The main objective of this work is to present a set of equations that relates exposure time to solar radiation, the ultraviolet index (UVI), and its effects, both positive (vitamin D synthesis) and negative (sunburns), including the influence of repeated doses and the incorporation of protective factors. To do so, expressions are proposed for both effects and a time range is specified where repeated doses can be applied. Moreover, we propose expressions (PROBIT equations) that relate the percentage of a population that would reach the recommended daily amount of vitamin D and the repeated dosage to which the population is exposed for skin types I to IV. For all the cases studied, protective factors, such as the use of clothes or sunscreens, were taken into account. Additionally, comparisons were made based on skin types between the generation of daily vitamin D and the onset of sunburn, being able to establish a percentage of those who would suffer from first-degree sunburns when this population reached the recommended daily amount of vitamin D. Thus, it can be noted that when a large percentage of a population wants to obtain the recommended daily allowance of vitamin D of 2000 IU or more, and by exposing a small portion of skin to solar radiation, a considerable percentage of that population would suffer from first-degree sunburn as vitamin D generation is related to the area of exposed skin. Therefore, by increasing the area of skin that is exposed to solar radiation, vitamin D needs can be safely met even with higher daily amounts.

Background and Objectives: The widespread use of tobacco has evolved with the popularity of vapes, especially among young people, despite the lack of clarity in warnings about their risks. Studies indicate the need for more effective communication about the oral risks of vaping. In addition to systemic, respiratory, and cardiovascular effects, vaping is associated with an increased risk of gingivitis and periodontal disease as well as reduced antioxidant capacity of saliva. The objectives of this narrative review are to summarize the existing information in the literature on the effects of vaping at the oral level and to bring together knowledge about the mechanism of action of vaping in oral tissues. Materials and Methods: In the present study, articles were searched in PubMed, Elsevier Scopus, and Web of Science using the keywords "oral health", "vaping", and "vape". Studies published in the last 6 years that addressed the effects of oral vaping were selected, including comparisons among vape users, smokers, and non-smokers. Repeated articles, prior to 2017 and in languages other than English, were excluded. Two review authors (A.M.I and M.F.E.M) independently selected the papers based on titles and abstracts and conducted a full review of the remaining papers. In cases of disagreement, a third reviewer was used. Results: A total of 113 results were obtained, distributed as 16 from PubMed, 35 from Web of Science, and 62 from Elsevier Scopus. After removing duplicates, 67 articles were filtered by reviewing titles and abstracts, and finally, 22 articles were selected for comprehensive reading. Subsequently, eight of these articles were chosen for qualitative synthesis and are presented in standardized tables. The sample size of all included studies was composed of 31,647 participants, (14,477 male and 17,170 female) with a mean of 35.016 ± 7.57 years of age. Conclusions: This review indicates that the use of vapes is associated with an increased risk of periodontitis and caries. Although users experience more oral problems than non-smokers, these are less severe than those of traditional smokers. The widespread prevalence, especially among young people, highlights the urgency of awareness campaigns to warn of risks and understand potential harm.

Peripheral nerve damage results in the loss of sensorimotor and autonomic functions, which is a significant burden to patients. Furthermore, nerve injuries greater than the limiting gap length require surgical repair. Although autografts are the preferred clinical choice, their usage is impeded by their limited availability, dimensional mismatch, and the sacrifice of another functional donor nerve. Accordingly, nerve guidance conduits, which are tubular scaffolds engineered to provide a biomimetic environment for nerve regeneration, have emerged as alternatives to autografts. Consequently, a few nerve guidance conduits have received clinical approval for the repair of short-mid nerve gaps but failed to regenerate limiting gap damage, which represents the bottleneck of this technology. Thus, it is still necessary to optimize the morphology and constituent materials of conduits. This review summarizes the recent advances in nerve conduit technology. Several manufacturing techniques and conduit designs are discussed, with emphasis on the structural improvement of simple hollow tubes, additive manufacturing techniques, and decellularized grafts. The main objective of this review is to provide a critical overview of nerve guidance conduit technology to support regeneration in long nerve defects, promote future developments, and speed up its clinical translation as a reliable alternative to autografts.

