RESUMEN

We report a new case of cutaneous leishmaniosis caused by Leishmania (Mundinia) martiniquensis in a horse in Florida, USA. A 10-year-old neutered male Quarter horse was presented with multifocal to coalescing, raised, ulcerated and oozing, non-healing wounds on both pinnae of several weeks' duration. After a few months, the lesions regressed spontaneously. Biopsies of the lesions were performed with microscopical findings of epidermal hyperplasia with multifocal ulceration and focally extensive, dermal pyogranulomatous inflammation with numerous intact and degenerate neutrophils being surrounded by epithelioid macrophages, lymphocytes and plasma cells, as well as rare eosinophils. Within the macrophages, and freely within the inflammatory infiltrate, were small (2-4 µm) round, basophilic protozoal organisms. Immunohistochemistry and colourimetric in-situ hybridization were positive for amastigote forms of Leishmania spp. The species L. martiniquensis was identified by polymerase chain reaction targeting the ITS-1 gene performed with extracts from formalin-fixed and paraffin wax-embedded samples of skin lesions. L. martiniquensis causes an ulcerative pyogranulomatous dermatitis in horses with spontaneous healing. This second autochthonous case in Florida, 5 years after the first case, suggests that this parasite may have become endemic in this state.

The purpose of the present work was to evaluate the accuracy of quantitative polymerase chain reaction (qPCR) performed on samples of fresh frozen tissue (FT) and formalin-fixed, paraffin-embedded (FFPE) healthy skin. This is a validation study conducted with samples from 46 dogs from an endemic area in Brazil. After sample collection, DNA extractions were conducted using commercial kits and qPCR was oriented to kinetoplast DNA (kDNA) targets of the Leishmania infantum species. The results obtained for the FFPE samples showed 63.6% sensitivity and 77.1% specificity, whereas those obtained for the FT samples showed 100% and 48.6%, respectively. Poor agreement was observed for the results of the qPCR technique with FT and FFPE samples. Our results suggest freezing as the most suitable conservation method for the formation of sample databases considering DNA recovery.(AU)


O objetivo deste trabalho foi avaliar a acurácia da PCR quantitativa (qPCR) realizada em amostras de pele íntegra congelada (FT) e parafinada (FFPE). Trata-se de um estudo de validação, com amostras provenientes de 46 cães de uma área endêmica no Brasil. Após as coletas de amostras, as extrações de DNA foram realizadas utilizando-se kits comerciais, e a qPCR foi orientada para alvos do kDNA da espécie Leishmania infantum. Os resultados obtidos para as amostras FFPE foram 63,6% de sensibilidade e 77,1% de especificidade; para as amostras FT, 100% e 48,6%, respectivamente. A concordância dos resultados da técnica de qPCR com amostras FT e FFPE foi pobre. Os resultados sugerem que o congelamento é o método mais adequado de conservação para banco de amostras para recuperação de DNA.(AU)

Phosphogypsum can be classified as a Naturally Occurring Radioactive Material (NORM) residue of the phosphate fertilizer industry. One of the main environmental concerns of its use as building material is the radon exhalation. The aim of this study is to measure the radon exhalation rate from plates and bricks manufactured with phosphogypsum from three installations of the main Brazilian producer, Vale Fertilizantes, in order to evaluate the additional health risk to dwellers. A simple and reliable accumulator method involving a PVC pipe sealed with a PVC pipe cover commercially available with CR-39 radon detector into a diffusion chamber was used for measuring radon exhalation rate from phosphogypsum made plates and bricks. The radon exhalation rate from plates varied from 0.19 ± 0.06 Bq m-2 h-1, for phosphogypsum from Bunge Fertilizers, from 1.3 ± 0.3 Bq m-2 h-1, for phosphogypsum from Ultrafertil. As for the bricks, the results ranged from 0.11 ± 0.01 Bq m-2 h-1, for phosphogypsum from Bunge Fertilizers, to 1.2 ± 0.3 Bq m-2 h-1, for phosphogypsum from Ultrafertil. The results obtained in this study for the radon exhalation rate from phosphogypsum plates and bricks are of the same order of magnitude than those from ordinary building materials. So, it can be concluded that the recycling of phosphogypsum as building material is a safe practice, since no additional health risk is expected from the radiological point of view.

