RESUMEN

The information about foundation, development and main areas of activity of All-Russian Service for Disaster Medicine is given. Almost 20 years professional staff members help to save lives and health of people injured in emergency situations in Russia and other countries. There are 81 territorial centers for disaster medicine. The main center is All-Russian center for disaster medicine Zaschita. Its organizational structure, performance indexes including development of information and communication technology, telehealth, organization of firs-aid in a traffic collision are considered. The main ways of improvement of the whole service for disaster medicine including service for disaster medicine of Ministry of Defense are shown.

RESUMEN

An analysis of in-patients' flow in the surgery sections of main military clinical hospitals shows that according to the experts' assessment, 89.3% patients were accommodated according to the profile of their illness. 5% patients needed accommodation in other sections (not surgical) and 5.7% should have been treated as out-patients in a military sanatorium (so called unreasonable hospitalization). Trauma sections were most often overloaded with patients that should be placed somewhere else (3.1%) but in the sections of purulent vascular and cavity surgery the number of misplaced patients was almost 10 times less--0.3%, 0.4%, 1.9% respectively. Elder people represent the majority of unreasonably hospitalized patients.

RESUMEN

Taking into account the lack of science-reasonable data about average number of days of bed capacity in year and average number of days of various patient treatment in the hospital, the attempt is undertaken to substantiate these parameters in reference to DM of RF central military medicine institutions. In result of lead research it was established, that average terms of bed capacity in central military hospitals during the last 5 years were 310 days. Average patient terms of treatment differ essentially depending on age and contingent, that is necessary to take into account to define hospital bed requirement norms.

RESUMEN

The experience of combat actions, and especially of the wars in century XX proves that specialized medical care occupies an important place in the system of medical and evacuation support. Some elements of the specialized medical care have been originated even during the wars of centuries XVIII-XIX, but they had appeared under the influence of current situation and by the initiative of non-government organizations or single persons. Specialized medical care have obtained strict organizational forms, and was formatted as scientifically developed system only during the years of the Great Patriotic war 1941-1945. The basic trends in its development were: the appearance of different types of specialized medical care and formation of specialized medical establishments (sections) on the basis of Army or Front hospital facilities (AHF & FHF). There were certain differences in AHF and FHF specialized medical care. Specialization in AHF was realized by temporal attachment of specialized groups form of a separate company of medical re-enforcement; as for FHF, medical specialists were assigned to the staff of military hospitals.

RESUMEN

In the work of a local medical holding unit a great role is assigned to the evacuation sections which carry out the admission of wounded and their transportation to the rear hospitals. The specificity of its tasks exerts influence on the scheme of its deployment and functional principles. The results of these studies, as well as the experience of military medical exercises prove that is rational to deploy evacuation (rail) section at a single site. The important elements in the work of evacuation section are: medical triage and its readiness for immediate medical care.