INTRODUCTION: The association between viral infections and pulmonary exacerbations in children with cystic fibrosis (cwCF) is well established. However, the question of whether cwCF are at a higher risk of COVID-19 or its adverse consequences remains controversial. METHODS: We conducted an observational, multicenter, cross-sectional study of cwCF infected by severe acute respiratory syndrome coronavirus-2 (SARS-CoV-2) between March 2020 and June 2022, (first to sixth COVID-19 pandemic waves) in Spain. The study aimed to describe patients' basal characteristics, SARS-CoV-2 clinical manifestations and outcomes, and whether there were differences across the pandemic waves. RESULTS: During study time, 351 SARS-CoV2 infections were reported among 341 cwCF. Median age was 8.5 years (range 0-17) and 51% were female. Cases were unevenly distributed across the pandemic, with most cases (82%) clustered between November 2021 and June 2022 (sixth wave, also known as Omicron Wave due to the higher prevalence of this strain in that period in Spain). Most cwCF were asymptomatic (24.8%) or presented with mild Covid-19 symptoms (72.9%). Among symptomatic, most prevalent symptoms were fever (62%) and increased cough (53%). Infection occurring along the sixth wave was the only independent risk factor for being symptomatic. Just eight cwCF needed hospital admission. No multisystem inflammatory syndrome, persisting symptoms, long-term sequelae, or deaths were reported. CONCLUSIONS: Spanish current data indicate that cwCF do not experience higher risks of SARS-CoV-2 infection nor worse health outcomes or sequelae. Changes in patients' basal characteristics, clinical courses, and outcomes were detected across waves. While the pandemic continues, a worldwide monitoring of COVID-19 in pediatric CF patients is needed.

INTRODUCTION: Elexacaftor/tezacaftor/ivacaftor (ETI) was used through the early access programme in Spain from December 2019 in cystic fibrosis (CF) patients with homozygous or heterozygous F508del mutation with advanced lung disease. METHODOLOGY: Multicentre, ambispective, observational, study in which 114 patients in follow-up in 16 national CF units were recruited. Clinical data, functional tests, nutritional parameters, quality of life questionnaires, microbiological isolates, number of exacerbations, antibiotic treatments and side effects were collected. The study also compared patients with homozygous and heterozygous F508del mutations. RESULTS: Of the 114 patients, 85 (74.6%) were heterozygous for F508del mutation, and the mean age was 32.2±9.96 years. After 30 months of treatment, lung function measured by FEV1% showed improvement from 37.5 to 48.6 (p<0.001), BMI increased from 20.5 to 22.3 (p<0.001), and all isolated microorganisms decreased significantly. The total number of exacerbations was also significantly reduced from 3.9 (±2.9) to 0.9 (±1.1) (p<0.001). All items in the CFQ-R questionnaire showed improvement, except for the digestive domain. Oxygen therapy use decreased by 40%, and only 20% of patients referred for lung transplantation remained on the active transplant list. ETI was well-tolerated, with only 4 patients discontinuing treatment due to hypertransaminemia. CONCLUSIONS: ETI decreases the number of exacerbations, increases lung function and nutritional parameters, decrease in all isolated microorganisms, for 30 months of treatment. There is an improvement in the CFQ-R questionnaire score except for the digestive item. It is a safe and well-tolerated drug.

Non-governmental organizations (NGOs) in dentistry seek to promote the improvement on oral health in the most disadvantaged regions. The objective of this study is to identify the level of knowledge, expectations, and motivations that dental school students have about volunteering in dentistry, as well as to evaluate possible differences in these variables depending on their level of dental training. During the month of September 2022, a voluntary and anonymous online survey was carried out among all the students at the Dentistry School of Oviedo University. There were 5 questions to judge knowledge about global oral health course. 12 additional questions were included to assess the willingness to volunteer in international setting, the volunteer profile, as well as the most effective means to improve oral health in host communities. None of the students from our center had participated as a volunteer in dental NGOs, but up to 64.4% of them had considered their collaboration. The level of knowledge about global oral health obtained was low, with the percentage of correct answers ranging between 14.4% (in the question about the ideal patient/dentist ratio) and 57.8% (in the question about the fluoride concentration in drinking water). Majority of dental students (98.9%) were not aware that basic package of oral care was created by WHO. Significantly, the students of the clinical courses showed a greater motivation to volunteer.

