INTRODUCTION: The concept of safe care permeates health institutions around the world, however, it is necessary to understand the safety culture of an institution to improve the provision of safety to patients and professionals. METHODOLOGY: Cross-sectional study with a quantitative approach. The sample was made up of 119 health professionals who made up the multidisciplinary team at the surgical center from August to September 2021, where data collection took place. The Hospital Survey on Patient Safety Culture (HSOPSC) instrument was used to evaluate the twelve dimensions that make up patient safety culture. Data analysis was carried out using descriptive statistics, to evaluate the reliability of the responses to the HSOPSC instrument, the Cronbachs Alpha test was used. RESULTS: Of the twelve dimensions evaluated, there was no dimension considered strong for patient safety in the unit. The dimensions with potential for patient safety were "Expectations and actions of the supervisor/manager to promote patient safety"; "Teamwork within units" and "Organizational learning - continuous improvement", while all other dimensions were evaluated as weak for patient safety. 39.50% of participants consider patient safety in the unit to be regular, despite this, 89.91% of participants reported not having made any event notifications in the last 12 months. CONCLUSION: The study highlighted the need to strengthen all dimensions of the patient safety culture by the team at the hospital studied, as none of them were identified as strong.

RESUMO Introdução: O conceito do cuidado seguro permeia as instituições de saúde no mundo todo, no entanto, é preciso entender a cultura de segurança de uma instituição para melhorar a oferta de segurança aos pacientes e aos profissionais. Metodologia: Estudo transversal com abordagem quantitativa. A amostra foi composta por 119 profissionais de saúde que compunham a equipe multiprofissional do centro cirúrgico nos meses de agosto a setembro de 2021, onde ocorreu a coleta de dados. Foi utilizado o instrumento Hospital Survey on Patient Safety Culture (HSOPSC), para avaliar as doze dimensões que compõem a cultura de segurança do paciente. A análise dos dados foi feita através de estatística descritiva, para avaliar a confiabilidade das respostas ao instrumento HSOPSC, utilizou-se o teste Alpha de Cronbach. Resultados: Das doze dimensões avaliadas, não houve nenhuma dimensão considerada forte para segurança do paciente na unidade. As dimensões com potencial para segurança do paciente foram "Expectativas e ações do supervisor/chefia para a promoção da segurança do paciente"; "Trabalho em equipe dentro das unidades" e "Aprendizado organizacional - melhoria contínua", enquanto todas as outras dimensões foram avaliadas como fracas para segurança do paciente. 39,50% dos participantes consideram a segurança do paciente na unidade como regular, apesar disso 89,91% dos participantes relataram não ter realizado nenhuma notificação de evento nos últimos 12 meses. Conclusão: O estudo apontou a necessidade do fortalecimento de todas as dimensões da cultura de segurança do paciente pela equipe do hospital pesquisado, uma vez que nenhuma delas foi apontada como forte.

BACKGROUND: Traditional methods for assessing individual energy consumption often involve lengthy and intricate procedures. This study aims to introduce an Energy Consumption Estimation Scale, utilizing Item Response Theory (IRT) for adolescents aged 18-19 years. METHODS: This psychometric investigation applies IRT to 93 items extracted from a validated food frequency questionnaire. The study encompasses a representative sample of 2515 adolescents from the São Luís birth cohort in Brazil. The latent trait, energy intake, is derived using IRT and subsequently validated through hierarchical multiple linear regression modeling. Significance was established at p < 0.05. RESULTS: A Samejima's model was successfully fitted (CFI and TLI > 0.9 and RMSEA < 0.08), effectively capturing variations across all energy consumption levels. Factors associated with the latent trait demonstrate consistent behavioral patterns. Adolescents with higher energy intake exhibited increased consumption of dairy products, artificially sweetened beverages, and seasonal fruits and vegetables. CONCLUSIONS: The proposed Energy Consumption Estimation Scale demonstrates a reliable measurement of energy intake and serves as a practical and concise alternative for assessing energy consumption among adolescents. These findings suggest the potential for adapting similar models for different age groups and incorporating diverse food items based on the obtained results.