The Waste Management Department of Nuclear and Energy Research Institute (IPEN) is responsible for the safety management of the waste generated at all internal research centers and that of other waste producers such as industry, medical facilities, and universities in Brazil. These waste materials, after treatment, are placed in an interim storage facility. Among them are (226)Ra needles used in radiotherapy, siliceous cake arising from conversion processes, and several other classes of waste from the nuclear fuel cycle, which contain Ra-226 producing (222)Rn gas daughter.In order to estimate the effective dose for workers due to radon inhalation, the radon concentration at the storage facility has been assessed within this study. Radon measurements have been carried out through the passive method with solid-state nuclear track detectors (CR-39) over a period of nine months, changing detectors every month in order to determine the long-term average levels of indoor radon concentrations. The radon concentration results, covering the period from June 2012 to March 2013, varied from 0.55 ± 0.05 to 5.19 ± 0.45 kBq m(-3). The effective dose due to (222)Rn inhalation was further assessed following ICRP Publication 65.

Recently, the idea of generating radon map of Brazil has emerged. First attempts of coordinating radon surveys--carried out by different groups across the country--and initial discussions on how to proceed on a larger scale were made at the First Brazilian Radon Seminary, Natal, September 2012. Conventionally, it is believed that indoor radon is no major problem in Brazil, because the overall benign climate usually allows high ventilation rates. Nevertheless, scattered measurements have shown that moderately high indoor radon concentrations (up to a few hundred Bq m⁻³) do occur regionally. Brazilian geology is very diverse and there are regions where an elevated geogenic radon potential exists or is expected to exist. Therefore, a Brazilian Radon Survey is expected to be a challenge, although it appears an important issue, given the rising concern of the public about the quality of its environment.

This study describes the epidemiological, clinical and mycological aspects of feline sporotrichosis cases attending the Laboratory of Clinical Research on Dermatozoonosis in Domestic Animals - Evandro Chagas Clinical Research Institute (LAPCLIN-DERMZOO/IPEC/FIOCRUZ), from 1998 to 2005. It was possible to get in contact with 147 (19.2%) cat owners. One hundred and thirteen (76.9%) cats were male, 117 (79.6%) had no defined race and 87 (59.2%) were sexually intact. The age ranged from 72 to 216 months (median = 108 months). Nineteen cats were reassessed: eleven (57.8%) were male, thirteen (36.8%) were breed and fifteen (47.3%) castrated. Fourteen (52.6%) animals lived at home and did not roamed the streets. Seven (36.8%) had normal clinical findings and negative mycological examination. Twelve (63.1%) cats had skin lesions compatible with sporotrichosis. Thirty-one (21%, n = 147) cats disappeared after abandoning treatment, 36 (24.5%, n = 147) were alive and 80 (54.4%, n = 147) had died. Causes of death informed by the owners were: sporotrichosis in 35 (43.7%, n = 80), accidental death in 27 (33.7%, n = 80) and other diseases in 18 (22.5%, n = 80). Withdrawal of treatment occurred mainly at the time of clinical improvement and may represent a serious obstacle to the control of sporotrichosis.

This study compared the sensitivity of acetate tape impression and skin squeezing with that of deep skin scraping for the diagnosis of demodicosis in dogs. Demodex canis was detected in 100% of acetate tape impressions obtained after skin squeezing and in 90% of deep skin scrapings. There was a significant difference (P < 0.001) between the techniques in the total number of mites detected. Acetate tape impression with skin squeezing was found to be more sensitive than deep skin scraping and is an alternative diagnostic method for canine demodicosis.

