RESUMEN

Professional practice guidelines (PPGs) are intended to promote a high level of professional practice and serve as an educational resource, providing pragmatic guidance in a clinical area for psychologists. Measurement-based care (MBC) is an evidence-based psychological practice with accumulating empirical support and alignment with patient-centered care. In connection with the American Psychological Association's Advisory Committee for Measurement-based Care and the Mental and Behavioral Health Registry, this article outlines various lines of support for the development and implementation of an MBC PPG. In addition to research evidence, we address the demonstrated need of this guideline across three domains: public benefit, professional guidance, and legal and regulatory issues. Consistent with the aspirational spirit of a PPG, this article proposes a draft PPG statement and highlights how an MBC PPG would improve service delivery, facilitate implementation of an evidence-based practice associated with symptom reduction, improved retention, and greater patient satisfaction, as well as create a framework that will better align changes in reimbursement models with patients' and providers' treatment goals. Current cognitive assessments suffer from limited scalability and high user burden. This study aimed to (1) examine the relationship between a brief eye-tracking-based visual paired-comparison (VPC) and gold standard cognitive assessments, (2) examine longitudinal stability of the VPC task, (3) determine the ability of the VPC task to differentiate between cognitively normal (CN) individuals and individuals with mild cognitive impairment (MCI). Fifty-five adults (n = 44 CN, n = 11 MCI; 56.4 ± 26.7 years) were tested on two occasions, separated by at least 14 days. Visit 1 included VPC, Montreal Cognitive Assessment (MoCA), Digit Symbol Coding test (DSC), and NIH Toolbox Cognitive Battery (NIHTB-CB). Visit 2 included VPC, DSC, NIHTB-CB, and dual-task (DT). Significant differences existed between baseline VPC scores for CN and MCI groups (p < .001). VPC scores remained stable over time in both groups (p < .05). Significant associations existed between VPC and MoCA (p < .01), DSC (p < .001), and various NIHTB-CB subtests at both time points. The VPC test significantly predicts cognitive outcomes (p < .05), with age and VPC being the only significant predictors. Additionally, area under the curve (receiver operator characteristic = 0.80) for VPC scores demonstrated good classification accuracy. VPC reliably predicted cognitive status while remaining stable over time and displayed significant associations with gold standard cognitive assessments. VPC is a less burdensome and more scalable assessment than traditional tests, enabling longitudinal monitoring of cognitive status in resource-limited environments.

Many older adults report difficulty performing one or more activities of daily living. These difficulties may be attributed to cognitive decline and as a result, measuring cognitive status among aging adults may help provide an understanding of current functional status. The purpose of the present investigation was to determine the association between cognitive status and measures of physical functioning. Seventy-six older adults participated in this study; 41 were categorized as normal memory function (NM) and 35 were poor memory function (PM). NM participants had significantly higher physical function as measured by Short Physical Performance Battery (SPPB; 9.4 ± 2.2 vs. 8.4 ± 2.0; p = .03) and peak velocity (0.67 ± 0.16 vs. 0.56 ± 0.19; p = .04) during a quick sit-to-stand task. Dual-task walking velocities were 22% and 126% slower between cognitive groups for the fast and habitual trials, respectively when compared to the single-task walking condition. Significant correlations existed between measures of memory and physical function. The largest correlations with memory were for peak (r = 0.42) and average (r = 0.38) velocity. The results suggest a positive relationship between physical function and cognitive status. However, further research is needed to determine the mechanism of the underlying relationships between physical and cognitive function.

The COVID-19 pandemic has created an unprecedented worldwide public health concern. Characterized by rapid and high frequency human-to-human transmission, the World Health Organization has recommended implementation of public health measures, including isolation of all suspected infectious individuals for a 14-day quarantine period, while governments have introduced "social distancing" and "lock-downs" of varying severity to curtail COVID-19 spread. Recent COVID-19 research further suggests there are major sleep problems and psychological disorders (e.g., stress, anxiety, depression) associated with the reduction of movement and activities, as well as the reduced social interaction. There have been no studies examining the effect of physical activity at home during such periods of isolation. However, based on previous research, potential tactics to overcome these negative effects include home-based exercise, exergaming, dancing to music, and participation in yoga. Adults should accumulate at least 150 min of moderate-intensity and at least 75 min of vigorous-intensity of activity divided in to 5-7 sessions per week. This training volume could be reduced by 30% for children and adolescents if replaced by recess or active play in and around the home. Additionally, exercises should be adapted to the fitness level of the participant and a progressive model of intensity and training volume should be utilized, preferably monitored by telephone applications and wearable sensors.

