Resumen Introducción: La Ulcera con radionecrosis debe ser resecada ampliamente y reparada con colgajos miocutáneos y fasciocutáneos. Objetivo: Presentar el tratamiento quirúrgico de las úlceras por radionecrosis mediante colgajos Miocutáneos y Fasciocutáneos pediculados y microquirúrgicos. Materiales y Método: La casuística es de 31 pacientes portadores de úlceras por radionecrosis resueltas con colgajos miocutáneos y fasciocutáneos. Ellos han sido pediculados y microquirúrgicos. Las edades fluctuaron entre 30 y 75 años operados entre el 2000 y el 2019. Resultados: Se realizaron 19 colgajos musculares o miocutáneos y 12 fasciocutáneos en diferentes zonas del organismo. Los colgajos correspondieron a: temporal, latissimus dorsi de pedículo central y en reversa, latissimus dorsi microquirúrgicos, recto abdominal, escapular microquirúrgico, para escapular, gracilis, gastrocnemius, rectus femoris, cubital microquirúrgico, fasciocutaneos: cone flap y de rotación local. Las complicaciones fueron dehiscencia de herida operatoria suturada y celulitis tratada con antibióticos. No hubo necrosis completa de los colgajos. El seguimiento fue de 3 a 6 meses. Discusión: En esta serie hemos utilizados los colgajos músculo cutáneos de preferencia, los fasciocutáneos los hemos reservados para aquellos casos en los que por razones anatómicas, de localización de las úlceras, era más adecuado usar estos últimos, como en las lesiones planas o poco profundas. Generalmente los colgajos fueron pediculados y en determinados casos, cuando el tejido vecino de reparación no estaba disponible, realizamos colgajos con microcirugía. Conclusión: El tratamiento de las úlceras por radionecrosis es efectivo cuando se realiza desbridamiento exhaustivo con cobertura inmediata con colgajos musculares o músculocutáneos y fasciocutáneos. Fueron colgajos pediculados y microquirúrgicos.


Introduction: The radionecrosis ulcer have to be widely resected and repaired trough flaps with good blood supply who contibute with oxigen, nutrients and antibiotics. Thinking on this the more suitable flaps are the myocutaneous ones and in special conditions the fasciocutaneous. Aim: The objetive of this paper is to show the surgical treatment of radionecrosis ulcers with myocutaneous and fasciocutaneous flaps. Materials and Method: Casuistic is 31 patients with radionecrosis ulcers treated with, mainly, myocutaneous flaps and in special areas with fasciocutaneous ones. The majority were pedicle flaps and in less number microsurgical flaps. The age of patients were betwee30 and 75 years old. Results: 19 muscle or myocutaneous flaps and 12 fasciocutaneous flaps were made. Flaps were latissimus dorsi, temporal, escapular, para escapular, rectus abdominis, rectus femoris, gracilis, gatrocnemius, cone flap and local fasciocutaneous flaps. Complications were sutured operative wound dehiscence and cellulitis treated with antibiotics. There were no complete necrosis of any flap. Discusion: In this serie we used muscle or myocutaneous flaps because they give good blood supply, oxigen, nutrients and arrival of antibiotic with filling of the deep ulcers. When the ulcers were shallow we use fasciocutaneous flaps. The majority of our flaps were pedicles and microsurgical flaps were used when there was no near avialable tissue. Conclusion: The treatment have to be wide debridement of the ulcer. The surgical treatment is with myocutaneous and fasciocutaneous flaps sometimes pediculates and in others conditions microsurgicals.

RESUMEN

Homeodomain interacting protein kinase-2 (HIPK2) is a member of the HIPK family of stress-responsive kinases that modulates cell growth, apoptosis, proliferation and development. HIPK2 has several well-characterised tumour suppressor roles, but recent studies suggest it can also contribute to tumour progression, although the underlying mechanisms are unknown. Herein, we have identified novel crosstalk between HIPK2 and the cytoprotective transcription factor NRF2. We show that HIPK2 is a direct transcriptional target of NRF2, identifying a functional NRF2 binding site in the HIPK2 gene locus and demonstrating for the first time a transcriptional mode of regulation for this kinase. In addition, HIPK2 is required for robust NRF2 responsiveness in cells and in vivo. By using both gain-of-function and loss-of-function approaches, we demonstrate that HIPK2 can elicit a cytoprotective response in cancer cells via NRF2. Our results have uncovered a new downstream effector of HIPK2, NRF2, which is frequently activated in human tumours correlating with chemoresistance and poor prognosis. Furthermore, our results suggest that modulation of either HIPK2 levels or activity could be exploited to impair NRF2-mediated signalling in cancer cells, and thus sensitise them to chemotherapeutic drugs.

