RESUMEN

Lethal force training requires individuals to make threat assessments, which involves holistic scenario processing to identify potential threats. Photorealistic targets can make threat/non-threat judgments substantially more genuine and challenging compared to simple cardboard or silhouette targets. Unfortunately, repeated target use also brings unintended consequences that could invalidate threat assessment processes conducted during training. Contextually rich or unique targets could be implicitly memorable in a way that allows observers to recall weapon locations rather than forcing observers to conduct a naturalistic assessment. Experiment 1 demonstrated robust contextual cueing effects in a well-established shoot/don't-shoot stimulus set, and Experiment 2 extended this finding from complex scene stimuli to simple actor-only stimuli. Experiment 3 demonstrated that these effects also occurred among trained professionals using rifles rather than computer-based tasks. Taken together, these findings demonstrate the potential for uncontrolled target repetition to alter the fundamental processes of threat assessment during lethal force training.

Ethical research follows numerous rules and regulations to ensure that all human subjects are protected during the collection and dissemination of research outcomes. Nevertheless, there is often a critical distinction drawn between human subjects research and non-human subjects research (NHSR). The latter can also be described as non-research activities, which typically reduces any oversight even if human subjects are involved. Despite the need to conduct ethical NHSR or non-research activities in an applied setting, there are several ways this determination can be used to circumvent regulatory oversight. In particular, the problem arises because one or more of several key functions become conflated in an applied setting, whereas they would be compartmentalized and independent in controlled or experimental settings. These functions include: 1) ethical oversight; 2) funding; 3) execution; and 4) peer review. The current discussion outlines how NHSR in an applied setting can allow these functions to overlap, and how personnel might stretch the boundaries of ethical conduct even while following existing regulations. As such, the goal is to guide future practices when conducting or reviewing NHSR in an applied setting so that unethical practices do not bias the results.

Military service members encounter numerous stressors that adversely affect their mental health. These pervasive stressors emphasize the need to continually surveil, identify, and mitigate negative factors before they can produce cascading consequences for the individual. The present study utilized a large sample (N = 13,666) to identify several factors that might lead individuals to have poor mental health days in an austere naval operating environment. One quarter of respondents (N = 3,484; 25.49%) indicated that they had 0 poor mental health days in the preceding month, whereas one in eight (N = 1,868; 13.57%) indicated experiencing poor mental health every day in the preceding month. This bimodal distribution allowed for binary logistic regression to determine the relative influence of various factors in identifying individuals who reported significant mental health concerns versus those who did not. Split-half analyses also permitted replication of the data through randomized sampling and dividing data by ship class. Gender emerged as the most prominent predictor of mental health quality with females reporting poorer mental health. Meanwhile, organizational caring (a service member's belief that higher organizational levels cared about them) emerged as a protective factor. Perceptions of caring among the organizational hierarchy depended upon organizational tier; that is, a connection to the larger organization functioned as an even more robust predictor than perceptions that their local and more salient organizational structure (e.g., direct supervisor) cared about them. Taken together, this evidence helps identify factors related to mental health issues that may negatively impact military personnel on active duty. Although the careful assessment and selection of leaders are important for an organization to succeed, selecting senior leaders can prove paramount given their potential to impact the entire organization. While private sector businesses expend substantial resources to recruit and retain senior leaders, the public sector struggles to do so, which in turn impacts the attitudes and retention of employees throughout the enterprise. For example, a recent survey of federal employees in the United States found that most respondents did not believe their senior leaders generated high levels of motivation and commitment in the workforce. Enhancing the senior leader selection process thus represents a prime opportunity to enhance organizational success in the public sector. The current discussion reviews four core topics for senior leader selection: (a) determine organizational needs, (b) agree upon competencies for a senior leader, (c) leverage employees from all organizational tiers via a selection committee, and (d) examine ethical issues in selecting senior leaders. Each topic contains an overview of the relative challenge while drawing a contrast between senior and junior leadership positions, as well as comparisons between the public and private sectors. These challenges are presented alongside best practices that should produce a more effective selection process. Taken together, this combined evidence should enable organizational success by ensuring that the highest quality candidates are selected into senior leadership positions. Many concepts describe how individuals sustain effort despite challenging circumstances. For example, scholars and practitioners may incorporate discussions of grit, hardiness, self-control, and resilience into their ideas of performance under adversity. Although there are nuanced points underlying each construct capable of generating empirically sound propositions, the shared attributes make them difficult to differentiate. As a result, substantial confusion arises when debating how these related factors concomitantly contribute to success, especially when practitioners attempt to communicate these ideas in applied settings. The model proposed here-psychological endurance-is a unified theory to explore how multiple concepts contribute to sustained goal-directed behaviors and individual success. Central to this model is the metaphor of a psychological battery, which potentiates and sustains optimal performance despite adversity. Grit and hardiness are associated with the maximum charge of the psychological battery, or how long an individual could sustain effort. Self-control modulates energy management that augments effort required to sustain endurance, whereas resilience represents the ability to recharge. These factors are constrained by both psychological and physiological stressors in the environment that drain the psychology battery. Taken together, these ideas form a novel framework to discuss related psychological concepts, and ideally, optimize intervention to enhance psychological endurance.

