RATIONALE/OBJECTIVE: The phase angle (PhA) is a measurement obtained from the resistance and reactance found in the bioelectrical impedance examination and is useful in assessing nutritional status and in assessing cardiometabolic diseases. Therefore, the objective was to correlate the phase angle to the anthropometric variables of cardiometabolic risk (CMR) in children and adolescents. SUBJECTS/METHODS: Weight, height, body mass index (BMI), waist circumference (WC) and neck circumference (NC), waist-to-height ratio (WHtR), and PhA of 1231 children and adolescents aged between 5 and 18 years were assessed. PhA was assessed in tertiles and CMR was identified using the anthropometric variables BMI, WC, NC, and WHtR. Linear regression models were used to verify the association between PhA tertiles and CMR anthropometric variables, stratified by sex. RESULTS: No relationship was found between PhA and CMR variables in children. In adolescents, lower values of NC (ß = -0.019, p = 0.005) and BMI (ß = -0.025, p = 0.023) were found in boys, WC (ß = -0.017, p = 0.020), WHtR (ß = -0.017, p = 0.020), NC (ß = -0.011, p = 0.027) and BMI (ß = -0.026, p = 0.009) in girls when they were in tertile 1 when compared to tertile 3 of the PhA, even after adjusting for age. CONCLUSION: The PhA was directly associated with CMR in the assessed adolescents.

Despite the extensive clinical and scientific advances in prevention, diagnostics and treatment, cardiovascular diseases (CVD) remain the leading cause of morbidity and mortality worldwide for people aged 65 and over. Of all ageing-related diseases, CVD are responsible for almost one-third of deaths in the elderly, being above all cancers combined. Age is an independent and unavoidable risk factor contributing to the impairment of heart and blood vessels. As the average age of the population in industrialized countries has doubled in the last century, and almost a fifth of the world's population is predicted to be over 65 in the next decade, we can assume that the burden of CVD will fall primarily on the elderly. Evidence from basic and clinical science has shown that sex significantly influences the onset and severity of CVD. In women, CVD usually develop later than in men and with atypical symptomatology. After menopause, however, the incidence and severity of CVD increase in women, reaching equality in both sexes. Although intrinsic sexual dimorphism in cardiovascular ageing may contribute to the sex differences in CVD progression, the molecular mechanisms associated with cardiovascular ageing and their clinical value are not known in detail. In this review, we discuss the scientific knowledge available, focusing on structural, hormonal, genetic/epigenetic and inflammatory pathways, seeking to transfer these findings to the cardiovascular clinic in terms of prevention, diagnosis, prognosis and management of these pathologies and proposing possible validation of target specifics.

Introduction: The article analyzed homeless people's (HP) access to health and social protection policies and tailored inter-sector care, including emergency measures, during the COVID-19 pandemic in Belo Horizonte (BH), capital of Minas Gerais state, Brazil. It intended to provide data on HP and evaluate existing public policies focused on vulnerable populations during this health emergency. Methods: The study adopted a mixed-methods design with triangulation of quantitative and qualitative data. Results: Social cartography showed that in the early months of the pandemic, the health administration had difficulty reordering the health system, which experienced constant updates in the protocols but was nevertheless consolidated over the months. The evidence collected in the study showed that important emergency interventions in the municipality of BH involved activities that facilitated access by HP to the supply of services. Discussion: The existence of national guidelines for inter-sector care for HP cannot be ruled out as a positive influence, although the municipalities are responsible for their implementation. Significantly, a health emergency was necessary to intensify the relationship between health and social protection services. Roving services were among those with the greatest positive evidence, with the least need for infrastructure to be replicated at the local level. In addition, the temporary supply of various inter-sector services, simultaneously with the provision of day shelters by organized civil society, was considered a key factor for expanding and intensifying networks of care for HP during the emergency phase. A plan exists to continue and expand this model in the future. The study concluded that understanding the inter-sector variables that impact HP contributes to better targeting of investments in interventions that work at the root causes of these issues or that increase the effectiveness of health and social protection systems.