Ramadan observance is characterized by several changes in behaviors, such as food and sleep, which could affect physical and cognitive performance. The aim of the present study was to investigate the effects of a 35-min nap (N35) opportunity on physical performance during the 5-m shuttle run test (5mSRT); attention; feelings; mood states; and perceptual measures of stress, fatigue, and muscle soreness during Ramadan observance. Fourteen physically active men (22 ± 3 years, 177 ± 4 cm, 76 ± 5 kg) were tested after a no-nap condition (N0), N35 15 days before Ramadan (BR), the last 10 days of Ramadan (DR), and 20 days after Ramadan (AR). Measures included the digit cancellation test (attention estimation), the profile of mood state (POMS), and the Hooper questionnaires. After a 5-min standard warm-up, participants performed the 5mSRT (6 × 30 s with 35 s in between; best distance (BD), total distance (TD), and fatigue index (FI) were recorded), along with the rating of perceived exertion (RPE) after each test repetition. After the 5mSRT test, participants responded to the feeling scale (FS). The results showed that TD and FI during the 5mSRT were not affected by Ramadan observance. However, BD was significantly lower than DR compared to AR after N0 (∆ = -4.3 ± 1.3%; p < 0.01) and N35 (∆ = -2.6 ± 1.0%; p < 0.05). After N0, attention decreased significantly at DR in comparison with BR (p < 0.05) and AR (p < 0.001). BD and TD improved after N35 compared to N0 at BR (∆ = +4.4 ± 2.1%, p < 0.05 for BD and ∆ = +4.8 ± 1.6%, p < 0.01 for TD), DR (∆ = +7.1 ± 2.2%, p < 0.05 for BD and ∆ = +5.1 ± 1.6%, p < 0.01 for TD), and AR (∆ = +5.5 ± 1.5%, p < 0.01 for BD and ∆ = +5.2 ± 1.2%, p < 0.001 for TD). A significant increase in attention was observed after N35 in comparison with N0 at DR (p < 0.01) and AR (p < 0.01). However, no changes were found for the perception of mood states, stress, sleep, muscle soreness, and the FI during the 5mSRT. Also, N35 was better than N0 for RPE at DR (p < 0.05), feelings at AR (p < 0.05), and fatigue estimation at AR (p < 0.01). A 35-min nap opportunity may have beneficial effects on physical and cognitive performances before, during, and after Ramadan.

This study examined the effects of different nap durations on attention and physical performance as well as mood states, sleepiness, perceived exertion (RPE), recovery (PRS), and muscle soreness (DOMS) in trained men. Fourteen amateur team sport players (age: 20.3 ± 3.0 years, height: 173.1 ± 6.7 cm, body-mass: 68.1 ± 6.6 kg) performed a maximal voluntary isometric contraction (MVIC) test, 5-m shuttle run, and the digit-cancellation (i.e., attention) test after a no-nap (N0) and 40-min (N40) and 90-min (N90) of nap opportunities. Subjective measurement of mood states, RPE, PRS and DOMS were determined. Compared to N0, both nap durations enhanced attention, MVIC, total distance (TD), and higher distance (HD) (p < 0.001), with a higher gain after N90 compared to N40 for attention (Δ = +3), MVIC (Δ = +30N) and TD (Δ = +35m) (p < 0.001). Total mood scores were better after N40 and N90 compared to N0 (p < 0.05), with lower scores after N90 compared to N40 (p < 0.05). DOMS and RPE scores were significantly lower and PRS was significantly higher after N40 and N90 compared to N0 and after N90 compared to N40 (p < 0.05). Although both nap opportunity durations were beneficial, N90 was better than N40 for improving physical performances and attention as well as the perception of recovery, reducing fatigue perception, muscle soreness, and negative mood states.

