RESUMEN

The outcomes in 12 patients who underwent revision surgery for a failed trapeziometacarpal joint arthroplasty were assessed. Multiple procedures were common (an average of 4.5 per patient), and associated with an overall complication rate of 27%. However, after an average follow-up of 5 years, nine of the 12 patients reported improved function and ability to complete normal daily tasks. Most patients were satisfied with their level of pain relief, their grip and pinch strength, and their overall final result. The subjective outcome was less satisfactory in those involved in workers' compensation litigation. All seven attempted scaphoid-thumb metacarpal fusions failed.

RESUMEN

Subluxation of the extensor digitorum communis tendons in the rheumatoid hand causes ulnar digital drift. If passively correctable, the digit may be realigned by soft tissue rebalancing and extensor centralization, which may preserve a more functional arc of motion than achieved with arthroplasty. A total of 71 centralization procedures were done in 15 rheumatoid patients with a mean age of 55 years and an average follow-up of 9 years. A distally based central-third strip of extensor tendon was used. Correction of ulnar drift deformity was from an average of 47 degrees preoperatively to 7.9 degrees postoperatively, and correction of active range of motion of the metacarpophalangeal joints was from an average of 38 degrees to 56.2 degrees. Reoperation and complication rates were low. This technique corrects and maintains ulnar drift in the rheumatoid hand. Range of motion at the metacarpophalangeal joint level is improved and converted to a more functional one by decreasing the extensor lag.

Juxta-epiphyseal fractures of the proximal phalanx are relatively common. A case report and literature review calls attention to the possibility of entrapment of either a flexor or extensor tendon in this type of fracture.

RESUMEN

A review of the orthopaedic literature suggests that traumatic ulnar physeal arrest associated with radial fracture is a rare occurrence. Twenty-three cases of traumatic ulnar physeal arrest have been reported in patients with distal radius fractures, and we report five additional cases. A classification system for the distal ulnar growth deformities is proposed, and the compensatory radial changes are reviewed. Surgical indications for treatment include cosmetic deformity, progressive carpal subluxation, and decreased range of motion. Surgical options for treatment are discussed and include epiphysiodesis, ulnar lengthening, radial osteotomy, and the Suave-Kapandji procedure.

Ulnar head resection for treatment of painful traumatic and arthritic conditions of the distal radioulnar joint has been performed for over 100 years. Although this is a time-honored procedure, several negative sequelae of the operation have been described. Most of these problems have been due to the instability of the ulna remnant with respect to surrounding structures, including the radius and extensor tendons. This report describes an operative technique to prevent and treat this problem. The pronator quadratus origin is transferred to the dorsum of the ulna, placing the muscle belly between the radius and the ulna remnant. Theoretically, this may provide a soft tissue cushion to prevent ulnoradial impingement. We have been performing this operation since 1985 and report here the results on 16 wrists in 15 patients who underwent surgery between 1985 and 1989. Patients included in this study had pain in the distal radioulnar joint due to osteoarthritis, post-traumatic arthritis, or incongruity. No patients with rheumatoid or other autoimmune arthritis were included. The average follow-up period was 8 years, with a range of 5-9 years. Two groups of patients were studied, including seven who had failure of prior ulna head resection and eight who underwent this procedure (one on both wrists) concomitantly with ulnar head resection.

In a combined study of three hand surgery practices, 78 patients younger than 16 years who had sustained flexor tendon lacerations in zone I or zone II of 95 digits were available for critical evaluation. The average postrepair follow-up period was 24 months (range, 3-144 months). Patient age was divided into three groups: 0-5 years, 6-10 years, and 11-15 years. Performance of all digits was assessed to determine the percentage return of normal digital function following repair. Data were analyzed to determine the effect of age, the effect of varying periods of postrepair immobilization, and the long-term changes in digital performance resulting from growth. All profundus repairs in zone I returned excellent function. Isolated profundus and combined profundus and superficialis repairs in zone II achieved comparable results when managed with an early passive motion program or following immobilization for 3 or 4 weeks. Immobilization for longer than 4 weeks resulted in an appreciable deterioration of function. Digital motion following zone II flexor digitorum profundus and superficialis injuries treated with less than 4 weeks of immobilization or early motion was not significantly different in the three age groups studied. Digits with associated digital nerve and/or palmar plate lacerations fared less favorably when compared with isolated tendon lacerations. In many digits, a modest improvement in digital motion was found when patients returned after several years of growth.

Fifty patients who underwent intercarpal arthrodesis were retrospectively reviewed at an average follow-up period of 34 months. Eighteen patients had a good result, 16 results were fair, and 13 were poor. Thirty-six patients experienced some complication, and 25 patients underwent further surgical therapy: The most common indication for secondary surgery was nonunion (16 patients). Good results were significantly decreased among patients who experienced nonunion, required secondary surgery, or experienced any complication. Patients with static carpal instability treated by scaphoid trapezoid trapezium arthrodesis had a significantly greater proportion of good results.

Nine patients with nonunited humeral shaft fractures were treated by open reduction and internal fixation with an intramedullary fibular bone graft and a compression plate. Fixation of the screws was enhanced by passing them through the fibula as well as the two humeral cortices (quadricortical fixation). Eight of the nine fractures united at an average of 3.5 months. Tests on cadaver bones showed that quadricortical fixation was as strong as methylmethacrylate augmentation and significantly better than bicortical fixation.