Right ventricular (RV) failure remains the strongest determinant of survival in pulmonary hypertension (PH). We aimed to identify relevant mechanisms, beyond pressure overload, associated with maladaptive RV hypertrophy in PH. To separate the effect of pressure overload from other potential mechanisms, we developed in pigs two experimental models of PH (M1, by pulmonary vein banding and M2, by aorto-pulmonary shunting) and compared them with a model of pure pressure overload (M3, pulmonary artery banding) and a sham-operated group. Animals were assessed at 1 and 8 months by right heart catheterization, cardiac magnetic resonance and blood sampling, and myocardial tissue was analyzed. Plasma unbiased proteomic and metabolomic data were compared among groups and integrated by an interaction network analysis. A total of 33 pigs completed follow-up (M1, n = 8; M2, n = 6; M3, n = 10; and M0, n = 9). M1 and M2 animals developed PH and reduced RV systolic function, whereas animals in M3 showed increased RV systolic pressure but maintained normal function. Significant plasma arginine and histidine deficiency and complement system activation were observed in both PH models (M1&M2), with additional alterations to taurine and purine pathways in M2. Changes in lipid metabolism were very remarkable, particularly the elevation of free fatty acids in M2. In the integrative analysis, arginine-histidine-purines deficiency, complement activation, and fatty acid accumulation were significantly associated with maladaptive RV hypertrophy. Our study integrating imaging and omics in large-animal experimental models demonstrates that, beyond pressure overload, metabolic alterations play a relevant role in RV dysfunction in PH.

This study analyzed the meteorological and hydrological droughts in a typical basin of the Brazilian semiarid region from 1994 to 2016. In recent decades, this region has faced prolonged and severe droughts, leading to marked reductions in agricultural productivity and significant challenges to food security and water availability. The datasets employed included a digital elevation model, land use and cover data, soil characteristics, climatic data (temperature, wind speed, solar radiation, humidity, and precipitation), runoff data, images from the MODIS/TERRA and AQUA sensors (MOD09A1 and MODY09A1 products), and soil water content. A variety of methods and products were used to study these droughts: the meteorological drought was analyzed using the Standardized Precipitation Index (SPI) derived from observed precipitation data, while the hydrological drought was assessed using the Standardized Soil Index (SSI), the Nonparametric Multivariate Standardized Drought Index (NMSDI), and the Parametric Multivariate Standardized Drought Index (PMSDI). These indices were determined using water balance components, including streamflow and soil water content, from the Soil Water Assessment Tool (SWAT) model, and evapotranspiration data from the Surface Energy Balance Algorithm for Land (SEBAL). The findings indicate that the methodology effectively identified variations in water dynamics and drought periods in a headwater basin within Brazil's semiarid region, suggesting potential applicability in other semiarid areas. This study provides essential insights for water resource management and resilience building in the face of adverse climatic events, offering a valuable guide for decision-making processes.

AIM: Physical exercise is part of the type 1 diabetes mellitus (T1DM) treatment. However, this practice is still neglected due to the wide variety of glycemic responses under the influence of anaerobic exercise. Therefore, this study aimed to investigate the influence of anaerobic exercise on biomarkers of T1DM. METHODS: The systematic review was conducted on PubMed, Lilacs, and Embase, according to PRISMA. For this purpose, three groups of descriptors were used: Adults with T1DM, anaerobic physical exercise, and glycemic control. The search filter was set to human beings older than 18 years of age, longitudinal and cross-sectional studies, with studies published from 2000 to 2023 in English, Spanish, or Portuguese. Titles and abstracts were read independently by two reviewers, and then the articles were selected for this review. The Kappa coefficient was measured to evaluate the selection. RESULTS: A total of 738 articles were identified, and five were selected to be part of the review after applying the steps of the procedure. Some benefits were observed in fatigue reduction, absence of diabetic ketoacidosis requiring hospitalization, and enhancement of glucose monitoring during exercise. In the anaerobic workouts of the groups with T1DM, glycemic mean values ranged from 124.5-185.0 mg/dl, and glycated hemoglobin records ranged from 6.7-8.1%. CONCLUSION: Anaerobic exercise improved the biomarkers of T1DM, especially glycemic control, and the reduction of symptomatic hypoglycemic episodes. Anaerobic exercise can be performed by individuals with T1DM, suggesting an individualized training prescription and encouraging its practice associated with aerobic exercise.

