RESUMEN

The aim of this study is to evaluate the efficacy and viability of hypnosis before and during a gastrointestinal endoscopy. Six Gastroscopies and 22 colonoscopies were carried out under hypnosis in a group of patients. The patients ranged in age from 20 and 67 years and have a history of previously incomplete and poorly tolerated examinations or expressed an active demand for sedation. For 6 of the patients who underwent a gastroscopy under hypnosis, the procedure was successfully completed, reaching the second part of the duodenum without difficulty for the endoscopist. Colonoscopy of the cecum was completed in 19 of 20 patients. All patients, except 1, considered their tolerance level as "good." Hypnosis facilitated an adequate endoscopy intervention without any discomfort in 85% of the cases examined. Avoidance of anaesthesia reduces risk to the patient. Hence, hypnosis for gastrointestinal endoscopy appears to provide a promising strategy.

RESUMEN

The attention deficit hyperactivity disorder (ADHD) is an heterogeneous, complex and common childhood disorder that causes significant impairment of child's functioning. This paper reviews the epidemiology, etiology, diagnosis, comorbidity, course and treatment of ADHD. Special attention is paid to sleep problems both because these 2 conditions frequently co-exist and because there is substantial evidence that ADHD psychopathology and sleep-wake regulation share common neurobiologic mechanisms. Moreover, there could be an overlap between symptoms of ADHD and certain sleep problems such as obstructive sleep apnea syndrome, restless leg syndrome and periodic limb movements of sleep. Children undergoing evaluation for ADHD should be systematically assessed for sleep disturbances because treatment of sleep disorders is often associated with improved symptomatology and decreased need for stimulants.

RESUMEN

El trastorno por déficit de atención con hiperactividad (TDAH) es un trastorno heterogéneo, complejo y frecuente en la infancia, que provoca un deterioro significativo en el funcionamiento del niño. En este artículo se revisan su epidemiología, etiología, diagnóstico, comorbilidad asociada, curso y tratamiento. Se presta especial atención a los trastornos del sueño, ya que estos 2 procesos presentan una alta comorbilidad y comparten mecanismos neurobiológicos comunes. Además, existe un solapamiento entre los síntomas del TDAH y ciertos trastornos del sueño como la apnea obstructiva, el síndrome de las piernas inquietas y el síndrome de movimientos periódicos de las extremidades. Debe evaluarse sistemáticamente a los niños con sospecha de TDAH en busca de alteraciones del sueño, ya que su tratamiento se asocia a menudo con una mejora en los síntomas y una disminución de la necesidad de medicación estimulante

RESUMEN

OBJECTIVE: The study reports on a 33-year-old Caucasian female with Down syndrome and narcolepsy-like symptoms. METHOD: After medical and genetic screening, docturnal polysomnography followed by a Multiple Sleep Latency Test and HLA typing were performed. The patient was medication free and reported a number of cataplexy attacks everyday. Each time that she came to the sleep disorders clinic, she was observed to have cataplexy. She also felt extremely drowsy. A mean sleep latency of 8.8 minutes with 4 sleep-onset rapid eye movement periods in the Multiple Sleep Latency Test, with no other sleep disorder that accounts for the symptoms, was obtained. The patient was DQB1*0301, DQB1*0602, as revealed by the last high-resolution serologic typing.