Shooting errors have multi-faceted causes with contributing factors that include sensorimotor activity and cognitive failures. Empirical investigations often assess mental errors through threat identification, yet other cognitive failures could contribute to poor outcomes. The current study explored several possible sources of cognitive failures unrelated to threat identification with live fire exercises. Experiment 1 examined a national shooting competition to compare marksmanship accuracy, expertise, and planning in the likelihood of hitting no-shoot or unintended targets. Experts demonstrated an inverse speed/accuracy trade-off and fired upon fewer no-shoot targets than lesser skilled shooters, yet overall, greater opportunity to plan produced more no-shoot errors, thereby demonstrating an increase in cognitive errors. Experiment 2 replicated and extended this finding under conditions accounting for target type, location, and number. These findings further dissociate the roles of marksmanship and cognition in shooting errors while suggesting that marksmanship evaluations should be re-designed to better incorporate cognitive variables.

Special Operations organizations have recently demonstrated their commitment to enhanced cognitive functioning and improving brain health through the development of a Cognitive Domain. However, as this new enterprise becomes supported by more resources and personnel, a critical question involves what cognitive assessments should be conducted to evaluate cognitive functions. The assessment itself forms a crux in the Cognitive Domain that could mislead cognitive practitioners if not properly applied. Here, the discussion addresses the most important criteria to satisfy in the development of a Special Operations cognitive assessment, including operational relevance, optimization, and speed. Cognitive assessments in this domain must incorporate the following: (1) a task with clear operational relevance to ensure meaningful results, (2) no ceiling effects so that performance can support cognitive enhancement initiatives, and (3) the task itself should impose a minimal time requirement to avoid creating a substantial logistical burden. A dynamic threat assessment task supported by drift diffusion modeling can meet all requisite criteria, while also providing more insight into decision parameters of Special Operations personnel than any currently used test. The discussion concludes with a detailed description of this recommended cognitive assessment task, as well as the research and development steps needed to support its application.

Special Operations medicine must provide highly reliable healthcare under intense and sometimes dangerous circumstances. In turn, it is important to understand the principles inherent to building a High Reliability Organization (HRO). These principles include (1) sensitivity to operations; (2) preoccupation with failure; (3) reluctance to simplify; (4) resilience; and (5) deference to expertise. Understanding them is crucial to turning good ideas into sound practical benefit in operational medicine. A prime teaching opportunity involves an interesting coincidence that occurred during the emergence of HROs. Specifically, United States Special Operations Command (USSOCOM) adopted five Special Operations Forces (SOF) Truths that contribute to success in Special Operations, including (1) humans are more important than hardware; (2) quality is better than quantity; (3) SOF cannot be mass produced; (4) competent SOF cannot be created after emergencies occur; and (5) most Special Operations require non-SOF support. These five Truths have more in common with the five HRO principles than merely quantity. They describe the same underlying ideas with a key focus on human performance in high-risk activities. As such, when presented alongside the five HRO principles, there is an opportunity to improve the overall health and performance of SOF personnel by integrating these principles across the range of Special Operations medicine from point of injury care to garrison human performance initiatives. The following discussion describes in greater detail the five HRO principles, the five SOF Truths, and how these similar ideas emerged as more than just a useful coincidence in illustrating the key concepts to produce high performance.