Objetivo: Verificar la validez de contenido de la propuesta del diagnóstico de enfermería “Sequedad ocular” en pacientes adultos internados en una Unidad de Cuidados Intensivos. Materiales y métodos: Se trata de un estudio metodológico de validación de contenido de la propuesta del diagnóstico de enfermería “Sequedad ocular”, operacionalizado a través de un grupo focal. La selección de especialistas tuvo en cuenta la experiencia clínica y/o académica en el área de diagnósticos de enfermería y/o sequedad ocular y/o ojo seco y/o salud ocular, así como el tiempo de actuación con el tema. Fueron invitados 13 enfermeros que cumplieron con los criterios descritos anteriormente, de los cuales 10 aceptaron participar. El enfoque de validación fue por consenso. Los datos fueron analizados mediante estadística descriptiva, nivel de especialización y análisis de elementos diagnósticos. Resultados: Luego del consenso final de los jueces de enfermería en relación a los elementos diagnósticos, se definió una propuesta del diagnóstico “Sequedad ocular” basada en la validez de contenido con una nueva definición, 14 características definitorias, 9 factores relacionados, 2 poblaciones en riesgo y 20 problemas asociados. Además, luego de juzgar la coherencia de los elementos en relación a la estructura diagnóstica, los jueces emitieron el consenso en relación a las definiciones conceptuales y operativas (AU)

Materiais e métodos: Trata-se de um estudo metodológico de validação de conteúdo da proposição diagnóstica de enfermagem Ressecamento ocular, operacionalizado mediante grupo focal. A seleção dos especialistas levou em consideração a experiência clínica e/ou acadêmica na área de diagnósticos de enfermagem e/ou ressecamento ocular e/ou olho seco e/ou saúde ocular, bem como o tempo de atuação na temática. Foram convidados 13 enfermeiros que se enquadravam nos critérios acima descritos, dos quais 10 aceitaram participar. A abordagem de validação foi por consenso. Os dados foram analisados mediante estatística descritiva, nível de expertise e análise dos elementos diagnósticos. Resultados: Após o consenso final dos enfermeiros juízes em relação aos elementos diagnósticos, foi definida uma proposta do diagnóstico Ressecamento ocular a partir da validade de conteúdo com nova definição, 14 características definidoras, 9 fatores relacionados, 2 populações em risco e 20 condições associadas. Após julgar a coerência dos elementos em relação a estrutura diagnóstica, os juízes emitiram o consenso sobre as definições conceituais e operacionais. Conclusões: O estudo permitiu verificar a validade do conteúdo por juízes da proposição diagnóstica de enfermagem Ressecamento ocular em pacientes em unidades de terapia intensiva, o que favorece o raciocínio diagnóstico do enfermeiro e o planejamento de intervenções efetivas relacionadas a esse diagnóstico, permitindo o manejo do paciente de maneira a proporcionar a integridade ocular (AU)


Objective: to verify the content validity of the nursing diagnosis proposal Ocular dryness in adult patients admitted to an intensive care unit. Materials and methods: this is a methodological study of content validation of the nursing diagnosis proposal Ocular dryness, operationalized through a focus group. The selection of specialists took into account the clinical and/or academic experience in the area of nursing diagnoses and/or ocular dryness and/or dry eye and/or ocular health, as well as the time of performance with the theme. Thirteen nurses who met the criteria described above were invited, of whom 10 agreed to participate. The validation approach was by consensus. Data were analyzed using descriptive statistics, level of expertise and analysis of diagnostic elements. Results: after the final consensus of the nurse judges in relation to the diagnostic elements, a proposal of the diagnosis Ocular dryness was defined based on the validity of content with a new definition, 14 defining characteristics, 9 related factors, 2 populations at risk and 20 associated conditions. In addition, after judging the coherence of the elements in relation to the diagnostic structure, the judges issued the consensus in relation to the conceptual and operational definitions. Conclusions: the study allowed verifying the validity of the content by judges of the nursing diagnosis proposal Ocular dryness in patients in intensive care units, which favors the diagnostic reasoning of nurses and the planning of effective interventions related to this diagnosis, allowing the management of the patients in order to provide ocular integrity (AU)

O estudo objetivou identificar os sentidos produzidos pela comunicação dirigida à população em situação de rua (PSR) durante a pandemia de Covid-19 na cidade de Belo Horizonte. Por meio da técnica de análise de conteúdo, analisaram-se 48 entrevistas realizadas com a PSR; gestores e trabalhadores do Sistema Único de Saúde e do Sistema Único de Assistência Social; e representantes da Pastoral do Povo da Rua e da Defensoria Pública. Cinco temáticas identificadas respaldam os resultados e a discussão: pandemia e seu surgimento; fechamento da cidade e consequências para PSR; desinformação e reprodução de estigmas; desinformação e vacinação; e infodemia. Os resultados indicaram que a comunicação relacionada à pandemia foi considerada como fator de menor importância no planejamento público, ocasionando impacto negativo no enfrentamento social da doença. Destaca-se a importância de estratégias de comunicação públicas, inclusivas, dialógicas - com identificação e escuta dos grupos populacionais vulneráveis - e adaptadas às suas necessidades.


