RESUMEN

Attempts at unifying the diagnostic and therapeutic procedures for transgender individuals experiencing gender dysphoria were first undertaken in Poland in the 1980s. Since then, there has been a change in the perception of transgenderism, almost paradigmatic, expressed in subsequent editions of the diagnostic systems (DSM, ICD), which is also associated with the fundamental changes in the principles of conducting trans-specific healthcare. This triggered the need to formulate recommendations for specialists practicing in Poland, which would at least partly reflect the evolution of views and guidelines on clinical care in transgender adults seeking help due to gender dysphoria.

Despite the fact that the group of sexual offenders remains a population which is still difficult to study, the results of current research are considered novel and interesting. Surprisingly, the very old descriptions applying to paraphilia, which is considered to be one of the reasons of sexual offences, appear to be accurate, especially in the context of similarities between impulsivity and pathologic sexual behaviors. Notably, the nomenclature concerning impulsivity enables a specific and reasonable description of behaviors associated with sexual offences. Moreover, the results of research studies show that it is the lack of inhibition, not pathologic arousal, which is the most important factor in the pathogenesis of forbidden sexual behaviors. In addition, it has been shown that behavioral manifestations of impulsivity (substance abuse, suicide attempts) appear commonly in sexual offenders. Mutual relationships between alcohol drinking, suicide attempts, history of child sexual abuse and sexual offences, both in symptomatologic and etiologic aspect, raise a suggestion that all these phenomena may share a common background of poor inhibitory control.

AIM: The aim of the research was to construct a questionnaire basing on the presently standing criteria of sexual dysfunctions and sexual disorders (according to ICD-10), that would enable the diagnosis of symptoms, as well as to verify its validity. METHOD: 102 patients (46 women and 56 men, mean age of 32.09, SD = 10.29) of sexological clinics who declared they had symptoms of sexual dysfunctions or disorders were asked to participate and fill in the questionnaire which was later evaluated in a clinical study on the basis of ICD-10 research criteria. RESULTS: Results show that using the questionnaire enables the diagnosis of 100% of gender identity disorders, almost all sexual dysfunctions and disorders connected to sexual orientation. The sensibility and specificity of the questionnaire were also examined and proved very high. CONCLUSIONS: The Sexological questionnaire for screening research presents with good validity, sensibility and specificity.

AIM: Assessment of body image satisfaction, sense of coherence and life satisfaction during the therapy of women with transsexualism. METHODS: 27 women with transsexualism (before therapy--9; in therapy but before surgery--7; after surgery--11), in different stages of therapy and 15 women and 15 men from the control group participated in the study. The Body Image Scale, the Sense of Coherence- SOC 29 and Satisfaction with Life Scale-- SWLS. RESULTS AND CONCLUSIONS: The results suggest that the increased satisfaction from the body image in the group of women with transsexualism is related with progress in therapy. Persons from the control group were significantly more satisfied form their own body image than persons with transexualism. The progress of therapy is related with the decrease of will to change the body. The mean scores on the meaningfulness subscale of Sense of Coherence Scale was significantly lower in the group of patients before the beginning of treatment than in the control group and in the group after surgery. There was no relation between the level of life satisfaction and stages of therapy.
