RESUMEN

This study aims to examine the relationship between burnout and dentists' intentions to change careers during the COVID-19 pandemic. The MBI-Human Services Survey for Medical Personnel-MBI-HSS (MP) was used to measure burnout levels and investigate how they relate to dentists' intentions to change their profession. The sample included 69 Romanian dentists, 56 of whom were women and 13 of whom were men. Self-reported questionnaires provided to the participants were used to collect the data. Female participants reported higher levels of emotional Exhaustion and Depersonalisation than males. However, there were no substantial differences in Personal Accomplishment levels between genders. As for the intentions to change careers, 41 expressed a clear intention to change their profession, 15 were still determining if they would choose the same speciality, and 28 indicated they would not choose the medical field. The study's findings provide insight into how dentists' thoughts about perceived burnout during the COVID-19 pandemic significantly influenced their attitudes regarding their career paths. The results suggest concerns regarding burnout in the dental field and emphasise the necessity for burnout interventions and support services, particularly during healthcare crises. Further research and interventions to mitigate burnout and promote well-being among dentists are needed to ensure the sustainability and quality of dental healthcare services in Romania.

RESUMEN

Obesity has become a widespread health problem influencing people's health, general well-being, and healthcare costs. It also represents an important risk factor for multiple comorbidities and malignancies. OBJECTIVES: the primary objective of this study was to provide notable insights to healthcare professionals regarding the management of patients with obesity, to highlight the effectiveness of bariatric surgical methods in losing excess weight, and to establish the relationship between weight loss and changes in quality of life (QoL). MATERIAL AND METHODS: our study evaluated the QoL of 76 patients following bariatric surgery at different postoperative stages using the 36-Item Short Form Survey (SF-36) and The World Health Organization Quality of Life-BREF (WHOQOL-BREF) questionnaires. RESULTS: regarding the type of bariatric procedure performed, out of the 76 respondents, 39.47% underwent gastric bypass surgery (RYGB), 56.57% underwent sleeve gastrectomy (LSG), and only 3.94% underwent single anastomosis duodeno-ileal switch (SADI-S). Pertaining to the SF-36 questionnaire, the lowest average scores were found in the energy/fatigue subscales and in the limitations due to mental health, which remained consistent across surgery types with a significant decrease in the SADI-S group. Concerning the WHOQOL-BREF questionnaire, the lowest mean scores were found in the environment (15.03 ± 2.37) and social relations (16.08 ± 2.22) subscales, whilst the highest average scores were in physical health (16.30 ± 2.03) and mental health (16.57 ± 2.16). CONCLUSIONS: the findings revealed that whilst bariatric surgery significantly improved physical health, it resulted in a decrease in mental health scores. Consequently, the study emphasizes the importance of adopting a holistic approach to managing obesity that considers improving both physical and mental health outcomes.

RESUMEN

Diabetes is a condition associated with multiple systemic secondary risk factors, besides pancreatic dysfunctions, affecting the population worldwide and with high costs impacting the healthcare systems. This paper aims to identify the major issues in patients' adherence to injectable diabetes treatment. After the interrogation of the Web of Science database, a scientometric map was generated, from which six directions of approach were identified as essential factors influencing the patient's adherence. These directions yielded clusters of related articles. Glycemic control with the endocrinology metabolic implications, lifestyle adjustments, the healthcare services, medication therapy algorithm, healthcare services digitalization and healthcare policies seem to have a major impact on injectable diabetes therapy and patient adherence. Further research on every one of the six directions is needed to identify the potential of increasing injectable treatment adherence in diabetes patients.

RESUMEN

Health economics refers to the analysis of medical institutions considering their economic and social efficacy, but also the regularity and the relationships that govern the phenomena and the processes from the field of health with the final purpose of achieving better results with the minimum of resources; it represents the study of health price in its complexity. The economics of the population's health needs and in particular the health needs in case of the poor groups of the population, consider health to be the main component of global human vulnerability. Health economics tries to change the simple interpretation of health price and disease cost into a wider consideration of a system administration similar to educational and social economics and the study of health in the context of the multiple specializations of the macro economy of the national group, as it is an instrument in the country's great economics symphony.

RESUMEN

PURPOSE: The aim of the study was to evaluate the correlation between electrocardiographic parameters and heart rate variability with cardiovascular events and mortality among chronic hemodialysis patients. METHODS: In this prospective study, we enrolled 116 asymptomatic patients in whom we performed ambulatory 24-h electrocardiographic Holter monitoring and before and after hemodialysis electrocardiographs. We measured the interval (PR, QRS, QTc, QTc dispersion) differences on the surface electrocardiographs and obtained frequency-domain measures from Holter monitoring (VLF, LF, HF and the LF/HF ratio). RESULTS: During the follow-up period, 13 participants died (11.2 %) and 16 (13.8 %) patients experienced a cardiovascular event. The pre-post-dialysis difference in QTc interval was the best predictor for cardiovascular events (95 % CI 0.453-0.786), while pre-dialysis QRS interval was the predictor for all-cause mortality (95 % CI 1.134-3.136). Also, both outcomes were predicted by pre-post-dialysis difference in PR interval and VLF. CONCLUSIONS: Interval changes during hemodialysis are predictive for cardiovascular events and mortality. Autonomic dysfunction and changes in PR should be monitored routinely, particularly in patients with suspected coronary artery disease.

RESUMEN

UNLABELLED: The quality of the health care system is an essential condition for in improving the quality of health services and satisfying both the patients and healthcare professionals. RESEARCH OBJECTIVES: To identify the characteristics of medical profession, the factors of professional satisfaction or dissatisfaction and health care team-related factors that influence the quality of care. MATERIAL AND METHODS: Qualitative study using a SurveyMonkey online questionnaire consisting of 14 open-ended questions. The respondents were 1013 health professionals in university and non-university hospitals with various unit profiles. RESULTS: According to healthcare professionals, medical profession is defined by: humanism and personal sacrifice (33.37%) and also high professional competence (33.07%). Satisfaction factors are: patient health (40.57%), high social status (36.33) and saved lives (33.07%). Dissatisfaction may be due to: low salary (39.98%), disorganization (38.10%) and lack of procedures (33.96%). Performance may decrease as a result of: stressful working conditions, lack of adequate medical supplies, lack of recognition at work, routine, and strained relations with superiors. CONCLUSIONS: The study shows the need for correlating the medical education system and health systems, and for an integrated analysis of both systems by determining the required skills and modeling the medical team behavior in accordance with the performance variables which take into account satisfaction among both patients and health care professionals.

RESUMEN

AIM: Marketing surveys of the health system collect useful information to develop effective management strategies. The research aim consisted in measuring patient satisfaction with health care quality. MATERIAL AND METHODS: The qualitative research was based on an online SurveyMonkey open-ended questionnaire. The analysis of patient satisfaction/dissatisfaction with healthcare professionals was performed in 1838 patients. Correlation analysis allowed the identification of some determinants associated with patient satisfaction. RESULTS: The variable most commonly associated with satisfaction was "I got adequate information about procedures/treatment" according to 32.2% of respondents. The patients who were dissatisfied most commonly complained that they were "Not adequately informed about maneuvers and treatment", reported by 40.0% of respondents. CONCLUSIONS: This study provides a basis for building an original model for determining the variables of an efficient healthcare system which to ensure a high degree of patient satisfaction.