Phosphogypsum, a fertilizer industry by-product, is being worldwide stockpiled, posing environmental concerns. Since this material contains natural radionuclides in significant concentrations, its use as a building material has radiological implications. In order to confirm the feasibility of the use of a new material mainly composed by phosphogypsum, an experimental house was built, having some of its rooms entirely lined with this material. Measurements of samples of phosphogypsum plates from different origins resulted in values of 0.2 to 2.6 for the external radiation index, thus justifying a more detailed investigation. In this paper, the application of a previously developed computational model to forecast external doses indoors is described. A comprehensive radiological evaluation is being performed, including measurement of the external gamma exposure and radon concentrations in one of the rooms of the house. The results show that the annual increment in the effective dose to an inhabitant of the house will remain below the 1 mSv limit for every reasonable scenario. The radon measurements were carried out over a period of 18 months, in order to determine the long-term average levels of the indoor radon concentrations. The results obtained are below 200 Bq m(-3), the recommended investigation level for radon.

Indoor radon concentrations have been measured in Santana cave, the most frequented cave of PETAR (High Ribeira River Tourist State Park), situated southern of Sao Paulo State, Brazil. The measurements were carried out with CR-39 detectors installed in four of the most frequently visited galleries. Preliminary results from November 2009 to June 2010 show radon concentrations varying from 1.9 ± 0.1 to 8.4 ± 0.6 kBq m(-3). The total annual effective dose for all galleries was 3.32 mSv. The complete evaluation will be concluded by September 2010.

Fatigue is an exceedingly common often treatable problem in cancer patients that profoundly affects all aspects of quality of life. Prevalence estimates have ranged from 50% to 90% of cancer patients overall. After addressing reversible or treatable contributing factors, such as hypothyroidism, anemia, sleep disturbance, pain, emotional distress, climacterium, medication adverse events, metabolic disturbances, or organ dysfunction such as heart failure, myopathy, and pulmonary fibrosis, patients may be screened with a brief fatigue self-assessment tool. All cancer patients should be screened regularly for fatigue. Those with moderate or severe fatigue may benefit from both pharmacologic and nonpharmacologic interventions, while mild fatigue that does not interfere with quality of life can be treated with nonpharmacologic measures alone. Physicians often have insufficient knowledge about fatigue and its treatments or underestimate the impact of fatigue on quality of life, while patients may consider it an unavoidable and untreatable side-effect and fear that reporting it may incite a change toward less aggressive cancer treatment. A practical review may therefore be useful to health care professionals in order to avoid the common barriers to its treatment that exist on the sides of both physicians and patients.

Termas de Araxá spa is located in Araxá, State of Minas Gerais. In this region, several minerals rich in uranium and thorium are found. The mineral waters and mud from Termas de Araxá spa have been used for therapeutic and recreation purposes. In this study, the committed effective dose was evaluated for workers and patients at Araxá spa due to (222)Rn and (212)Pb inhalation. Radon measurements were carried out through the passive method with solid state nuclear track detectors (Makrofol E) over a period of 21 months, with results varying from 258 +/- 20 to 1634 +/- 111 Bq m(-3). The (212)Pb air concentration was assessed through the modified Kusnetz's method, the results varying from 0.3 +/- 0.1 to 2.1 +/- 0.2 Bq m(-3). Doses received by the spa workers are below 20 mSv.y(-1), suggested by ICRP 60 as an annual effective dose limit for occupational exposure. The radiation doses for the patients are below the mean annual effective dose due to natural sources estimated to be 2.4 mSv.y(-1).

At the Instituto de Pesquisas Energeticas e Nucleares (IPEN), Sao Paulo, Brazil, there is a facility (thorium purification plant) where materials with high thorium concentrations are manipulated. In order to estimate afterwards the lung cancer risk for the workers, the thoron daughter (212Pb) levels were assessed and the committed effective and lung committed equivalent doses for workers in place. A total of 28 air filter samples were measured by total alpha counting through the modified Kusnetz method, to determine the 212Pb concentraion. The committed effective dose and lung committed equivalent dose due to 212Pb inhalation were derived from compartmental analysis following the ICRP 66 lung compartmental model, and ICRP 67 lead metabolic model.
