RESUMEN

The present article analyzed the process of precarious work relations in the health sector, demonstrating how this situation generated illness and suffering among workers. The text sought to register how the institutional adoption of neoliberal economic conceptions, of the reduction of State social policies, coupled with a history of cultural rejection of labor rights, provided the attraction of mechanisms of precarious work for the public sector, notably, the outsourcing of labor and the provision of services. In a second moment, the text points out the legal improprieties committed, presenting the consequences of the weakening of the legal bond in the working conditions of these workers, mainly regarding working hours and remuneration, also highlighting how much the dissemination of these links represented a factor of a general reduction in wages and labor and social security rights in the sector. Data from Fiocruz surveys on working conditions during the pandemic were also presented. The article concludes by showing the urgency of rescuing the legal labor institutes that were abandoned in the historical path studied in order to guide the public health policies in a new direction.


O artigo analisou o processo de precarização das relações de trabalho no setor da saúde, demonstrando como essa situação gerou sofrimento dos trabalhadores. INTRODUCTION: Cerebrospinal fluid analysis contributes to the diagnosis and neuropathogenesis of neuroinvasive arboviruses. Neurological complications caused by dengue, Zika, and chikungunya infections have high clinical relevance because of their high potential to cause death or neurological deficits. We aimed to evaluate the use of cerebrospinal fluid assays for diagnostic support in neurological disorders associated with dengue, chikungunya, and Zika infections. METHODS: A systematic review was carried out by searching the electronic databases LILACS, PubMed, Scopus, and Embase for articles written in English, Portuguese, or Spanish in the last 19 years. Published studies were reviewed using the terms "dengue," "Zika", "chikungunya", alone or in combination with "cerebrospinal fluid" in the period from 2000 to 2019. RESULTS: A total of 98,060 studies were identified; of these, 1.1% (1,041 studies, 58,478 cases) used cerebrospinal fluid assays for neurological investigations. The most frequent neurological disorders included encephalitis (41.4%), congenital syndromes (17%), and microcephaly associated with Zika virus infections (8.9%). Neuroinvasive disorders were confirmed in 8.03% of 58,478 cases by specific cerebrospinal fluid analyses. The main methods used were IgM-specific antibodies (66%) and reverse transcription-polymerase chain reaction (10%). The largest number of scientific papers (29%) originated from Brazil, followed by India (18.4%) and the United States (14.4%). CONCLUSIONS: Although cerebrospinal fluid analysis is of great importance for increasing neurological diagnostic accuracy and contributes to the early diagnosis of neuroinvasive dengue, chikungunya, and Zika infections, it is underused in routine laboratory investigations worldwide.

Intestinal mucositis, characterized by inflammatory and/or ulcerative processes in the gastrointestinal tract, occurs due to cellular and tissue damage following treatment with 5-fluorouracil (5-FU). Rutin (RUT), a natural flavonoid extracted from Dimorphandra gardneriana, exhibits antioxidant, anti-inflammatory, cytoprotective, and gastroprotective properties. However, the effect of RUT on inflammatory processes in the intestine, especially on mucositis promoted by antineoplastic agents, has not yet been reported. In this study, we investigated the role of RUT on 5-FU-induced experimental intestinal mucositis. Swiss mice were randomly divided into seven groups: Saline, 5-FU, RUT-50, RUT-100, RUT-200, Celecoxib (CLX), and CLX + RUT-200 groups. The mice were weighed daily. After treatment, the animals were euthanized and segments of the small intestine were collected to evaluate histopathological alterations (morphometric analysis); malondialdehyde (MDA), myeloperoxidase (MPO), and glutathione (GSH) concentrations; mast and goblet cell counts; and cyclooxygenase-2 (COX-2) activity, as well as to perform immunohistochemical analyses. RUT treatment (200 mg/kg) prevented 5-FU-induced histopathological changes and reduced oxidative stress by decreasing MDA concentrations and increasing GSH concentrations. RUT attenuated the inflammatory response by decreasing MPO activity, intestinal mastocytosis, and COX-2 expression. These results suggest that the COX-2 pathway is one of the underlying protective mechanisms of RUT against 5-FU-induced intestinal mucositis.

