BACKGROUND: High salt (HS) intake induces an augmented hypertensive response to nitric oxide (NO) inhibition, though it causes minimal changes in blood pressure (BP) in NO intact condition. The cause of such augmentation is not known. HS induces tumor necrosis factor-alpha (TNFα) production that causes natriuresis via activation of its' receptor type 1 (TNFR1). We hypothesized that NO deficiency reduces renal TNFR1 activity, leading to enhanced sodium retention and hypertension. METHODS: We examined the changes in renal TNFR1 protein expression (Immunohistochemistry analyses) after HS (4% NaCl) intake in wild-type mice (WT, C57BL6) treated with a NO synthase (NOS) inhibitor, nitro-L-arginine methyl ester (L-NAME; 0.05 mg/min/g; osmotic mini-pump), as well as in endothelial NOS knockout mice (eNOSKO) and compared the responses in WT mice with normal salt (NS; 0.3% NaCl) intake. BP was measured with tail-cuff plethysmography and 24-hour urine collections were made using metabolic cages. RESULTS: HS alone did not alter mean BP in untreated mice (76±3 to 77±1 mmHg) but induced an augmented response in L-NAME treated (106±1 vs 97±2 mmHg) and in eNOSKO (107±2 vs 89±3 mmHg) mice. The percentage area of TNFR1 expression in renal tissue was higher in WT+HS (4.1 + 0.5%) than in WT+NS mice (2.7±0.6%). However, TNFR1 expression was significantly lower in L-NAME treated WT+NS (0.9±0.1%) and in eNOSKO+NS (1.4±0.2%) than in both WT+NS and WT+HS mice. CONCLUSION: These data indicate that TNFR1 activity is downregulated in NO deficient conditions, which facilitates salt retention leading to augmented hypertension during HS intake.

Isomaltulose, a slowly digested isocaloric analog of sucrose, and allulose, a noncaloric fructose analog, are promoted as "healthful" sugar alternatives in human food products. Here we investigated the appetite and preference conditioning actions of these sugar analogs in inbred mouse strains. In brief-access lick tests (Experiment 1), C57BL/6 (B6) mice showed similar concentration dependent increases in licking for allulose and fructose, but less pronounced concentration-dependent increases in licking for isomaltulose than sucrose. In Experiment 2, B6 mice were given one-bottle training with a CS+ flavor (e.g., grape) mixed with 8% isomaltulose or allulose and a CS- flavor (e.g., cherry) mixed in water followed by two-bottle CS flavor tests. The isomaltulose mice showed only a weak CS+ flavor preference but a strong preference for the sugar over water. The allulose mice strongly preferred the CS- flavor and water over the sugar. The allulose avoidance may be due to gut discomfort as reported in humans consuming high amounts of the sugar. Experiment 3 found that the preference for 8% sucrose over 8% isomaltulose could be reversed or blocked by adding different concentrations of a noncaloric sweetener mixture (sucralose + saccharin, SS) to the isomaltulose. Experiment 4 revealed that the preference of B6 or FVB/N mice for isomaltulose+0.01%SS or sucrose over 0.1%SS increased after separate experience with the sugars and SS. This indicates that isomaltulose, like sucrose, has postoral appetition effects that enhances the appetite for the sugar. In Experiments 5 and 6, the appetition actions of the two sugars were directly compared by giving mice isomaltulose+0.05%SS vs. sucrose choice tests before and after separate experience with the two sugars. In general, the initial preference the mice displayed for isomaltulose+0.05%SS was reduced or reversed after separate experience with the two sugars although some strain and sex differences were obtained. This indicates that isomaltulose has weaker postoral appetition effects than sucrose.

