RESUMEN

INTRODUCTION: Pectus excavatum is a frequent anomaly. It represents a challenge for adjuvant radiotherapy in the conservative treatment of breast cancer. Primary objective of this study is to compare dosimetric outcomes, normal tissue complication probability (NTCP), and integral dose using four radiation techniques. Secondary objective is to describe acute toxicity and setup errors. METHODS AND MATERIALS: A 57-year-old female patient with an inner quadrant, left breast, ductal carcinoma in situ, was identified. Whole breast was prescribed with 50 Gy in 25 fractions. Boost planning target volume (PTV) was prescribed with 60 Gy in 30 fractions for sequential boost (SB) plans or 57.5 Gy in 25 fractions in the simultaneous integrated boost (SIB) plan. All plans were normalized to deliver 47.5 Gy to 95 % of the breast PTV. Daily image-guided radiotherapy (IGRT) was performed. Setup deviations were described. RESULTS: Constraints were not accomplished for heart when using intensity modulated radiotherapy (IMRT) + SB or conformal radiotherapy with three photon fields and SB. Left lung constraint was not achieved by any of the techniques in comparison. IMRT + SIB and conformal photons and electrons + SB plan were closer to the objective. Integral doses were lower with IMRT for heart and ipsilateral lung; however, it were higher for contralateral breast and lung. Coverage and tumoral conformity indexes were similar for all techniques in comparison. Greater inhomogeneity was observed with the photons and electrons + SB. IMRT + SIB treatment was administered daily with grade I skin toxicity. The highest setup error was observed in Y direction. CONCLUSION: Planning target volume coverage was similar with the four techniques. Homogeneity was superior with both IMRT plans. A good balance between dose constraints for organs at risk, PTV coverage, homogeneity, and NTCP was observed with IMRT + SIB. The documented daily setup error justifies the use of online IGRT (AU)

RESUMEN

This article describes the optimization of a patient telemonitoring platform based on the ISO/IEEE11073 (X73) standard to enable medical device interoperability. In order to achieve this, principal advantages and remaining improvements are evaluated to include in further upgrades towards the new profile evolution, oriented to ubiquitous environments and wearable devices (Personal Health Devices, X73-PHD), and opened to additional plug-and-play features and remote management. After evaluating the possibilities, we describe the platform porting process, a required step to adapt it to the new functionalities and allowing the development of end-to-end standard based systems. The paper details the implementation of the agent-manager architecture, particularized on the X73-PHD communication protocol between a Medical Device (MD) and a central gateway (Compute Engine, CE). Lastly, the obtained results are evaluated, oriented to constitute an X73-PHD tester to prove the challenges currently under discussion in the European Standardization Committee (CEN).

A growing body of evidence has demonstrated that reperfusion injury may be mediated, in part, by mitochondrial Ca2+ overload that promotes non-selective permeability of the inner membrane. In this regard it is known that mitochondria from hypothyroid rats are resistant to membrane damage as induced by Ca2+. The purpose of this study was to evaluate the sensitivity of hearts from hypothyroid rats, to the damage by reperfusion, after an ischemic period of 5 min. The results were compared with those from control and hyperthyroid rats. Hypothyroidism was established by surgical removal of the thyroid gland; in turn hyperthyroidism was induced after a daily injection of 2 mg/kg of 3,5,3'-triiodothyronine for 4 days. ECG tracings from hypothyroid rats showed a total absence of post-reperfusion arrhythmias conversely to what was observed in control and hyperthyroid rats. The release of creatine kinase and aspartate amino transferase to the plasma in hypothyroid rats was found to be lower than that found in hyperthyroid and euthyroid rats. The histological studies showed that myocardial fibers from hypothyroid rats were in good condition and retained their striae and a remarkable near absence of edema was clearly observed.