There seems to be a consensus that adolescents worldwide are not getting enough sleep. This study aimed to evaluate the psychometric properties of the Epworth Sleepiness Scale (ESS) in adolescents using the item response theory. A psychometric study was conducted with 2206 adolescents aged 18 and 19 years in the city of São Luís, Maranhão, Brazil. The dimensionality of the ESS was assessed by principal component analysis. A Samejima's graded response model (SGRM) was fitted to it. The findings of this study showed a good internal consistency and the unidimensionality of the ESS. Considering the latent trait continuum, we obtained three levels, with anchor items. For the item 'Sitting still in a public place', the adolescents presented a small possibility of dozing in level 1, and a medium and great possibility in level 2. The item 'Sitting around talking to someone' presented small, medium, and great possibilities of dozing in level 3. The ESS with five items showed satisfactory psychometric properties. In addition, the results from the SGRM showed that adolescents with high levels of sleepiness are likely to nod off or sleep sitting up while talking to someone. This study allows us to understand excessive daytime sleepiness in adolescents.

OBJECTIVE: To identify the factors associated with Upright Delivery (UD) performed in hospitals linked to the Rede Cegonha (RC) in Brazil. METHODS: Cross-sectional study with 3,073 parturients who had vaginal delivery in 606 health facilities in Brazil, located in health regions with a regional action plan approved in the RC. Socioeconomic, demographic, and obstetric characteristics of the parturients, organizational and management aspects of maternity hospitals, and work processes in childbirth care were evaluated. The multivariate logistic regression model with a hierarchical approach was adjusted to identify the variables associated with UD (outcome), estimating Odds Ratios (OR) with a significance level of 5%. RESULTS: Of the evaluated parturient, 6.7% gave birth in the vertical position. The following were associated with a greater chance of PPV: being black (OR=2.07); having 13 or more years of study (OR=3.20); giving birth in a high-risk hospital (OR=1.58); giving birth in PPP rooms (which assisted with labor, delivery, and puerperium in the same environment) in Obstetric Centers (OR=2.07) or in-hospital Normal Delivery Centers (OR=1.62); being assisted by an obstetrician nurse (OR=1.64) or by a midwife (OR=7.62) when compared to a doctor; receiving massage during labor and delivery (OR=1.89); using a stool (OR=4.16) and among women who did not ask for/not receive analgesia (OR=3.15). CONCLUSION: The UD is an event related to racial aspects and the education of the parturient, being stimulated in health establishments where good practices of childbirth care are implemented, with adequate ambiance, and with multidisciplinary teams comprising midwives and obstetric nurses.

This study aimed to develop an intelligible scale to measure the level of supply of best practices in labor, childbirth and delivery in maternity units in the Rede Cegonha (RC) based on the workers' perception. The scale consisted of seventeen items related to best practices, based on the instrument used in the "Evaluation of care during delivery and birth in maternity hospitals in the RC" research The three-parameter logistic model of Item Response Theory was used to create the scale and analyze the items. The scale consisted of three levels. In the first level, there are maternity hospitals that adequately offered strategies for welcoming and encouraging the pregnant woman to circulate during labor. The second level maternity wards also included the adequate offer of the right to a companion of choice, massage, ball and different birth positions. Finally, the third level maternity hospitals also offered an adequate delivery stool, as well as the items already mentioned. The findings of this study showed the contribution of each item in measuring the level of supply of best practices in care for labor, childbirth and delivery and the construction of an intelligible scale to assess RC maternity hospitals.