This study aimed to identify meanings produced by communication directed at the homeless population during the Covid-19 pandemic in Belo Horizonte. Forty-eight interviews with homeless people, Brazilian National Health System and Brazilian National Social Assistance System managers and workers, and representatives of the Pastoral Ministry for the Homeless and Public Defender's Office were analyzed using content analysis. The results and discussion are structured around five core themes: the pandemic and its onset; closure of the city and the consequences for homeless people; disinformation and reproduction of stigmas; disinformation and vaccination; and the infodemic. The results suggest that communication related to the pandemic was seen as a minor factor in public planning that negatively impacted coping with the disease. The findings highlight the importance of identifying and listening to vulnerable groups and promoting inclusive and dialogical public communication strategies tailored to their specific needs.


El objetivo del estudio fue identificar los sentidos producidos por la comunicación dirigida a la Población que Vive en la Calle (PSR) durante la pandemia de Covid-19 en la ciudad de Belo Horizonte. Por medio de la técnica de análisis de contenido se analizaron 48 entrevistas realizadas con esa población, gestores y trabajadores del Sistema Brasileño de Salud y Sistema Brasileño de Asistencia Social, representantes de la Pastoral de la Población que vive en la Calle y la Defensoría Pública. Cinco temáticas identificadas respaldan los resultados y la discusión: la pandemia y su aparición; cierre de la ciudad y consecuencias para la Población que vive en la Calle; desinformación y reproducción de estigmas; desinformación y vacunación; e infodemia. Los resultados indicaron que la comunicación relacionada con la pandemia se consideró como factor de menor importancia en la planificación pública, ocasionando impacto negativo en el enfrentamiento social de la enfermedad. Se destaca la importancia de estrategias de comunicación públicas, incluyentes y dialógicas, con identificación y escucha de los grupos poblacionales vulnerables y adaptadas a sus necesidades.

RESUMO Objetivo: identificar os marcadores de violência contra pessoa idosa na perspectiva de enfermeiros. Método: estudo qualitativo conduzido no estado da Paraíba - Brasil, com a participação de nove enfermeiros, em dois grupos focais, durante junho e setembro de 2021. O material empírico foi transcrito na íntegra, transformado em corpus textual com análise de similitude. Resultados: no núcleo central da árvore máxima foi possível observar termos comuns a mais de uma tipificação de violência: "ciclo de violência", "confiança", "dominar", "sofrimento psíquico". Na violência física, "fraturas", "quebrar" e "jogar"; na psicológica, "ameaçar", "medo", "vergonha", "lesão", "corrimento vaginal" e "vaginal"; na financeira, "saque", "dependência" e "pegar"; na negligência, "falta", "insumo" e "higiene"; no abandono, "depressão", "solidão" e "tristeza". Conclusão: esta pesquisa valoriza a Enfermagem como ciência e profissão, especialmente na área forense, ao investigar a violência contra a pessoa idosa. A percepção do enfermeiro torna o processo de enfermagem mais confiável e útil na identificação, notificação e resolução das ocorrências.


ABSTRACT Objective: to identify the markers of violence against older adults from the nurses' perspective. Method: a qualitative study conducted between June and September 2021 in the state of Paraíba (Brazil), with the participation of nine nurses in two focus groups. The empirical material was transcribed in full and transformed into a text corpus with similarity analysis. Results: in the central core of the maximum tree, it was possible to observe terms common to more than one typification of violence, such as: "cycle of violence", "trust", "dominate", "psychological distress". In physical violence: "fractures", "break" and "throw"; in psychological violence: "threat", "fear", "shame", "injury", "vaginal discharge" and "vaginal"; in financial violence: "withdrawal", "dependence" and "take"; in neglect: "lack", "supply" and "hygiene"; and in abandonment: "depression", "loneliness" and "sadness". Conclusion: his research values Nursing as a science and profession, especially in the forensic area, by investigating violence against older adults. The nurses' perception makes the Nursing Process more reliable and useful in identifying, reporting and solving incidents.