The present study centers on the synthesis of ultra-small silver nanoparticles (AgNPs) with antibacterial properties using citrus peel residues (orange, lemon, and grapefruit) as reducing and stabilizing agents, and on assessing their antibacterial activity against multidrug-resistant clinical Staphylococcus aureus. The synthesized AgNPs were analyzed by various techniques, including UV-Vis spectroscopy, SAED, TEM, XRD, FTIR, and Raman. The results demonstrate the formation of ultra-small, monodisperse, quasi-spherical AgNPs with an average particle size of 2.42 nm for AgNPs produced with mixed extracts. XRD analysis indicated that the AgNPs have a crystal size of 9.71 to 16.23 nm. The AgNPs exhibited potent inhibitory activity against resistant S. aureus, with a minimum inhibitory concentration (MIC) of 15.625 to 62.50 ppm. The findings suggest that the ultra-small nanometer size of the AgNPs could be attributed to the synthesis method that employs ambient conditions and the presence of polyphenolic compounds from citrus peel. Consequently, AgNPs obtained through sustainable green synthesis hold significant potential in combating clinical multi-resistant bacterial strains that are challenging to treat and eradicate. This approach also contributes to the revaluation of citrus residues in the region, which is an ongoing environmental issue today.

Severe asthma has an important impact on patients and healthcare resources. Recently, the new specific treatments have defined a new scenario in which person-focused care and specialist multidisciplinary teams are necessary. Our Severe Asthma Unit (SAU) started the ASfarMA project along with an external human-centered design company to understand patients' vision of their illness, treatment, and healthcare experience, and to define the ideal SAU by performing a core group session, in-depth semistructured interviews and co-creation workshop. Herein, a series of tips classified as either 'transformative solutions' or 'quick wins', according to a value versus effort matrix are presented. Successful implementation of the proposed solutions will be valuable for patients and healthcare professionals, optimising patient care and resources. These findings can also be helpful to other SAUs or other humanisation projects involving complex, chronic and multidisciplinary pathologies.

INTRODUCTION: Cancer is the second leading cause of death worldwide, with 70% of cancer deaths occurring in low- or middle- income countries. To mitigate the mortality of this disease, it is recommended the evaluation of multiple high-penetrance genes. METHODS: We used a multi-gene panel testing to identify germline variants in a unique case of a breast cancer patient with a family history of five different neoplasm types. The patient, at the age of 50 years, was diagnosed with a high-grade cribriform ductal carcinoma in situ in her left breast. RESULTS: We identified two heterozygous mutations, one classified as pathogenic/likely pathogenic in RAD50 and the other classified as a variant of uncertain significance (VUS) in ATM. CONCLUSION: In conclusion, the use of the multi-gene panel leads to the identification of a double heterozygous mutation in RAD50 and ATM in a breast cancer patient from a Peruvian family with several cancer types. This data helps our physician team and the patient to choose a treatment following the post-test genetic counseling.