Este estudo objetivou elaborar uma escala interpretável para mensurar o nível de oferta das boas práticas no trabalho de parto e parto em maternidades da Rede Cegonha (RC) a partir da percepção dos trabalhadores. A escala foi composta por dezessete itens relacionados às boas práticas, obtidos a partir do instrumento utilizado na pesquisa "Avaliação da atenção ao parto e nascimento em maternidades da RC". O modelo logístico de três parâmetros da Teoria de Resposta ao Item foi utilizado para criação da escala e análise dos itens. A escala foi composta por três níveis âncoras. RESUMO Objetivo: Identificar os fatores associados aos partos na posição vertical realizados em hospitais vinculados à Rede Cegonha no Brasil. Métodos: Estudo transversal com 3.073 parturientes que tiveram parto vaginal em 606 estabelecimentos de saúde no Brasil, localizados em regiões de saúde com plano de ação regional aprovado na Rede Cegonha. Foram avaliadas características socioeconômicas, demográficas e obstétricas das parturientes, aspectos organizacionais e de gestão das maternidades e processos de trabalho na atenção ao parto. Modelo de regressão logística multivariada com abordagem hierarquizada foi ajustado para identificar as variáveis associadas ao parto na posição vertical (desfecho), estimando-se odds ratio (OR) com nível de significância de 5%. Resultados: Do total de parturientes avaliadas, 6,7% das mulheres tiveram parto na posição vertical. Estiveram associados à maior chance de ocorrência do parto na posição vertical: ser preta (OR=2,07); ter 13 ou mais anos de estudo (OR=3,20); parir em hospital de alto risco (OR=1,58); parir em quartos PPP (que dispunham de assistência ao trabalho de parto, parto e puerpério no mesmo ambiente) em centros obstétricos (OR=2,07) ou em centros de parto normal intra-hospitalares (OR=1,62); ser assistida por enfermeiro obstetra (OR=1,64) ou por obstetriz (OR=7,62) quando comparado ao médico; receber massagem durante o trabalho de parto e parto (OR=1,91); utilizar banqueta (OR=4,35) e entre mulheres que não pediram/não receberem analgesia (OR=3,33). Conclusão: O parto na posição vertical é um evento relacionado a aspectos raciais e à escolaridade da parturiente, sendo estimulado em estabelecimentos de saúde onde estão implantadas boas práticas de assistência ao parto, com ambiência adequada e com equipes multiprofissionais contendo obstetriz e enfermeiro obstetra.

Cronbach's Alpha, Pearson's Chi-Square or Fischer's Exact tests (5% significance level) were performed. RESULTS: The final sample consisted of a total of 163 nursing professionals. No strong areas were observed for patient safety. The dimensions "teamwork in the unit", "expectations and actions of the supervisor/manager for the promotion of patient safety" and "organizational learning and continuous improvement" had the highest rates of positive responses, whereas the dimensions "opening for communication" and "feedback and communication about the error" had the lowest percentages. The general degree of patient safety was considered very good 72 (47%) and underreporting of the events was observed, most of them were carried out by nurses. CONCLUSION: No dimension assessed was considered to be a strong area. However, most were shown to be potential areas for patient safety culture.

OBJECTIVE: To determine the factor structure of the instrument Alcohol Use Disorders Identification Test (AUDIT) in a representative sample of adolescents aged 18 to 19 years. METHODS: Cross-sectional study performed with adolescents born in São Luís (MA). The internal consistency of the instrument was determined by the Cronbach's alpha coefficient, and the validity of the construct was assessed by Confirmatory Factor Analysis (CFA). The Kaiser-Meyer-Olkin (KMO) was estimated to analyze the adequacy of the sample. The fit quality of the factor model was analyzed according to the indexes of the Chi-square adjustment test, comparative fit index (CFI), Tucker-Lewis index (TLI) and root mean square error of approximation (RMSEA). RESULTS: The sample of the study was composed of 1,002 adolescents aged from 18 to 19 years, being 56.8% girls, 68.5% with 18 years, 63.3% brown, 48.6% belonging to class C, 15.4% did not work or did not study, and 52.1% had divorced parents. The sample was suitable for confirmatory factor analysis (KMO = 0.79); Cronbach's alpha coefficient was 0.70, demonstrating satisfactory internal consistency with factor loads above 0.5, except for item 9, "was injured or someone else was injured due to drinking." Confirmatory factor analysis revealed the validity of the three-factor model for the studied sample based on the indices of psychometric adjustments. CONCLUSION: The three-factor AUDIT factor structure was confirmed for the population of adolescents between 18 and 19 years old living in São Luís, ratifying the original conceptual domains proposed by the World Health Organization. AUDIT proved to be a reliable instrument to identify the consumption of alcohol.

RESUMO Objetivo Analisar a cultura de segurança do paciente entre profissionais de enfermagem da terapia intensiva. Método Estudo transversal realizado em Unidades de Terapia Intensiva de hospital público. A coleta de dados foi realizada de setembro a outubro de 2017 com profissionais de enfermagem, aplicando o questionário Hospital Survey on Patient Safety Culture . Foram realizados os testes Alpha de Cronbach, Qui-Quadrado de Pearson ou Exato de Fischer (nível de significância de 5%). Resultados A amostra final foi composta por 163 profissionais de enfermagem. Não foram observadas áreas fortes para a segurança do paciente. As dimensões "trabalho em equipe na unidade", "expectativas e ações do supervisor/chefia para a promoção da segurança do paciente" e "aprendizado organizacional e melhoria contínua" apresentaram os maiores índices de respostas positivas, enquanto as dimensões "abertura para comunicação" e "retorno das informações e da comunicação sobre o erro" obtiveram os menores percentuais. O grau geral de segurança do paciente foi considerado muito bom (72 = 47%) e observou-se subnotificação dos eventos, sendo a maioria realizada pelos enfermeiros. Conclusão Nenhuma dimensão avaliada foi considerada área forte, porém, a maioria mostrou-se como áreas em potenciais para a cultura de segurança do paciente.