El material empírico fue transcrito de forma completa, transformado en corpus textual y sometido a análisis de similitud. Resultados: en el núcleo central del árbol máximo fue posible observar términos comunes a más de un tipo de violencia: "ciclo de violencia", "confianza", "dominar", "sufrimiento psíquico". Los términos relacionados con la violencia física fueron, "fracturas", "romper" y "arrojar"; con la psicológica, "amenazar", "miedo", "vergüenza", "lesión", "secreción vaginal" y "vaginal"; con la financiera, "extracción", "dependencia" y "agarrar"; con la negligencia, "falta", "insumo" e "higiene"; con el abandono, "depresión", "soledad" y "tristeza". Conclusión: esta investigación enaltece la Enfermería como ciencia y profesión, especialmente en el área forense, al investigar la violencia contra las personas mayores. La percepción del enfermero hace que el proceso de enfermería sea más confiable y útil para identificar, informar y resolver casos.

OBJECTIVE: to analyze the concept of elder abuse and identify its respective antecedents, attributes and consequents. METHODS: this is a conceptual analysis according to Walker and Avant's proposition. The search for the concept was mediated by an integrative review in the LILACS, PubMed, CINAHL, Web of Science and BDENF databases. RESULTS: as antecedents, female, family member, low social support and low income or socioeconomic conditions stand out. Attributes were threats and/or intimidation, intentional use of physical force, using resources without authorization, unwanted sexual activity, offering low insufficient amount of nutrients for older adults and not meeting older adults' affective/emotional needs. Consequents were psychological disorders, dependence on aggressors, environment of insecurity and damage/loss of human rights or human dignity. FINAL CONSIDERATIONS: the phenomenon under discussion is broad and multifaceted, suggesting expansion of studies related to the theme in order to explore it in detail.

Objetivo: investigar as possibilidades de atuação de uma clínica jurídica em direitos humanos diante de uma emergência sanitária com efeitos dramáticos para mulheres e meninas vulneráveis. Metodologia: estudo de caso da atuação do Cravinas, projeto de extensão da Universidade de Brasília (UnB) que atua como uma clínica jurídica em direitos sexuais e reprodutivos, durante a pandemia de COVID-19. Resultados: a grave crise de saúde gerada pela COVID-19 reposicionou as prioridades do Cravinas para evidenciar cuidados em saúde abertamente atacados pelo governo federal. A atuação da clínica priorizou ações de acesso à informação, de formação em saúde e direitos e de litígio estratégico, em uma constelação de estratégias atenta e engajada na defesa de populações que nunca estiveram nas prioridades da agenda pública. Conclusão: diante de um cenário de crise, as clínicas jurídicas podem ajudar a propor urgências contra-hegemônicas capazes de responder a problemas e a cuidar de pessoas que estão fora da agenda prioritária das políticas públicas ligadas à pandemia.


Objective: to explore the potential actions of intervention by a legal clinic focusing on human rights in the context of a health emergency with severe repercussions for vulnerable women and girls. Methods:the research adopts a case study approach, examining the activities of Cravinas, an extension project at the University of Brasilia (UnB) that operates as a pro bono office specializing in sexual and reproductive rights. These strategies collectively constituted a conscientious and committed effort to defend populations that historically found themselves outside the ambit of public policy priorities. Conclusion: in the face of a crisis, legal clinics possess the potential to propose counter-hegemonic urgencies, offering responses to challenges and providing support for individuals not prioritized in public policies linked to the pandemic.


Objetivo: investigar las posibilidades de actuación de una clínica jurídica en derechos humanos ante una emergencia sanitaria con efectos dramáticos para mujeres y niñas vulnerables. Metodología: estudio de caso del trabajo de Cravinas, proyecto de extensión de la Universidad de Brasilia (UnB) que presta asistencia legal gratuita por los derechos sexuales y reproductivos, durante la pandemia de COVID-19. Resultados: la grave crisis sanitaria generada por el COVID-19 reposicionó las prioridades de Cravinas para poner de relieve la atención sanitaria abiertamente atacada por el gobierno federal. Las acciones de la clínica priorizaron el acceso a la información, la capacitación en salud y derechos y el litigio estratégico, en una constelación de estrategias atentas y comprometidas con la defensa de poblaciones que nunca habían estado en las prioridades de la agenda pública. Conclusión: frente a un escenario de crisis, las clínicas jurídicas pueden ayudar a proponer urgencias contra hegemónicas capaces de responder a los problemas y atender a las personas que están fuera de la agenda prioritaria de las políticas públicas vinculadas a la pandemia.

