OBJECTIVE: To identify the Leishmania species being responsible for cutaneous leishmaniasis in Multan. METHOD: Parasites were isolated from clinically and parasitologically confirmed lesions of cutaneous leishmaniasis from 30 patients by fine needle aspiration (FNA). The bioptical materials were then cultured in Evans Tobie's medium and parasites isolated were identified by isoenzyme electrophoresis technique. RESULTS: Successful Leishmania isolates were obtained from 16 patients. All strains were identified by biochemical techniques as belonging to Leishmania tropica zimodeme MON7 variant PGD. CONCLUSION: The causative species was identified as Leishmania tropica.

A case is reported in which a 46-year-old normotensive male patient presented with large right-sided painless abdominal mass. On sonography there was a large cyst in the lower pole of right kidney. Intravenous urography showed distortion of pelvicollecting system and medial shift of upper right ureter. Exploration revealed a cystic mass (18 x 15 cm) located in lower pole of right kidney. It contained brownish fluid and because of suspicion of malignancy radical nephrectomy was carried out. Histological examination showed the cystic mass to be adrenal tissue infiltrated by neoplastic cells. Characteristic organoid nests of malignant cells ("Zellaballen" were present. Cells stained positively for chromogranin, neurone specific enolase (NSE), neurofilament, and synaptophysin. Sustentacular cells showed positive reaction to S-100 protein. No further treatment was offered. Patient remains well at 30 months follow-up. On medline search there is record of three cases of intra-renal paraganglioma in English literature and the present case appears to be the fourth one.

OBJECTIVE: Cutaneous leishmaniasis is endemic in Multan. Therefore this study was undertaken to elucidate the epidemiological features with main emphasis on the geographical distribution of the infection in Multan. SETTINGS: Multan city. Dermatology Outpatient Department, Nishtar Medical College. METHOD: Records of 173 diagnosed patients of cutaneous leishmaniasis were retrospectively reviewed according to the age, sex, number and site of lesions, results of the smear for LD bodies and residential area. RESULTS: The disease was more common in the children and young adults, with male to female ratio of 3:2. The maximum number of lesions were on the lower legs. Sixty five percent of the patients were from the interior old city. CONCLUSION: The main focus of infection is in old interior part of the city with microfoci of the disease scattered throughout the city.

OBJECTIVE: To study the chemical composition of upper renal tract (renal and ureteric) calculi in Multan. SETTING: Department of Urology, Nishtar Hospital, Multan. STUDY PERIOD: September 1992 to February 1999. MATERIAL AND METHODS: A total of 700 renal and ureteric calculi were analyzed by chemical method of Hodgkinson. RESULTS: The commonest were uric acid (28.1%) calculi, followed in frequency by calcium oxalate calculi (26.1%), mixed calculi containing calcium oxalate and uric acid (21.8%) and calculi containing calcium oxalate and calcium phosphate (10.4%). Other variety of calculi were less common. CONCLUSION: Uric acid, calcium oxalate and mixed uric acid and calcium oxalate calculi are the main types in Multan region (JPMA 50:145, 2000).

In order to identify and characterise the organisms responsible for Cutaneous Leishmaniasis, parasites were isolated from active lesions, grown in-vitro cultures and identified by iso-enzyme characterisation. Thirteen isolates from different patients were typed as L. tropica. Seven of these isolates were from Afghan refugees encamped in the suburbs of Islamabad, 3 were from patients in Multan, 1 was from a patient from Azad Jammu and Kashmir and 1 was from Besham (Swat, NWFP). The study confirms the presence of anthroponotic Cutaneous Leishamaniasis caused by L. Tropica in Pakistan.