OBJECTIVE: The primary aim was to analyze the association of moderate-to-vigorous physical activity (MVPA) with menopause symptomatology and its overall impact. A secondary/exploratory aim was to assess the association of different components of physical fitness with menopause symptomatology. METHODS: This cross-sectional study comprised 191 perimenopausal women from Southern Spain (age range 45-60 years, mean age 52.6 ± 4.5 years). The Blatt-Kupperman Menopausal Index was used to evaluate menopause symptomatology. Objectively measured MVPA was registered with accelerometry. Physical fitness was assessed with the Senior Fitness Test battery plus handgrip strength and sit-and-reach tests. RESULTS: After adjustment for multiple confounders, MVPA was only inversely associated with vertigo (r = -0.185, p < 0.05) and palpitations (r = -0.148, p < 0.05). Upper-body flexibility was inversely associated with the Blatt-Kupperman Menopausal Index global score (r = -0.147, p < 0.05). This test was also inversely associated with vertigo (r = -0.230, p < 0.01) and arthralgia (r = -0.168, p < 0.05). Lower-body muscle strength was associated with lower nervousness (r = -0.171, p < 0.05). Cardiorespiratory fitness was inversely associated with paresthesia (r = -0.158, p < 0.05), vertigo (r = -0.219, p < 0.01), fatigue (r = -0.159, p < 0.05) and arthralgia (r = -0.180, p < 0.05). CONCLUSION: The main findings of the present study indicate a weak association of objectively measured MVPA with menopause symptomatology. Exploratory analyses suggest that upper-body flexibility was associated with slightly lower overall menopause impact whereas neither MVPA nor any physical fitness components studied were associated with vasomotor symptoms.

BACKGROUND AND AIMS: The Mediterranean diet (MD) has been associated with reduced morbidity from cardiovascular diseases in the general population. The aim of this study was to assess whether different degrees of adherence to the MD were associated with the cardiometabolic risk in peri and menopausal women. METHODS AND RESULTS: This cross-sectional study included 198 peri and menopausal women participating in the Flamenco project. Validated questionnaires were used to assess menopause health-related quality of life and degree of adherence to the MD (low, medium and high). The following cardiometabolic risk factors were assessed: fat mass percentage, waist circumference, blood pressure and resting heart rate, plasma markers (total cholesterol, high and low-density lipoprotein cholesterol [HDL-C and LDL-C, respectively], total cholesterol/HDL ratio, triglycerides, C-reactive protein and fasting glucose), Physical activity levels and smoking status. The degree of adherence to the MD among the study sample was 27%, 40% and 30% for low, medium and high adherence, respectively. After controlling for potential confounders, women with a high adherence to the MD showed lower plasma total cholesterol (p = 0.025), resting heart rate (p = 0.005), LDL-C (p = 0.019), triglycerides (p = 0.046) and C-reactive protein (p = 0.009) compared to those with a low adherence. Likewise women with high adherence to the MD showed lower total cholesterol/HDL-C ratio (p = 0.020) compared to those with a medium adherence. The high MD adherence group also showed lower clustered cardiometabolic risk (p = 0.004). Moreover, when analysing specific MD components, whole grain cereals, pulses (both p < 0.05) and red wine (p < 0.01) consumption were inversely associated with the clustered cardiometabolic risk. CONCLUSION: The present findings suggest that a high but not medium adherence to the MD is associated with a cardioprotective effect in peri and menopausal women. As a low percentage of the sample showed a high adherence to the MD, future research aimed at increasing the adherence to this dietary pattern for a better cardiometabolic status during peri and menopause is warranted.

