RESUMEN

With over 80 million people forcibly displaced worldwide, providing safe, healthy, and supportive places for refugees has become an imperative for national governments, aid organizations, and host communities. While much has been written about the needs of these displaced people, organizations and practitioners tend to focus on essential material needs, medical care, and food and water provisioning. Yet a growing body of evidence points to the potential role of social capital - the bonding, bridging, and linking social ties that connect us to one another - as a critical resource for these refugees. We have little data about social capital interventions at individual and community levels to assist with mental health for this vulnerable population, and even less methodical evidence about such interventions' impact. This systematic review analyzes nearly 400 articles to find patterns in the literature on how social-capital-based interventions can improve the mental health of refugees. Within the studies of interventions that met our filtering criteria, the reinforcement or creation of social capital, especially bridging and linking types, serves as a crucial resource to help this vulnerable group. Specifically, our review showed that community and multilevel social capital interventions are key to curbing mental health symptoms among refugees. Given this scanty evidence base among a group so vulnerable to mental health problems, this review serves as an explicit invitation for researchers to further examine social capital interventions among refugees.

AbstractConservation translocations are important in maintaining viable wildlife populations of vulnerable species within their indigenous ranges. To be effective, population restoration efforts (e.g., head start programs) must consider the species' life history, regional ecology, and physiology and the health status of wild and translocated populations. The decline of Blanding's turtles (Emydoidea blandingii) has prompted the initiation of head start programs, but the health and short-term survival of head-started juveniles released to the wild is largely unknown. From May to October 2016 and 2017, we radio tracked captive-reared, recently released juvenile Blanding's turtles and monitored their survivorship and monthly physiological health. We aimed to (1) compare physiological metrics of juveniles before and after release from captivity and between head-started cohorts, (2) identify seasonal trends in physiological metrics of recently released juveniles, (3) compare physiological metrics of recently released and formerly released juveniles, and (4) identify predictors of juvenile survivorship after release from captivity. Juvenile short-term survival was low compared with other studies. Most physiological metrics did not change after release from captivity, negating significant juvenile stress before or after release. Physiological metrics for recently released cohorts varied seasonally, suggesting that these juveniles were likely in good health. Some physiological metrics differed between recently released and formerly released juveniles, demonstrating a potential postrelease acclimatization period. Finally, no physiological metrics significantly predicted survival, but surviving juveniles had a higher percentage of fat. In all, juvenile deaths were not due to poor turtle health but rather to predation from human-subsidized mesocarnivores. Therefore, head-started juvenile Blanding's turtles released in suburban areas may benefit from antipredator training and mesocarnivore control at release sites.

RESUMEN

A 38-year-old man presented to the emergency room in the trauma bay for multiple ballistic injuries to the right neck. He was hemodynamically stable, protecting his airway, and neurologically intact. Computed tomography angiography (CTA) revealed absent filling the right internal carotid artery from its origin to the circle of Willis, which was intact, as well as absent petrous carotid canal on the right. The patient was diagnosed with right internal carotid artery (ICA) agenesis and discharged in several days. This report demonstrates the importance of an in-depth knowledge of vascular embryology and anatomy. The patient has agreed to have images and case details published.

Pickleball, a paddle sport that combines aspects of tennis, badminton, and table tennis, is one of the fastest growing sports in America. The sport is especially popular with older adults as it provides a means for both competition and exercise. Limited research is available on the motivation and perceived benefits of participants in this unique and growing community. A total of 3,012 participants completed a survey instrument that included items from the Sport Motivation Scale, the Task and Ego Orientation in Sport Questionnaire, and the Quality and Importance of Recreational Services. Participants reported being more task oriented than ego oriented, suggesting that pickleball may effectively satisfy the need for persons to be engaged in physical activities that allow for the ongoing development of personal mastery. This finding is supported by respondents noting that their primary motivation to participate in pickleball involved mastering difficult training techniques. However, participants cited competition as the top perceived benefit to their participation. This result supports two possible conclusions that motivation and benefit are different and potentially disconnected constructs or that the benefit of competition among this sample of older persons is understood through the lens of personal mastery, whereby the demonstration of that mastery is only possible through competition.