RESUMEN

OBJECTIVE: to analyze the concept associated with diagnostic proposition Ocular dryness in adult patients hospitalized in an Intensive Care Unit, identifying its attributes, antecedents and consequences. METHODS: a methodological study carried out through concept analysis, operationalized by scoping review. RESULTS: the analysis of 180 studies allowed the identification of two attributes, 32 antecedents and 12 consequences. The attributes were tear film deficiency and ocular signs and/or symptoms. The prevalent antecedents were incomplete eyelid closure (lagophthalmos) and blinking mechanism decrease. Major consequences included conjunctival hyperemia and decreased tear volume. CONCLUSIONS: this study allowed constructing nursing diagnosis Ocular dryness, part of domain 11, class 2, with 12 defining characteristics, 12 related factors, seven populations at risk and 13 associated conditions. This problem-focused proposal may provide targeted care by promoting early detection and implementing interventions that reduce the risk of ocular damage.

BACKGROUND: The coronavirus disease 2019 (COVID-19) pandemic has imposed significant burden on Brazil's health system. This study aimed to examine clinical characteristics, overall vaccine uptake, and to assess healthcare resource utilization (HCRU) and costs associated with acute COVID-19 in Brazil during the Omicron predominant period. METHODS: A nationwide retrospective study was conducted using various Brazilian databases including, COVID-19 related databases, public health systems, and other surveillance/demographic datasets. Individuals with positive COVID-19 test results between January 1 2022 and April 30 2022, during Omicron BA.1/BA.2 wave, were identified. Patients' demographics, vaccine uptake, HCRU and corresponding costs were described by age groups. RESULTS: A total of 8,160,715 (3.80%) COVID-19 cases were identified in the study cohort, ranging from 2.43% in <5 years to 62.05% in 19-49 years. The uptake of partial (Dose 1) or full immunization (Dose 2) was less than 0.1% in children aged <5 years, whereas in individuals ≥ 19 years, it exceeded 89.78% for Dose 1 and 84.07% for Dose 2. Overall booster vaccine uptake was 38.06%, which was significantly higher among individuals aged ≥ 65 years, surpassing 74.79%. Regardless of vaccination status, 87.2% cases were symptomatic, and 1.48% were hospitalized due to acute COVID-19 (<5 years: 2.33%, 5-11 years: 0.99%, 12-18 years: 0.32%, 19-49 years: 0.40%; 50-64 years: 1.50%, 65-74 years: 5.43%, and ≥ 75 years: 17.89%). Among the hospitalized patients (n = 120,450), 32.57% were admitted to ICU, of whom 31,283 (79.75%) individuals required mechanical ventilation (MV) support. The average cost per day in normal ward and ICU without MV in public/general hospital settings was $104.36 and $302.81, respectively. While average cost per day in normal ward and ICU with MV was $75.91 and $301.22 respectively. CONCLUSIONS: This study quantified the burden of COVID-19 in Brazil, suggesting substantial healthcare resources required to manage the COVID-19 pandemic.

The rapid rollout of vaccines to combat the coronavirus disease 2019 (COVID-19) pandemic over the past 2 years has resulted in the use of various vaccine platforms and regional differences in COVID-19 vaccine implementation strategies. The aim of this narrative review was to summarize evolving COVID-19 vaccine recommendations in countries in Latin America, Asia, and Africa and the Middle East across various vaccine platforms, age groups, and specific subpopulations. Nuances in primary and booster vaccination schedules were evaluated, and the preliminary impact of such diverse vaccination strategies are discussed, including key vaccine effectiveness data in the era of Omicron-lineage variants. Primary vaccination rates for included Latin American countries were 71-94% for adults and between 41% and 98% for adolescents and children; rates for first booster in adults were 36-85%. Primary vaccination rates for adults in the included Asian countries ranged from 64% in the Philippines to 98% in Malaysia, with corresponding booster rates varying from 9% in India to 78% in Singapore; for adolescents and children, primary vaccination rates ranged from 29% in the Philippines to 93% in Malaysia. Across included African and Middle Eastern countries, primary vaccination rates in adults varied widely from 32% in South Africa to 99% in the United Arab Emirates; booster rates ranged from 5% in South Africa to 60% in Bahrain. Evidence from the regions studied indicates preference of using an mRNA vaccine as a booster on the basis of safety and effectiveness of observed real-world data, especially during circulation of Omicron lineages. Vaccination against COVID-19 remains of paramount importance to reduce the burden of disease; strategies to overcome vaccine inequity, fatigue, hesitancy, and misinformation and to ensure adequate access and supply are also important.

