RESUMEN

The objective of this study was to perform a review of the pathological features and treatment of intraosseous myofi broma of the jaw. Electronic searches were performed of MEDLINE, via Pubmed (from 1950 to November 2012), EMBASE and Cochrane Wiley, on November 27th, 2012, with no language restrictions. The electronic search yielded 41 hits, of which 27 articles were considered relevant after evaluation of titles and abstracts and underwent eligibility assessment according to the eligibility criteria Of these, 10 did not meet one or more inclusion criteria and were rejected. The 17 articles selected included three retrospective studies, 13 case reports and one case series. These articles reported on a total of 24 cases of solitary myofi broma in the jaw. Intraosseous myofi bromas of the jaw are rare lesions and are part of the wide group of spindle-cell neoplasms and are generally observed in the posterior mandible. It is important to establish the appropriate treatment to avoid unnecessary aggressive therapy. Conservative surgery is the preferred option. Keywords: Myofi broma; Mandibular Neoplasm; Treatment.


O objetivo deste estudo foi realizar uma revisão de literatura sobre miofi broma intraósseo da mandíbula, suas características patológicas e tratamento. Foi realizada uma busca eletrônica nas base de dados MEDLINE via Pubmed (1950 a novembro de 2012), EMBASE e Wiley Cochrane, no dia 27 de novembro de 2012 sem restrição de linguagem. A busca eletrônica resultou em 41 citações, e 27 artigos foram considerados relevantes pela leitura do título e pela avaliação do resumo. Desses 27 artigos, 10 não atenderam um ou mais critérios de inclusão e foram excluídos. Entre os 17 artigos selecionados, havia três estudos retrospectivos, 13 relatos de caso e uma série de casos. Vinte e quatro casos de miofi broma solitário intraósseo da mandíbula foram identifi cados. Miofi bromas intraósseos dos maxilares são lesões raras que fazem parte do grupo de lesões neoplásicas fusiformes e são comumente encontrados na região posterior da mandíbula. É importante estabelecer o tratamento apropriado para evitar terapia agressiva desnecessária. Cirurgia conservadora é a opção desejada.

PURPOSE: The purpose of the present study is to describe a surgical technique for the treatment of condylar fractures using an intramedullary screw. MATERIALS AND METHODS: The treatment of these fractures remains very controversial. At the time of the choice of the surgical treatment, a wide range of accesses and of techniques of reduction and fixation is used, depending on the type of the fracture, the experience of the surgeon, and aesthetical issues. Among the difficulties posed by this type of fracture is the correct repositioning of the fractured condylar portion for which we have developed a technique that uses a neck screw, with intramedullary insertion. Because this is a round-headed screw, it is necessary to install a plate, which may be straight or "L" shape, and is placed perpendicular and superjacent to the screw so that it does not allow the rotation of the condylar portion. RESULTS: Neck screw can be used, resulting in satisfactory stability, for the reduction and fixation of these fractures.

RESUMEN

Fraturas de côndilo são comuns, e a melhor maneira de tratá-las tem sido extensivamente discutida na literatura. Há basicamente dois métodos distintos para tratar fraturas de côndilo, o método funcional e o método cirúrgico. No presente estudo, o paciente foi submetido à anestesia geral para realizar o método de redução aberta com fixação interna. Após ser realizada uma abordagem submandibular para acessar a região da fratura, uma perfuração, usando broca, foi realizada na base do segmento condilar e um parafuso longo (11 mm) foi inserido, deixando uma parte (6 mm) extra-ósseo. Um sulco foi feito na parte lateral do ramo mandibular para alocação da parte extra-óssea do parafuso. Após redução e estabilização do segmento condilar, uma miniplaca de 2.0 mm foi instalada por cima do parafuso deixando a cabeça do parafuso mais inferiormente para maximizar a retenção. O paciente teve uma recuperação pós-operatória rápida, e função a função mastigatória foi reestabelecida...


Mandibular condyle fractures are common and the best approach to treating them has been extensively discussed in the literature. There are basically two different approaches to treatment of condyle fractures: the functional method and the surgical method. In the case described here, the patient underwent general anesthesia for open reduction and internal fixation. After a submandibular approach to access the fracture site, a bur was used to make a perforation in the base of the condylar segment and a long screw (11 mm) was inserted in place leaving a portion protruding from the bone (6 mm). A groove was made in the lateral part of the mandibular ramus to accommodate the extra osseous portion of the screw. After reduction and stabilization of the condylar segment, a 2.0 mm miniplate was installed over the screw leaving the screw head in an inferior position to maximize retention. The patient enjoyed a rapid postoperative recovery and early jaw function was restored in a short period of time...

Objective: The present study aims to develop a histolo-gical descriptive analysis on the repair of bone defects filled with ?-TCP, associated to pure titanium implants into cavities prepared in the calvarial bone of rats. Ma-terials and method: Eighteen male adult rats were ran-domly selected and divided into three groups of six rats each, according to their period of death: 15, 30, and 60 days. Four bone defects were made into each one of the calvarial bones of rats and filled with: (a) ?-TCP, (b) clot, (c) ?-TCP + pure titanium screw, and (d) pure titanium screw. The samples were cut and histologically assessed. Presence and volume of loose connective tis-sue and newly formed bone tissue were analyzed, as well as the presence or absence of remaining material (?-TCP) into the defect. Results: It was verified an acce-leration bone neoformation process in the presence of ?-TCP. Conclusion: The results suggest the biocompa-tibility and osteoconductivity of the biomaterial, even when associated to pure titanium implants.