Intestinal mucositis is a common complication associated with 5-fluorouracil (5-FU), a chemotherapeutic agent used for cancer treatment. Troxerutin (TRX), a semi-synthetic flavonoid extracted from Dimorphandra gardneriana, has been reported as a potent antioxidant and anti-inflammatory agent. In the present study, we aimed to evaluate the effect of TRX on 5-FU-induced intestinal mucositis. Swiss mice were randomly divided into seven groups: Saline, 5-FU, TRX-50, TRX-100, TRX-150, Celecoxib (CLX), and CLX + TRX-100. The weight of mice was measured daily. After treatment, the animals were euthanized and segments of the small intestine were collected to evaluate histopathological alterations (morphometric analysis), levels of malondialdehyde (MDA), myeloperoxidase (MPO), glutathione (GSH), mast and goblet cell counts, immunohistochemical analysis, and cyclooxygenase-2 (COX-2) activity. Compared to the saline treatment, the 5-FU treatment induced intense weight loss and reduction in villus height. TRX treatment (100 mg/kg) prevented the 5-FU-induced histopathological changes and decreased oxidative stress by decreasing the MDA levels and increasing GSH concentration. TRX attenuated inflammatory process by decreasing MPO activity, intestinal mastocytosis, and COX-2 expression. TRX also reversed the depletion of goblet cells. Our findings suggest that TRX at a concentration of 100 mg/kg had chemopreventive effects on 5-FU-induced intestinal mucositis via COX-2 pathway.

Objetivo: descrever uma abordagem fisioterapêutica na reabilitação neuropsicomotora e respiratória pediátrica em paciente com transposição das grandes artérias. Relato de caso: paciente R. S. C. R, sexo masculino, nascido de parto cesáreo, com diagnóstico clínico de malformações congênitas, portador de Transposição das Grandes Artérias ­ TGA, Comunicação Interatrial ­ CIA e Comunicação Interventricular Ampla ­ CIV. Avaliado aos 09 meses de idade com diagnóstico fisioterapêutico de atraso no desenvolvimento neuropsicomotor. Conclusão: esse relato de caso evidenciou efetiva abordagem fisioterapêutica na reabilitação neuropsicomotora e respiratória de uma criança com TGA + CIA + CIV em acompanhamento clínico, resultando em melhora neuropsicomotora identificada pelo melhor controle cervical e maior tempo na posição sentada, e melhora do desconforto respiratório passando de moderado para leve

BACKGROUND: The tuberous breast syndrome is a condition that affects young women and can bring on serious disorders of self-esteem. There are numerous surgical techniques to correct this condition, but most of them include the use of breast implants to replace lack of volume. Nowadays, patients are increasingly becoming more demanding to get a definitive solution with minimal aesthetic sequelae. METHODS: We present a series of 11 patients with tuberous breast deformities treated with lipofilling technique. Fat harvest was performed by conventional lipoaspiration with 3 mm cannulas, centrifuged at 2000 rpm for 2 minutes and injected in the breast were was needed with 1.9 mm blunt cannulas. RESULTS: The patient's average age was 24 year old with a BMI of 23.4 kg/m2. Volumes between 80 and 250 ml per breast were injected in every session, requiring a total volume of 413 ml per breast. Most patients required up to two procedures to achieve the complete breast correction. The mean follow-up was 29.7 months. All patients had good aesthetics results with minimal complications. CONCLUSION: Natural breast mound with excellent cosmetic result can be achieved with fat grafting. Fat grafting main advantage is to respond to physiological changes in weight over time. In addition, lipofilling do not carry the inherent complications of implants. We strongly believe it is a safe and easy technique to reproduce in properly selected patients and with minimal morbidity it can cause the best natural result.

AIMS: This study aims to evaluate whether there is a difference between the effects of vildagliptin and gliclazide MR (modified release) on glycemic variability (GV) in women with type 2 diabetes (T2DM) as evaluated by continuous glucose monitoring (CGM). METHODS: An open-label, randomized study was conducted in T2DM women on steady-dose metformin monotherapy which were treated with 50 mg vildagliptin twice daily or 60-120 mg of gliclazide MR once daily. CGM and GV indices calculation were performed at baseline and after 24 weeks. RESULTS: In total, 42 patients (age: 61.9 ±â€¯5.9 years, baseline glycated hemoglobin (HbA1c): 7.3 ±â€¯0.56) were selected and 37 completed the 24-week protocol. Vildagliptin and gliclazide MR reduced GV, as measured by the mean amplitude of glycemic excursions (MAGE, p = 0.007 and 0.034, respectively). The difference between the groups did not reach statistical significance. Vildagliptin also significantly decreased the standard deviation of the mean glucose (SD) and the mean of the daily differences (MODD) (p = 0.007 and 0.030). CONCLUSIONS: Vildagliptin and gliclazide MR similarly reduced the MAGE in women with T2DM after 24 weeks of treatment. Further studies are required to attest differences between vildagliptin and gliclazide MR regarding glycemic variability.