BACKGROUND: Fibromyalgia (FM) is a complex multidimensional disorder with pain as its main symptom. Fibromyalgia imposes a psychosocial burden on individuals that negatively impacts quality of life. The relationship of dietary habits with these psychosocial aspects is still unclear. OBJECTIVE: The purpose of this cross-sectional study was to assess dietary habits in a representative sample of women with FM and to explore their association with mental health, depression, and optimism in this population. DESIGN: A cross-sectional study was conducted between November 2011 and January 2013. PARTICIPANTS: The study sample comprised 486 women (ages 35 to 65 years) with FM from Andalucía (southern Spain). MAIN OUTCOME MEASURES: Mental health, depression, and optimism were evaluated by means of the mental component scale of the 36-item Short-Form Health Survey, the Beck Depression Inventory (BDI-II), and the Life Orientation Test Revised, respectively. A short form of a validated food frequency questionnaire was used to assess dietary habits. STATISTICAL ANALYSES PERFORMED: Analysis of covariance was used to assess associations between dietary habits and mental health, depression, and optimism. The presence of severe depression (BDI-II ≥29) as a function of dietary habits was examined with logistic regression. RESULTS: A daily or almost-daily consumption of fruit and vegetables and a moderate consumption of fish (2 to 5 servings per week) were associated with higher scores in mental health (P<0.001, P<0.05, and P<0.001, respectively) and lower levels of depression (P<0.001, P<0.01, and P<0.01, respectively). A daily or almost-daily consumption of vegetables and a moderate consumption of dairy products and fish were associated with higher levels of optimism (P<0.05, P<0.05, and P<0.001, respectively). A daily or almost-daily consumption of cured meats and sweetened beverages were associated with higher levels of depression and lower levels of optimism, respectively (both P<0.05). CONCLUSION: The results this study suggest that a daily or almost-daily intake of fruit and vegetables and a moderate intake of fish may be associated with more favorable psychosocial outcomes in women with FM. Conversely, excessive intake of cured meats and sweetened beverages was related to worse scores in optimism and depression outcomes. Future research analyzing dietary patterns as well as intervention studies evaluating the effects of healthy dietary patterns on psychosocial and physical outcomes in individuals with FM are warranted.

OBJECTIVE: To study the association between physical fitness and body-size phenotypes, and to test which aspects of physical fitness show the greatest independent association with cardiometabolic risk in perimenopausal women. STUDY DESIGN: This cross-sectional study involved 228 women aged 53±5years from southern Spain. MAIN OUTCOME MEASUREMENTS: Physical fitness was assessed by means of the Senior Fitness Test Battery (additionally including handgrip strength and timed up-and-go tests). Anthropometry, resting heart rate, blood pressure and plasma markers of lipid, glycaemic and inflammatory status were measured by standard procedures. The harmonized definition of the 'metabolically healthy but obese' (MHO) phenotype was employed to classify individuals. RESULTS: The overall prevalence of the MHO phenotype was 13% but was 43% among the obese women. Apart from traditional markers, metabolically healthy non-obese women had lower levels of C-reactive protein than women with the other phenotypes (p<0.001), and levels of glycosylated haemoglobin were lower in MHO women than in metabolically abnormal non-obese women (overall p=0.004). Most of the components of physical fitness differed with body-size phenotypes. The 6-min walk and the back-scratch tests presented the most robust differences (both p<0.001). Moreover, the women's performance on the back-scratch (ß=0.32; p<0.001) and the 6-min walk (ß=0.22; p=0.003) tests was independently associated with the clustered cardiometabolic risk. The back-scratch test explained 10% of the variability (step 1, p<0.001), and the final model, which also included the 6-min walk test (step 2, p=0.003), explained 14% of the variability. CONCLUSION: Low upper-body flexibility was the most important fitness indicator of cardiometabolic risk in perimenopausal women, but cardiorespiratory fitness also played an important role.

OBJECTIVE: To assess the association of different components of physical fitness with HRQoL in early postmenopause and to test which physical fitness components are independently associated with the physical and mental components of HRQoL. METHODS: The final sample comprised 67 early postmenopausal women. Physical fitness was assessed with the Senior Fitness Test battery (additionally including handgrip strength test), and HRQoL was evaluated with the Short-Form Health Survey-36 (SF-36). We also analyzed plasma gonadotropic hormones and estradiol. RESULTS: Overall, most of the fitness components were positively associated with HRQoL. Lower-body flexibility, upper-body muscle strength and cardiorespiratory fitness were the fitness components more strongly associated with HRQoL (r range from 0.28 to 0.56). Static balance was especially associated with mental health (r = -0.46, P < 0.001). Lower-body flexibility (assessed with the chair sit-and-reach test) and upper-body muscle strength (assessed with handgrip dynamometry) were independently associated with the SF-36 Physical Component Summary (both, P < 0.001). Upper-body muscle strength (P < 0.01) and cardiorespiratory fitness (assessed with the 6-min walk test, P < 0.05) were independently associated with the SF-36 Mental Component Summary. CONCLUSIONS: Higher physical fitness is associated with better HRQoL in early postmenopause. Lower-body flexibility and upper-body muscle strength were the most important independent fitness indicators, explaining ~30 % of HRQoL.

