COPDIII/IV; and overlap III/IV. RESULTS: Of the 268 subjects screened, 31 were included. The overlap group exhibited higher values for peak carbon dioxide (COPD: 830 [678-1157]; overlap: 1127 [938-1305] mm Hg; p < 0.05), minute ventilation (COPD: 31 [27-45]; overlap: 48 [37-55] L; p < 0.05), and peak systolic blood pressure (COPD: 180 [169-191]; overlap: 220 [203-227] mm Hg; p <; 0.001) and peak diastolic blood pressure COPD: 100 [93-103]; overlap: 110 [96-106] mm Hg; p < 0.001). COPD severity associated with OSA produced a negative impact on exercise time (COPDIII/IV: 487 ± 102; overlap III/IV: 421 ± 94 s), peak oxygen uptake (COPDIII/IV: 12 ± 2; overlap III/IV: 9 ± 1 ml.Kg.min-1 ; p < 0.05) and circulatory power (COPDIII/IV: 2306 ± 439; overlap III/IV: 2162 ± 340 ml/kg/min.mmHg; p < 0.05). CONCLUSION: Overlap syndrome causes greater hemodynamic and ventilatory demand at the peak of dynamic exercise. In addition, OSA overlap in individuals with more severe COPD impairs CRF.

Multiple sclerosis (MS) is an immune-mediated, inflammatory disease characterized by repeated episodes of demyelization. Objective: The present study aimed to trace the epidemiological and functional profile of patients with Multiple Sclerosis treated at a Reference Hospital in Manaus-AM. Method: 80 charts were selected for screening and subsequent application of the Functional Independence Measure (MIF) scale. Results: 32 patients were included in the study, 23 females (74%), whose mean age was 35 (± 12) years, with a relationship between females and males of 2.5. The mean diagnostic time for men was 7.8 years and for women of 5.3 years. The mean score on the Total MIF scale was 110.9 (± 17.5). Conclusion: Epidemiological characteristics are in line with most similar studies, but lack further studies aimed at assessing the functionality of individuals with MS.


A esclerose múltipla (EM) é uma doença imuno-mediada, inflamatória, caracterizada por repetidos episódios de desmielinização. Objetivo: Traçar o perfil epidemiológico e funcional dos pacientes com Esclerose Múltipla atendidos em um Hospital de Referência de Manaus-AM. Método: Foram selecionados 80 prontuários para triagem e posterior aplicação da escala de Medida de Independência Funcional (MIF). Resultado: Foram incluídos no estudo 32 pacientes, 23 do sexo feminino (74%) cuja média de idade era de 35 (± 12) anos verificando uma relação entre mulheres e homens de 2.5. O tempo de diagnóstico médio para os homens foi de 7.8 anos e para as mulheres de 5,3 anos. A média do escore na escala de MIF Total foi de 110,9 (± 17,5). Conclusão: As características epidemiológicas estão em consonância com a maioria dos estudos semelhantes, mas carecem de mais estudos voltados para a avaliação da funcionalidade de indivíduos com EM.