RESUMEN

PURPOSE: Myofibroma is a rare benign spindle cell neoplasm, and the aim of the present study was to carry out a literature review and present a clinical case of a patient with a myofibroma in the submandibular region and its management. CONCLUSIONS: Diagnosis of myofibroma can be reached by a histopathologic and immunohistochemical analysis and surgical excision is the treatment of choice.

RESUMEN

Introduction: osteoradionecrosis (ORN) of facial bones is a serious complication of radiotherapy. Objective: this literature review was performed to study maxillary osteoradionecrosis, its incidence, classification, risk factors, physiopathology, prevention, and current treatment. Literature review: literature search using Medline was performed for all relevant articles with specific keywords from January 2000 to November 2011 aiming to find current data available on ORN and the paradigm change in physiopathology and prevention of the disease. The literature reviewed showed lack of either controlled or randomized trials, a few prospective studies, and a great amount of retrospective studies. Final considerations: adjunctive hyperbaric oxygen (HBO) therapy may be applied for osteoradionecrosis prevention, before and after dental procedures, with satisfying results. Nonetheless, several studies indicated that HBO therapy is not beneficial.

RESUMEN

PURPOSE: The objective of this paper is to report the clinical case of a patient with a Class II division 1 malocclusion with traumatized central incisors and the treatment option of extracting them, followed by space closure with lateral incisors replacement. CASE DESCRIPTION: A female patient aged 10 years old reported a previous facial trauma due to a bike accident resulting in avulsion and reimplantation of both central incisors when she was 9 years old. The treatment plan included extraction of central incisors, pulling the lateral incisors to the central incisor position and restoring them to improve esthetics. CONCLUSIONS: Extraction of maxillary central incisors is not a usual treatment protocol. However, for patients with traumatized central incisors with internal and external root reabsorption and Class II division 1 malocclusion it might be a good alternative for correction of the increased overjet.


OBJETIVO: O objetivo deste trabalho é relatar o caso clínico de uma paciente portadora de maloclusão Classe II divisão 1 com incisivos centrais superiores traumatizados com reabsorção interna e externa e a opção de tratamento de extraí-los, seguido de fechamento do espaço com a mesialização dos incisivos laterais superiores. DESCRIÇÃO DO CASO: Paciente do sexo feminino com 10 anos de idade relatou trauma facial, após queda de bicicleta, e avulsão e reimplante dos incisivos centrais superiores aos 9 anos de idade. Optou-se pela extração dos incisivos centrais superiores mesializando os incisivos laterais para a posição dos centrais, restaurando-os para ficar esteticamente aceitável. CONCLUSÃO: A extração dos incisivos centrais superiores não é um protocolo de tratamento comum. Porém, para pacientes portadores de maloclusão Classe II divisão 1 com incisivos centrais traumatizados e com reabsorção radicular externa e interna pode ser uma boa solução para a correção do sobrepasse aumentado.

RESUMEN

PURPOSE: To analyze, in vitro, the influence of a quartz-tungsten-halogen (QTH) and a plasma arc (PAC) on the degree of conversion and hardness of a composite resin, as well as the heat generated by the units. METHODS: Transbond XT disks were prepared and light-cured for 10, 20 and 30 seconds with a QTH (Curing Light XL 3000) or with a PAC (Apollo 95E) for 1, 2 and 3 seconds. The composite resin polymerization was evaluated by infrared spectrophotometry (FTIR) and Knoop hardness number (KHN). The temperature at the curing tip was evaluated. The results were analyzed using ANOVA and Tukey test (α=0.05). RESULTS: According to FTIR and KHN, the polymerization of the composite resin was statistically higher with the QTH. The temperature obtained with the QTH 20 s (45.44ºC) and 30 s (45.84ºC) was statistically higher than the QTH 10 s (39.90ºC). The PAC 1 s (27.12ºC), 2 s (28.48ºC) and 3 s (29.96ºC) presented the lowest temperature and did not differ statistically among them. CONCLUSION: Transbond XT light-activated for 10, 20 and 30 seconds with the QTH presented higher degree of conversion and hardness in comparison with those obtained with the PAC for 1, 2 and 3 seconds, and the QTH generated more heat than the PAC.


OBJETIVO: Analisar, in vitro, as fontes luminosas Halógena (QTH) e Xenon (PAC) no grau de conversão e dureza de uma resina composta e também o calor gerado por estas unidades. METODOLOGIA: discos com resina Transbond XT foram fotoativados por 10, 20 e 30 segundos com QTH (Curing light XL 3000) ou com PAC (Apollo 95E) por 1, 2 e 3 segundos. A resina composta foi analisada por meio de espectrofotometria infravermelha (FTIR) e pelo teste de dureza Knoop (KHN). A temperatura na ponta dos aparelhos foi também avaliada. Os resultados foram analisados usando ANOVA e Tuckey test (α=0.05). RESULTADOS: Conforme FTIR e KHN a polimerização do compósito foi estatisticamente maior com QTH. A temperatura obtida do QTH com 20 segundos (45,44ºC) foi estatisticamente maior do que com 10 segundos (39,90ºC). O PAC com 1, 2 e 3 segundos apresentou as temperaturas mais baixas e não diferem estatisticamente entre si. CONCLUSÃO: A resina Transbond XT fotoativada por 10, 20 e 30 segundos com QTH apresentou maior grau de conversão e dureza em comparação com PAC por 1, 2 e 3 segundos. O QTH produziu mais calor que o PAC.