Este estudo teve por objetivo desenvolver um modelo matemático que a partir da extensão do osso esterno fornecesse o tamanho do pulmão compatível para o receptor. Foram coletadas as medidas antropométricas do tórax de 250 indivíduos, através de exame de tomografia computadorizada. Os resultados apontam que a medida do osso esterno (distância da incisura jugular até processo xifóide) apresenta correlação positiva com todas as outras medidas do tórax (medida ântero-posterior e látero-medial entre II e III costela, e ápice à base de ambos os pulmões). Entretanto, o volume pulmonar e sua relação com o osso esterno apresentam discrepâncias quando analisados sob a correlação de Pearson, pois a relação entre a medida da incisura jugular ao processo xifóide e a medida do ápice à base do pulmão direito e esquerdo, apresenta correlação positiva média (0,31-0,6). Já a medida da incisura jugular ao processo xifóide com a medida ântero-posterior e látero-medial do tórax, apresenta correlação significativa baixa (0-0,3). Então, a análise estatística da correlação de Pearson demonstrou ser inviável o desenvolvimento da fórmula, pois esta não seria confiável já que funcionaria para cerca de apenas 39% dos pacientes. Assim, o melhor método para determinar o doador para o transplante, continua sendo a análise de fatores de risco, a capacidade vital forçada do doador e receptor com estatura maior do que a do doador.


This study aimed at developing a mathematical model that can provide the compatible lung size for the recipient from the length of the sternum bone. Anthropometric chest measurements of 250 individuals were collected through a CT scan. The results indicate that the measurement of the sternum bone (distance from the jugular notch to the xiphoid process) shows a positive correlation with all other thorax measurements (antero-posterior and medial-posterior measurement between ribs II and III, and apex-to-base on both lungs). However, lung volume and its correlation to the sternal bone present discrepancies when analyzed under Pearson's correlation, since the relation between the jugular notch measurement and the apex measurement at the base of the right and left lungs shows a positive correlation mean (0.31-0.6). The measurement of the jugular notch in the xiphoid process with the anterior-posterior and medial-medial measurements, presents a low significant correlation (0-0.3). Therefore, the statistical analysis of the Pearson's correlation showed that the formula could not be applied since it would not be reliable since it would work for only 39% of the patients. Thus, the best method to determine the donor for transplantation remains the analysis of risk factors, the forced vital capacity of the donor, and the recipient being taller than the donor.