Pseudomonas pseudoalcaligenes CECT5344 is an alkaliphilic bacterium that can use cyanide as nitrogen source for growth, becoming a suitable candidate to be applied in biological treatment of cyanide-containing wastewaters. The assessment of the whole genome sequence of the strain CECT5344 has allowed the generation of DNA microarrays to analyze the response to different nitrogen sources. The mRNA of P. pseudoalcaligenes CECT5344 cells grown under nitrogen limiting conditions showed considerable changes when compared against the transcripts from cells grown with ammonium; up-regulated genes were, among others, the glnK gene encoding the nitrogen regulatory protein PII, the two-component ntrBC system involved in global nitrogen regulation, and the ammonium transporter-encoding amtB gene. The protein coding transcripts of P. pseudoalcaligenes CECT5344 cells grown with sodium cyanide or an industrial jewelry wastewater that contains high concentration of cyanide and metals like iron, copper and zinc, were also compared against the transcripts of cells grown with ammonium as nitrogen source. This analysis revealed the induction by cyanide and the cyanide-rich wastewater of four nitrilase-encoding genes, including the nitC gene that is essential for cyanide assimilation, the cyanase cynS gene involved in cyanate assimilation, the cioAB genes required for the cyanide-insensitive respiration, and the ahpC gene coding for an alkyl-hydroperoxide reductase that could be related with iron homeostasis and oxidative stress. The nitC and cynS genes were also induced in cells grown under nitrogen starvation conditions. In cells grown with the jewelry wastewater, a malate quinone:oxidoreductase mqoB gene and several genes coding for metal extrusion systems were specifically induced.

INTRODUCCIÓN: La creación de lugares de trabajo sin humo sigue siendo la medida de mayor implantación frente al tabaquismo. WinMedtra es la aplicación informática corporativa del Servicio Andaluz de Salud para la Vigilancia de la Salud. OBJETIVO: Aprovechando dicha aplicación, planteamos mostrar su utilidad para calcular la prevalencia de tabaquismo en un Hospital de referencia. MATERIAL Y MÉTODO: Estudio observacional a partir de los exámenes de salud realizados en 2013 (n=1672). Se extrajeron los datos relativos al consumo de tabaco de WinMedtra realizándose una estadística descriptiva e inferencial (prueba «t» de Student, análisis de varianza simple y Ji-Cuadrado). RESULTADOS: La aplicación informática nos ha permitido obtener datos del hábito tabáquico en la población laboral estudiada. La prevalencia de sujetos fumadores activos fue de un 22,8% (IC95%:20,79%- 24,81%) y el 19,8% eran ex fumadores, por lo que el 77,2% no fumaba en el periodo del estudio (IC95%: 75,19%-79,21%). El consumo acumulado fue mayor en el grupo de los hombres que en el de las mujeres (21,08 ± 16,31 versus 15,38±13,28 paquetes-año) (t: 3,61, p < 0,001). CONCLUSIONES: La aplicación informática puede ser útil para tener un conocimiento epidemiológico más exacto de la magnitud de conductas adictivas como el tabaquismo en centros de trabajo, obteniéndose en nuestro estudio una prevalencia de tabaquismo activo inferior a la del entorno


INTRODUCTION: creating smoke-free workplaces still remains the most implemented action concerning tobacco smoking. WinMedtra is the Andalusian Health Service corporate software for the medical surveillance. AIM: by using this application, we intend to show its utility in calculating the smoking prevalence in a reference hospital. MATERIAL AND METHODS: an observational study was performed based on health examinations conducted in 2013 (n = 1672). After extracting data related with the tobacco consumption from WinMedtra, a descriptive and inferential statistics (student, simple anova and Chi-square statistical tests) were performed. We calculated arithmetic means and standard deviations for quantitative variables; and absolute and relative frequencies for qualitative variables. Confidence intervals up to 95% were calculated for the main estimates. RESULTS: the application allowed us to obtain smoking habits data in the interviewed working population. The smoking prevalence active subjects were 22.8% (95% CI: 20.79%-24.81%); 19.8% were former smokers so 77.2% did not smoke during the period study (95% CI: 75.19% -79.21%). The accumulated smoking intake was higher in the group of men than in that of women (21.08 ± 16.31 versus 15.38 ± 13.28 packs/year) (t: 3.61, p < 0.001). Significant differences (p <0.001) were obtained by comparing the average age of nonsmokers and that of smokers, (48.33 ± 9.82 versus 50.55 ± 7.62 years) and former smokers (48.33 ± 9.82 versus 50.65 ± 8.32 years). Regarding occupational status, the highest "Non Smokers" percentage belonged to the resident workers (87%) followed by the chief group (75%). The group of administrative assistants (34.7%) and the supervisors (33.3%) had the highest "Smokers" percentage (Chi-square = 131.23, p < 0.001). CONCLUSIONS: the software application may be useful to have an accurate epidemiological knowledge about the magnitude of addictive behavior like smoking in workplaces. We have obtained a lower smoking prevalence compared to the area in our study