Alzheimer's Disease (AD) incidence is increasing and with no disease modifying agents available, preventative measures through lifestyle factors are being investigated. Combined with the prevention of AD risk factors such as heart disease, diabetes, and with more recent evidence, microbiome dysfunction, there is a substantial foundation for diet as a modifiable risk factor and preventative measure for AD. Recent evidence suggests AD associated pathologies, such as oxidative stress and inflammation, can be modulated by the lipids, vitamins, and polyphenols obtained through nutritional intake. Furthermore, epidemiological and preclinical evidence has uncovered certain compounds within foods that may have beneficial effects in the prevention of AD, including omega-3 fatty acids, vitamin E, and resveratrol among others. However, clinical data examining specific compounds are often inconsistent and fail to replicate the preclinical data. On the other hand, dietary patterns such as the Mediterranean or MIND diet have shown promise in terms of clinical outcomes for patients, indicating a reductionist approach to diet is not as effective as a holistic dietary pattern. In this review, we summarize some of the biological mechanisms of key compounds in their relation to AD and how they fit into a dietary pattern that supports the role of diet as a risk reducing factor for AD.

Physical exercise is often associated with increases in muscle damage markers and inflammation. However, biomarkers of muscle damage and inflammation responses to the 5-m shuttle run test (5mSRT) have not yet been evaluated. The aim of the present study was to investigate effects of the 5mSRT on muscle damage markers, inflammation, and perception of fatigue and recovery in healthy male athletes. Fifteen male amateur team sports players (age: 20 ± 3 yrs, height: 173 ± 7 cm, body-mass: 67 ± 7 kg) participated in this study. Blood biomarkers were collected at rest, 5 min after, and 72 h after the 5mSRT to measure muscle damage (i.e., creatinine kinase (CK), lactate dehydrogenase (LDH), aspartate aminotransferase (ASAT), and alanine aminotransferase (ALAT)) and inflammation (i.e., C-reactive protein (CRP)). Best distance (BD), total distance (TD), fatigue index (FI), and percentage decrement (PD) during the 5mSRT were assessed. Perceived recovery (PRS) and delayed onset muscle soreness (DOMS) were recorded before, 5 min after, and 72 h after the 5mSRT; perceived exertion (RPE) was recorded before, during, and 72 h after the 5mSRT. Muscle damage biomarkers post 5mSRT showed a significant increase compared to pre 5mSRT (p < 0.001) levels ((i.e., CK (190.6 ± 109.1 IU/L vs. 234.6 ± 113.7 IU/L), LDH (163.6 ± 35.1 IU/L vs. 209.9 ± 50.8 IU/L), ASAT (18.0 ± 4.4 IU/L vs. 21.7 ± 6.2 IU/L), and ALAT (10.2 ± 3.4 IU/L vs. 12.7 ± 3.8 IU/L)) and 72 h post 5mSRT (p < 0.001) levels ((CK (125.3 ± 80.5 IU/L vs. 234.6 ± 113.7 IU/L), LDH (143.9 ± 36.6 IU/L vs. 209.9 ± 50.8 IU/L), ASAT (15.0 ± 4.7 IU/L vs. 21.7 ± 6.2 IU/L), and ALAT (8.6 ± 2.4 IU/L vs. 12.7 ± 3.8 IU/L)). CRP was also significantly higher post 5mSRT compared to pre 5mSRT (2.1 ± 2.5 mg/L vs. 2.8 ± 3.3 mg/L, p < 0.001) and 72 h post 5mSRT (1.4 ± 2.3 mg/L vs. 2.8 ± 3.3 mg/L, p < 0.001). Significant correlations were reported between (i) physical performance parameters (i.e., PD, FI, TD, and BD), and (ii) markers of muscle damage (i.e., CK, LDH, ASAT, and ALAT) and inflammation (i.e., CRP). Similarly, DOMS and RPE scores were significantly higher post 5mSRT compared to pre 5mSRT (2.4 ± 1.0UA vs. 6.7 ± 1.1UA and 2.1 ± 0.6 UA vs. 8.1 ± 0.6 UA, respectively p < 0.001) and 72 h post 5mSRT (1.9 ± 0.7 UA vs. 6.7 ± 1.1 UA and 1.5 ± 0.6 UA vs. 8.1 ± 0.6 UA, respectively p < 0.001). PRS scores were significantly lower post 5mSRT as compared to pre 5mSRT (6 ± 1 UA vs. 3 ± 1 UA, p < 0.001) and 72 h post 5mSRT (7 ± 1 UA vs. 3 ± 1 UA, p < 0.001). Significant correlations existed between (i) performance parameters (PD, FI, TD, and BD) and (ii) RPE, PRS, and DOMS. The 5mSRT increased biomarkers of muscle damage and inflammation, as well as the DOMS and RPE and reduced the PRS. Seventy-two hours was sufficient for fatigue recovery induced by the 5mSRT. PD is better than FI for the calculation of performance decrements during the 5mSRT to represent fatigue.