OBJECTIVES: Few data are available to describe the changes in incidence of pediatric-onset inflammatory bowel disease (IBD). The aim of this study was to describe changes in incidence and phenotypic presentation of pediatric-onset IBD in northern France during a 24-year period. METHODS: Pediatric-onset IBD (<17 years) was issued from a population-based IBD study in France between 1988 and 2011. Age groups and digestive location were defined according to the Paris classification. RESULTS: 1,350 incident cases were recorded (8.3% of all IBD) including 990 Crohn's disease (CD), 326 ulcerative colitis (UC) and 34 IBD unclassified (IBDU). Median age at diagnosis was similar in CD (14.4 years (Q1=11.8-Q3=16.0)) and UC (14.0 years (11.0-16.0)) and did not change over time. There were significantly more males with CD (females/males=0.82) than UC (females/males=1.25) (P=0.0042). Median time between onset of symptoms and IBD diagnosis was consistently 3 months (1-6). Mean incidence was 4.4/105 for IBD overall (3.2 for CD, 1.1 for UC and 0.1 for IBDU). From 1988-1990 to 2009-2011, a dramatic increase in incidences of both CD and UC were observed in adolescents (10-16 years): for CD from 4.2 to 9.5/105 (+126%; P<0.001) and for UC, from 1.6 to 4.1/105 (+156%; P<0.001). No modification in age or location at diagnosis was observed in either CD or UC. CONCLUSIONS: In this population-based study, CD and UC incidences increased dramatically in adolescents across a 24-year span, suggesting that one or more strong environmental factors may predispose this population to IBD.

AIM: To determine whether bacteriological analysis of a wound swab is supportive in the clinical assessment of infection of a chronic wound. METHODS: Patients attending an outpatient wound clinic who had endured a chronic wound for more than 3 weeks were clinically assessed for infection. In addition, standardized wound swabs were taken according to the Levine technique and the microbiological findings of the swabs compared with the clinical assessment of the wounds. RESULTS: There was no significant relationship between the clinical assessments of the chronic wounds and the qualitative or quantitative bacteriological results of the swabs. CONCLUSION: Microbiological analysis of wound swabs taken from chronic wounds to support clinical assessment of the wounds is waste of time and money. It may be preferable to assess chronic wounds clinically, however, validation studies of these signs and symptoms are needed.

The aim of this study was to measure the prevalence of (infected) chronic wounds in Dutch nursing homes and to explore which signs and symptoms are used to diagnose infected chronic wounds. Moreover, it was to determine which structural quality indicators related to chronic wound care at ward and institutional levels were fulfilled. In April 2012, as part of the annual National Prevalence Measurement of Care Problems of Maastricht University [Landelijke Prevalentiemeting Zorgproblemen (LPZ)], a multi-center cross-sectional point-prevalence measurement was carried out together with an assessment of relevant care quality indicators. The prevalence was 4·2%; 16 of 72 (22%) chronic wounds were considered to be infected. Increase of exudate (81·3%; n = 13), erythema (68·8%; n = 11), pain (56·3%; n = 9) and wound recalcitrance (56·3%; n = 9) were considered to be diagnostic signs and symptoms of a chronic wound infection. Although at institutional level most quality indicators were fulfilled, at ward level this was not the case. Despite the relatively low number of residents, we consider our population as representative for the nursing home population. It may be an advantage to appoint specific ward nurses and to provide them specifically with knowledge and skills concerning chronic wounds.

A metagenomic approach was applied using 454-pyrosequencing data analysis for the profiling of bacterial communities in the brine samples of the water reclamation plant. Some physicochemical characteristics of brine samples were also determined using standard methods. Samples ranged from being lightly alkaline to highly alkaline (pH 7.40-10.91) throughout the various treatment stages, with the salinity ranging from 1.62 to 4.53 g L(-1) and dissolved oxygen concentrations ranging from 7.47 to 9.12 mg L(-1). Phenotypic switching was found to occur due to these physicochemical parameters. Microbial diversities increased from those present in Stage I reactor (six taxonomic groups) to those in Reverse Osmosis (RO) stage I (17 taxonomic groups), whereas in the second phase of the treatment, it increased in Stage II clarifier (14 taxonomic groups) followed by a decrease in RO stage II (seven taxonomic groups). Overall, seven phyla were detected, apart from many bacterial sequences that were unclassified at the phylum level. The most dominant phylum found was Proteobacteria accounting for 59% of the total sequences. A blastn sequence similarity search showed that the majority of the sequences (56%) were homologous to the uncultured bacterial species, underlining the vast untapped bacterial diversity.

OBJECTIVE: The goal of this review was to investigate the usefulness of a wound swab (using the Levine or Z technique) in comparison with a biopsy as a reliable method for the diagnosis of a chronic wound infection. METHOD: A literature review using the electronic databases PubMed, CINAHL, and MEDLINE were searched by strategy. A total of 6 articles fulfilled the inclusion criteria. MAIN RESULTS: The Levine technique detects more organisms in acute wounds, as well as in chronic wounds, than the Z technique. Comparing both with the biopsy as criterion standard, the diagnostic accuracy to diagnose a chronic wound infection by the Levine technique was higher in comparison to the Z technique. At a threshold of 3.7 × 10(4) microorganisms per swab, the Levine technique had a sensitivity of 0.90, a specificity of 57%, and a positive predictive value and negative predictive value of 0.77 and 0.91, respectively. Description of the method of swab taking was diverse and not uniform. DISCUSSION: Only a few studies in the literature compare wound swabs with biopsies for the diagnosis of chronic infected wounds. Until now, the Levine technique has been considered as the most reliable and valid method, but there is an urgent need for a well-designed study with a sufficient number of patients to optimize the diagnostic accuracy of chronic infected wounds. CONCLUSION: The best sampling technique for taking a swab has not yet been identified and validated. Until then, the authors recommend the Levine technique.