Opioid and dopamine (DA) D1 receptor antagonists differentially reduce nutritive and non-nutritive sweetener intakes in inbred mouse strains. Sucrose intake was more effectively reduced by naltrexone in C57BL/6 (B6) mice relative to 129P3 (129) mice, but more effectively reduced by SCH23390 in 129 mice relative to B6 mice. Opioid and DA D1 antagonists differentially reduced saccharin intakes in B6 mice relative to other strains. Given these differential patterns in sweetener intake in B6 and 129 mice, the present study examined whether systemic naltrexone (0.01-5 mg/kg) and SCH23390 (50-1600 nmol/kg) reduced intakes of 10 % sucrose or 0.2 % saccharin solutions over a 120 min time course in first-generation hybrid mice (B6:129) of B6 and 129 parents and reduced low-nutritive sweetener intakes in 129 mice. Naltrexone (5 mg/kg) significantly reduced 10 % sucrose intake in B6:129 hybrid mice more like that of 129 than B6 mice. In contrast, SCH23390 (400-1600 nmol/kg) reduced 10 % sucrose intake in B6:129 hybrid mice more effectively than that observed in B6 or 129 parental strains. Because 129 mice consumed relatively low amounts of 0.2 % saccharin, they were tested with a more attractive low-nutritive solution containing 0.2 % saccharin and 2 % sucrose. Naltrexone failed to reduce saccharin intake in B6:129 hybrid mice but suppressed saccharin+sucrose intake in 129 mice more like that observed in B6 mice. SCH23390 similarly inhibited saccharin or saccharin+sucrose intakes in hybrid B6:129, 129, and B6 mice with B6 mice more resistant to the lowest SCH23390 dose. Thus, whereas sucrose intake in B6:129 hybrid mice exhibited similar sensitivity to opioid and to a lesser degree DA D1 antagonism to their 129, but not B6 parents, opioid and DA D1 mediation of low- and non-nutritive sweet intake produced unique profiles among B6:129 hybrid and B6 and 129 strains which does not support a simple heritability explanation.

Inbred mouse strains differ in their postoral appetite stimulating response (appetition) to fructose as demonstrated in intragastric (IG) sugar conditioning and oral sugar vs. nonnutritive conditioning experiments. For example, FVB and SWR strains show experience-induced preferences for 8% fructose over a 0.1% sucralose + 0.1% saccharin (S + S) solution, whereas C57BL/6 (B6) and BALB/c strains do not. All strains, however, learn to prefer 8% glucose to S + S after experience, which is attributed to the potent appetition actions of this sugar. The present study extended this analysis to DBA/2 (DBA) and 129P3 (129) inbred mice. In Experiment 1A, ad libitum fed DBA and 129 mice preferred S + S to fructose before and after separate experience with the two sweeteners, indicating an indifference to the postoral nutrient effects of the sugar. When food restricted (Experiment 1B), 129 mice continued to prefer S + S to fructose while DBA mice showed equal preference for the sweeteners after experience, indicating some sensitivity to fructose appetition. In Experiment 1C, both strains acquired significant preferences for glucose over S + S after experience, confirming their sensitivity to postoral glucose appetition. Experiment 2 revealed that C57BL/6 × 129P3 (B6:129) hybrid mice responded like inbred B6 mice and 129 mice in acquiring a preference for glucose but not fructose over S + S. This is of interest because sweet "taste-blind" P2 × 2 / P2 × 3 double-knockout (DKO) mice on a B6:129 genetic background prefer fructose to water in 24 h tests, which is indicative of fructose appetition. Whether differences in the genetic makeup of DKO and B6:129 hybrid mice or other factors explain the fructose appetition of the DKO mice remains to be determined.

In hypertension induced by angiotensin II (AngII) administration with high salt (HS) intake, intrarenal angiotensinogen (AGT) and tumor necrosis factor-alpha (TNF-α) levels increase. However, TNF-α has been shown to suppress AGT formation in cultured renal proximal tubular cells. We examined the hypothesis that elevated AngII levels during HS intake reduces TNF-α receptor type 1 (TNFR1) activity in the kidneys, thus facilitating increased intrarenal AGT formation. The responses to HS diet (4% NaCl) with chronic infusion of AngII (25 ng/min) via implanted minipump for 4 weeks were assessed in wild-type (WT) and knockout (KO) mice lacking TNFR1 or TNFR2 receptors. Blood pressure was measured by tail-cuff plethysmography, and 24-h urine samples were collected using metabolic cages prior to start (0 day) and at the end of 2nd and 4th week periods. The urinary excretion rate of AGT (uAGT; marker for intrarenal AGT) was measured using ELISA. HS +AngII treatment for 4 weeks increased mean arterial pressure (MAP) in all strains of mice. However, the increase in MAP in TNFR1KO (77 ± 2 to 115 ± 3 mmHg; n = 7) was significantly greater (p < 0.01) than in WT (76 ± 1 to 102 ± 2 mmHg; n = 7) or in TNFR2KO (78 ± 2 to 99 ± 5 mmHg; n = 6). The increase in uAGT at 4th week was also greater (p < 0.05) in TNFR1KO mice (6 ± 2 to 167 ± 75 ng/24 h) than that in WT (6 ± 3 to 46 ± 16 ng/24 h) or in TNFR2KO mice (8 ± 7 to 65 ± 44 ng/24 h). The results indicate that TNFR1 exerts a protective role by mitigating intrarenal AGT formation induced by elevated AngII and HS intake.