To evaluate the effects of Ramadan observance on dietary intake, body mass and body composition of adolescent athletes (design: systematic review and meta-analysis; data sources: PubMed and Web of Science; eligibility criteria for selecting studies: single-group, pre-post, with or without control-group studies, conducted in athletes aged <19 years, training at least 3 times/week, and published in any language before February 12th, 2020). Studies assessing body mass and/or body composition and/or dietary intake were deemed eligible. The methodological quality was assessed using 'QualSyst'. Of the twelve selected articles evaluating body mass and/or body composition, one was of strong quality and eleven were rated as moderate. Ten articles evaluated dietary intake; four were rated as strong and the remaining moderate in quality. Continuation of training during Ramadan did not change body mass from before to the first week (trivial effect size (ES) = -0.011, p = 0.899) or from before to the fourth week of Ramadan (trivial ES = 0.069, p = 0.277). Additionally, Ramadan observance did not change body fat content from before to the first week (trivial ES = -0.005, p = 0.947) and from before to the fourth week of Ramadan (trivial ES = -0.057, p = 0.947). Lean body mass remained unchanged from before to the fourth week of Ramadan (trivial ES = -0.025, p = 0.876). Dietary data showed the intake of energy (small ES = -0.272, p = 0.182), fat (trivial ES = 0.044, p = 0.842), protein (trivial ES = 0.069, p = 0.720), carbohydrate (trivial ES = 0.075, p = 0.606) and water (trivial ES = -0.115, p = 0.624) remained essentially unchanged during as compared to before Ramadan. Continued training of adolescent athletes at least three times/week during Ramadan observance has no effect on body mass, body composition or dietary intake.

Currently, it is well accepted that physical exercise-induced oxidative stress may damage biological structures and impair cellular functions. However, it is still unclear which type of exercise results in the greatest oxidative stress responses among a healthy untrained population. The aim of the present study was to compare the acute oxidative stress response (i.e., 0 to 20 min) following different types of exercise (anaerobic, aerobic, and combined). Ten healthy, untrained males (19.5 ± 1.7 years) performed three randomized exercise bouts: anaerobic (30 s Wingate test), aerobic (30 min at 60% maximal aerobic power (MAP)) or combined (anaerobic and aerobic). Venous blood samples were collected before, as well as at 0 (P0), 5 (P5), 10 (P10), and 20 (P20) min after each session. Rates of malondialdehyde (MDA) and antioxidant activities (i.e., glutathione peroxidase (GPX), superoxide dismutase (SOD), glutathione reductase (GR), α-tocopherol, and total antioxidant status (TAS)) were assessed. Independent of exercise type, plasma MDA, GPX, SOD, and GR contents increased above baseline, whereas plasma α-tocopherol decreased under baseline after the test sessions (p < 0.05). Aerobic and anaerobic exercises generated faster responses (at P0) when compared to the combined exercise (P5 to P10) for the majority of the tested parameters. Plasma TAS content only increased following the aerobic exercise at P10 (p = 0.03). Five to twenty-minutes post exercise, the highest MDA response was registered in the aerobic condition, and the highest GPX and SOD responses were recorded in the anaerobic (at P5) and aerobic (at P20) conditions (p < 0.05). In conclusion, aerobic, anaerobic, or combined exercises have the potential to acutely increase oxidative stress and antioxidant activities, but with different responses magnitude. These findings confirm that oxidative stress response seems to be dependent on the intensity and the duration of the physical exercise and may help in understanding how varying exercise bouts influence the degree of oxidative stress among healthy untrained young adults.