The in vivo pyrogen test is the main toxicological assay used in the quality control of injectable products, especially immunobiologicals. Pharmacopoeias state that, before the main test, a preliminary test must be conducted on all animals, and must follow the same conditions as the main test. The aim of this study was to determine the normal temperature variation in New Zealand white rabbits during restraint and propose a limit value for considering an animal as suitable for testing. Results of the temperature variation in 4,689 rabbits during preliminary tests were obtained from the routine database of the Pharmacology and Toxicology Department of the National Institute of Quality Control in Health (INCQS/FIOCRUZ), Brazil. From these preliminary tests, 3,364 rabbits were considered suitable for testing according to the Brazilian Pharmacopoeia criteria (temperature variation < 0.5 °C). Results showed that about 92 per cent of the rabbits presented a normal individual temperature variation equal to or below 0.30 °C. Animals presenting a temperature variation close to the fever temperature must not be included in the main test, since they can be stressed or sick. Consequently, the temperature variation of 0.30 °C could be adopted by pharmacopoeias as a limit temperature to be considered in the preliminary test to determine which animals can be used in the main rabbit pyrogen test. Animals can be pre-tested until presenting this safe variation, in order to ensure they are healthy and minimise interference in the result.

Background: Incomplete development and growth of the pinna can lead to a small or deformed pinna, called microtia. Aim: To report and evaluate the effectiveness of a surgical technique previously described for the treatment of microtia, based on a new anatomical-surgical classification. Materials and Methods: Retrospective study of patients treated at two plastic surgery departments from January 2008 to December 2010. We recruited a total of 15 patients aged 9 to 25 years. Thirteen (87 percent) had unilateral and 2 (13 percent) bilateral microtia. All patients underwent the surgical reconstructive technique described by Firmin. Results: Among patients with unilateral microtia, seven belonged to Firmin type I, five to type II and one to type IIIa. Both patients with bilateral microtia, were classified as type IIIb. In all cases with unilateral microtia, a good initial projection of the cartilage frame was achieved during the first surgical procedure. Six patients were subjected to a second operation. During 18 months follow up, four patients (16 percent) had complications. Conclusions: This surgical technique that is based on an anatomical surgical classification, achieves favorable and satisfactory results.


Objetivos: Reportar y evaluar la efectividad de una técnica quirúrgica, previamente descrita para el tratamiento de microtias, basado en un nuevo concepto de clasificación anátomo-quirúrgica. Material y Método: Estudio retrospectivo de pacientes tratados en el Hospital Militar y Clínica Alemana de Santiago en el Servicio de Cirugía Plástica desde enero de 2008 hasta diciembre de 2010. Se reclutaron a un total de 15 pacientes, 13 (87 por ciento) de los cuales eran unilaterales y 2 (13 por ciento) bilaterales. A todos los pacientes se les realizó la reconstrucción mediante técnica quirúrgica de Firmin. Resultados: Se operó a 15 pacientes de rango de edad entre 9 y 25 años, con una mediana de 11 años. De los casos unilaterales; 7 correspondían a tipo I de Firmin, 5 casos a tipo II Firmin y un caso tipo IIIa. De los casos bilaterales, ambos tenían microtia tipo IIIb. Se logró en la totalidad de los casos unilaterales una buena proyección inicial del marco cartilaginoso durante el primer tiempo quirúrgico. Seis casos fueron sometidos a un segundo tiempo quirúrgico. Durante el seguimiento mínimo de 18 meses, se observaron complicaciones en 4 (16 por ciento) pacientes. Conclusiones: Se comprueba que mediante una técnica basada en la clasificación anátomo-quirúrgica, los resultados a corto plazo fueron favorables y satisfactorios.

Allele frequencies for the high polymorphic short tandem repeats (STR) loci PentaE, PentaD, D18S51, D21S11, TH01, D3S1358, FGA, D16S539, D7820, D13S317, vWA and D81179 were analysed in an native Amerindian population from Mato Grosso do Sul state named Terena. Deviations from Hardy-Weinberg expectations were evaluated and the results showed no differences from equilibrium in all loci. The combined power of discrimination and the combined power of exclusion for the 12 tested STR loci were 0.99999999 and 0.999999 respectively. The Terena population data were compared to other from 11 Brazilian populations (Amazônia, Pernambuco, Mato Grosso do Sul, São Paulo, Rio Grande do Sul, Alagoas, Sergipe, Rio Grande do Norte, Santa Catarina, Rondônia and Rio de Janeiro) representing the major Brazilian geographic regions. The F(ST) comparative analysis showed no significant differences between all those populations except when comparing Terena with the remained ones.