OBJECTIVE: The general objective of this study is to analyze the change in mental health and post-traumatic symptoms over 6 months during the COVID-19 pandemic in a Spanish community sample, with a particular focus on individual differences in longitudinal change in symptoms and its predictors. METHOD: This longitudinal prospective study surveyed a Spanish community sample three times (T1: during the initial outbreak, T2: after 4 weeks and T3: after 6 months). Four thousand one hundred and thirty-nine participants from all the Spanish regions completed the questionnaires. However, the longitudinal analysis was performed only with participants which responded at least two times (1,423 participants). Mental health assessments included depression, anxiety, and stress (measured by the Depression, Anxiety and Stress Scale (DASS-21) and post-traumatic symptoms were assessed by the Impact of Event Scale-Revised (IES-R). RESULTS: All the mental health variables achieved worse results at T2. Depression, stress, and posttraumatic symptoms did not recover at T3 when compared with the initial measure, while anxiety was practically stable across the timeline. Women, younger age, having a previous mental health diagnosis and contact with individuals with COVID-19 were linked to worse psychological evolution during the 6-months period. A good perception of one's physical health may be a protective factor. CONCLUSIONS: After 6 months of the pandemic, the general population's mental health was still worse than at the initial outbreak for most of the variables analyzed. RESUMO Objetivo: construir uma terminologia especializada de enfermagem para o cuidado com crianças e adolescentes com epidermólise bolhosa. Método: estudo metodológico, documental, construído entre os meses de janeiro a dezembro de 2021, em parceria com o Centro CIPE®, em João Pessoa - Brasil. Os dados foram analisados com utilização da ferramenta PorOnto e dos seguintes referenciais teóricos metodológicos: a Classificação Internacional para Prática de Enfermagem 2019/2020; e o mapeamento humano seguindo a ISO/TR 12300:2016 e o método de validação dos termos. Resultados: o processo de validação com juízes resultou em 480 termos para criança e adolescente com epidermólise bolhosa, os quais foram submetidos ao mapeamento, resultando em 207 constantes na CIPE® e 273 não constantes na CIPE®. Conclusão: evidencia-se um quantitativo maior de termos não constantes, isto pode ser justificado pela peculiaridade da doença e cuidados específicos com a criança e adolescentes. A construção da terminologia fortalece a elaboração de diagnósticos, resultados e intervenção de enfermagem.

Results: The validation process with judges resulted in 480 terms for children and adolescents with epidermolysis bullosa, which were submitted to mapping, resulting in 207 constant ICNP® terms and 273 non-constant ICNP® terms. Conclusion: a higher number of non-constant terms is evidenced, which can be justified by the peculiarity of the disease and specific care with children and adolescents. The construction of terminology strengthens the development of diagnoses, outcomes, and nursing intervention.

Introduction: The lower rates of cardiovascular disease in Southern Europe could be partially explained by the low prevalence of lipid-rich atheroma plaques. Consumption of certain foods affects the progression and severity of atherosclerosis. We investigated whether the isocaloric inclusion of walnuts within an atherogenic diet prevents phenotypes predicting unstable atheroma plaque in a mouse model of accelerated atherosclerosis. Methods: Apolipoprotein E-deficient male mice (10-week-old) were randomized to receive a control diet (9.6% of energy as fat, n = 14), a palm oil-based high-fat diet (43% of energy as fat, n = 15), or an isocaloric diet in which part of palm oil was replaced by walnuts in a dose equivalent to 30 g/day in humans (n = 14). All diets contained 0.2% cholesterol. Results: After 15 weeks of intervention, there were no differences in size and extension in aortic atherosclerosis among groups. Compared to control diet, palm oil-diet induced features predicting unstable atheroma plaque (higher lipid content, necrosis, and calcification), and more advanced lesions (Stary score). Walnut inclusion attenuated these features. Palm oil-based diet also boosted inflammatory aortic storm (increased expression of chemokines, cytokines, inflammasome components, and M1 macrophage phenotype markers) and promoted defective efferocytosis. Such response was not observed in the walnut group. The walnut group's differential activation of nuclear factor kappa B (NF-κB; downregulated) and Nrf2 (upregulated) in the atherosclerotic lesion could explain these findings. Conclusion: The isocaloric inclusion of walnuts in an unhealthy high-fat diet promotes traits predicting stable advanced atheroma plaque in mid-life mice. This contributes novel evidence for the benefits of walnuts, even in an unhealthy dietary environment.