ABSTRACT: Jensen, AE, Bernards, JR, Hamilton, JA, Markwald, RR, Kelly, KR, and Biggs, AT. Do not shoot me: potential consequences of force-on-force training modulate the human stress response. J Strength Cond Res 37(9): 1761-1769, 2023-Close-quarters combat (CQC) engagements trigger the "fight-or-flight" response, activating the sympathetic nervous system and hypothalamic-pituitary-adrenal axis in response to perceived threats. However, it has yet to be shown if a force-on-force (FoF) CQC training environment will lead to adaptations in the physiological stress response or performance. United States Marines and Army infantry personnel participated in a 15-day CQC training program. The CQC program focused heavily on FoF training with the use of nonlethal training ammunition (NLTA). Data collections occurred on training days 1 and 15, during a simulated FoF-hostage rescue (HR) scenario and photorealistic target drill. For the FoF-HR, subjects were instructed to clear the shoot house, rescue the hostage, and only shoot hostile threat(s) with NLTA. The photorealistic target drills were similar, but replaced the role players in the FoF-HR with paper targets. Salivary alpha-amylase (sAA) and salivary cortisol were obtained immediately before entering and exiting the shoot house. Time to completion significantly decreased, between days 1 and 15, for both the FoF-HR and the photorealistic drills by 67.7 and 54.4%, respectively ( p < 0.05). Analyses revealed that the change in sAA, nonsignificantly, doubled from day 1 to 15 during FoF-HR ( p > 0.05), whereas the change in sAA decreased during the photorealistic drills across days ( p < 0.05). Cortisol was significantly higher during the FoF-HR in comparison to the photorealistic drills ( p < 0.05). These data suggest that potential consequences of FoF training heighten the stress response in conjunction with enhanced performance.

BACKGROUND: Our institution was experiencing a respiratory therapy staffing crisis during the COVID-19 pandemic, in part due to excessive workload. We identified an opportunity to reduce burden by limiting use of 3% hypertonic saline and/or N-acetylcysteine nebulizer therapies (3%HTS/NAC). METHODS: Leveraging the science of de-implementation, we established a policy empowering respiratory therapists to discontinue 3%HTS/NAC not meeting the American Association for Respiratory Care (AARC) Clinical Practice Guideline: Effectiveness of Pharmacologic Airway Clearance Therapies in Hospitalized Patients. After a 3-month period of educating physicians and advanced practice practitioners the policy went to into effect. Outcomes measured included monthly number of treatments, orders, and full-time employees associated with administering nebulized 3%HTS/NAC. RESULTS: Post policy activation, the monthly mean 3%HTS/NAC treatments were significantly reduced to 547.5 ± 284.3 from 3,565.2 ± 596.4 (P < .001) as were the associated monthly mean of full-time employees, 0.8 ± 0.41 from 5.1 ± 0.86 (P < .001). The monthly mean 3%HTS/NAC orders also fell to 93.8 ± 31.5 from 370.0 ± 46.9 (P < .001). Monthly mean non-3%HTS/NAC treatments remained stable; post policy was 3,089.4 ± 611.4 and baseline 3,279.6 ± 695.0 (P = 1.0). CONCLUSIONS: Implementing a policy that empowers respiratory therapists to promote adherence to AARC Clinical Guidelines reduced low-value therapies, costs, and staffing needs.