Trauma can lead to isolated or associated distal radioulnar joint disorders. Instability or incongruity of the joint frequently leads to pain, weakness, and limited forearm rotation. A knowledge of the anatomy and biomechanics is helpful to understand the pathology and to formulate a treatment protocol. This article reviews the surgical options that have been proposed to correct derangements of the distal radioulnar joint resulting from trauma.

The collagen arrangement of the triangular fibrocartilage complex was studied in 20 fresh cadaver wrists by means of standard and polarized light microscopy and scanning electron microscopy. The collagen fibres in the articular disk are arranged in undulating sheets oriented at oblique angles to each other. The fibers of the radioulnar ligaments are oriented longitudinally from the radial origin to the ulnar insertion. The origin of the articular disk from the radius is characterized by thick fibers 1 to 2 mm in length radiating from the radius into the articular disk. Five specimens were also injected with india ink. The radioulnar ligaments and the peripheral 15% to 20% of the articular disk are well vascularized, whereas the central 80% of the articular disk is avascular.

Aseptic necrosis of the metacarpal head is extremely rare and has been described in association with systemic lupus erythematosus, steroid use, trauma, and other sites of bone infarction. The long metacarpal is the most commonly involved. The extrinsic and intrinsic vascular anatomy of the metacarpal is described in fifty metacarpal specimens. No definite conclusions can be drawn about anatomic variations in blood supply between the different metacarpals. However, in 35% of the specimens, a main arteriole in the distal epiphysis was absent, making these metacarpal heads solely dependent on small circumferential pericapsular arterioles. This group of metacarpals, when associated with other risk factors, such as trauma, systemic lupus erythematosus, or steroid use, may be at increased risk for the development of aseptic necrosis.

Eleven patients had elbow manipulation under general anesthesia to improve a dysfunctional range of motion. Six (55%) patients improved their motion, three (27%) patients had no significant change, and two (18%) patients lost motion. Type of injury, extremity dominance, previous surgical procedures, delay to manipulation, or use of postoperative steroid injections and continuous passive motion did not correlate with successful results. Two transient sensory ulnar neuropathies were the only complications and no heterotopic bone formation or exacerbation was observed. Manipulation under anesthesia should be considered in patients with a dysfunctional elbow range of motion after failure of well-supervised rehabilitation.

Over the last 15 years, vascularized bone grafts have proved to be a valuable method of reconstructing skeletal defects in the extremities. Successful skeletal reconstruction has been reported following tumor resection, select traumatic skeletal defects, congenital pseudarthrosis, and both infected and uninfected skeletal nonunions unresponsive to conventional methodology. Incorporation of the graft into the recipient is not as dependent on an uninfected well-vascularized host as a conventional avascular autogenous graft is. Vascularized autografts are also capable of achieving union across large skeletal defects with fewer stress fractures than when avascular autografts are used to span a defect greater than 6 cm. With patency of the vascular anastomoses and union at both ends of the graft, hypertrophy of the segmental graft is not unusual. The overall success rate of ultimate bony union is approximately 80% and is mostly dependent on the recipient location. As discussed in this article, vascularized autografts are a salvage procedure to be used in reconstructing extremities in which conventional reconstructive techniques have failed or are not applicable.

Six patients who injured their wrists had radiographs documenting a dorsal, 5- to 10-mm oblong fragment of bone immediately proximal to the base of the fourth and/or fifth metacarpal bones. The fragment was seen on the pronation oblique and/or lateral projections, but not on the posteroanterior projection. The radiographic appearance of the fragment was remarkably similar in all cases. In the one patient in which it was performed, pluridirectional tomography demonstrated that the fragment originated from the dorsal surface of the hamate. Five of the six patients also had associated posterior dislocation of the fourth and/or fifth metacarpals. We conclude that this fragment represents a coronal fracture through the body of the hamate resulting from posterior dislocation or subluxation of the fourth and/or fifth metacarpal.

Numerous fixation techniques have been described to obtain successful proximal interphalangeal (PIP) joint arthrodesis. By use of the Herbert screw for compression, 50 (98%) out of 51 PIP joints were fused within 6 weeks. Biomechanically, PIP joint arthrodeses using the Herbert screw in cadaver joints were comparable to tension band arthrodeses evaluated by three-point bending. Herbert screw arthrodesis of the PIP joint achieves a high fusion rate with minimal external protection provided certain technical details are followed.

Fractures of the hook of the hamate frequently fail to unite. In symptomatic nonunion, surgical excision of the hook has provided relief of pain in the few reported cases. In a survey of surgeons, 133 cases of excision of the hook to treat nonunion were reviewed retrospectively. The complication rate associated with excision is three percent. These complications, such as injury to the ulnar nerve or the superficial palmar arch and median nerve paresthesias secondary to retraction are due more to the surgical exposure than the actual excision.

RESUMEN

Dislocations of the DRUJ, when diagnosed promptly, may be successfully treated by closed reduction and immobilization. Symptomatic tears of the TFCC may be decompressed by ulnar shortening with or without debridement of the tear. Several operations are available for the treatment of derangements of the DRUJ. The Lauenstein procedure maintains TFCC function and restores stable painful forearm rotation.