Intravenous infusion of relatively higher doses of angiotensin II (AngII) elicits natriuresis as opposed to its usual anti-natruretic response. As AngII can induce tumor necrosis factor-α (TNFα) production which elicits natriuresis via its action on TNFα receptor type 1 (TNFR1), we hypothesize that the concomitant release of TNFα contributes to the natriuretic response to AngII. Responses to AngII infusion (1 ng min-1  g-1 for 75 min, iv) were evaluated in anesthetized knockout (KO) mice lacking TNFR1 (n = 6) and TNFR2 (TNFα receptor type 2; n = 6) and compared these responses with those in wild type (WT; n = 6) mice. Arterial pressure (AP) was recorded from a cannula placed in the carotid artery. Renal blood flow (RBF) and glomerular filtration rate (GFR) were measured by PAH and inulin clearances, respectively. Urine was collected from a catheter placed in the bladder. AngII caused similar increases (p < 0.05 vs basal values) in AP (WT, 37 ± 5%; TNFR1KO, 35 ± 4%; TNFR2KO, 30 ± 4%) and decreases (p < 0.05) in RBF (WT, -39 ± 5%; TNFR1KO, -28 ± 6%; TNFR2KO, -31 ± 4%) without significant changes in GFR (WT, -17 ± 7%; TNFR1KO, -18 ± 7%; TNFR2KO, -12 ± 7%). However, despite similar changes in AP and renal hemodynamics, AngII induced increases (p < 0.05) in urinary sodium excretion in WT (3916 ± 942%) were less in the KO strains, more or less in TNFR1KO (473 ± 170%) than in TNFR2KO (1176 ± 168%). These data indicate that TNF-α receptors, particularly TNFR1 are involved in the natriuretic response that occur during acute infusion of AngII and thus, plays a protective role in preventing excessive salt retention at clinical conditions associated with elevated AngII level.

Resumen Introducción: En el hábitat natural, el estado nutricional insatisfactorio de un número elevado de ejemplares es expresión de la influencia de factores desfavorables. Objetivo: Determinar variaciones espacio-temporales en el estado nutricional de langostas Panulirus argus, relacionarlas con variaciones reportadas en la comunidad bentónica y demostrar, mediante la aplicación de indicadores analíticos y morfométricos, que la variación de factores ambientales afecta a ambos grupos de índices; y que no todos los índices de condición nutricional evidencian por igual el impacto generalizado del proceso de deterioro ambiental en este golfo. Métodos: Las langostas se capturaron en seis áreas de pesca. La condición nutricional se estimó mediante tres índices no destructivos, poco costosos y de fácil y rápida aplicación (extensivos): índice de refracción de la hemolinfa (IRH), relación peso total / largo total = Klt, y factor de condición (FCA). Las variaciones a largo plazo (60 años) se determinaron mediante índices morfométricos (Klt y FCA). Se analizaron datos de los períodos 1963-1964 (N = 29 001), 1983-1993 (N = 3 123) y 2011-2017 (N = 3 600), separándolos en épocas de Lluvia y Seca. Resultados: En todos los períodos la condición nutricional varió significativamente entre áreas, pero sin similitudes entre períodos. Esto indica que los factores que impactan en el estado nutricional tienen una influencia estocástica más característica de factores ambientales. Aunque los tres índices fueron menores en 2017, sólo IRH disminuyó gradualmente entre 2011 y 2017, lo que sugiere que este índice, y los morfométricos, expresan diferente información. Todas las correlaciones fueron estadísticamente significativas, los coeficientes más altos se establecieron entre los índices morfométricos. El peso total y el Klt no mostraron diferencias entre Seca y Lluvia. Sin embargo, IRH y FCA resultaron mayores en Seca, hecho que se constató para cada sexo. FCA y Klt no presentaron diferencias entre el período 1963-1964 y el período 1983-1993, pero aumentaron (P < 0.05) en el período 2011-2017, lo que, dado el carácter morfométrico de estos índices, se atribuye a la presencia de langostas de mayor peso en las áreas de pesca. Conclusiones: Los menores valores en 2017 y la tendencia gradual al decrecimiento de IRH, corroboran el carácter generalizado del deterioro ambiental en el golfo de Batabanó, lo cual no fue igualmente expresado por los tres índices.


