BACKGROUND: Considering the tortuous course of the facial canal that houses the facial nerve, the stylomastoid artery and the homonymous vein, its morphological features are of great clinical significance in otologic, maxillofacial, oncologic, reconstructive and plastic surgery of the head and neck. The aim of this paper was to determine the individual specific features of the mastoid segment of facial canal and of the stylomastoid foramen. MATERIAL AND METHODS: The study was carried out on 82 temporal bones (41 right/41 left), at the Department of anatomy and clinical anatomy of Nicolae Testemitanu State University of Medicine and Pharmacy of the Republic of Moldova. The morphometry of the mastoid segment of the facial canal and of the stylomastoid foramen was performed. The morphometric parameters were statistically analyzed by descriptive and inferential statistics methods. RESULTS: The mastoid segment exited the facial canal by an obtuse, right and sharp angles, with a mean value of 113.1 ± 21.80° (right/left - 112.1 ± 23.85°/114.1 ± 19.76°), p = 0.701. The mean length of the mastoid segment was 15.1 ± 3.78 mm (right/left - 15.7 ± 3.66 mm/14.5 ± 3.84 mm), p = 0.153. The longitudinal diameter of the stylomastoid foramen had a mean of 3.0 ± 0.93 mm (right/left - 3.3 ± 0.96 mm/2.7 ± 0.81 mm), p = 0.007. The transverse diameter had a mean of 2.6 ± 0.74 mm (right/left - 2.9 ± 0.80 mm/2.4 ± 0.60 mm), p = 0.012. CONCLUSIONS: In otologic surgery and particularly in mastoidectomy, it should be taken into consideration that the mastoid segment of the facial canal could exit the temporal bone by a sharp, right and obtuse angles that along with high morphological variability of the stylomastoid foramen might be a predisposal factor for Bell's palsy.

The goals of this research were to determine the influence of several factors on implants' biological and technical complications in posterior fixed implant prosthetic therapy. MATERIALS AND METHODS: The study group consisted of 67 edentulous patients (mean age: 63.88 ± 11.709 yrs; 20 males, 47 females) with implant prosthetic therapy for posterior edentulism. A total of 76 implant-supported fixed partial dentures (IP-FPDs) and 178 implants were assessed using clinical and paraclinical assessments. Risk factors for biological complications (peri-implantitis) and technical complications were determined by using the Pearson Chi-squared test and multivariate analysis. RESULTS: The implant success (the absence of biological and mechanical/technical complications) was 66.30%. The prevalence of biological complications was 13.5%. The prevalence of technical complications was 28.70%. Variables that were associated with a higher risk of peri-implantitis were poor oral hygiene and bruxism. In univariate analysis, poor oral hygiene increased the risk of peri-implantitis 5.778 times and bruxism 5.875 times. Variables that were associated with a higher risk of mechanical/technical complications were age group > 60 yrs, smoking, history of periodontal disease, and bruxism. In univariate analysis, the risk of technical complications increased 4.14 times for patients in the age group > 60 years (vs. age group 40-60 years) and 20.5 times for patients with bruxism. Bruxism and smoking were significant predictors of mechanical/technical complications in the multivariate model. CONCLUSIONS: In univariate models, patients with poor oral hygiene and bruxism have an increased risk of peri-implantitis. In multivariate models, we did not identify significant predictors of peri-implantitis. Age group > 60 yrs, smoking, history of periodontal disease, bone grafting, and bruxism are risk factors for the increase in the mechanical/technical complication rate. In the multivariate model, smoking and bruxism are significant predictors of the mechanical/technical complications.

Background and Objectives: Implant rehabilitation of complete edentulous arches has become more and more popular because of the increased access of the population to this type of treatment. Furthermore, the development of new rehabilitation procedures can be applied in most clinical cases, including in those with severe atrophy. Hence, this study aimed to assess the functional changes that can occur in the stomatognathic system after implant rehabilitation procedures. Materials and Methods: A total of 63 patients were accepted in the study. They were divided into a first control (dentate) group (CG) and a second study group (edentulous, SG). For the latter, 30 patients received 204 two-stage implants immediately loaded with provisional prostheses. Surface electromyography (EMG) was assessed at the time of prostheses fixation, while for some patients it was applied six months after the fixation of the fixed prostheses, as well. These supplemental investigated patients formed a third, follow-up study group (FSG). All assessments were performed during the processes of clenching and mastication. The obtained data of the two study groups, SG and FSG, were compared with those of the control group, CG. Results: No statistical differences were found in the electrical muscular activity between the study and control groups during both clenching and mastication (p > 0.05). In addition, there were no differences within the same study group, both initially and after 6 months. The only changes were noticed between static and dynamic values for the right masseter muscle in the follow-up group FSG (p = 0.008). Deviations of the overlapping coefficients were similar for all groups (p = 0.086): for CG, 20.5%, median 11.1 (min. 0, max. 104); for SG, 21.4%, median 12.2 (min. 0, max. 103); for FSG, 36.1%, median 26.9 (min. 0, max. 160). This revealed no neuromuscular adaption to the prostheses. Conclusions: Implant-prosthetic rehabilitation led to an EMG activity that was similar to that of dentate patients immediately after the placement of the fixed implant-supported prostheses. Moreover, the measured values did not change after six months of functioning for all evaluated parameters. This may point to an immediate restoration of the muscle contraction capacity, without the necessity of adaptation over time. The study serves as an argument for the application and reliability of the immediate fixed implant-supported prostheses from the perspective of muscle adaptation and functioning.