The United States Navy is a high-reliability organization that must maintain optimum performance under challenging conditions. One key challenge for sailors is obtaining sufficient sleep, which can lead to fatigue and other outcomes that compromise operational readiness. Identifying sleep issues and their causes is critical for military leaders to care for their personnel, and to make informed, risk-based operational decisions. Though previous studies in shipboard environments have implicated factors responsible for insufficient sleep (e.g. poor sleep environment and work demands), there has been less research into characterizing the complex interplay among such factors in relation to sleep and work-related fatigue outcomes. This study seeks to address this gap. Data were drawn from the Afloat Safety Climate Assessment Survey of 7617 sailors from 73 ships. The survey included demographic characteristics and measures of crew endurance (e.g. sleep, occupational impairment due to fatigue). Descriptive analyses characterized the presence and severity of sleep issues across subpopulations and operational settings (e.g. the type of ship); structural equation modelling techniques characterized and quantified the statistical associations among factors. The results indicate that sleep deficits are widespread, holding across subpopulations and operational settings. Though sleep deficits varied across subpopulations, no group obtained an average of more than 7 hr of sleep per night. Fatigue-induced occupational functional impairment was directly related to sleep deficiency, and sleep environment and job-related factors were contributors to sleep deficiency. Moreover, job-related factors emerged as potentially more consequential. Lastly, factors may exist aboard a ship that could help promote better sleep.

Asymptomatic transmission complicates any public health strategies to combat a pandemic, which proved especially accurate in the case of COVID-19. Although asymptomatic cases are not unique to COVID-19, the high asymptomatic case rate raised many problems for developing effective public health interventions. The current modeling effort explored how asymptomatic transmission might impact pandemic responses in four key areas: isolation procedures, changes in reproduction rate, the potential for reduced transmission from asymptomatic cases, and social adherence to public health measures. A high rate of asymptomatic cases effectively requires large-scale public health suppression and mitigation procedures given that quarantine procedures alone could not prevent an outbreak for a virus such as SARS-CoV-2. This problem only becomes worse without lowering the effective reproduction rate, and even assuming the potential for reduced transmission, any virus with a high degree of asymptomatic transmission will likely produce a pandemic. Finally, there is a concern that asymptomatic individuals will also refuse to adhere to public health guidance. Analyses indicate that, given certain assumptions, even half of the population adhering to public health guidance could reduce the peak and flatten the curve by over 90%. Taken together, these analyses highlight the importance of taking asymptomatic cases into account when modeling viral spread and developing public health intervention strategies.

INTRODUCTION: Vaccine technology has improved substantially since the first smallpox vaccine, developed more than 200 years ago. As technology improves, vaccines can be produced more safely and reliably for many different pathogens. A recent breakthrough saw the first full deployment of mRNA vaccines to fight a pandemic. Despite the technological and logistical feat of developing a viable vaccine in an abbreviated time frame, there have been many questions about this new approach to vaccine development. The current review will provide descriptions about different types of vaccines as well as answers to some common questions about mRNA vaccines. The purpose is to provide military medical professionals with the information needed to better convey the importance and function of these new vaccines to service members. MATERIALS AND METHODS: There were no explicit inclusion or exclusion criteria for articles describing mRNA vaccine technology. References included here were intended to illustrate important principles or empirical evidence in demonstrating the safety, efficacy, and function of mRNA vaccines. DISCUSSION: The review describes three different types of vaccines: whole-pathogen, subunit, and nucleic acid. Each vaccine type has different implications for the development and production of a vaccine line. For example, whole-pathogen and subunit vaccines often require growing significant amounts of the vaccine sample in laboratory before the material can be incorporated into the vaccine. Nucleic acid vaccines instead provide cells the opportunity to produce key proteins without needing to reproduce the virus and attenuate it in a laboratory setting. This approach has a notable advantage of speed in moving from genome sequencing to vaccine production, but it also creates some potential confusion. The discussion covers three questions with regard to this confusion. First, was the vaccine developed too quickly? Speed here is a byproduct of the new technology and unprecedented government interdepartmental cooperation. No steps were skipped in development or production. Second, does the vaccine modify DNA? No, the mRNA vaccines never enter the cell nucleus and therefore cannot modify DNA. The discussion clarifies how mRNA enters cells and produces the key proteins required to stimulate an immune system response. Third, how long will immunity last? Because mRNA vaccines are new, long-term immunity cannot be projected without significant further study. Still, the discussion does cover issues in determining vaccine efficacy in clinical laboratory trials versus field effectiveness in the real world. CONCLUSIONS AND FUTURE USES: These mRNA vaccines are the newest and most sophisticated defensive tool military medicine has against emerging biological threats. Evolving dangers, such as synthetic biology and engineered pathogens, further enhance the importance of having defensive countermeasures that can be rapidly deployed in response. Current evidence suggests high safety and effectiveness for a biological countermeasure, decades in the making, and military medical personnel should feel confident using and recommending this technology to ensure force health protection.