Abstract Introduction: Nutritional condition indices are morphophysiological estimators that quantify the nutritional status of organisms. In the natural habitat, unsatisfactory nutritional stage of numerous individuals is an expression of unfavorable factors. Objectives: Determine spatio-temporal variations in the nutritional condition of lobsters Panulirus argus, relate them to variations reported in the benthic community, and to demonstrate, through the application of analytical and morphometric indices, that variations of environmental factors affects both groups of indices due to its potential magnitude and its generalized nature. Methods: Lobsters were captured at six sites. Nutritional condition was estimated by three non-destructive, inexpensive and quick indices also easy to apply, which it means extensive indices: blood refractive index (BRI), relationship between total weight and total length (K), and condition factor (CFA). Because there are no historical records of BRI, long-term variations (60 years) were determined using morphometric indices (Klt and CFA). Data from the periods 1963-1964 (N = 29 001), 1983-1993 (N = 3 123) and 2011-2017 (N = 3 600) were analyzed, separating them in the rainy and dry seasons. Results: In all the periods the nutritional condition varied significantly between sites, but without similarities between periods. Despite this, it increased progressively in three sites. This indicates that factors impacting nutritional condition have a stochastic influence that is constantly changing, which is more characteristic of environmental factors than anthropogenic factors. Although the three indices showed a lower nutritional condition in 2017, only BRI gradually decreased between 2011 (16.6) and 2017 (14.2), which suggests that this index, and the morphometrics (Klt and CFA), express different information. All correlations were statistically significant but greatest coefficients were established between morphometric indices. The total weight and the Klt index did not show differences between rainy and dry seasons. However, BRI and CFA were statistically greater in dry season, a fact that was found for each sex. Both CFA and Klt decreased slightly (P > 0.05) between the first (1963-1964) and the second (1983-1993) period and then increased (P < 0.05) in the current period (2011-2017). Because they both are morphometric indices, this was attributed to the presence of greater weight lobsters. Conclusions: The lowest values in 2017 and gradually decreasing trend in BRI, corroborate the generalized nature of the environmental deterioration in the Gulf of Batabanó, but this was not equally expressed by the three indices.

Background: Skeletal muscle lacerations are a relatively common injury. Compared with nonrepaired lacerations, surgically repaired muscle lacerations regenerate faster, develop less scar tissue, have a higher return to baseline strength, and have lower incidence of hematomas. Despite the benefits of repair, the optimal repair technique is still unknown. The purpose of this study was to examine the biomechanical properties of common muscle repair techniques to determine the optimal repair. Methods: Forty-two fusiform porcine muscle specimens were dissected and used for this study. Three suture techniques were used for comparative analysis: Figure-eight, Mason Allen, and Perimeter. Each muscle was transected and then repaired using one of the 3 techniques. Fourteen muscle-tendon specimens were prepared for each group and tested for tensile failure using a material testing system. Biomechanical properties, including peak failure point and stiffness, were compared for differences between the suture groups by 1-way analysis of variance. The average time per repair technique was also recorded. Results: The Perimeter technique showed a statistically significant higher peak failure point than the Mason Allen technique (P = .03). Both the Figure-eight (P = .047) and Perimeter techniques (P < .001) were significantly stiffer than the Mason Allen technique. The repair time was comparable across all 3 techniques. Conclusions: The Figure-eight and Perimeter repairs were found to be similar in peak failure point and stiffness, whereas the Mason Allen technique showed significantly lower stiffness and peak failure point. The Figure-eight was the quickest repair to perform. The Figure-eight technique may be strongly considered for muscle laceration repairs due to its simplicity and efficiency.

