RESUMEN

Autotransplanted teeth in the maxillary anterior region should be restored or reshaped as soon as possible for functional and esthetic reasons as well as the well-being of the patient. However, such tooth restorations are frequently not carried out immediately after tooth transplantation because the process could have a negative impact on the healing process. The development of a simple, immediate, and atraumatic esthetic interim restoration would be of great benefit to both the clinician and patient and address all the concerns caused by immediately preparing the tooth postoperatively and cementing a permanent restoration. This report describes the use of CAD-CAM technology to create poly (methyl methacrylate) veneers preoperatively that can be cemented extraorally on the extracted donor tooth for the immediate and interim recontouring of autotransplanted anterior teeth.

RESUMEN

The trial restoration increases outcome predictability in restorative treatments, enhances communication among specialists and patients, and guides tooth preparations. It should reproduce the planned design precisely because many decisions will be made based on the transferred design. Traditionally, a diagnostic waxing design is transferred to the mouth with a flexible silicone matrix. However, a rigid matrix would ensure an accurate transfer of the planned design by avoiding the deformation of the silicone index. A step-by-step technique for fabricating a computer-aided design and computer-aided manufactured (CAD-CAM) rigid matrix relined with polyvinyl siloxane material to fabricate a trial restoration is presented. The technique ensures accurate detail reproduction and dimensional stability, avoiding deformation and ensuring a predictable transfer of the planned design.

BACKGROUND: The interarch space is defined as the vertical space between the edentulous ridge and the occlusal or incisal aspect of the opposing arch. Measuring the interarch space in a patient requiring an implant-supported fixed prosthesis is crucial for determining the prostheses to be used in each clinical situation. Depending on the measurements and other factors, such as the need for lip support or pink esthetics issues, decisions about the most convenient type of implant-supported prosthesis can be taken. Analogic workflow to measure the interarch space can represent a time-consuming and expensive procedure that may lead to inaccuracies. OBJECTIVE: To describe a step-by-step protocol to measure the upper and lower interarch space with open-access software, starting from the digital scan of the patient's complete dentures obtained with an intraoral scanner. METHODS: An extraoral scan (using an intraoral scanner) of the existing complete removable prosthesis is required to assess the interarch space for making an implant-supported prosthesis treatment planning. If the existing complete denture does not fulfill the required functional and esthetic parameters, a complete interim denture, a wax trial denture, or a printed denture prototype should be previously fabricated. The antagonist arch and the intermaxillary record scan also need to be obtained. Finally, all STL files are imported into an open-access software for measuring the interarch space. RESULTS: Open-source software can be used to measure the upper and lower interarch space from existing complete dentures following the step-by-step protocol outlined in this paper. CONCLUSION: Interarch space of edentulous patients rehabilitated with complete dentures can be measured with a 360 degrees scanning of the existing complete denture and open-access software by applying the protocol outlined in this paper. CLINICAL RELEVANCE: The clinician can assess and digitally measure the interarch space of edentulous patients requiring an implant-supported fixed prosthesis using an open access software.

