RESUMEN

Penile calciphylaxis is a rare cause of penile gangrene and is typically associated with multiple comorbidities, most commonly diabetes mellitus and hyperparathyroidism. It demonstrates a high mortality rate of 64% and is seen almost exclusively in patients with end-stage renal disease on hemodialysis. Underreporting of this disease likely occurs, contributing to a paucity of data and lack of formal therapeutic guidelines and approved treatments. Conflicting guidance exists regarding effective treatment strategies, with most formal literature existing in the form of case reports. Herein, we describe a 44-year-old man with end-stage renal disease on hemodialysis who presented with a 2-month history of nonhealing wounds on his penis and lower extremities.

RESUMEN

We report the first case of an 82-year-old woman with five basal cell carcinomas within a large nevus sebaceous of the trunk. Nevus sebaceous of Jadassohn is a congenital cutaneous hamartoma affecting 0.3% of newborns. Clinical diagnosis is based on its distinctive hairless, orange-brown appearance. Up to 51% of nevus sebaceous may exhibit secondary neoplasia, and basal cell carcinoma is thought to occur in approximately 1% of lesions. Our patient presented with an asymptomatic, pigmented verrucous plaque on the upper right back that had been present since birth. Histopathology of concerning papules within the nevus sebaceous were consistent with five basal cell carcinomas and two follicular adnexal neoplasms. The patient was treated with local excisions, topical imiquimod, and close monitoring.

RESUMEN

Levamisole-contaminated cocaine toxicity is a serious emerging public health concern, and providers should be aware of its presentation and management. Most cases of levamisole-induced vasculitis/vasculopathy (LIV) are associated with high antineutrophil cytoplasmic antibodies (ANCA). We describe a unique case of a cocaine user who presented with an acute purpuric eruption and negative ANCA laboratory findings. A brief clinical overview of LIV, spanning from patient presentation to treatment, is provided. In addition, we present a summary of all cases of ANCA-negative vasculitis identified via a PubMed literature review.

RESUMEN

Bacteriophage Pookie is a novel podophage, isolated from soil, which infects Bacillus megaterium. B. megaterium is an important host for large-scale recombinant protein production. Here, we present the complete genome of phage Pookie and describe its core features.

RESUMEN

The complete annotated genome sequence of Bacillus megaterium bacteriophage Slash is described here. Several key features related to morphogenesis, replication/recombination, DNA metabolism, and lysis are described. Slash also encodes a homolog of SleB, a germination-specific cell wall amidase.
