RESUMEN

Anaerobic nonclostridial infection of pelvis are analysed in 89 patients that were on treatment in the department of festering surgery in 1999-2012 years. Clinical classification of disease is offered and it is applied in practice for dividing of research array into clinically-morphological groups with the aim of the differentiated going near a surgical treatment.

RESUMEN

The results of surgical treatment of 232 injured persons, suffering closed abdominal trauma, were analyzed. In 117 injured persons a standard surgical tactic was applied, and in 115--the improved one the treatment-diagnostic tactic, what have had permitted to reduce the complications occurrence rate from 62.4 to 54.8%, total lethality--from 35.0 to 27.8%.

Our group previously demonstrated a strong association between elevated plasma soluble CD13 enzyme activity and newly diagnosed extensive chronic GVHD (cGVHD) in children. As cytotoxic anti-CD13 Abs have been documented after blood and marrow transplant (BMT) in association with CMV infection and cGVHD, we hypothesized that soluble CD13 contributes to cGVHD pathogenesis by induction of CD13 reactive Abs and that anti-CD13 Abs could be additional biomarkers for newly diagnosed pediatric extensive cGVHD. Using prospectively collected plasma samples from pediatric allogeneic BMT (allo-BMT) subjects with cGVHD and controls without cGVHD enrolled in a large multi-institution Children's Oncology Group cGVHD therapeutic trial, we evaluated whether soluble CD13 correlates with induction of anti-CD13 Abs. We found that CD13 reactive Abs are present in a proportion of patients after allo-BMT, but did not seem to correlate with the presence of soluble CD13. Anti-CD13 Abs also did not meet our criteria as a diagnostic biomarker for cGVHD. These data do not confirm that induction of CD13 reactive Abs is a mechanism for cGVHD in children nor are part of the pathogenesis of cGVHD associated with elevated soluble CD13. The exact role of CD13 in cGVHD remains to be determined.

The hemoglobinopathies represent a genetically heterogeneous group of disorders. Clinically important hemoglobin variants have been increasingly reported in the USA. Consequently, rapid and accurate testing methods are needed to address the growing diagnostic challenges of identifying these variants. To evaluate the utility of the Luminex LabMAP system for hemoglobinopathy testing, we adapted single base primer extension (SBPE) to this platform to detect 11 clinically important hemoglobin variants. Clinical samples from 11 individuals were tested for five beta-globin mutations (C-Harlem, D-Iran, Fannin-Lubbock and Hope) and six alpha-globin mutations (J-Toronto, Hasharon, G-Philadelphia, G-Norfolk, Constant-Spring and Quong-Sze). Two separate multiplexed SBPE assays were developed. Biotinylated amplification products were hybridized to fluorescent microspheres tagged with allele-specific capture probes and analyzed by flow cytometry on the Luminex100 instrument. The median fluorescent intensity (MFI) ranged from 1255 to 7478 fluorescence units (FU) and from 282 to 2609 FU above background for all positive beta-globin and alpha-globin alleles, respectively. Using the highest background MFI + 3 SD as a conservative threshold, MFI values uniformly discriminated wild type from mutant alleles, and genotypes were correctly identified in all samples tested. This pilot study demonstrates the potential application of the Luminex LabMAP genotyping platform to newborn screening for definitive hemoglobinopathy testing.

RESUMEN

During ten-year period 1191 patients with perforative pyloroduodenal ulcer were operated on. Palliative intervention was conducted in 206 patients, conventionally radical one--in 677 and radical--in 308. Postoperative mortality have constituted 5.2%.

The article discusses a complex of methods for the diagnosis of injuries to the duodenum in 18 patients. Laboratory and X-ray findings and laparocentesis with introduction of a "feeling" catheter into the abdominal cavity with lavage of the cavity facilitated the discovery of blood and intestinal contents and allowed the correct diagnosis to be established before the operation. In inspection of the duodenum mobilization after Kocher is insufficient, in rupture of the posterior wall in the region of the inferior horizontal part the duodenum must be mobilized for its whole length. It is advisable that the defect in the duodenum is closed with a double-row suture applied with an atraumatic needle (the first inner row of interrupted sutures). The methods for applying hemostatic sutures suggested by the authors by means of created devices provide reliable hemostasis in combined injuries to the parenchymatous organs. The operation was completed by leaving glove-tube drains. The postoperative mortality was 16.6%.

RESUMEN

The experience with treatment of acute ileus for the 5 years in 1032 patients is summarized. Operated on were 269 patients. The different types of operative interventions with a wide use of intubation, adsorbents, laparostomy were performed. The postoperative lethality was 9.2%.

In examination of 286 patients, it was established that in 78.0% of them after isolated selective proximal vagotomy (SPV) by day 31-35, and after SPV with the draining stomach operations by day 40-45, the indices of homeostasis, functions of the alimentary organs normalize, regeneration of a postoperative scar occurs, the physical working ability restores.

RESUMEN

The results of treatment of 235 sufferers with open (142) and closed (93) intestinal injuries are analysed. One hundred and twenty one patients had isolated, 114-combined intestinal injuries. Suturing of the small, large intestine and other hollow organs was performed in 192 sufferers, resection of the small intestine-in 28, resection of the large intestine-in 5, resection of the large intestine with unloading cecostomy-in 4, right hemicolectomy-in 3, colostomy-in 3. Twenty eight patients developed complications after the operation, 14 (6.4%) died.

Examined were 164 patients 3-7 years after surgical intervention for chronic pancreatitis. The numerous diseases of the alimentary organs and complications leading to reduction and loss of working ability in a significant number of the patients operated on were revealed. Preservation of working ability in this population of patients largely depends on a degree of the use of a rehabilitative (medical, social and vocational) potential.

The investigation of factual terms of temporary invalidity after selective proximal vagotomy without draining operations of the stomach in a number of regions of the Ukrainian SSR has shown their considerable variability (56.4 +/- 13.8 days). Under observation were 178 patients. It has been established that normal indices of homeostasis, functions of the gastro-intestinal tract, regeneration of the postoperative scar and recovery of their physical labour capacity is taking place during 30-33 days. Criteria for the determination of terms of temporary invalidity after selective proximal vagotomy for the ulcer disease of the duodenum have been elaborated.