BACKGROUND: Several antidiabetic therapies affect bone metabolism. Sulfonylureas have the lowest impact on bone among oral antidiabetics. The objective of this study is to compare the effects of vildagliptin and gliclazide modified release (MR) on bone turnover markers (BTMs) and bone mineral density (BMD) in postmenopausal women with uncontrolled type 2 diabetes (T2D). METHODS: Forty-two postmenopausal women with uncontrolled T2D were randomly allocated into vildagliptin or gliclazide MR (control) groups. The primary endpoint was the change in the BTMs in months 6 and 12 compared with the baseline. The secondary endpoint was the variation in the BMD, which was assessed via dual-energy X-ray absorptiometry at the lumbar spine, femoral neck and total hip at baseline and month 12. RESULTS: After a 12-month treatment, the BTM serum carboxy-terminal telopeptide of type 1 collagen increased 0.001 ± 0.153 ng/mL in the vildagliptin group versus 0.008 ± 0.060 ng/mL in the gliclazide MR group (p = 0.858). The serum osteocalcin, serum amino-terminal propeptide of procollagen type I and urinary amino-terminal telopeptide of type 1 collagen remained stable in both groups, and there was no statistically significant difference between the effect of vildagliptin and gliclazide MR on these variables. The lumbar spine BMD did not change in the vildagliptin or gliclazide MR groups after a 12-month treatment (0.000 ± 0.025 g/cm2 versus -0.008 ± 0.036, respectively, p = 0.434). Furthermore, there was a similar lack of change in the femoral neck and total hip BMD values in both treatments. CONCLUSIONS: Bone turnover markers and BMD remained unchanged after a 12-month treatment in both groups, which suggests that vildagliptin has the same safety profile as gliclazide MR on bone metabolism. O emprego de dietas para cães contendo carne crua tem sido uma prática popular frequente. Estas, quando contaminadas por Salmonella, podem oferecer risco para a saúde dos animais e dos seres humanos. Este trabalho é uma revisão sistemática da literatura efetuada para avaliar a frequência de dietas para cães que continham carne crua contaminada por Salmonella, e se os cães que as consumiam eliminariam a bactéria em suas fezes. A pesquisa foi realizada em duas bases de dados, Web of Knowledge e PubMed, durante janeiro de 2015. Participaram da pesquisa trabalhos sobre dietas para cães contendo alimentos crus e seu consumo; treze artigos foram selecionados. Em 11 trabalhos foram conduzidas análises microbiológicas para Salmonella nas dietas contendo carne crua, sendo que em quatro desses estudos também foi analisada a presença da bactéria nas fezes dos animais. Em 679 dietas contendo carne crua, 141 delas (20,76%) estavam contaminadas, sendo o sorotipo mais encontrado foi Salmonnela Typhimurium (24,60% do total). Dos estudos que relataram a eliminação da bactéria após o consumo de dietas contendo carne crua, um total de 141 amostras fecais foram analisadas, das quais 73 (51,77%) estavam contaminadas, e o sorotipo mais encontrado foi Salmonella Newport (72,60% do total). Os resultados obtidos permitem afirmar que os proprietários dos cães alimentados por dietas (caseiras ou comerciais) a base de carne crua contaminada estão expostos ao risco de infecção por Salmonella tanto pela manipulação de alimentos como pelas fezes dos animais.(AU)


Feeding dogs raw meat diets is an increasingly popular trend, and when contaminated with Salmonella they may present a risk to the health of both animals and humans. The present investigation is a systematic review of literature to assess the frequency of raw meat diets for dogs contaminated with Salmonella and whether dogs consuming these diets can eliminate it in their faeces. A search was made using two databases: "Web of Knowledge" and "PubMed" in January 2015. Only studies related to raw food diets and consumption by dogs were included. Thirteen articles were selected after refining the search. In 11 studies microbiological analysis was conducted for Salmonella in diets containing raw meat, so that in four of these studies Salmonella in the faeces was also analysed. In the analysis of 679 raw meat diets 141 diets (20.76%) were contaminated with Salmonella, in which the serotype most commonly found was S. Typhimurium (24.60%). Of the studies that reported the number of dogs that eliminated Salmonella in faeces after consumption of diets containing raw meat, 141 faecal samples were analysed, of which 73 (51.77%) were contaminated with Salmonella and the serotype most commonly found was S. Newport (72.60%). The available data suggest that the owners who feed their animals with diets containing contaminated raw meat, either commercial or homemade, are exposed to risk of Salmonella contamination, either by the manipulation of food or by handling the dog's stool.(AU)

ABSTRACT: This study aimed to evaluate the inclusion of Yucca schidigera extract and zeolite (Clinoptilolite) added to the diets for dogs and its effect on apparent digestibility coefficients (ADC) of diet components (dry matter, organic matter, crude protein, acid hydrolyzed fat, and energy) and urinary pH. Twenty-one adult Beagles, males and females, body weight mean of 12.5±1.5kg and four and a half years old, distributed in a completely randomized design with seven replicates per treatment within three experimental treatments: control (no addition of Yucca schidigera and zeolite), Yucca schidigera (375ppm) and zeolite (1%), administered in the encapsulated form of the supplement. The ADC of diet components and urinary pH were not affected by the inclusion of additives (P>0.05), except the ADC acid hydrolyzed fat showed reduction with Yucca schidigera supplementation compared to the other treatments (P<0.05). The addition of 375ppm of Yucca schidigera extract reduced the digestibility of dietary fat by dogs and both Yucca schidigera extract and zeolite did not affect the urinary pH of dogs.


