RESUMEN

The purpose of the study was to review quantitative and qualitative radiological manifestations of coxarthrosis and gonarthritis in patients aged over 60 years. Radiography, computed tomography and multislice computed tomography were performed for 48 patients aged 60-70 years with deforming arthritis of the hip and knee joints. Idiopathic and dysplastic cases prevailed. The results of the study showed that roentgen anatomical changes in the hip and knee joints were accompanied by changes in the density of femoral head, typical restructuring of the femoral and tibial condyles, patella that could earlier be detected with computed tomography. A decrease in the total density of the femoral and tibial condyles and a relative increase in subchondral density with varus of the knee in particular were shown to be a specific feature of quantitative changes in the density of bone tissue in patients with deforming arthritis.

RESUMEN

Computed tomography was used to study the X-ray morphological characteristics of femoral and humoral bone cysts and the course of a reparative process after surgery in 23 patients with varying activity of solitary bone cysts. Treatment using transosseous osteosynthesis, intraosseous armoring, and therapeutic and diagnostic punctures in patients with solitary bone cysts could yield the most active osteoproductive effect and cause a considerable increase in the density of cyst contents in active and activity-losing stages, which was most evident around the intraosseous wires. Patients with late-stage cysts were found to have an osteogenic effect, mainly in an area of osteotomy.

RESUMEN

To study the age-related features of growth areas and to detect the x-ray background of recurrent deformities in patients with rickets-like diseases, the investigators analyzed knee arthrograms in 74 patients aged 4 to 16 years (28 and 46 patients with vitamin D-deficiency or vitamin D-resistant rickets (phosphate diabetes), respectively) prior to treatment and in the late period.

RESUMEN

A Somatom AR.HP computed tomograph (Siemens) was used to make studies in 54 patients with injury to the vertebral column and spinal cord in order to determine the formation of a bony block and its density in different portions of an injured segment at the stages of treatment. During therapy, the highest values of bony block density were observed in the dorsal portions of an injured segment (mean 377.32 +/- 184.57 HU). This may be accounted for by a slight effect of artefacts of an external transpedicular fixation apparatus, the main components of which are present at the posterior supporting complex. After treatment, the highest values of bony block density were noted in the central portions of an injured segment (mean 353.11 +/- 208.01 HU). This may be associated with rather serious intervertebral disk damage in this portion. The bony block density index was 339.75 +/- 139.41 HU in the left lateral portions and as high as 312.56 +/- 135.07 HU in the right ones. In the late period, the highest bony block density index (mean 397 +/- 193.07 HU) was seen in the right portions of an injured segment, which may be due to slight scoliotic deformity with the angle being open to the right.

The Academician G.A. Ilizarov Russian Research Center "Reconstructive Traumatology and Orthopedics" treated 40 patients with double humeral fractures. On examining their X-ray films, the patients were divided into groups according to the location of an intercalated fragment. The Eletto procedure was used to estimate the levels of fractures. Comparison of the X-ray morphometric studies, the diagram of the humeral architectonics, and clinical observations revealed a relationship between the time of consolidation and the location of an intercalated fragment, which is explained by the degree of preservation of the latter's intramedullary blood flow. While treating patients with double fragments of the shoulder, it is expedient to use the Eletto index to determine the levels of the fragments.

Computed tomography was used to study soft tissues of the femur in 78 patients with its chronic recurrent osteomyelitis and the studied parameters were estimated. The fibrous changes were found to be more pronounced in posttraumatic osteomyelitis; the thickening of the subcutaneous fat and its atrophy were equally marked; the density gradients between the intact muscle, infiltrated muscle tissue, and purulent leakage were calculated.

RESUMEN

The paper studies the age-related X-ray features of the spine in patients with achondroplasia. It gives the time course of changes in the shape of vertebrae, the specific features of apophyseal ossification, provides a quantitative account of the shorter caudal lumbar vertebral arch root distance symptom. The time course of changes in the size of the lumbosacral angle was examined. The findings suggest that there are not only considerable static changes in the spine of patients with achondroplasia, but also significant age-related features of vertebral tissue growth and differentiation.

RESUMEN

Muscles and subcutaneous cellular tissue of upper and lower limbs have been studied in 14 patients with achondroplasia treated after Ilizarov in various periods of lengthening employing contrast roentgenography. The technique applied has allowed to reveal anatomic and topographic features of muscles and subcutaneous cellular tissue in achodroplasia cases and observe changes of morphological parameters in different stages of elongation.

RESUMEN

The paper is concerned with the main methodological principles and potentialities of a radiocontrast study of subcutaneous fat to detect pathological changes in it. The proposed method consists in slow administration in the middle fat layers of 20-25% solution of verografin from not less than two points, and subsequent roentgenography in a necessary projection. The method was used in 15 patients with various orthopedic diseases for diagnosis of inflammatory, tumorous and degenerative-dystrophic diseases.

In process of crus lengthening, following the method, given by Ilizarov G. A., with 22 adult patients have been studied the peculiarities of metabolic processes of muscle tissue of the lengthened segment as well as the muscle tissue structural changes. As the tests were used; determination of lactate dehydrogenase content and its isoenzymatic spectrum in the blood serum, the residual products of lactic and pyruvic acids glycolysis, relating to the higher indications of the internal medium of an organism; carboxylase activity detection. Energy provision of an organism was estimated according to the ATP content; anatomical state of muscles of the lengthened extremity segment and muscle tissue structure were estimated by means of contrast myography. Dynamics of biochemical indexes suggests that the sparing character of an operation, patient mobility from the first days, preservation of locomotive and supporting function contribute to prevention of metabolic disease and ensure earlier transformation of catabolic reaction to anabolic one. Carboxylase and lactate-dehydrogenase activity increase indicates the biochemical adaptation of an organism. Metabolic changes well correlate with the detected structural changes in the lengthened crus muscles.

On the basis of analysis of the roentgen contrast examination of femoral muscles of the patients with extension contractures of knee joint, roentgen systematics of the muscle changes at given pathology is presented and the criteria of differential diagnosis of contractures of different types are proposed. 55 patients (125 examinations) with extension contractures of knee joint (arthrogenic, immobilizing, due to myofascio-tenodesis) of different etiology were subjected to roentgen contrast myography. As a consequence, this offered a means of objective evaluation of the muscle state and enabled one to discover the changes of muscles, specific for contractures in general as well as for each type in particular. The table of roentgenologic signs has been compiled which permits to make differential diagnosis, determine the tactics of treatment, select the type of surgical intervention on the basis of the muscle condition.