CONTEXT: As the food industry is continually involved in the development of new attractive alternative therapeutic agents, the evaluation of the beneficial impact of (poly)phenols on cognitive and brain function during aging has gained increasing interest. OBJECTIVE: This systematic review and meta-analysis aimed to evaluate the acute and chronic effects of (poly)phenol-rich diet supplementation on cognitive function and brain health in aging adults. DATA SOURCES: PubMed and Web of Science databases were searched for relevant randomized placebo-controlled trials (RCTs) published from inception to July 2019. STUDY SELECTION: Two researchers independently screened 4303 records, using the PICOS criteria: Participants were aging adults; Intervention was based on acute and/or chronic (poly)phenols-rich supplementation; Comparator was any comparator; Outcomes included cognitive function and neuroprotective measures; and Study design was RCTs. A third researcher was consulted when discrepancies arose. Fifteen high-quality (mean PEDro score = 8.8 ± 0.56) RCTs (total participants: 918 healthy older adults) were included in the final sample. DATA EXTRACTION: Information on study design, employed treatment, characteristics of participants, outcomes, and the correspondent assessing methods were extracted. Preferred Reporting Items for Systematic Reviews and Meta-Analyses (PRISMA) guidelines were followed. Data Analysis and Results: A random-effects meta-analysis was used to pool estimates across studies. Effect size (ES) and its 95% confidence interval (CI) was calculated. Pooled results yielded a trivial ES (-0.2 to 0.03) for brain-derived neurotrophic factor and neuroinflammatory parameters and small (0.36) to moderate (0.82) ES for executive functions. CONCLUSION: This meta-analysis failed to provide evidence regarding the neuroprotective and anti-inflammatory effect of (poly)phenols supplementation in aging adults. However, findings from individual studies, included in this systematic review, suggest polyphenol-rich supplementation may improve some cognitive and brain functions in older adults. The beneficial effect of polyphenols seems to depend on ingested dose and bioavailability. Results suggest at least an intermediate dose (≥500 mg), and intermediate (≈9%) to high (43%) bioavailability rates are needed to cross the brain blood barrier and to exert a significant effect on cognitive health.

BACKGROUND: As the global prevalence of dementia continues to rise, multidomain lifestyle interventions that address modifiable risk factors associated with pathological cognitive decline are increasing. Although some digital options have been developed to increase the reach and scalability of these programs, because of cultural differences, the efficacy of the programs in one population cannot easily be generalized to populations in other countries. OBJECTIVE: This investigation aimed to examine the usability and engagement of a digitally delivered multidomain cognitive lifestyle intervention developed in the United States for a Japanese population. METHODS: This feasibility investigation utilized a quasi-experimental, single-arm, nonrandomized, longitudinal design where participants engaged in the behavioral intervention on a smartphone. Of the 559 participants that initially enrolled (age: mean 51 years, SD 7.5 years; 51.7% female [289/559]), 242 completed the final testing trial. Participants enrolled in a multidomain lifestyle program that consisted of (1) psychoeducational material, (2) physical activity tracker, (3) nutrition tracker, (4) audio-based meditations, and (5) health coaching. Engagement with the program was assessed through the total number of app sessions and the use of the exercise, diet, and meditation tracking features within the app. The total number of minutes exercised was collected through subjective user inputs, and nutrition was quantified by the Mediterranean-DASH Intervention for Neurodegenerative Delay diet adherence score. RESULTS: Significant relationships existed between overall nutrition score and frequency of nutrition tracking (r=0.18), frequency of physical activity tracking (r=0.19), and the total number of minutes exercised (r=0.22). Total minutes exercised was significantly correlated with total app sessions (r=0.57), frequency of physical activity tracking (r=0.85), frequency of nutrition tracking (r=0.64), number of times participants meditated (r=0.46), and total lessons read (r=0.36). The number of completed lessons was significantly related to frequency of physical activity tracking (r=0.40), frequency of nutrition tracking (r=0.43), the total number of times participants meditated (r=0.35), and total minutes exercised (r=0.33). Dividing the cohort into two groups based on lesson completion (<10 lessons completed vs ≥10 lessons completed), significant differences were observed between the total minutes exercised, frequency of physical activity tracking, frequency of nutrition tracking, and total number of times participants meditated (all P values <.01). CONCLUSIONS: Overall, this cross-cultural feasibility study in Japanese users demonstrated that the various engagement metrics were significantly correlated, and greater engagement was related to improved nutrition scores and increased time exercising. In addition, the relationships between lesson completion and other engagement metrics suggest that there may be value in exploring mechanisms that enhance lesson completion. Future research should examine the program in randomized control trials to more rigorously evaluate program efficacy.