High salt (HS) intake is usually considered as an aggravating factor to induce inflammatory renal injury. However, the changes in the renal levels of inflammatory cytokines during HS intake is not yet clearly defined. We hypothesize that HS increases renal levels of tumor necrosis factor-alpha (TNF-α) and interleukin-6 (IL-6) but decreases interleukin-10 (IL-10; anti-inflammatory cytokine) and these responses exacerbate in NO deficient conditions. Both wild-type (WT) and endothelial NO synthase knockout (eNOSKO) mice (~8 weeks old, n = 6 in each group) were given normal-salt (NS; 0.3% NaCl) and HS (4% NaCl) containing diets for 2 weeks. Systolic blood pressure (SBP) was determined by tail-cuff plethysmography and urine collections were made using metabolic cages. Basal SBP was higher in eNOSKO than WT mice (131 ± 7 vs 117 ± 3 mmHg; p < .05). HS intake for 2 weeks increased SBP in eNOSKO (161 ± 5 mmHg) but not in WT mice. In NS groups, the cytokine levels in renal tissues (measured using ELISA kits and expressed in pg/mg protein) were significantly higher in eNOSKO than WT mice (TNF-α, 624 ± 67 vs. 325 ± 73; IL-6, 619 ± 106 vs. 166 ± 61; IL-10, 6,087 ± 567 vs. 3,929 ± 378). Interestingly, these cytokine levels in HS groups were significantly less both in WT (TNF-α, 114 ± 17; IL-6, 81 ± 14; IL-10, 865 ± 130) and eNOSKO (TNF-α, 115 ± 18; IL-6, 56 ± 7; IL-10, 882 ± 141) mice. These findings indicate that HS induces downregulation of cytokines in the kidney. Such HS-induced reduction in cytokines, particularly TNF-α (a natriuretic agent), would facilitate more salt-retention, and thus, leading to salt-sensitive hypertension in NO deficient conditions.

Corynespora cassiicola (Burk. & M.A. Curtis) C.T. Wei. is an anamorphic fungus that affects more than 530 plant species, including economically important crops. Several lineages of this pathogen have been recognized, but the classification of isolates into clades is time-consuming and still sometimes leads to unclear results. In this work, eight major phylogenetic clades (PhL1-PhL8) including 245 isolates of C. cassiicola from 44 plant species were established based on a Bayesian inference analysis of four combined C. cassiicola genomic loci retrieved from GenBank, i.e., rDNA internal transcribed spacer (ITS), actin-1,ga4, and caa5. The existence of PhL1-PhL5 and PhL7 as clonal lineages was further confirmed through the analysis of full-genome single-nucleotide polymorphisms of 39 isolates. Haplotypes of the caa5 locus were PhL specific and encode isoforms of the LDB19 domain of a putative α-arrestin N-terminal-like protein. Evolution of the Caa5 arrestin is in correspondence with the PhLs. ga4 and caa5 PhL consensus sequences and a cleaved amplified polymorphic sequence (CAPS) procedure were generated based on the conserved nucleotide sequences and enzyme restriction patterns observed among isolates from the same lineage, respectively. The CAPS method was validated in silico, and its practical use allowed us to differentiate between tomato and papaya isolates, as well as to reveal the prevalence of PhL1 among isolates infecting soybean in Brazil. This novel approach could be useful in the efforts to control the diseases associated with C. cassiicola.