ABSTRACT Introduction: Rowers are especially prone to oral health problems due to the massive consumption of supplements and isotonic drinks that lead eating disorders, due to the need for weight control, and to the development of teeth clenching caused by the constant activation of the chewing muscles during sports activity. Objective: To evaluate the oral health status of a group of Brazilian rowers and to identify the main risk factors influencing their oral health. Methods: One hundred and twenty rowers (84 men and 36 women) with an age of 24.16 ± 5.74 years participated in this study; an average height of 1.79 ± 0.09 cm and body mass of 75.02 ± 11.64 kg were detected by means of a questionnaire focused on the main factors that influence the deterioration in athletes' oral health. Results: The dental results revealed a significant number of rowers with the oral breathing pattern (46.7%), malocclusion (65.8%), bruxism (33.3%) and noise and/or pain in the TMJ (temporomandibular joint) (20.8%). The use of supplements and isotonic drinks (48.3%) and the incidence of bleeding gums (30.83%) were also found. Conclusion: The results show the importance of making rowers aware of the relevance of oral health and the need for prevention systems, ideally including sports dentistry specialists within the sports medicine departments of rowing clubs. RESUMEN Introducción: Los remeros son especialmente propensos a sufrir problemas de salud bucodental debido al uso masivo de suplementos e isotónicos, con tendencia a trastornos alimentarios por la exigencia de control de peso y el apretamiento dental, por la constante activación de los músculos masticatorios durante la actividad deportiva. Objetivo: Evaluar el estado bucal de un grupo de remeros brasileños e identificar los principales factores de riesgo para la salud bucal de estos deportistas. Material y método: En este estudio participaron 120 remeros (84 hombres y 36 mujeres) con una edad de 24,16 ± 5,74 años; una altura de 1,79 ± 0,09 cm y una masa corporal de 75,02 ± 11,64 kg, por medio de un cuestionario que indagó sobre los principales problemas de la salud bucodental de los deportistas. Resultados: Los resultados relativos al área dental revelaron un número significativo de remeros con patrón de respiración oral (46,7%), con historia de maloclusión (65,8%), con parafunción de bruxismo (33,3%) y con ruido y/o dolor de la ATM (articulación temporomandibular) (20,8%). El uso de suplementos e isotónicos (48,3%) y la incidencia de sangrado gingival (30,83%) son también datos relevantes encontrados. Conclusión: Los resultados obtenidos muestran la importancia de concientizar a esta categoría deportiva sobre la relevancia de la salud bucodental y establecer sistemas de prevención, preferiblemente con la inclusión de un especialista en odontología deportiva en el departamento de medicina deportiva de los clubes de remo. RESUMO Introdução: Os remadores são particularmente propensos a problemas de saúde oral devido ao uso generalizado de suplementos e isotônicos, tendência a distúrbios alimentares pela exigência do controle de peso e ao cerramento dos dentes causado pela ativação constante dos músculos mastigatórios durante a atividade esportiva. Objetivo: Avaliar os distúrbios orais dos remadores brasileiros e identificar os principais fatores de risco de saúde bucal desses atletas. Métodos: Participaram deste estudo 120 remadores (84 homens e 36 mulheres) com idade de 24,16 ± 5,74 anos, estatura de 1,79 ± 0,09 cm e massa corporal de 75,02 ± 11,64 kg, que foram avaliados com um questionário que investigou os principais problemas de comprometimento da saúde oral dos atletas. Resultados: Os resultados odontológicos revelaram um número significativo de remadores com padrão respiratório oral (46,7%), com histórico de maloclusão (65,8%), com parafunção oclusal de bruxismo (33,3%) e com ruído e/ou dor na ATM (articulação temporomandibular) (20,8%). O uso de suplementos e isotônicos (48,3%) e a incidência de sangramento gengival (30,83%) também são achados relevantes. Conclusão: Os resultados obtidos mostram a importância de conscientizar essa categoria esportiva sobre a relevância da saúde oral e de estabelecer sistemas de prevenção, de preferência, com a inclusão de um especialista em odontologia do esporte no departamento de medicina esportiva dos clubes de remo. BACKGROUND: An interim hollow shell (IHS) is a temporary prosthesis that adapts like a cap over a prepared tooth abutment. Using a conventional protocol to fabricate IHS from casts of the initial situation or the wax-up can be challenging, time-consuming, and sometimes frustrating. A digital workflow makes this process quicker and more convenient. The IHS must be first designed with computer-aided design (CAD), then fabricated with computer-aided manufacture (CAM). Proprietary dental software is commonly used for the design process but needs to be purchased. OBJECTIVE: To describe a step-by-step technique for designing an IHS for posterior relining with open-source software. METHODS: This paper describes a straightforward procedure to design an IHS from a dental scan of the initial situation or a digital wax-up for an esthetic and functional temporary rehabilitation. RESULTS: An IHS can be quickly designed using open-source software by copying an existing restoration or a conventional or digital wax-up. Then, the design can be 3D printed using a biocompatible resin. CONCLUSIONS: The clinician can use open-source software to design IHSs, which are then 3D printed using a biocompatible resin. CLINICAL RELEVANCE: The clinician can design IHSs from a dental scan of the initial situation or a wax-up using open-source software by following the step-by-step protocol outlined in this paper. The restoration can then be 3D printed using a biocompatible resin.