In a recent study, the theory of pulse generation with fiber optical parametric amplification using sinusoidal (clock) intensity modulated pump was revisited. This work showed that the pulses generated through such parametric interaction exhibit a shape which depends on the signal detuning with respect to the pump position (i.e. linear phase mismatch). A near Gaussian shape can only be achieved over a small region of the gain spectrum, close to the maximum gain location. Towards the extremities of the gain spectrum, the generated pulses take a near Sinc shape which can have many potential applications such as for all-optical Nyquist limited transmitters and/or receivers. In this paper we experimentally verify the theory at repetition rates up to 40 GHz. We also discuss the impact of noise, pump saturation and walk-off on the generated pulses.

BACKGROUND: Lymphomatoid papulosis is a rare lymphoproliferative T cell CD30+ disease with excellent prognosis which affects almost exclusively adult patients, being rarely in the childhood; thus the clinic and pathologic spectrum and the risk of evolution to another type of lymphoma are not well defined in the pediatric group. OBJECTIVE: The aim of this article is to analyze the characteristics of infantile lymphomatoid papulosis and review the literature. MATERIAL AND METHOD: A retrospective study analyzing the characteristics of 9 patients aged up to 18 diagnosed of lymphomatoid papulosis attended in our department from 1995 to 2009 was performed. RESULTS: The study included 7 boys and 2 girls aged between 2 and 17. Pityriasis lichenoides acuta's lesions appeared associated before and after lymphomatoid papulosis' development in 2 and 1 cases respectively. The lesions resolved spontaneously, leaving a postinflammatory hyperpigmentation (77%) or hypopigmentation (23%). The development of varioliform scars occurred in over 77% of cases. Histologically, all cases showed features compatible with type A of lymphomatoid papulosis. Molecular studies showed monoclonality in the 3 cases in which this technique was done. CONCLUSIONS: Infantile lymphomatoid papulosis is a rare entity clinically manifested as the adult form. This lymphoproliferative disease, which is occasionally associated with pityriasis lichenoides acuta, shows features compatible with the type A or histiocytoid pattern in the histological analysis. The development of other lymphoproliferative disorders is less frequent in the infantile form than in the adulthood. The prevalent association among pityriasis lichenoides and lymphomatoid papulosis observed in our analysis, as well as the difficulties which supposed to differentiate between these two pathologies in various cases, suggest that those entities could be part of a common clinical and pathological spectrum.

Introducción: La papulosis linfomatoide es un proceso linfoproliferativo de células T CD30+ poco frecuente y de pronóstico excelente que generalmente afecta a adultos y, en menor medida a niños, por lo que tanto el espectro clínicopatológico como el riesgo de progresión a otro tipo de linfoma en el grupo pediátrico no está bien establecido. Objetivo: Analizar las características de la papulosis linfomatoide infantil a partir de la descripción de nuevos casos y de la revisión de la literatura. Material y método: Se realizó un estudio retrospectivo de 9 pacientes menores de 18 años diagnosticados de papulosislinfomatoide atendidos en nuestro servicio entre 1995 y 2009. Resultados: Se incluyeron 7 niños y 2 niñas de edades entre 2 y 17 años. Las lesiones de papulosis linfomatoide se vieron precedidas en 2 casos y seguidas en 1 de otras compatibles con pitiriasis liquenoide aguda. La resolución de las lesiones fue espontánea, dejando hiperpigmentación (77%) o hipopigmentación postinflamatoria (23%) y cicatrices en el 77% de los casos. Histológicamente todos los casos presentaron el patrón tipo A de papulosis linfomatoide. El estudio molecular mostró monoclonalidad en los 3 casos en los que fue realizado. Conclusiones: La papulosis linfomatoide infantil es una entidad rara que se manifiesta clínicamente como la forma adulta. Esta enfermedad linfoproliferativa, que ocasionalmente se asocia a pitiriasis liquenoide aguda, muestra hallazgos histológicos compatibles con el patrón histiocitoide o tipo A. El desarrollo de otros procesos linfoproliferativos malignos en el seguimiento posterior es menos frecuente en la papulosis linfomatoide infantil comparado con la variante adulta. La frecuente asociación de pitiriasis liquenoide y de papulosis linfomatoide encontrada en nuestro análisis, así como la dificultad que supone en algunos casos el diferenciar entre ambos procesos, permite sugerir que ambas patologías podrían formar parte de un espectro clínico-patológico común (AU)