Alzheimer's disease (AD) affects the memory and cognitive function of approximately 5.7 million Americans. Early detection subsequently allows for earlier treatment and improves outcomes. Currently, there exists a validated 30-min eye-tracking cognitive assessment (VPC-30) for predicting AD risk. However, a shorter assessment would improve user experience and improve scalability. Thus, the purposes were to (1) determine convergent validity between the 5-min web camera-based eye-tracking task (VPC-5) and VPC-30, (2) examine the relationship between VPC-5 and gold-standard cognitive tests, and (3) determine the reliability and stability of VPC-5. This prospective study included two healthy cohorts: older adults (65+ years, n = 20) and younger adults (18-46 years, n = 24). Participants were tested on two separate occasions. Visit 1 included the Montreal Cognitive Assessment (MoCA), Digit Symbol Coding test (DSC), NIH Toolbox Cognitive Battery (NIHTB-CB), VPC-30, and VPC-5. Visit 2 occurred at least 14 days later; participants completed the VPC-5, DSC, NIHTB-CB, and dual-task walking assessments (DT). VPC-30 significantly correlated with VPC-5 at the first (p < .001) and second (p = .001) time points. VPC-5 and DSC (p < .01) and Pattern Comparison Processing Speed Test (PSPAC) (p = .01) were also correlated on day 1. Significant associations existed between VPC-5 and DSC (p < .001), Flanker Inhibitory Control Test (p = .05), PSPAC (p < .001), and Picture Sequence Memory Test (p = .02) during day 14 testing session. The test retest reliability of VPC-5 was significant (p < .001). VPC-5 displayed moderate convergent validity with the VPC-30 and gold-standard measures of cognition, while demonstrating strong stability, suggesting it is a valuable assessment for monitoring memory function.

RESUMEN

With the rapid growth in the aging population, there has been a subsequent increase in the rates of Alzheimer's disease and related dementias (ADRD). To combat these increases in ADRD, scientists and clinicians have begun to place an increased emphasis on preventative methods to ameliorate disease rates, with a primary focus area on dietary intake. Protein/amino acid intake is a burgeoning area of research as it relates to the prevention of ADRD, and consumption is directly related to a number of disease-related risk factors as such low-muscle mass, sleep, stress, depression, and anxiety. As a result, the role that protein/amino acid intake plays in affecting modifiable risk factors for cognitive decline has provided a robust area for scientific exploration; however, this research is still speculative and specific mechanisms have to be proven. The purpose of this review is to describe the current understanding of protein and amino acids and the preventative roles they play with regard to ADRD, while providing future recommendations for this body of research. Additionally, we will discuss the current recommendations for protein intake and how much protein older adults should consume in order to properly manage their long-term risk for cognitive decline.

Many emotional functions are relatively preserved in aging despite declines in several cognitive domains and physical health. High levels of happiness exist even among centenarians. To address the hypothesis of whether preservation of emotional function in healthy aging may relate to different rates of age-related volume loss across brain structures, we performed two volumetric analyses on structural magnetic resonance neuroimaging of a group of healthy aging research participants using Freesurfer version 5.1. Volumes selected as supporting cognition included bilateral midfrontal and lateral frontal gyri, lateral parietal and temporal cortex, and medial temporal lobes. Volumes supporting emotion included bilateral amygdala, rostral anterior cingulate, insula, orbitofrontal cortex, and nucleus accumbens. A cross-sectional analysis was performed using structural MRI scans from 258 subjects. We found no difference in proportional change between groups. A longitudinal mixed effects model was used to compare regional changes over time in a subset of 84 subjects. Again, there was no difference in proportional change over time. While our results suggest that aging does not collectively target cognitive brain regions more than emotional regions, subgroup analysis suggests relative preservation of the anterior cingulate cortex, with greater volume loss in the nucleus accumbens. Implications of these relative rates of age-related volume loss in healthy aging are discussed and merit further research.