This study observed the mosquito population in a rural eutrophised dam. Larvae of L3 and L4 stages and pupae were dipped out during twelve month collections and the reared to the adult stage for identification. The collections were done along nine metres from the edge of the dam divided in three parts (P1, P2 and P3), each part being 3 m long. P1 did not have vegetation (grass) along its edge,which would reach or sink into the water to promote some shade on the marginal water. A total of 217 adults of four species was identified with the following constancies and frequencies: Culex quinquefasciatus (Say, 1823) (83% and 40.6%), Anopheles (Nyssorhynchus) evansae (Brèthes, 1926) (92% and 26.7%), Anopheles (Nyssorhynchus) rangeli (Gabaldon, Cova Garcia and Lopez, 1940) (83% and 14.3%) and Culex nigripalpus (Theobald, 1901) (33% and 18.4%). C. quinquefasciatus, A. evansae, A. rangeli and C. nigripalpus were more frequent in the quarters Nov./Dec./Jan. (85.7%), May/June/July (75%), Aug./Sept./Oct. (29.4%) and Aug./Sept./Oct. (23.5%) particularly in the months of December (88.4%) Sept.tember (48.94), (38.3) and August (47.62) respectively. The presence of C. quinquefasciatus and the high incidence of Daphinia sp. and also the levels of Organic Nitrogen (0.28 mg/L) and of total Phosphorus (0.02 mg/L) are indications of the eutrophication of the dam. There was a difference regarding the total of Anopheles (A. avansae + A. rangeli) and Culex species (C. quinquefasciatus + C. nigripalpis) between P1 and P2 (χ(2) = 0.0097), P1 and P3 (χ(2) = 0.0005), but not between P2 and P3 (χ(2) = 0.2045).The high C. quinquefasciatus constancy and frequency were confirmed to be a good biological indicator for a eutrophised environment and A. evansae showed a good potential for this environment. Vegetation can be an important factor for anopheline population dynamic also in eutrophic breeding sites.

Introducción: El cáncer testicular representa entre el 1,5 por ciento de los cánceres en varones y el 5 por ciento de los tumores urológicos en general. Los tumores paratesticulares representan el 4 por ciento de los tumores testiculares. Material y Método: Se revisaron informes de anatomía patológica y las fichas clínicas de pacientes con diagnóstico de patología testicular durante el período entre enero de 2004 a marzo de 2009. Se analizaron las siguientes variables: edad, testículo involucrado, tipo histológico y tratamiento. Objetivo: Es describir los tumores paratesticulares diagnosticados en el Servicio de Urología del Hospital Clínico San Borja Arriarán. Resultado: Se revisaron 149 casos, de los cuales 110 casos se confirmaron como tumores testiculares. Los tumores germinales representan el 90 por ciento. Los demás tipos histológicos, tres corresponden a metástasis testiculares, dos linfomas y una leucemia. Sólo cuatro casos correspondían a tumores paratesticulares representando el 3,6 por ciento de los tumores tratados. La descripción histológica de los tumores encontrados fue de dos Sarcomas (Rabdomiosarcoma y fibrosarcoma), Hemangiopericitoma y un Linfoma. La mediana de edad es de 62 años. El promedio de evolución de las lesiones fue de 5 meses. Discusión: Las neoplasias paratesticulares del cordón espermático son raras y representan sólo el 4 por ciento de los tumores testiculares. Los más frecuentes son tumores benignos en 2/3 de los casos, siendo el lipoma el tipo histológico más común. Conclusión: Los tumores paratesticulares son neoplasias infrecuentes. La presentación clínica no varía respecto a los demás tumores testiculares excepto en los pacientes inmunodeprimidos.


Introduction: Testicular cancer accounts for 1.5 per cent of cancers in men and 5 per cent of general urologic tumors. Paratesticular tumors represent 4 per cent of testicular tumors. Materials and Methods: We reviewed pathology reports and clinical records of patients diagnosed with testicular pathology during the period January 2004 to March 2009. We analyzed the following variables: age, testicular involvement, histological type and treatment. Aim: You describe paratesticular tumors diagnosed in the urology department of Hospital San BorjaArriarán. Results: We reviewed 149 cases, of which 110 cases were confirmed as malignancy. Germ cell tumors account for 90 percent. Other histologic types, three correspond to metastatic testicular, two lymphomas and leukemia. Only four cases were paratesticular tumors accounting for 3.6 per cent of treated tumors. The histological description of the tumors found were two sarcomas (rhabdomyosarcoma and fibrosarcoma), hemangiopericytoma and a lymphoma. The median age was 62 years. The average evolution of lesions was 5 months. Discussion: Paratesticular neoplasms of the spermatic cord are rare and represent only 4 per cent of testicular tumors. The most common are benign tumors in 2/3 of cases, the lipoma being the most common histologic type. Conclusion: Paratesticular tumors are rare neoplasms. The clinical presentation varies in relation to other testicular tumors except in immunosuppressed patients.