OBJECTIVE: To compare cognitive impairments in dementia with Lewy bodies (DLB) and Parkinson's disease dementia (PDD), to discriminate between the two entities. METHODS: 10 DLB and 12 PDD consecutive patients performed a neuropsychological battery designed to assess several cognitive domains: verbal and visual memory (Delayed Matching to Sample (DMS)-48), language, gnosia, praxia and executive functions. RESULTS: DLB patients had poorer performances in orientation (p<0.05), Trail Making Test A (p<0.05) and reading of names of colours in the Stroop Test (p<0.05). Their scores were also lower in the visual object recognition memory test (DMS-48), in both immediate (p<0.05) and delayed recognition (p<0.05). No differences were observed in the other tests. CONCLUSION: Despite global similarities in cognitive performances between DLB and PDD patients, we observed important differences: in particular, DMS-48, a test of visual object recognition memory and visual storage capacity, was poorer in DLB patients.

AIM: The aim of this report is to show that eccentric exercise under well-controlled conditions is an alternative model, to chemical and mechanical analyses, and analyse the process of degeneration/regeneration in mouse soleus. METHODS: For this, mice were submitted to a single bout of eccentric exercise on a treadmill down a 14 degrees decline for 150 min and the soleus muscle was analysed at different times following exercise by histology and in situ hybridization in comparison with cardiotoxin-injured muscles. RESULTS: We analyse the regenerative process by detection of the accumulation of transcripts coding for the two myogenic regulatory factors, Myf-5 and MyoD, which are good markers of the activated satellite cells. From 24 h post-exercise (P-E), clusters of mononucleated Myf-5/MyoD-positive cells were detected. Their number increased up to 96 h P-E when young MyoD-positive myotubes with central nuclei began to appear. From 96 to 168 h P-E the number of myotubes increased, about 10-fold, the new myotubes representing 58% of the muscle cells (168 h P-E). CONCLUSION: These results show that this protocol of eccentric exercise is able to induce a drastic degeneration/regeneration process in the soleus muscle. This offers the opportunity to perform biochemical and molecular analyses of a process of regeneration without muscle environment defects. The advantages of this model are discussed in the context of fundamental and therapeutical perspectives.

Given the importance of the myogenic regulatory factors (MRFs) for myoblast differentiation during development, the aims of this work were to clarify the spatial and temporal expression pattern of the four MRF mRNAs during soleus regeneration in mouse after cardiotoxin injury, using in situ hybridization, and to investigate the influence of innervation on the expression of each MRF during a complete degeneration/regeneration process. For this, we performed cardiotoxin injury-induced regeneration experiments on denervated soleus muscle. Myf-5, MyoD, and MRF4 mRNAs were detected in satellite cell-derived myoblasts in the first stages of muscle regeneration analyzed (2--3 days P-I). The Myf-5 transcript level dramatically decreased in young multinucleated myotubes, whereas MyoD and MRF4 transcripts were expressed persistently throughout the regeneration process. Myogenin mRNA was transiently expressed in forming myotubes. These results are discussed with regard to the potential relationships between MyoD and MRF4 in the satellite cell differentiation pathway. Muscle denervation precociously (at 8 days P-I) upregulated both the Myf-5 and the MRF4 mRNA levels, whereas the increase of both MyoD and myogenin mRNA levels was observed later, in the late stages of regeneration (30 days P-I). This significant accumulation of each differentially upregulated MRF during soleus regeneration after denervation suggests that each myogenic factor might have a distinct role in the regulatory control of muscle gene expression. This role is discussed in relation to the expression of the nerve-regulated genes, such as the nAChR subunit gene family. (J Histochem Cytochem 49:887-899, 2001)

RESUMEN

Multicentric thyroid papillary carcinomas were present in two sisters with familial polyposis of the colon; in one the tumor was found at age 29 and in the other at 26 years of age, respectively 13 and 5 years after colectomy. This association is not fortuitous and should lead to regular examination of thyroid gland in patients with familial polyposis or in Gardner's syndrome patients. It might even be wise to search for adenomas of the colon in young patients with thyroid carcinoma.

RESUMEN

A general analysis of the halftone process for nonlinear transformations in optical signal processing is presented. The analysis considers the effects of the nonideal characteristics of the recording medium. The results predict output errors due to different parts of the recording medium characteristic curve for any nonlinear transformation. A synthesis method for a discrete halftone screen density profile is also described. This produces an optimum halftone screen density profile for any form of recording medium characteristic curve and any type of nonlinearity in the sense that it minimizes the mean-square difference between desired and degraded outputs. The results of a computer simulation for logarithmic and level slice functions are given.