RESUMEN Objetivo Analizar la cultura de seguridad del paciente entre los profesionales de enfermería de cuidados intensivos. Método Se trata de un estudio transversal realizado en Unidades de Cuidados Intensivos de un hospital público. La recolección de datos se llevó a cabo con el Cuestionario Hospitalario sobre la Cultura de Seguridad del Paciente entre septiembre y octubre de 2017, con profesionales de enfermería. Se aplicaron las pruebas Alfa de Cronbach, Chi-cuadrado de Pearson o Exacta de Fischer (nivel de significación del 5%). Resultados La muestra final estaba compuesta por 163 profesionales de enfermería. No se observaron áreas fuertes en la seguridad del paciente. Las dimensiones "trabajo en equipo de la unidad", "expectativas y acciones del supervisor/liderazgo para promover la seguridad del paciente" y "aprendizaje organizativo y mejora continua" mostraron los índices más altos de respuestas positivas, mientras que las dimensiones "apertura a la comunicación" y "respuestas a la información ( feedback ) y comunicación sobre el error" obtuvieron los porcentajes más bajos. El nivel de seguridad general del paciente se consideró como muy bueno, 72 (47%), y se observó una infradeclaración de sucesos, la mayoría de los cuales realizados por los enfermeros. Conclusión Ninguna dimensión evaluada ha sido considerada un área fuerte; sin embargo, la mayoría se mostró como un área potencial para la cultura de seguridad del paciente.

RESUMEN

ABSTRACT OBJECTIVE: To determine the factor structure of the instrument Alcohol Use Disorders Identification Test (AUDIT) in a representative sample of adolescents aged 18 to 19 years. METHODS: Cross-sectional study performed with adolescents born in São Luís (MA). The internal consistency of the instrument was determined by the Cronbach's alpha coefficient, and the validity of the construct was assessed by Confirmatory Factor Analysis (CFA). The Kaiser-Meyer-Olkin (KMO) was estimated to analyze the adequacy of the sample. The fit quality of the factor model was analyzed according to the indexes of the Chi-square adjustment test, comparative fit index (CFI), Tucker-Lewis index (TLI) and root mean square error of approximation (RMSEA). RESULTS: The sample of the study was composed of 1,002 adolescents aged from 18 to 19 years, being 56.8% girls, 68.5% with 18 years, 63.3% brown, 48.6% belonging to class C, 15.4% did not work or did not study, and 52.1% had divorced parents. The sample was suitable for confirmatory factor analysis (KMO = 0.79); Cronbach's alpha coefficient was 0.70, demonstrating satisfactory internal consistency with factor loads above 0.5, except for item 9, "was injured or someone else was injured due to drinking." Confirmatory factor analysis revealed the validity of the three-factor model for the studied sample based on the indices of psychometric adjustments. CONCLUSION: The three-factor AUDIT factor structure was confirmed for the population of adolescents between 18 and 19 years old living in São Luís, ratifying the original conceptual domains proposed by the World Health Organization. AUDIT proved to be a reliable instrument to identify the consumption of alcohol.