RESUMO: O objetivo deste trabalho foi avaliar a inclusão do extrato de Yucca schidigera e da zeólita (Clinoptilolita) nas dietas para cães e seus efeitos sobre os coeficientes de digestibilidade aparente (CDA) dos componentes da dieta (matéria seca, matéria orgânica, proteína bruta, extrato etéreo em hidrólise ácida e energia) e no pH urinário. Foram utilizados 21 cães Beagles, machos e fêmeas, com peso médio de 12,5±1,5kg e quatro anos e meio de idade, distribuídos em delineamento inteiramente casualizado com sete repetições para cada um dos três tratamentos experimentais: controle (sem adição de Yucca schidigera e zeólita), Yucca schidigera (375ppm) e zeólita (1%), administrados na forma de suplemento encapsulado. Os CDA dos componentes da dieta e o pH urinário não foram afetados pela inclusão dos aditivos (P>0,05), exceto o CDA do extrato etéreo em hidrólise ácida, que reduziu com a suplementação de Yucca schidigera em comparação aos demais tratamentos (P<0,05). A adição de 375ppm de extrato de Yucca schidigera reduziu a digestibilidade da gordura dietética pelos cães e tanto a adição de Yucca schidigera como a de zeólita não alteraram o pH urinário.

Background/Aims: To measure skeletal muscle lipid infiltration, its association with insulin resistance (IR) lean mass and function, in Chilean men differing in age and body composition. Our hypothesis was that muscle lipid accumulation would be higher among older and heavier individuals and this would deteriorate insulin sensitivity (IS) and decrease muscle mass and function, both features of the ageing process. Methods: Healthy men (38 < 55 and 18 > 65 years), underwent anthropometric measurements, body composition assessment through radiologic densitometry, Nuclear Magnetic Resonance spectroscopy at the tibialis anterioris muscle to measure intra (IMCL) and extramyocellular lipids (EMCL), quadriceps and handgrip strength, 12 minute walking distance and serum biochemistry (haemoglobin, lipoproteins, creatinine, ultrasensitive C Reactive Protein, fasting and post glucose insulin and glucose concentrations, to assess IS). Physical activity was estimated by actigraphy. Results: 23 men were eutrophic, 26 were overweight and 7 were obese and mostly sedentary, independent of age. Both IMCL and EMCL were higher in overweight/ obese men. Abdominal fat was negatively associated with IS and positively correlated with muscle lipid accretion (both IMCL and EMCL), but not with age. As expected, older individuals had lower muscle mass and strength, but not more adipose tissue nor intramyocellular lipids, yet were more glucose intolerant. Conclusions: central obesity was associated with IMCL and EMCL infiltration and IR. This type of lipid accretion was not related with ageing nor age-related sarcopenia. Older individuals were more glucose intolerant, which was explained by a decrease of insulin secretion more than adiposity-related IR (AU)


Introducción/Objetivos: medir la infiltración grasa en el músculo esquelético, su asociación con resistencia a la insulina (RI) y con masa y función muscular, en hombres chilenos de diferente edad y composición corporal. Nuestra hipótesis era que habría mas acumulación de grasa en el tejido muscular entre las personas de mayor edad y peso, lo cual deterioraría la sensibilidad a la insulina (SI) y afectaría negativamente la masa y la función muscular, ambas características del proceso de envejecimiento. Métodos: se estudiaron hombres sanos (38 < 55 anos y 18 > 65 anos), que fueron sometidos a mediciones antropométricas, evaluación de la composición corporal mediante densitometría radiológica (DEXA), espectroscopia de resonancia nuclear magnética en el músculo tibial anterior para medir lípidos intra (LIM) y extramiocelulares (LEM), fuerza de mano y cuadriceps, test de 12 minutos y bioquímica sérica (glicemia, hemoglobina, lipoproteínas, creatinina y proteína C reactiva ultrasensible en ayunas, ademas de glucosa e insulina post carga de glucosa para evaluar SI). La actividad física se estimo mediante actigrafía. Resultados: 23 hombres eran eutróficos, 26 tenían sobrepeso y 7 eran obesos, todos eran sedentarios según el registro actigráfico, independiente de la edad. Tanto LIM como LEM resultaron mas altos entre los hombres con sobrepeso / obesidad. La grasa abdominal se asocio negativamente con la SI y se correlaciono positivamente con la acumulación de grasa en el músculo (tanto LIM como LEM), pero no con la edad. Como era de esperar, las personas mayores tenían menor masa magra y fuerza, pero no mas tejido adiposo ni lípidos intramiocelulares, aunque eran mas intolerantes a la glucosa. Conclusiones: La obesidad central se asocio con infiltración de grasa intramuscular y con RI. Esta distribución adiposa no se relaciono con edad ni con sarcopenia asociada al envejecimiento. Las personas mayores resultaron mas intolerantes a la glucosa, explicable por una disminución de la secreción de insulina mas que por RI relacionada con mayor adiposidad (AU)