Background: Malaria is one of the biggest impediments to global progress. In Peru, it is still a major public health problem. Measures of health and economic burden due to malaria are relevant considerations for the assessment of current policies. Methods: We used estimates from the Global Burden of Diseases Study 2019 for malaria in Peru, grouped by gender and age, from 1990 to 2019. Results are presented as absolute numbers and age-standardized rates with 95% uncertainty intervals (UI). We collected economic data from the World Bank and The National Institute of Statistics and Informatics of Peru and Loreto to calculate the economic burden of productivity loss (EBPL) using the human capital approach. Economic values were presented in constant dollars, soles, and percentages. Findings: Rates of deaths, years of life lost (YLLs), years lived with disability (YLDs), and disability-adjusted life years (DALYs), as well as the EBPL, were drastically reduced from 1990 to 2019. DALYs had a greater percentage of YLDs in 2019 than in 1990. DALYs rates showed no preference between sexes, but the "< 1 year" age group had the highest DALYs values over the study period. We found that the EBPL due to malaria for Loreto was considerably higher than Peru's in terms of GDP percentage. Interpretation: Our study shows that the fight against malaria in Peru reduced remarkably the impact of the disease since 1990; however, during the last decade the estimates were stable or even increased. Our results help to measure the malaria impact on the health status of the Peruvian population as well as the economic pressure that it exerts, constituting remarkable tools for policymaking aimed at reducing the burden of this disease. Strengthening the malaria elimination program is important to achieve the elimination of the disease in the coming years. OBJECTIVE: E-parenting is an opportunity to provide parental support as a universal prevention strategy. This study analysed the extent to which the promotion actions improve universal use of the online course 'Positive parenting: Gaining health and wellbeing from birth to three' (GH&W) (http://aulaparentalidad-msssi.com/) at the primary care centers. METHODS: The profile of participants and the adherence to the course in a national sample of families using the GH&W course were compared with another Canarian sample distributed in three groups: only GH&W (level 1), GH&W + face-to-face group activities (level 2), and users who also received individual support in the medical checking (level 3). It was carried out in 20 health centers on the islands of Tenerife and Gran Canaria randomly assigned to one of the three levels. Participants were 175 national parental figures and 160 parental figures users of the Canary Health Service, both with children from 0 to 3 years. The sociodemographic profile and the completion rate of the GH&W were registered, as well as a template of implementation quality indicators for innovative experiences. To compare the sociodemographic profile differences between groups, a Chi-square contingency analysis with standardized residuals was performed. RESULTS: Compared to the national sample mainly of high educational level, primiparous mothers and 14% completion, the Canarian sample attracted different educational levels and multiparous mothers, with a higher completion rate at levels 2 and 3 (62.5% and 67.5%) than at level 1 (38.5%). CONCLUSIONS: The universal use of the course and its adherence improve due to the inclusion of face-to-face activities. The implementation process in the selected health centers satisfactorily meets the quality indicators of an innovative initiative.


OBJETIVO: La educación parental en línea es una oportunidad de proporcionar apoyo parental como estrategia de prevención universal. Se pretendió analizar en qué medida las acciones de dinamización mejoran el uso universal del curso a distancia "Parentalidad positiva: Ganar salud y bienestar de 0 a 3 años" (GSB) (http://aulaparentalidad-msssi.com/) en los centros de Atención Primaria. METODOS: Se comparó el perfil de participantes y la adherencia al curso en una muestra nacional de familias usuarias del curso GSB (175), con otra muestra canaria (160) usuaria de 20 centros de salud de las islas de Tenerife y Gran Canaria asignados aleatoriamente en tres grupos: sólo GSB (nivel 1), GSB + actividades presenciales grupales (nivel 2), y usuarios que además recibían apoyo individual en la consulta (nivel 3). Se analizó el perfil sociodemográfico y el porcentaje de finalización del curso GSB, así como una plantilla de indicadores de calidad de la implementación para iniciativas innovadoras. Se realizó un análisis de contingencia Chi Cuadrado con residuos tipificados para comparar el perfil sociodemográfico entre los grupos. RESULTADOS: Respecto a la muestra nacional, con un nivel educativo más alto y madres primíparas con el 14% de finalización, la muestra canaria estuvo compuesta de distintos niveles educativos y madres multíparas con un porcentaje de finalización mayor en los niveles 2 y 3 (62,5% y 67,5%) que en el nivel 1 (38,5%). CONCLUSIONES: El uso universal del curso y su adherencia mejora por la inclusión de actividades presenciales. El proceso de implementación en los centros de salud seleccionados cumple satisfactoriamente con los indicadores de calidad de una iniciativa innovadora.

In this work, we report the synthesis of copper nanoparticles (Cu NPs), employing the chemical reduction method in an aqueous medium. We used copper sulfate pentahydrate (CuSO4·5H2O) as a metallic precursor; polyethylenimine (PEI), allylamine (AAM), and 4-aminobutyric acid (AABT) as stabilizing agents; and hydrated hydrazine as a reducing agent. The characterization of the obtained nanoparticles consisted of X-ray, TEM, FTIR, and TGA analyses. Through these techniques, it was possible to detect the presence of the used stabilizing agents on the surface of the NPs. Finally, a zeta potential analysis was performed to differentiate the stability of the nanoparticles with a different type of stabilizing agent, from which it was determined that the most stable nanoparticles were the Cu NPs synthesized in the presence of the PEI/AAM mixture. The antimicrobial activity of Cu/PEI/AABT toward P. aeruginosa and S. aureus bacteria was high, inhibiting both bacteria with low contact times and copper concentrations of 50-200 ppm. The synthesis method allowed us to obtain Cu NPs free of oxides, stable to oxidation, and with high yields. The newly functionalized Cu NPs are potential candidates for antimicrobial applications.

