RESUMEN

Carbon fiber implants were used to fill osteochondral defects created on the articular surface of the patella of 36 rabbits, for the purpose of studying the long-term histological changes of the repair process. Six months after surgery the defect was filled by fibrous tissue, where the superficial area was organized parallel to the joint surface. Fibrocartilage developed after 9 months and, after 12 months, the defects were covered by hyaline cartilage tissue.

OBJECTIVE: To analyse the repair of lesions of articular cartilage with periosteum-free implants and follow-up at 6 months. DESIGN: Thirty-six New Zealand rabbits, 4-6 weeks old, were used. Full-thickness articular cartilage defects in the medial femoral condyle were created. Spontaneous evolution occurred in 18 animals; the other 18 animals were treated with a free autogenous periosteal tibial implant fixed with Tissucol. Animals were killed in groups of six at 8, 12 and 24 weeks. Macroscopic, histologic and histochemical results were evaluated and analysed statistically using the Mann-Whitney U-test. RESULTS: The spontaneous evolution of the lesion did not lead to complete repair in any case. The periosteum-free implant provided complete repair of the lesion and statistically significant restoration of the articular surface. CONCLUSIONS: In the rabbit, this study confirms the incomplete spontaneous repair of articular cartilage and the chondrogenic potential of tibial periosteum-free implants, with long-term maintenance of the macroscopic, histologic and histochemical characteristics of neo-cartilage. This raises the possibility of its use as an alternative method in the repair of circumscribed osteochondral lesions in young patients.

RESUMEN

Se diseña un modelo experimental consistente en la realización de una úlcera condral de 6 mm, en la superficie articular de la rótula, en 48 conejos distribuidos en dos series. Mientras en la serie control (24 conejos) dicha úlcera se dejó a evolución espontánea, en la experimental (24 conejos) fue cubierta con plastia libre de periostio tibia¡, para poder investigar la posibilidad de regeneración del cartilago articular. Los resultados se valoraron microscópicamente a intervalos de 1, 2,4 y 8 semanas,comprobando la ausencia de regeneración espontánea y el desarrollo de un cartílago hialino a partir de la plastia de periostio, con lo que se demuestra la capacidad condrogénica de la misma y se sugiere la posibilidad de aplicación clínica en defectos del cartilago articular (AU)

The purpose of this study was to compare the chondrogenic potential of free perichondrial with free periosteal grafts in the resurfacing of full-thickness defects of patellar articular cartilage in rabbits. We used adolescent New Zealand rabbits weighing between 2.4 and 3.6 kg. A 6-mm wide and 3-mm thick defect was created on the patellar articular surface. A total of 30 rabbits were randomly divided into a control group and two test groups. One test group received free perichondrial grafts (PC); the other received free periosteal grafts (PO). All the animals were killed 8 weeks after surgery. All the histological samples were scored from 0 to 17 according to a standard scoring system. Differences in the quality of the regenerated tissue were only found between the control and the test groups. There were no statistically significant histological differences between the grafted defects of the PC and the PO groups that there are not on any of the variables. The results of this study support that there are not significant differences in the quality of the repair tissue when using these two types of biological grafts.

Recently, the demonstration of DAP IV activity in thyroid cells aspirates has been proposed as an useful tool for the diagnosis of malignancy. We have studied the enzymatic activity of DAP IV, using the modified method of Lodja, in a series of 336 selected aspirates of the thyroid gland with the following cytologic diagnosis: 236 nodular hyperplasias, 60 follicular proliferations, eight Hashimoto's thyroiditis, eight Hürthe-cell proliferations, 20 papillary carcinomas, two anaplastic carcinomas, and two medullary carcinomas. The results were subjectively evaluated on the basis of staining intensity and extension in a minimum of 200 cells. Strong-to-moderate enzymatic activity with an extension of more than 40% of the cells were exclusively seen in follicular-cell derived carcinomas (papillary carcinoma, Hürthle-cell carcinoma, and follicular carcinoma). Medullary carcinoma, anaplastic carcinoma, and benign conditions were negative or weakly stained. Cytohistologic correlation in 88 patients operated on showed the following results: 26 nodular hyperplasia (18 nodular hyperplasia and eight follicular adenomas), 36 follicular proliferation (24 nodular hyperplasia, six, adenomas, three papillary carcinomas, three follicular carcinomas), two Hürthle-cell proliferation (one Hürthle-cell adenoma and one Hürthle-cell carcinoma), 20 papillary carcinomas, two medullary carcinomas, and two anaplastic carcinomas. DAP IV staining was moderate to strong and extensive in all malignant tumors initially diagnosed as follicular or Hürthle-cell proliferations. We conclude that DAP IV activity is present in malignant differentiated thyroid tumors of follicular cells (papillary carcinoma, follicular carcinoma, Hürthle-cell carcinoma), but it is identified neither in medullary carcinoma nor in anaplastic carcinoma. Therefore, its usefulness is restricted to the diagnosis of follicular-cell malignancies.

Forty-one cases of typical melanocytic skin lesions (15 intradermal nevi, 14 Spitz nevi and 12 malignant melanomas) were used to investigate the value of staining of nucleolar organizer regions (NORs) in the differential diagnosis of such pigmented lesions. Histologic sections were stained by the silver colloid (Ag) method, with and without the prior use of a melanin blocking agent. There were statistically significant differences in the mean numbers of AgNORs per nucleus between the groups of lesions studied (1.658 for intradermal nevi, 3.0042 for Spitz nevi and 6.669 for malignant melanomas). Sections treated with potassium permanganate (melanin blocking agent) prior to staining showed an obvious increase in the AgNOR scores in all groups; this increase was highest for Spitz nevi. Although AgNOR staining allows a distinction to be made between intradermal nevi and malignant melanomas, the striking overlap between the counts for Spitz nevi and malignant melanomas precludes the use of this technique as the sole method for establishing the diagnosis of malignancy. Other clinical and morphologic data are especially required to make the diagnosis of Spitz nevi.

RESUMEN

Giant cell arteritis (GCA) is a vasculitis which involves in a systemic fashion the mean and medium sized arteries. Intracranial vessels are exceptionally involved. Thus, stroke is an uncommon complication. We report a patient with typical GCA with blindness due to occlusion of the central artery of the retina, who developed an imitation and utilization behavior attributable to bilateral frontal infarction without accompanying neurological symptoms. Although histological study was not available, this case fulfilled all of Russi's criteria for GCA localized in the CNS. This lends support to the suspicion that the bilateral frontal infarction might be due to it.

A case of clinical and dermopathological characteristic cystic lymphangioma is reported. The atypical localization of the tumor and its delayed apparition were pointed out. A relation with any precipitating factor could not be established. Treatment was surgical remove.

RESUMEN

We report a familial case of Lowe's syndrome with histological and ultrastructural examination of the renal biopsy. The patient was an eleven years old boy with operated congenital bilateral cataracts, mental and psychomotor retardation, hyperexcitability, muscular hypotonia, proteinuria, generalised aminoaciduria, proximal tubular acidosis and reduced glomerular filtrate. The renal biopsy showed, in addition to the alterations in the glomerular corpuscle (mesangial proliferation), proximal tubules (atrophy, dilatation, hyalinous or calcerous cylindres and mitochondrial abnormalities) and interstitium (fibrosis, lymphocytic infiltrate), large number of cortical microcysts, many of with corresponded to Bowman's cystic capsules with small glomeruloid projections.
