RESUMEN

Two prominent personalities in Medicine, Lorenz Heister (1683-1758) and Xavier Bichat (1771-1802), compete for the attribution of the discovery of the buccal fat pad (BFP). A careful reading of the original texts reported here, seems to identify Bichat as the first to describe the BFP. However, Heister most probably was the first to describe an accessory parotid gland.

RESUMEN

One of the eponyms most frequently cited in dental morphology texts, together with the Carabelli tubercle of the first permanent maxillary molars, is the Zuckerkandl tubercle of deciduous molars. However, references about Emil Zuckerkandl in the field of dental history and this particular entity are scarce. The reason this dental eponym was pushed "into the shadows" probably lies in the many other anatomical parts (including another tubercle, the pyramidal one of the thyroids), which took their names from this great anatomist.

Current knowledge indicates that the molecular cross-talk between stem cells and biomaterials guides the stem cells' fate within a tissue engineering system. In this work, we have explored the effects of the interaction between the poly(l-lactide) acid (PLLA) polymer film and human adult adipose stem cells (hASCs), focusing on the events correlating the materials' surface characteristics and the cells' plasma membrane. hASCs were seeded on films of pristine PLLA polymer and on a PLLA surface modified by the radiofrequency plasma method under oxygen flow (PLLA+O2). Comparative experiments were performed using human bone-marrow mesenchymal stem cells (hBM-MSCs) and human umbilical matrix stem cells (hUCMSCs). After treatment with oxygen-plasma, the surface of PLLA films became hydrophilic, whereas the bulk properties were not affected. hASCs cultured on pristine PLLA polymer films acquired a spheroid conformation. On the contrary, hASCs seeded on PLLA+O2 film surface maintained the fibroblast-like morphology typically observed on tissue culture polystyrene. This suggests that the surface hydrophilicity is involved in the acquisition of the spheroid conformation. Noteworthy, the oxygen treatment had no effects on hBM-MSC and hUCMSC cultures and both stem cells maintained the same shape observed on PLLA films. This different behavior suggests that the biomaterial-interaction is stem cell specific.

OBJECTIVE: To estimate the association between periodontitis (PD) and preterm low birth weight (LBW). STUDY DESIGN: Observational study in a University clinical setting on a population of 120 pregnant women. RESULTS: One specialist in periodontics examined all the patients. Two thresholds for definition of PD were used, which detected respectively 29 women (24%) and 4 (3%) as affected by the pathology. Six patients were lost to follow-up. Out of the remaining 114, 1 had an abortion, while 20 (17%) delivered pre-term. Eleven per cent of all women delivered low-weight babies. Ten women (9%) delivered preterm LBW babies. No association was detected between any of the two definitions of PD and any of the adverse pregnancy outcomes. CONCLUSION: This study was unable to provide evidence of association between PD and preterm LBW.

As the quality of clinical information contained in referral letters for specialist care may determine the initial decisions as to appropriate treatment for patients, it is important that this information is objective and accurate. The use of a standardised questionnaire for this purpose improves the quality of information received, but it is not known how accurate this information is if used in the decision-making process for treatment planning. The aim of this study was to compare the clinical assessment data derived from a standardised questionnaire used by referring patients to a specialist periodontal unit. Equivalent data recorded by dentists and periodontists were compared to establish whether the data were consistently reproducible, and therefore usable for deciding on the appropriate initial phase of treatment for patients. Data from 50 patients referred for periodontal treatment planning and specialist care in which the questionnaires from the general dental practitioner and the subsequent periodontal assessment were available were compared. All of the patients were medically healthy and were not taking any long-term medication. The patients were selected based on the retrospective availability of equivalent clinical data from the questionnaires and consultation in the periodontal department at the Eastman Dental Hospital, London. The equivalence or disparity in the identification of different cofactors, and the community periodontal index of treatment need (CPITN) (basic periodontal examination, BPE) scores recorded by referring dentists (GDP), and periodontists (EDH, Eastman Dental Hospital) were annotated and compared. A level of equivalence of over 50% of assessments was demonstrated between referring dentists and periodontists in the variables of oral hygiene, smoking, bleeding on probing and restorative factors, as well as in the recorded scores of the CPITN (BPE) screening system. It can be concluded that the use of questionnaires for referral for specialist services is useful to not only improve and standardise the quality and extent of the relevant clinical information received, but may also be a valuable resource that may be used in the initial decision-making process for screening patients to the appropriate level of periodontal care. However, the limitations of this observational study preclude drawing any definitive conclusions in the absence of a more carefully controlled study designed specifically to overcome these limitations.

OBJECTIVE: Dental calculus is considered to be simply a "plaque-retentive factor", and therefore only a secondary aetiological factor in the pathogenesis of periodontitis. Recent studies have suggested a more active role for calculus. Our objective was to demonstrate the presence of periodontal pathogens in the non-mineralised areas of supra- and subgingival dental calculus. METHODS: Subjects for the study were derived from patients with substantial amounts of supragingival calculus in the lower anterior region who had moderate periodontal disease, having been referred to the periodontal department at the Eastman Dental Hospital for periodontal care. Calculus was removed in as large pieces as possible by the use of a sickle or a push scaler placed underneath the apical or facial border of the calculus and fracturing it from the tooth surface in a single stroke. The orientation and absence of dental plaque was confirmed using light microscopy for each sample prior to inclusion in this study. Samples were prepared for transmission electron microscopic (TEM) observation after immunogold staining with polyclonal antibodies for the presence of Actinobacillus actinomycetemcomitans (A. a.), Porphyromonas gingivalis (P. g.) and Treponema denticola (T. d.). RESULTS: Most of the samples contained at least one of the bacterial species examined, either in the lacunae or in the covering dental plaque. T. d. was the most frequently identified species and was found in nearly all of the subgingival samples, whilstA. a. was rarely observed. CONCLUSIONS: In this limited study, supra- and subgingival dental calculus appears to be capable of maintaining periodontal pathogens within the deep recesses of its structural lacunae and channels. Therefore, calculus could possibly play a relevant role in the aetiology and pathogenesis of periodontitis. The presence of T. d. in the majority of specimens requires further investigation as its pathogenic potential may be underestimated in current published microbiological research, and further work is required to determine its role in the pathogenesis of periodontal diseases. However, further work by way of a large-scale definitive study is necessary to confirm the results of this investigation.