Heart development requires a precise temporal regulation of gene expression in cardiomyoblasts. Therefore, the transcriptional changes in differentiating cells can lead to congenital heart diseases. Although the genetic mutations underlie most of these alterations, exposure to environmental contaminants, such as bisphenol A (BPA), has been recently considered as a risk factor as well. In this study we investigated the genotoxic and epigenotoxic effects of BPA throughout cardiomyocyte differentiation. H9c2 cells (rat myoblasts) were exposed to 10 and 30 µM BPA before and during the last two days of cardiac-driven differentiation. Then, we have analysed the phenotypic and molecular modifications (at transcriptional, genetic and epigenetic level). The results showed that treated myoblasts developed a skeletal muscle cell-like phenotype. The transcriptional changes induced by BPA in genes codifying proteins involved in heart differentiation and function depend on the window of exposure to BPA. The exposure before differentiation repressed the expression of heart transcription factors (Hand2 and Gata4), whereas exposure during differentiation reduced the expression of cardiac-specific genes (Tnnt2, Myom2, Sln, and Atp2a1). Additionally, significant effects were observed regarding DNA damage and histone acetylation levels after the two periods of BPA exposure: in cells exposed to the toxicant the percentage of DNA repair foci (formed by the co-localization of γH2AX and 53BP1) increased in a dose-dependent manner, whereas the treatment with the toxicant triggered a decrease in the epigenetic marks H3K9ac and H3K27ac. Our in vitro results reveal that BPA seriously interferes with the process of cardiomyocyte differentiation, which could be related to the reported in vivo effects of this toxicant on cardiogenesis.

The objective of this study was to determine the oxidative stress and the physiological and antioxidant responses of coriander plants (Coriandrum sativum) grown for 58 days in soil with zinc oxide nanoparticles (ZnO NPs) and zinc sulfate (ZnSO4) at concentrations of 0, 100, 200, 300, and 400 mg of Zn/kg of soil. The results revealed that all Zn compounds increased the total chlorophyll content (CHLt) by at least 45%, compared to the control group; however, with 400 mg/kg of ZnSO4, chlorophyll accumulation decreased by 34.6%. Zn determination by induction-plasma-coupled atomic emission spectrometry (ICP-AES) showed that Zn absorption in roots and shoots occurred in plants exposed to ZnSO4 at all concentrations, which resulted in high levels of hydrogen peroxide (H2O2) and malondialdehyde (MDA). Only at 400 mg/kg of ZnSO4, a 78.6% decrease in the MDA levels was observed. According to the results, the ZnSO4 treatments were more effective than the ZnO NPs to increase the antioxidant activity of catalase (CAT), ascorbate peroxidase (APX), and peroxidases (POD). The results corroborate that phytotoxicity was higher in plants subjected to ZnSO4 compared to treatments with ZnO NPs, which suggests that the toxicity was due to Zn accumulation in the tissues by absorbing dissolved Zn++ ions.

This manuscript reports on the application of copper thermal spraying in the manufacturing process of an electrical connection between Nb3Sn cables for superconducting magnets of fusion reactors. The joint is realized through diffusion bonding of the sprayed coating of the two cables. The main requirement for such a connection is its electrical resistance, which must be below 1 nΩ at B = 8 T, I = 63.3 kA and T = 4.5 K. Micrographs of the joint prototype were taken to relate the joint resistance with its microstructure and to provide feedback on the manufacturing process. Optical microscopy (OM) was used to evaluate the grain size of the coating, presence of oxide phases and to analyze the jointed surfaces. Scanning electron microscopy (SEM) and, in particular, energy-dispersive X-ray spectroscopy (EDX) were used to confirm the elemental composition of specimens extracted from the prototype. It is shown that the copper coating has an oxide concentration of 40%. Despite this, the resistance of the prototype is 0.48 nΩ in operating conditions, as the oxides are in globular form. The contact ratio between the jointed surfaces is about 95%. In addition, residual resistivity ratio (RRR) measurements were carried out to quantify the electrical quality of the Cu coating.