Despite the importance of being both fast and accurate in lethal force decisions, there is little empirical evidence to identify how speed impacts threat-related decisions and perception. Two experiments used speeded and unspeeded manipulations to determine how the speed imperative impacted threat assessments. Experiment 1 used drift diffusion modeling to quantify decision parameters, including rate of information processing, decision threshold, bias, and non-decisional processes. Speeded conditions reduced the information threshold needed to make decisions and shortened non-decisional processes, yet this manipulation had no impact on the rate of information processing or starting bias. Experiment 2 explored perceptual differences in threat assessment. Participants confidently made threat assessments despite only 30 ms exposure to stimuli with little impact on their subjective threat ratings based on exposure duration. Taken together, these results document the influence of speed on decision-making parameters of threat assessments while demonstrating little impact on threat perception.

Hyperbaric oxygen therapy is emerging as a potential treatment for critical medical and psychological issues, including mild traumatic brain injury, traumatic brain injury, and post-traumatic stress disorder. Based on the promising results from numerous case studies, randomized clinical trials generated conflicting interpretations despite frequent improvements in patient symptoms. The primary debate concerns whether the therapeutic benefits could be attributed to placebo effects or sham conditions that actually induce a therapeutic state. In part, the contention has been exacerbated by experimental designs which could not properly account for extraneous variables, such as the potential for differing patient expectations to influence the outcome. The current discussion addresses five methodological challenges that complicate any determination of clinical significance due to experimental design. These challenges include: 1) not properly addressing or controlling patient expectations prior to the experimental sessions; 2) the challenge of experimental masking in clinical designs that require pressurized environments; 3) patient subjectivity in the primary dependent variables; 4) potential fluidity in patient symptoms or data, such as regression to the mean; and 5) the potential for nocebo effects to exaggerate treatment benefits by lowering performance expectations during pre-treatment assessments. Each factor provides an influential means by which placebo effects could complicate results and prevent the combined data from reaching a threshold of clinical significance. The discussion concludes with methodological best practices with which future research could minimize placebo effects and produce more conclusive results.

