[The first European-style hospital in Georgia and its surgical activities].

The article reviews activities of Tbilisi Military Hospital (MTH) during the period from 1808 to 2007. The information disseminated in periodicals and other sources of information was studied. The surgical activity of the hospital is the main subject for the review. The MTH played significant role in the development of social medicine study and research of relevant curative climate resources, in the establishment of Borzhomi, and other. The hospital stuff was active member of Imperial Caucasian Medical Society. The high experience of the stuff gained hospital the name of highest, 4-th level degree. The surgical activities promoted further development of narcosis. first surgical department in Tbilisi State University, the high level of medical services at the time of Second World War and for the peace time also. The three main periods of TMH's activities are covered in the article.

[Experimental and clinical ground of the use of radiosurgical unit "Surgitron" in gastrointestinal surgery].

The authors represent results of experimental study showing objective differences and advantages of radioscalpel "Surgitron" vs. widely used physical cutting methods. This unit significantly decreases traumatism of gastrointestinal organs and tissues, and increases efficiency and safety of surgical operations. The results of surgical treatment in 313 patients, operated with the use of radioscalpel "Surgitron" purposely to cut and coagulate tissues, represented in the article. Use of radioscalpel in clinical practice for gastrointestinal and interintestinal anastomosis formation favors primary intention healing, decreases risk of postoperative complications (bleeding, anastomositis, insufficiency of anastomosis stitches) and lethality. These results give reason for wide practical application of radioscalpel "Surgitron" in surgical clinics.

[The post-closure syndromes of the duodenal ulcers].

The objective of the study was the determination of the ulcerorhaphy in perforated duodenal ulcers, a clinical course of the disease in the post-operational period and a delineation of the treatment tactics in the case of various morpho-functional abnormalities in the post-ulcerorhaphic period. To this effect, the method of retrospective analysis, allowing comparison of the pre- and post- operational pictures of the disease has been applied. As a result the post-closure syndromes of the perforated ulcer were detected in 42 (37,9%) patients. In the case of the presence of the syndrome, the disease is aggressive, emerging as a recurrent refractive ulcer and various severe complications. This kind of the disease is resistant to a conservative therapy, thus, requiring a drastic surgery.

[Results of the morphological examination of stomach in the different terms after stitching of a perforation duodenal ulcer].

Analyses of 184 cases of morphological examination of resected part of stomach in the different terms after stitching of a perforative duodenal ulcer is presented. The authors confirm, that ulcerative disease of duodenum and it's various complications including perforation is associated with antrum gastritis in all cases, which has superficial character. The authors are confirming that a gastric metaplasia of a mucosa of duodenum assists to the helicobacterial colonization, which represents one of the main factors of an ulcer formation along with a fast transition of acid gastric contents into the duodenum. The authors consider the gastric metaplasia as preulcerative pathological condition of duodenum.

[Free-tension plastic surgery of inguinal canal in hernias].

Fifty-one patients with inguinal hernia underwent tension-free hernioplasty, 32 of them were operated according to an original method which protects the spermatic cord against contact with net graft. This method permits to avoid recurrence, inguinodinia and dis-ejaculation.

[Results of a surgical treatment of perforative gastroduodenal ulcers].

The article gives an analysis of the surgical treatment of perforative gastroduodenal ulcers. Out of 185 patients in 143 (77,2%) cases took place operation on taking in a perforative hole when 9 (6,2%) patients, five of them above 60, couldn't get it over. It has to be noted that the mentioned 9 patients were taken to the hospital after 521 (five hundred and twenty one) hours the perforation had started, each of them after 58 (fifty eight) hours in average. At the same time for patients below 60 were taken to the hospital after 38 (thirty eight) hours, patients above 60 (sixty) after 74 (seventy four) hours. So one of the main causes of death was later hospitalization. This approach considerably reduces ulceroraphy due to the active implementation of the radical treatment methods. [Use of absorbable antimicrobial suture material at the gastroenteric anastomosis in patients with gastric and duodenal disease]. / Primenenie rassasyvaiushchegocia antimikrobnogo shovnogo materiala pri formirovanii zheludochno-kishechnogo anastomoza u patsientov s iazvennoi bolezníu zheludka i dvenadtsatiperstnoi kishki.

The authors in creation of the internal layer of sutures of gastroenteric anastomoses in 80 patients have used the synthetic antimicrobial absorbable material. No complications related to use of the suture material were revealed. The results obtained permit to recommend the new suture material for more wide use in the gastrointestinal surgery.

[Acute diseases of the appendices epiploicae]. / Ostrye zabolevaniia zhirovyk podveskov.

There were 5 patients with acute pathological conditions of the appendices epiploicae of the colon. Three of them had inflammation of the appendices one had necrosis due to strangulation of the appendix, and one patient had perforation of the colon in the region of the appendix. From their personal experience the authors conclude that all patients with acute diseases (inflammation, torsion, necrosis) of the appendices epiploicae should be treated by an emergency operation for removal of the appendices, closure of their beds with double-row interrupted sutures and plastics with a pedicled omentum graft.

[Disruption of the morphofunctional state of hepatocytes and microcirculation in the liver during prolonged artificial circulation in dogs]. / Narushenie morfofunktsional'nogo sostoianiia gepatotsitov i mikrotsirkuliatsii v pecheni pri dlitel'nom i skusstvennom krovoobrashchenii u sobak.

Qualitative and quantitative analysis was made of morpho-functional and enzymatic changes in the dog liver during long-term extracorporeal circulation. Pronounced necrotic alterations of hepatocytes and microcirculatory disorders were detected by the end of 3-hour extracorporeal circulation. The data obtained indicate the need for correction of circulation in the terminal parts of the vascular bed.