Canine distemper virus (CDV) has recently emerged as an extinction threat for the endangered Amur tiger (Panthera tigris altaica). CDV is vaccine-preventable, and control strategies could require vaccination of domestic dogs and/or wildlife populations. However, vaccination of endangered wildlife remains controversial, which has led to a focus on interventions in domestic dogs, often assumed to be the source of infection. Effective decision making requires an understanding of the true reservoir dynamics, which poses substantial challenges in remote areas with diverse host communities. We carried out serological, demographic, and phylogenetic studies of dog and wildlife populations in the Russian Far East to show that a number of wildlife species are more important than dogs, both in maintaining CDV and as sources of infection for tigers. Critically, therefore, because CDV circulates among multiple wildlife sources, dog vaccination alone would not be effective at protecting tigers. We show, however, that low-coverage vaccination of tigers themselves is feasible and would produce substantive reductions in extinction risks. Vaccination of endangered wildlife provides a valuable component of conservation strategies for endangered species.

RESUMEN

La actual pandemia de COVID-19 se ha convertido en un grave problema de salud pública mundial, contándose casi un millón y medio de casos y decenas de miles de muertes a la fecha. La genética por su parte está teniendo un papel protagónico en la identificación, manejo y tratamiento de enfermedades. En el presente artículo se detalla sobre la importancia de la genética como ciencia para afrontar la amenaza global de la COVID-19, desde diferentes enfoques. Se resaltan los aportes que ha tenido y seguirá teniendo la genética en la identificación del nuevo virus SARS- CoV-2, en el desarrollo de nuevas técnicas diagnósticas, en la prevención de contagios y del desarrollo de cuadros graves, así como en el diseño de vacunas y en la propuesta y evaluación de tratamientos para la COVID-19.


The current COVID-19 pandemic has become a major global public health problem with almost one and a half million cases and tens of thousands of deaths until now. Genetics is playing a leading role in the identification, management and treatment of diseases. This article details the importance of genetics as a science to face the global threat of COVID-19, from different approaches. The contributions that genetics have had and will continue to have in the identification of the new SARS-CoV-2 virus, in the development of new diagnostic techniques, in the prevention of infections and development of severe symptoms, as well as in the design of vaccines and in the proposal and evaluation of treatments for COVID-19.

The purpose of this study was to generate novel chitosan hydrogels (CHs) loaded with silver nanoparticles (AgNPs) and ampicillin (AMP) to prevent early formation of biofilms. AgNPs and CHs were characterized by UV-Vis, DLS, TEM, rheology, FT-IR, Raman, and SEM. The antibiofilm effect of the formulations was investigated against four multidrug-resistant and extensively drug-resistant pathogens using a colony biofilm, a high cell density and gradients model. Also, their hemostatic properties and cytotoxic effect were evaluated. Rheology results showed that CHs with AgNPs and AMP are typical non-Newtonian pseudoplastic fluids. The CH with 25 ppm of AgNPs and 50 ppm AMP inhibited the formation of biofilms of Acinetobacter baumannii, Enterococcus faecium and Staphylococcus epidermidis, while a ten-fold increase of the antimicrobial's concentration was needed to inhibit the biofilm of the ß-lactamase positive Enterobacter cloacae. Further, CH with 250 ppm of AgNPs and 500 ppm AMP showed anticoagulant effect, and it was shown that all formulations were biocompatible. Besides to previous reports that described the bioadhesion properties of chitosan, these results suggest that AgNPs and AMP CHs loaded could be used as prophylactic treatment in patients with central venous catheter (CVC), inhibiting the formation of biofilms in their early stages, in addition to their anticoagulant effect and biocompatibility, those properties could keep the functionality of CVC helping to prevent complications such as sepsis and thrombosis.