OBJECTIVE: To compare ethylenediamine tetraacetic acid (EDTA) tubes and plasma preparation tubes (PPT) for evaluating maternal plasma during the first trimester of pregnancy. METHODS: A cross-sectional study was conducted on 24 male fetuses in women between 6 and 14 weeks of pregnancy. Blood samples (10 mL) were collected and stored in EDTA and PPT tubes. Subsequently, the samples were centrifuged and sent for free fetal DNA extraction by means of the polymerase chain reaction (PCR) technique. The reactions were performed in a real time PCR machine for detecting the amplification products. The genome region chosen for performing the PCR reactions was a target specific for the Y chromosome, in which the DYS-14 marker was amplified only when the DNA was of male sex. The free fetal DNA concentration was given by the threshold cycle (TC). To compare the tubes, the paired Student t-test was used. RESULTS: The mean gestational age was 11.08 ± 2.30 weeks (range: 6-14). The mean TC for PPT was 30.08 ± 1.05 (range: 27.08-32.61) and for EDTA, 30.23 ± 0.96 (range: 28.01-32.09), but without statistical significance (p=0.357). CONCLUSION: We did not observe any statistically significant difference in free fetal DNA concentration between the EDTA and PPT tubes.

We report on a prenatal diagnosis of ring chromosome 15 in a fetus with left congenital diaphragmatic hernia (CDH) and severe intrauterine growth restriction (IUGR). A 31-year-old woman, gravida 2 para 1, was referred because of increased nuchal translucency at gestational age of 13 weeks. Comprehensive fetal ultrasound examination was performed at 19 weeks revealing an early onset IUGR, left CDH with liver herniation, and hypoplastic nasal bone. Three-dimensional ultrasound (rendering mode) showed low set ears and depressed nasal bridge. Amniocentesis was performed with a result of a 46,XX,r(15) fetus after a cytogenetic study. A 1,430 g infant (less than third percentile) was born at 36 weeks. The infant presented with respiratory failure and died at 2 h of life. Postnatal karyotype from the umbilical cord confirmed the diagnosis of 15-ring chromosome. We described the main prenatal 2D- and 3D-ultrasound findings associated with ring chromosome 15. The interest in reporting the present case is that CDH can be associated with the diagnosis of 15-ring chromosome because the critical location of the normal diaphragm development is at chromosome 15q26.1-q26.2.

OBJECTIVES: The purpose of this study was to establish reference ranges for 2-dimensional sonographic measurements of fetal lungs from longitudinal data. METHODS: A total of 214 fetal lung measurements were longitudinally evaluated in 62 healthy fetuses between 20 and 36 weeks' menstrual age. Both right and left lung areas were measured in the heart 4-chamber view using lung area tracing and axis diameter methods. Multilevel modeling was used to evaluate the expected values and variability with respect to menstrual age and to generate reference ranges for the lung area, lung-to-head ratio, quantitative lung index, and observed-to-expected lung-to-head ratio for both lungs. RESULTS: The expected values varied with menstrual age for all parameters. Variance was menstrual age dependent for all parameters except the longest diameter area measurements and their lung-to-head ratios. CONCLUSIONS: Models are presented for expected 2-dimensional sonographic lung size parameters and their variance as a function of menstrual age. These data have been used to generate age-specific reference ranges for both measurements and indices.

Epignathus teratomas are rare tumors that originate in the region of the palate or pharynx and are known as Rathke pouch. They may be associated with other malformations such as a cleft palate and bifid tongue or nose. The prenatal diagnosis can be established by two-dimensional ultrasonography showing a heterogeneous mass protruding through the mouth of the fetus. The three-dimensional ultrasonography in rendering mode permits better understanding of this anomaly by the parents, facilitating the counseling. The importance of prenatal diagnosis lies in the fact that this tumor may obstruct the airways, thus leading to death at birth due to respiratory insufficiency. We report a case of prenatal diagnosis of epignathus teratoma in the 30th week of pregnancy by two-dimensional ultrasonography. We focus on the importance of three-dimensional ultrasonography in rendering mode for demonstrating the spatial relationships of the tumor with the oral cavity and provide correlations between the ultrasound images and the anatomopathological findings.

