RESUMEN

Considering the rising incidence of allergic asthma, the symptomatic treatments that are currently applied in most cases are less than ideal. Specific immunotherapy is currently the only treatment that is able to change the course of the disease, but suffers from a long treatment duration. A gene based immunization that elicits the targeting of allergens towards dendritic cells in a steady-state environment might have the potential to amend these difficulties. Here we used a replication deficient adenovirus to induce the mucosal expression of OVA coupled to a single-chain antibody against DEC-205. A single intranasal vaccination was sufficient to mitigate an OVA-dependent asthmatic phenotype in a murine model. Invasive airway measurements demonstrated improved lung function after Ad-Dec-OVA treatment, which was in line with a marked reduction of goblet cell hyperplasia and lung eosinophilia. Furthermore OVA-specific IgE titers and production of type 2 cytokines were significantly reduced. Together, the here presented data demonstrate the feasibility of mucosal expression of DEC-targeted allergens as a treatment of allergic asthma.

AIM: The assessment of the postoperative outcome following laparoscopic sacrocolpopexy using anterior and posterior mesh. MATERIAL AND METHODS: In the study were included one hundred and ten women (mean age 62 years with range from 34 to 78) who had laparoscopic sarcoplexy the period 2001-2005. They were contacted and completed postal questionnaires more than one year after surgery and had a follow up in the uro-gynaecology clinic. RESULTS: The median follow up was 21 months. Eighty-three of them (75.4%) answered the postal questionnaire. Good satisfaction was defined as complete disappearance of all genito-urinary symptoms. Moderate satisfaction was defined as a partial disappearance of symptoms, or de novo less annoying symptoms. Dissatisfaction was defined as no change in symptoms, and /or de novo important symptoms. The overall rate of good satisfaction was 74.7% (62/83), moderate satisfaction 15.6% (13/83), and only 9.6% (8/83) were not satisfied. There was no statistical difference among the six different groups (sacrocolpopexy only, plus TVT, plus TVT and hysterectomy, sacrocolpopexy and previous hysterectomy, sacrocolpopexy and TVT with previous hysterectomy), concerning the pre and post operative clinical signs and post operative symptoms. There was a statistically significant difference (p=0.038) regarding dissatisfaction and prolapse relapse between the group that had a previous total hysterectomy combining sacrocolpopexy with TVT and all other groups. The most frequent post operative symptoms were stress incontinence, dysuria and constipation. No severe complications and mesh erosion were observed, despite the two cases of mesh detachment. CONCLUSIONS: Laparoscopic double synthetic mesh sacrocolpopexy seems to be a safe and effective treatment of genitourinary prolapse, with good overall long term outcomes and benefits of the minimal access approach. The presence of the remaining cervix after subtotal hysterectomy, seems to enhance the results of laparoscopic sacrocolpopexy.. Further randomised studies are needed to confirm our results and to compare this method to open and/or vaginal approach.

In the first paper of this series, we proposed a definition for the concept of 2-D optical cellular processors. In this second part we present an optical experimental setup capable of implementing a binary pattern recognizer. A 2-D hologram array is used in conjunction with a multiplexed holographic illumination for the interconnection and makes the system easily programmable. Thick phase holograms on dichromated gelatin cause distortion and their field of view is constrained by the Bragg effect. These problems are investigated in relation to all the system aberrations and a set of optimal parameters for one distortion-free configuration is given. Experimental results are shown.

RESUMEN

General characteristics and advantages of 2-D optical cellular processors are listed and discussed, with reference to the concepts of cellular automata, symbolic substitution, and neural nets. The role of optical interconnections and of quasilinear processing combining linear array operations and pointwise nonlinearities is highlighted. An architecture for optical implementation of cellular automata is introduced; it features high density 3-D optical shift-invariant interconnections and programmability of the interconnection pattern through adequate use of holographic connectors.

RESUMEN

Stochastic optical phenomena can be used for generation of random-bit arrays with a prescribed probability law that may vary in space and in time. This is useful for implementation of Monte Carlo algorithms, including simulated annealing, with a high recursion rate by monolithic massively parallel arrays of processing elements. Each processing element includes a photosensor. The use and advantages of speckle and of microchannel-plate image intensifiers amplifying single-photon events are considered.
