RESUMEN

Fifty subjects divided into three groups were studied: Group I: 10 normal adult subjects as controls. Group II: 20 patients with schistosomal hepatic fibrosis (SHF) without ascites (compensated stage). Group III: 20 patients with SHF with ascites. In all these patients plasma atrial natriuretic factor (ANF), serum aldosterone and serum osmolality were determined. There was a significant decrease in the plasma ANF in groups II and III compared to group I. There was a significant increase in the serum aldosterone level in group III compared to groups I and II. No significant difference was noted in serum sodium and potassium levels among the three groups. There was a significant decrease in serum osmolality in groups II and III compared to group I.

Liver biopsies from 20 schistosomal patients in the compensated stage were studied by immunoalkaline phosphatase technique for the relative distribution of T-cell subpopulations. T-cell subsets were defined for OKT3 (+) (pan T-lymphocytes), OKT4 (+) (helper-inducer), and OKT8 (+) (suppressor-cytotoxic) cell by using mouse hybridoma monoclonal antibodies. Sections showed marked pan T-lymphocytic infiltration predominantly in the portal tract within liver lobules and in the granulomatous lesions. Marked relative increment of suppressor/cytotoxic OKT8 (+) lymphocytes over the helper/inducer OKT4 (+) were observed. Alkaline phosphatase activity showed inverse significant correlation with OKT3 (+) and OKT8 (+) % and a direct significant correlation with OKT4 (+) % lymphocytes. This may suggest that suppressor cytotoxic OKT8 lymphocytes may be responsible for the modulation of the granulomata and stabilization of the morbidity state.

RESUMEN

20 patients of schistosomal hepatic fibrosis and splenomegaly (SHF) with and without haematemesis were examined. Typing for HLA-A, B and C antigens in these patients were compared with those of a group of 100 Egyptian controls. The study showed the presence of an association between HLA-A1 and B5 antigens in SHF cases. However, there was no significant association between HLA antigens and SHF cases with haematemesis.

RESUMEN

Thirty-two female mice infected with Schistosoma mansoni and 16 non-infected control mice were studied. They were killed by cervical dislocation, dissected and their ovaries examined histopathologically and histochemically. Ovaries of infected mice showed definite structural damage. No ova, worms or specific granulomata were detected. The study points to a possible immunological mechanism producing such changes simulating those occurring in schistosomal nephropathy. Detection of immune complexes in such organs is recommended.