ANTECEDENTES: En la cirugía del cáncer mamario, asociando técnicas de mamoplastía reductiva a la resección oncológica, ha permitido aumentar la extensión de las resecciones previniendo las deformidades secundarias. Es lo que llamamos oncoplástica mamaria o reconstrucción mamaria tumor específica. OBJETIVO: Presentar una serie descriptiva retrospectiva de 60 pacientes en quienes usamos oncoplástica mamaria como alternativa a la mastectomía. RESULTADOS: Un 86% consideró el resultado cosmético como satisfactorio. La mediana de seguimiento fue de 48 meses. Dos pacientes presentaron recurrencia local, una asociada a falla sistémica sincrónica a los 18 meses; otra paciente se diseminó a los 6 años. Ambas fallecieron. Una tercera se encuentra en tratamiento hormonal por metástasis óseas. No hubo complicaciones importantes en la serie, ni retardo significativo de las terapias secuenciales. CONCLUSIÓN: Esta técnica permite evitar mastectomías y prevenir deformidades en cirugía conservadora estándar.


BACKGROUND: In breast cancer surgery, the association of reductive mammoplasty to oncological excisión, allows more extensive excisions, reducing radical surgery and preventing secondary deformities. This is called oncoplastic or tumor specific mammary reconstruction. OBJECTIVE: To report 60 patients in whom oncoplastic surgery was used. METHODS: Retrospective descriptive series of 60 patients aged 34 to 65 years, subjected to oncoplastic mammary surgery, that were followed for a median of 48 months. RESULTS: Eighty six percent of patients considered the cosmetic result of surgery as satisfactory. Two patients had a local relapse, one associated to systemic dissemination at 18 months and die. Another patient died with systemic disease and other is under hormonal treatment for bone metastases. No significant complications or delays in oncological therapies were recorded. CONCLUSIONS: Oncoplastic surgery may avoid mastectomy and prevent deformities in breast cancer surgery.

We determined whether ADRbeta3 polymorphism is associated with obesity-related traits in 140 obese patients. Molecular analysis was performed by PCR and RFLP. Individuals carrying the Arg64 allele had a lower waist-to-hip ratio, higher adiponectin and high-density lipoprotein cholesterol levels, and a tendency towards lower blood pressure. In contrast, Trp64/Trp64 carriers were at greater risk for dysmetabolic phenotypes (odds ratio = 2.88, P = 0.03).

Background: In breast cancer surgery, the association of reductive mammoplasty to oncological excisión, allows more extensive excisions, reducing radical surgery and preventing secondary deformities. This is called oncoplastic or tumor specific mammary reconstruction. Aim: To report 37 patients in whom oncoplastic surgery was used. Material and Methods: Retrospective descriptive series of 37 patients aged 38 to 65 years, subjected to oncoplastic mammary surgery, that were followed for a median of 42 months. Results: Eighty six percent considered the cosmetic result of surgery as satisfactory. One patient had a local relapse associated to systemic dissemination at 16 months. One patient died and one is under hormonal treatment for bone metastases. No significant complications or delays in oncological therapies were recorded. Conclusions: Oncoplastic surgery may avoid mastectomy and prevent deformities in breast cancer surgery.


Conservar la mama se considera uno de los objetivos importantes en el tratamiento del cáncer. Frecuentemente la relación entre el tamaño y la localización tumoral impide la cirugía conservadora con un buen resultado estético u obliga a realizar una mastectomía, lo que empobrece la calidad de la sobrevida. Asociar técnicas de mamoplastía reductiva a la resección oncológica ha permitido aumentar la extensión de las resecciones, disminuyendo las cirugías radicales o previniendo las deformidades secundarias. Es lo que llamamos oncoplástica mamaria (OPM) o reconstrucción mamaria tumor específica. Presentamos una serie descriptiva retrospectiva de 37 pacientes en quienes usamos OPM como alternativa a la mastectomía. Un 86 por ciento consideró el resultado cosmético como satisfactorio. La mediana de seguimiento fue 42 meses. Una paciente ha tenido recurrencia local asociada a diseminación sistémica a los 16 meses. Una paciente falleció y otra está en tratamiento hormonal por metástasis óseas. No hubo complicaciones importantes en la serie ni retardo significativo de las terapias secuenciales. Concluimos que esta técnica permite evitar mastectomías y prevenir deformidades en cirugía conservadora estándar.