α-Adrenergic receptors are crucial regulators of vascular hemodynamics and essential pharmacological targets for cardiovascular diseases. With aging, there is an increase in sympathetic activation, which could contribute to the progression of aging-associated cardiovascular dysfunction, including stroke. Nevertheless, there is little information directly associating adrenergic receptor dysfunction in the blood vessels of aged females. This study determined the role of a-adrenergic receptors in carotid dysfunction of senescent female mice (accelerated-senescence prone, SAMP8), compared with a nonsenescent (accelerated-senescence prone, SAMR1). Vasoconstriction to phenylephrine (Phe) was markedly increased in common carotid artery of SAMP8 [area under the curve (AUC), 527 ± 53] compared with SAMR1 (AUC, 334 ± 30, P = 0.006). There were no changes in vascular responses to the vasoconstrictor agent U46619 or the vasodilators acetylcholine (ACh) and sodium nitroprusside (NPS). Hyperactivity to Phe in female SAMP8 was reduced by cyclooxygenase-1 and cyclooxygenase-2 inhibition and associated with augmented ratio of TXA2/PGI2 release (SAMR1, 1.1 ± 0.1 vs. SAMP8, 2.1 ± 0.3, P = 0.007). However, no changes in cyclooxygenase expression were seen in SAMP8 carotids. Selective α1A-receptor antagonism markedly reduced maximal contraction, whereas α1D antagonism induced a minor shift in Phe contraction in SAMP8 carotids. Ligand binding analysis revealed a threefold increase of α-adrenergic receptor density in smooth muscle cells (VSMCs) of SAMP8 vs. SAMR1. Phe rapidly increased intracellular calcium (Cai2+) in VSMCs via the α1A-receptor, with a higher peak in VSMCs from SAMP8. In conclusion, senescence intensifies vasoconstriction mediated by α1A-adrenergic signaling in the carotid of female mice by mechanisms involving increased Cai2+ and release of cyclooxygenase-derived prostanoids.NEW & NOTEWORTHY The present study provides evidence that senescence induces hyperreactivity of α1-adrenoceptor-mediated contraction of the common carotid. Impairment of α1-adrenoceptor responses is linked to increased Ca2+ influx and release of COX-derived vasoconstrictor prostanoids, contributing to carotid dysfunction in the murine model of female senescence (SAMP8). Increased reactivity of the common carotid artery during senescence may lead to morphological and functional changes in arteries of the cerebral microcirculation and contribute to cognitive decline in females. Because the elderly population is growing, elucidating the mechanisms of aging- and sex-associated vascular dysfunction is critical to better direct pharmacological and lifestyle interventions to prevent cardiovascular risk in both sexes.

INTRODUCTION AND OBJECTIVES: Cardiovascular disease remains a leading cause of global morbidity and mortality. The administration of low doses of aspirin in secondary prevention of atherosclerotic cardiovascular disease (ASCVD) has been clearly established. However, the most recent guidelines do not recommend aspirin in primary prevention, reserving it for high-risk patients and after a risk/benefit assessment. The aim of this study was to assess adherence to European guidelines for the use of aspirin in primary and secondary prevention of ASCVD in primary health care. METHODS: The study population consisted of individuals aged >50 years registered at two primary health care units without (primary prevention) and with previous ASCVD events (secondary prevention). RESULTS: We studied a total of 1262 individuals, 720 in primary prevention and 542 in secondary prevention. A total of 61 individuals (8.5%) were under aspirin therapy in primary prevention, most of them taking 150 mg/day (57%). In secondary prevention, 195 patients (27%) were receiving aspirin only, most taking 150 mg/day (52%), and 166 patients (31%) were not under any antithrombotic or anticoagulant therapy. The 100 mg dosage was predominant in patients with ischemic heart disease with (64%) and without (64%) angina, as well as those with myocardial infarction (61.5%) and peripheral vascular disease (62%). CONCLUSIONS: In this study, the prevalence of aspirin use in primary prevention was 8.5%. We found that 30% of patients were not taking either antithrombotic or anticoagulation therapy in secondary prevention. In both primary and secondary prevention, the 150 mg dosage was predominant.