OBJECTIVE: To evaluate the accuracy of serum IGF-1 in the detection of isolated (IGHD) or combined growth hormone deficiency (CGHD) at the transition phase. SUBJECTS AND METHODS: Forty nine patients with GHD during childhood [16 with IGHD (10 men) and 33 with CGHD (24 men); age 23.2 ± 3.5 yrs.] were submitted to an insulin tolerance test (ITT) with a GH peak < 5 µg/L used for the diagnosis of GHD at the transition phase. Pituitary function and IGF-1 measurements were evaluated in the basal sample of the ITT. Transition patients were reclassified as GH-sufficient (SGH; n = 12), IGHD (n = 7), or CGHD (n = 30). RESULTS: Five (31%) patients with IGHD and 32 (97%) with CGHD at childhood persisted with GHD at retesting. One patient with IGHD was reclassified as CGHD, whereas 3 patients with CGHD were reclassified as IGHD. Mean GH peak was 0.2 ± 0.3 µg/L in the CGHD, 1.3 ± 1.5 µg/L in the IGHD, and 18.1 ± 13.1 µg/L in the SGH group. Serum IGF-1 level was significantly higher in the SGH (272 ± 107 ng/mL) compared to IGHD (100.2 ± 110) and CGHD (48.7 ± 32.8) (p < 0.01). All patients reclassified as CGHD, 86% reclassified as IGHD, and 8.3% reclassified as SGH had low IGF-1 level, resulting in 97.3% sensitivity and 91.6% specificity in the detection of GHD at the transition period; the cutoff value of 110 ng/mL showed 94.5% sensitivity and 100% specificity. Mean IGF-1 values did not differ in IGHD or CGHD associated with one, two, three, or four additional pituitary deficiencies. CONCLUSION: IGF-1 measurement is accurate to replace ITT as initial diagnostic test for IGHD and CGHD detection at the transition phase.


OBJETIVO: Avaliar a acurácia da dosagem sérica de IGF-1 no diagnóstico da deficiência de hormônio de crescimento isolada (DGHI) ou combinada (DGHC) na fase de transição. SUJEITOS E MÉTODOS: Quarenta e nove pacientes com DGH na infância [16 DGHI (10 homens) e 33 DGHC (24 homens); idade 23,2 ± 3,5 anos] realizaram teste de tolerância à insulina (TTI), com pico de GH < 5 µg/L considerado diagnóstico de DGH na transição. Função hipofisária e níveis de IGF-1 foram determinados na amostra basal do TTI e os pacientes foram reclassificados em GH suficientes (SGH; n = 12), DGHI (n = 7) ou DGHC (n = 30). RESULTADOS: Cinco (31%) pacientes com DGHI e 32 (97%) com DGHC na infância persistiram com DGH no reteste. Um paciente com DGHI foi reclassificado como DGHC e três com DGHC como DGHI. Os picos médios de GH foram 0,2 ± 0,3 µg/L (DGHC), 1,3 ± 1,5 µg/L (DGHI) e 18,1 ± 13,1 µg/L (SGH). O nível médio de IGF-1 foi maior no grupo SGH (272 ± 107 ng/mL) comparado com DGHI (100,2 ± 110) e DGHC (48,7 ± 32,8) (p < 0,01). IGF-1 baixo foi observado em todos os pacientes reclassificados como DGHC, 86% dos DGHI e 8,3% dos SGH, resultando em sensibilidade de 97,3% e especificidade de 91,6% para detecção de DGH na transição; valor de corte de 110 ng/mL mostrou 94,5% sensibilidade e 100% especificidade. RESUMEN

Treacher-Collins syndrome (TCS) is a rare dominant autosomal anomaly resulting from malformation or disruption of the development of the first and second branchial arches. It is characterized by micrognathia, malar hypoplasia, and malformations of the eyes and ears. The prenatal diagnosis using two-dimensional ultrasonography (2DUS) is characterized by identification of facial malformations together with polyhydramnios. Three-dimensional ultrasonography (3DUS) has the capacity to spatially display these facial malformations, thus making it easy for the parents to understand them. We present a case of TCS diagnosed in the 33rd week using 3DUS, with postnatal confirmation using cranial computed tomography and anatomopathological analysis.