Introducción: El bentos constituye un eficiente indicador de disturbios ambientales, de biodiversidad y de dinámica trófica. En Cuba, los moluscos bentónicos han sido reportados como el principal alimento de la langosta Panulirus argus. Objetivos: Estimar variaciones espacio-temporales en la comunidad bentónica de fondos blandos del golfo de Batabanó, e inferir, haciendo énfasis en los moluscos, el efecto de tales variaciones en la disponibilidad de alimento para las langostas. Métodos: Se realizó un análisis histórico de las variaciones en la densidad y biomasa de megazoobentos (organismos ≥ 4 mm) y particularmente de moluscos megazoobentónicos, tomando como referencia los estudios realizados desde 1990 (muestreos del año 1983) y datos actuales (2007-2018) registrados in situ mediante metodologías comparables. Resultados: Se encontró que tanto la densidad como la biomasa eran significativamente mayores al final del pasado siglo y muestran tendencia a disminuir gradualmente hacia la actualidad. Respecto a los valores reportados en 1983, la densidad de organismos mostró un decrecimiento del 73 %, sin embargo, la biomasa experimentó una disminución aún mayor (86 %). Las áreas de mayor densidad y biomasa, tanto de bentos como de moluscos, han variado respecto a lo reportado para 1983 y actualmente se ubican hacia el centro del golfo. A pesar de ello, los bivalvos continúan estando mejor representados que los gastrópodos. En general, los moluscos mostraron un significativo decrecimiento (55 % y 88 % respectivamente). Conclusiones: Se corroboró el deterioro de la comunidad bentónica en las áreas pesqueras del golfo de Batabanó y se demostró, basado en el megazoobentos, que la malacofauna puede utilizarse como un indicador fiable de los parámetros generales de la comunidad bentónica de fondos blandos. Se estimó que la disponibilidad de alimento para las langostas ha disminuido en un 55.6 % respecto a 1983, lo cual, unido a las variaciones espaciales del megazoobentos, puede contribuir a explicar los actuales cambios en la abundancia y distribución de las langostas.


Introduction: Benthos is an efficient indicator of environmental disturbances, biodiversity and trophic dynamics. In Cuba, benthic mollusks have been reported as the main food for the lobster Panulirus argus. Objectives: To estimate spatio-temporal variations in the soft-bottom benthic community of the Gulf of Batabanó, and to infer, with emphasis on mollusks, the effect of such variations on the availability of food for spiny lobsters. Methods: A historical analysis of the variations in the density and biomass of megazoobenthos (organisms ≥ 4 mm) and particularly of megazoobenthic mollusks was carried out, taking as a reference the studies published in 1990 (with samples from 1983) and recent data (2007-2018) recorded on site using comparable methodologies based on sediment drags. Abundance was given as density (ind/m2) and biomass in g/m2. Results: It was found that both the density and the biomass were significantly higher at the end of the last century and show a tendency to gradually decrease towards recent times. Regarding the values reported in 1983, the density of organisms showed a decrease of 73 %, however, the biomass experienced an even greater decrease (86 %). The areas with the highest density and biomass, both benthos and mollusks, have varied from 1983 and are currently located towards the center of the gulf. Despite this, bivalves continue to be better represented than gastropods. In general, mollusks showed a significant decrease (55 % and 88 % respectively). Conclusions: The deterioration of the benthic community in the fishing areas of the Gulf of Batabanó was corroborated and it was demonstrated, based on megazoobenthos, that malacofauna can be used as a reliable indicator of the general parameters of the soft-bottom benthic community. The availability of food for lobsters have decreased 55.6 % compared to 1983. This could help to explain the current changes in the abundance, distribution and nutritional condition of lobsters, which could also be related to the recent megazoobenthos variations.

The genus Dichorhavirus includes plant-infecting rhabdoviruses with bisegmented genomes that are horizontally transmitted by false spider mites of the genus Brevipalpus. The complete genome sequences of three isolates of the putative dichorhavirus clerodendrum chlorotic spot virus were determined using next-generation sequencing (Illumina) and traditional RT-PCR. Their genome organization, sequence similarity and phylogenetic relationship to other viruses, and transmissibility by Brevipalpus yothersi mites support the assignment of these viruses to a new species of dichorhavirus, as suggested previously. New data are discussed stressing the reliability of the current rules for species demarcation and taxonomic status criteria within the genus Dichorhavirus.