RESUMO OBJETIVO: Determinar a estrutura fatorial do instrumento Alcohol Use Disorders Identification Test (Audit) em uma amostra representativa de adolescentes de 18 a 19 anos. MÉTODOS: Estudo transversal realizado com adolescentes nascidos em São Luís (MA). A consistência interna do instrumento foi determinada pelo coeficiente alfa de Cronbach e a validade do construto foi avaliada por meio da Análise Fatorial Confirmatória (AFC). O índice de Kaiser-Meyer-Olkin (KMO) foi calculado para analisar a adequação da amostra. A qualidade de ajuste do modelo fatorial foi analisada de acordo com os índices dos testes qui-quadrado de ajustamento, comparative fit index (CFI), Tucker-Lewis index (TLI) e root mean square error of approximation (RMSEA). RESULTADOS: A amostra do estudo foi composta por 1.002 adolescentes entre 18 e 19 anos, sendo 56,8% meninas, 68,5% com 18 anos, 63,3% pardos ou mulatos, 48,6% pertencentes à classe C, 15,4% não trabalhavam e não estudavam e 52,1% tinham pais separados. A amostra foi adequada para a análise fatorial confirmatória (KMO = 0,79) e o coeficiente do alfa de Cronbach foi de 0,70, demostrando consistência interna satisfatória com cargas fatoriais acima de 0,5, com exceção do item 9 "Ficou ferido ou ficou alguém ferido por ter bebido". A análise fatorial confirmatória revelou a validade do modelo de três fatores para a amostra em estudo com base nos índices de ajustes psicométricos. CONCLUSÃO: A estrutura fatorial do Audit com três fatores foi confirmada para a população de adolescentes entre 18 e 19 anos residentes em São Luís, ratificando os domínios conceituais originais propostos pela Organização Mundial da Saúde. O Audit apresentou-se como um instrumento confiável para a identificação do consumo de álcool.

RESUMEN

Objetivo: compreender a cultura de segurança fundamentada nos conceitos discutidos na Teoria da Diversidade e Universalidade do Cuidado Cultural e no Modelo Conceitual do Cuidado de Enfermagem Transcultural. Método: estudo reflexivo a partir da Dissertação do Programa de Pós-Graduação em Enfermagem, da Universidade Federal do Maranhão, intitulada "Cultura de segurança do paciente em terapia intensiva na perspectiva de profissionais de Enfermagem". Resultados: percebe-se a aproximação da Enfermagem transcultural na avaliação da cultura de segurança do paciente tendo em vista que ela deve ser baseada em uma comunicação eficaz e por práticas que envolvam as culturas dos gestores, organizações, profissionais de Enfermagem e pacientes sustentando o planejamento de ações e a necessidade de prevenção de erros e danos. Conclusão: os conceitos apresentados são perfeitamente apropriados no contexto da cultura de segurança por remeter à necessidade de considerar os aspectos culturais contribuindo, assim, para a prática do cuidado seguro.(AU)

RESUMEN

Os estudos relacionados à hanseníase pouco abordam a questão humana, os sentimentos ou as experiências vivenciadas nessa trajetória, portanto esse trabalho tem como objetivos descrever as implicações na sexualidade do portador de hanseníase e analisar os aspectos que envolvem a sexualidade do portador. Trata-se de um estudo descritivo com abordagem qualitativa desenvolvido no Centro Maria Imaculada, que presta assistência aos portadores de hanseníase na prevenção, diagnóstico, tratamento e reabilitação, em Teresina-PI. Para a produção dos dados utilizou-se como instrumento um roteiro de entrevista semi-estruturada, os dados foram analisados de acordo com a Análise de Conteúdo de Minayo. As análises dos relatos culminaram no surgimento de quatro categorias: mudanças na atividade sexual após o diagnóstico de hanseníase; medo da transmissão da doença pelo contato sexual; sentimento de inferioridade e mudanças no cotidiano familiar. Verificou-se a dimensão do sofrimento causado pela doença no grupo de pessoas entrevistadas e as mudanças acarretadas em sua vida sexual ocasionados pela doença, impedindo que seus portadores vivenciem uma vida sexual ativa, sejam por problemas de ordem orgânica e/ou psicológica.


Studies related to leprosy give little attention to human affairs, feelings or experiences along the way, therefore, this paper aims to describe the effects leprosy has on the sexuality of leprosy patients and analyze the aspects which involve the sexuality of the bearer. This is a descriptive study with qualitative approach, developed at the "Centro Maria Imaculada", which provides assistance to leprosy patients in the prevention, diagnosis, treatment and rehabilitation, in Teresina-PI. A series of semi-structured interviews was used as an instrument so as to produce data; they were analyzed according to Minayo's Content Analysis. Analysis of the reports resulted in the emergence of four categories: changes in sexual activity after leprosy diagnosis, fear of disease transmission through sexual contact, feelings of inferiority, and changes in family's daily life. The extent of suffering caused by the disease in the group of interviewed people was noticed, and so were the changes brought about in their sexual life caused by the disease, preventing bearers from experiencing an active sexual life, be it due to organic and/or psychological matters.