INTRODUCTION: Emerging biological threats represent a serious challenge for force health protection (FHP). Against a novel biological threat, medical countermeasures are the first line of defense. However, as exposed by global pandemic conditions, there are significant complications when administering medical countermeasures against novel threats. One such limitation involved the lack of any guiding structure to discuss and deliberate upon the relative value of employing different countermeasures either alone or in tandem. For example, both personal protective equipment and prophylactic medication can provide some protection, but how are individual protections weighed against operational capabilities and FHP initiatives? The goal of this review is to provide a hierarchical organizing structure to the different medical countermeasures available in response to emerging biological threats. MATERIALS AND METHODS: This review used no explicit inclusion or exclusion criteria for its literature review. References are provided for illustrative purposes to represent different components of the medical hierarchy. DISCUSSION: The hierarchy presented here is organized around a resource-durability structure that functions as a push-pull mechanism. Specifically, lower levels of the hierarchy require more resources to sustain FHP conditions while also providing less durable protection. These countermeasures require continual reapplication, and so resources become conceived as much in time and effort to apply as in exhaustible supplies. Higher-order countermeasures require less resources investment as a single application can provide weeks, months, or years of protection. Moreover, higher-order protections are less likely to interfere with military operational capabilities, which further support their classification as superior countermeasures to sustain FHP. The five levels of the hierarchy are presented here as follows, organized from lowest to highest value as a medical countermeasure: (1) Sanitization, (2) personal protective equipment, (3) prophylaxis, (4) vaccines, and (5) optimized immune system. Rationale and descriptions identify how each higher-order countermeasure is superior to its lower-order counterparts while noting that optimizing FHP will likely require employing multiple countermeasures at different levels. The discussion concludes with an overview as to how different countermeasures were employed to various degrees during a novel global pandemic. CONCLUSIONS AND FUTURE USES: Identifying medical countermeasures is important to optimizing FHP. Different countermeasures have different advantages, and the hierarchy distinguishes between inferior and superior countermeasures through the push-pull style mechanism of resource-durability assessment. Future deployment and development should focus on superior countermeasures to maximize medical protections and operational readiness while understanding the relative value and complications inherent with different countermeasures.

Hyperbaric oxygen therapy is an existing and approved treatment to address multiple medical conditions, including decompression sickness, air or gas embolism, carbon monoxide poisoning, and profound blood loss when transfusion cannot be accomplished. However, recent efforts have emerged to promote hyperbaric oxygen therapy for other purposes. The most controversial applications have been utilizing this therapy as a treatment for mild traumatic brain injury and post-traumatic stress disorder. As evidence accumulates and the debate continues about whether published studies have satisfied the threshold of clinical significance, a common issue is raised regarding current clinical practices and health insurance coverage as allowed or recommended by the Department of Defense and other federal agencies. This review describes the current federal policies regarding medical insurance issues for providers and clinical practice guidelines as they pertain to alternative uses of hyperbaric oxygen therapy. First, the current policies are explored for what is reimbursable under federal insurance as approved clinical or research usages. Second, these policies are compared to the clinical practice guidelines to determine what might be clinical best practice versus exploratory research. Third, the evidence from government reports is reviewed as supporting documentation for these positions. As such, the current discussion addresses what can and cannot be covered under health insurance and where various federal health care organizations stand currently on using hyperbaric oxygen therapy as an alternative therapeutic technique. The primary goal is informing military healthcare practitioners and prospective patients about the treatment options available to them under current federal guidelines.

Previous work has demonstrated a link between cognitive abilities, specifically inhibitory control and lethal force decision-making performance. However, many previously used approaches to simulating shoot/don't shoot scenarios have lacked ecological validity. There is a need to investigate how inhibitory control impacts shoot/don't decisions using realistic simulations to better translate the findings to military and law enforcement settings. This study used multiple cognitive control tasks incorporating discrete judgements in go/no-go and stop signal tasks as well as subjective judgements in go/no-go tasks with both colour stimuli and emotional faces. These combined tasks provided a comprehensive evaluation of inhibitory control abilities. To ensure ecological validity in shooting performance, existing military training scenarios incorporated realistic weaponry and aiming behaviours across different shoot/don't-shoot simulations. The inhibitory control battery identified five principal components from the various tasks, including: stopping ability, response speed, emotion detection, colour detection, and emotional biases. These principal inhibitory control components were entered into hierarchical linear regressions with the dependent variables of unintended casualties inflicted and lethal rounds fired, respectively. Stopping ability better predicted the likelihood of inflicting an unintended casualty, whereas response speed better predicted the number of lethal rounds fired. These regression models included baseline metrics of marksmanship and shots fired, which supports a role for inhibitory control above and beyond basic shooting abilities or strategy. These collective findings provide mechanistic support for the relationship between inhibitory control and errors in shoot/don't-shoot decision-making while using realistic military training scenarios.