IL-10 (interleukin-10) has been suggested to play a protective role in angiotensin II (AngII)-induced cardiovascular disorders. This study examined the role of endogenous IL-10 in salt-sensitive hypertension and renal injury induced by AngII. Responses to chronic AngII (400 ng/min per kilogram body weight; osmotic minipump) infusion were evaluated in IL-10 gene knockout mice fed with either normal salt diet (0.3% NaCl) or high salt (HS; 4% NaCl) diet, and these responses were compared with those in wild-type mice. Normal salt diets or HS diets were given alone for the first 2 weeks and then with AngII treatment for an additional 2 weeks (n=6 in each group). Arterial pressure was continuously monitored by implanted radio-telemetry, and a 24-hour urine collection was performed by metabolic cages on the last day of the experimental period. Basal mean arterial pressure was lower in IL-10 gene knockout mice than in wild-type (98±3 versus 113±3 mm Hg) mice. Mean arterial pressure responses to normal salt/HS alone or to the AngII+normal salt treatment were similar in both strains. However, the increase in mean arterial pressure induced by the AngII+HS treatment was significantly lower in IL-10 gene knockout mice (15±5% versus 37±3%) compared with wild-type mice. Renal tissue endothelial nitric oxide synthase expression (≈3-folds) and urinary excretion of nitric oxide metabolites, nitrate/nitrite (1.2±0.1 versus 0.2±0.02 µmol/L/24 hours) were higher in IL-10 gene knockout mice compared with wild-type mice. These results indicate that an increase in nitric oxide production helps to mitigate salt-sensitive hypertension induced by AngII and suggest that a compensatory interaction between IL-10 and nitric oxide exists in modulating AngII-induced responses during HS intake.

The os acetabuli is thought to arise from unfused secondary ossification centers or as rim fractures in the setting of dysplastic hips or hips with femoroacetabular impingement. Resection of a large os acetabuli can lead to structural instability of the joint, and in these cases, osteoplasty of the impingement, reduction and internal fixation of the osseous fragment, and labral repair have been described in the literature. Anchor fixation for labral repair in the surrounding zone of the osteosynthesis might bring some technical problems. We aim to describe a technical modification improving labral lesion treatment while addressing the rim fracture. The addition of a suture to the screw addresses both lesions because it simultaneously has the function of a screw and an anchor. A suture-on-screw technique for os acetabuli fixation helps surgeons to gain versatility and is more cost-effective for the patients and health services.

BACKGROUND: Young female subjects are known to have the highest baseline menstrual hormone levels of any female age group. Studies have found an association between hormone levels and wound healing. This has been researched in the orthopedic, gynecologic, and dermatologic literature, and more recently, in young patients undergoing augmentation mammaplasty. The purpose of this study was to determine whether the timing of surgery relative to the menstrual cycle plays a role in surgical complications following bilateral reduction mammaplasty. METHODS: All female patients aged 25 years or younger with a documented last menstrual cycle undergoing a bilateral reduction mammaplasty from 2005 to 2013 were reviewed. Surgical timing and postoperative complications relative to the last menstrual cycle were recorded. The preovulatory phase referred to days 1 to 14 after the patient's last menstrual cycle, whereas the postovulatory phase referred to days 15 to 28. RESULTS: Forty-nine patients met inclusion criteria. Undergoing bilateral reduction mammaplasty during the postovulatory phase was associated with development of wound dehiscence and hypertrophic scarring (p < 0.005), which were the most common postoperative complications. Surgery in the preovulatory or postovulatory phase did not affect hematoma, seroma, wound infection, or nipple-areolar complex necrosis rates (p > 0.05). Age, race/ethnicity, body mass index, large resection mass, and medical comorbidities did not affect wound dehiscence or scar hypertrophy rates (p > 0.05). CONCLUSIONS: Young patients undergoing bilateral reduction mammaplasty during the postovulatory phase of the menstrual cycle have an increased risk of wound healing issues and poor scarring. This may be attributable to hormonal fluxes occurring during this phase and the already high hormone levels in this population.