Background: Gynecomastia is common among men. Pseudogynecomastia, caused by an increased subcutaneous fat in the mammary area, can be consisted with gynecomastia. Aim: To report the experience with the surgical treatment of both. Material and Methods: Review of medical records of 106 patients, aged 15 to 50 years, operated. Results: Thirty two percent of patients had a pure gynecomastia, 30 percent had pseudogynecomastia, and 39 percent had both conditions. All were mild or moderate. A mastectomy plus liposuction was performed in 50 percent of patients, solely mastectomy in 26 percent and solely liposuction in 23 percent. Four percent had surgical complications such as hematomas or ecchymoses. Conclusions: The surgical treatment of gynecomastia requires liposuction in most patients to achieve a good result.


La Ginecomastia, mamas femeninas, es la anomalía más frecuente en el sexo masculino. La pseudoginecomastia es un cuadro que a menudo se confunde con la ginecomastia y consiste en el aumento de tejido graso en la región pectoral. El equipo de cirugía plástica del Hospital Militar de Santiago, por atender una población cautiva de jóvenes que realizan el servicio militar obligatorio, ha tenido una gran experiencia en el manejo de esta enfermedad; es por esto que los objetivos de nuestro estudio fueron evaluar el manejo de los pacientes operados por esta patología en el Hospital Militar y la Clínica Santa María de Santiago, e intentar ofrecer pautas para el manejo de esta patología de la cirugía general a nivel nacional. Pacientes y Métodos: Es un estudio retrospectivo y descriptivo, realizado a 106 pacientes operados con el diagnóstico de Ginecomastia o pseudoginecomastia, atendidos entre Enero 1987 a Julio 2005 en las instituciones antes mencionadas. Resultados: Promedio de edad 24 años (rango: 15 a 50 años); 32 por ciento ginecomastia pura, 29,4 por ciento pseudoginecomastia y 38,8 por ciento ambas. Todos fueron grado I y Ha de la clasificación de Simón. En el 50 por ciento se realizó adenectomía más lipoaspiración, adenectomía sola en 26,4 por ciento y lipoaspiración exclusiva a 23,4 por ciento. Complicados un 3,8 por ciento con hematomas y equimosis. Discusión: La ginecomastia es una enfermedad frecuente en la adolescencia; además de provocar alteraciones estéticas, provoca alteraciones psicológicas en el paciente. La liposucción se consagra como una técnica quirúrgica importante a ser aplicada en esta anormalidad.

La cobertura de heridas complejas en extremidades inferiores es un desafío de difícil manejo. Se realizó un estudio retrospectivo, analizando el resultado y las complicaciones en el uso del colgajo sural a pedículo distal para cobertura de defectos de extremidad inferior. Material y Método: Revisión de fichas clínicas de todos los pacientes sometidos a colgajo sural en los Hospitales Militar y Trabajador de Santiago entre los años 1995 y 2005, analizando características epidemiológicas, técnica quirúrgica y complicaciones del procedimiento. Resultados: Colgajos surales totales: 40 casos. Etiología de lesión: Fracturas: 33 casos (82,5%), heridas complejas (infectadas, inestables) 4 casos (10%), desforrramiento: 2 casos (5%) y quemadura: 1 caso (2,5%). En 20 casos no hubo complicación, y dentro de las complicaciones se encontraron necrosis parcial: 8 casos (20%), infección: 5 casos (12,5%), epidermolísis: 4 casos (10%) y seroma en 3 casos (7,5%). No se reportaron casos de necrosis total. Conclusión: El colgajo sural a pedículo distal es una alternativa válida de cobertura en lesiones complejas de extremo distal de extremidad inferior, con complicaciones de manejo aceptables, y con buen resultado a largo plazo.


Background: The covering of complex wounds of lower extremities is a difficult challenge. Aim: To analyze the results and complications on the use of sural flaps with distal pedicle for the covering of lower extremity defects. Material and Methods: We reviewed the clinical records of all the patients that had sural flaps between 1995 and 2005 at the Military Hospital and Workers Hospital of Santiago, Chile. We analyzed the epidemiologic characteristics, surgical techniques and complications of the procedure. Results: We found 40 cases of sural flaps. The etiology of the lesions were fractures in 33 cases (82.5%), complex wounds: four cases (10%), skin tearing and muscle laceration in 2 cases (5%) and burns 1 case (2.5%). In 20 cases there were no complications. Eight cases had partial necrosis (20%), five had infections (12.5%), four had epidermolysis and three had a seroma (7.5%). There were no reports of total necrosis. Conclusions: Sural flaps with a distal pedicle are a valid alternative for the covering of complex wounds of the distal portion of lower extremities, with a low rate of complications and good long term results.