BACKGROUND: Deep gluteal syndrome (DGS) is an underdiagnosed entity characterized by pain and/or dysesthesias in the buttock area, hip or posterior thigh and/or radicular pain due to a non-discogenic sciatic nerve entrapment in the subgluteal space. Multiple pathologies have been incorporated in this all-included "piriformis syndrome", a term that has nothing to do with the presence of fibrous bands, obturator internus/gemellus syndrome, quadratus femoris/ischiofemoral pathology, hamstring conditions, gluteal disorders and orthopedic causes. METHODS: This article describes the subgluteal space anatomy, reviews known and new etiologies of DGS, and assesses the role of the radiologist and orthopaedic surgeons in the diagnosis, treatment and postoperative evaluation of sciatic nerve entrapments. CONCLUSION: DGS is an under-recognized and multifactorial pathology. The development of periarticular hip endoscopy has led to an understanding of the pathophysiological mechanisms underlying piriformis syndrome, which has supported its further classification. The whole sciatic nerve trajectory in the deep gluteal space can be addressed by an endoscopic surgical technique. Endoscopic decompression of the sciatic nerve appears useful in improving function and diminishing hip pain in sciatic nerve entrapments, but requires significant experience and familiarity with the gross and endoscopic anatomy. La valoración del pronóstico es una parte esencial en la evaluación de todo paciente con cirrosis hepática. En la actualidad se sigue desarrollando nuevos modelos para optimizar la precisión del pronóstico de mortalidad que se calcula mediante el score Child-Turcotte-Pugh (CTP) y el modelo para la enfermedad hepática en etapa terminal (MELD). Objetivo: Comparar la precisión pronóstica de mortalidad hospitalaria y mortalidad a corto plazo del CTP, MELD y demás modelos en pacientes con cirrosis hepática descompensada. Material y Métodos: Estudio descriptivo prospectivo, el cual incluyó 84 pacientes. El score CTP, MELD y demás modelos fueron calculados en el primer día de hospitalización. La precisión pronóstica de mortalidad se evaluó mediante el área bajo la curva ROC (AUROCs) del score CTP, MELD y los demás modelos. Resultados: La mortalidad hospitalaria y la mortalidad en el seguimiento a corto plazo fue de 20 (23,8%) y 44 (52,4%), respectivamente. Los AUROCs del CTP, MELD, MELD Na, MESO, iMELD, RefitMELD y RefitMELD Na para predecir mortalidad hospitalaria fue de 0,4488, 0,5645, 0,5426, 0,5578, 0,5719, 0,5598 y 0,5754; y para predecir mortalidad a corto plazo fue de 0,5386, 0,5747, 0,5770, 0,5781, 0,5631, 0,5881 y 0,5693, respectivamente. Al comparar entre si los AUROCs del score CTP, MELD y demás modelos ninguno mostró ser mejor que el otro (p>0,05). Conclusiones: El presente estudio no ha demostrado la utilidad predictiva del score CTP, MELD y los otros modelos (MELD Na, MESO, iMELD, Refit MELD y Refit MELD Na) para la evaluar la mortalidad hospitalaria o mortalidad a corto plazo en una muestra de pacientes con cirrosis hepática descompensada del Hospital Cayetano Heredia...


Introduction: The assessment of prognosis is an essential part of the evaluation of all patients with liver cirrhosis. While it is clearly recognized that increased intrarenal nitric oxide (NO) levels elicit natriuresis, confounding data showing that systemic nitric oxide synthase inhibition (NOSi) also increases sodium excretion (UNaV) poses a conundrum. This response has been attributed to the associated increases in arterial pressure (AP); however, the increases in AP and in UNaV are temporally dissociated. The changes in regional renal haemodynamics induced by NOSi could also contribute to the alterations of UNaV. To evaluate the roles of AP and non-AP mechanisms mediating the natriuresis, N ω-nitro-L-arginine methyl ester hydrochloride (L-NAME) was infused i.v. at doses ranging from 5 to 50 µg/kg/min in anaesthetized rats. UNaV, perfusion of the cortex (cortical blood flow, CBF) and medulla (medullary blood flow, MBF) with laser-Doppler flowmetry and glomerular filtration rate (GFR) were measured. UNaV increased from 0.6 ± 0.2 to 1.6 ± 0.1 µmol/kg/min (P < 0.05) with the lower nonpressor doses. With the higher doses, AP increased from 116 ± 4 to 122 ± 4 mmHg and UNaV increased from 1.1 ± 0.3 to 3.3 ± 0.7 µmol/min/g (P < 0.002). UNaV increased similarly in a group where renal AP was maintained at baseline levels. The associated reductions in CBF (17 ± 5 and 38 ± 5 %) and MBF (27 ± 6 and 52 ± 6 %) would be expected to attenuate rather than contribute to the natriuresis. Plasma atrial natriuretic peptide (ANP) concentrations increased significantly following NOSi. Anantin, a natriuretic peptide receptor-A blocker, prevented or reversed the L-NAME-induced natriuresis without altering the L-NAME-induced changes in AP or CBF. The results indicate that increased ANP and related natriuretic peptides mediate the AP-independent natriuresis, at least partly, elicited by systemic L-NAME infusion and help resolve the conundrum of natriuresis during systemic NOSi.
