RESUMEN

Patients with implanted pacemakers (PM) should pass outpatient monitoring at definite intervals given in the PM certificate. The paper shows that the monitoring periods in the patients are frequently violated, therefore the voltage of the PM battery is less than the critical level at which the PM is to be replaced. As the discharge of the battery is less than the critical level, the electronic unit of the PM begins first to regenerate an irregular sequence of stimulating pulses and then the generation of pulses stops. Thus, if the outpatient monitoring intervals in patients with implanted PM are over those given in the PM certificate, this will cause errors in encoding the causes of replacement of a PM, followed by dangerous changes in its parameters.

RESUMEN

A portable "STICAR-39" apparatus for emergency transesophageal and endocardial pacing is described. The apparatus is the smallest, most light-weighed, and power-efficient transesophageal cardiac stimulator in the world. It is effective in emergency medical care, urgent and critical cases, surgery, resuscitation, and intensive therapy. The high cost-efficiency of the apparatus is achieved by using the original scheme of an impulse accumulator-converter of constant power supply voltage to the voltage of stimulating impulses.

RESUMEN

Until recently heart transplantation has been the only method of surgical treatment of severe myocardial heart failure. In the recent years operations of cardiomyoplasty have been performed more and more often for treatment of patients with this disease. The article presents the literature and personal data on the operation technique, indications and contraindications for it. First clinical results of the operative interventions fulfilled since 1994 are discussed.

Modern methods for the evaluation of principal respiration indexes were analysed to formulate the demands for methodological characteristics of the devices used in lung functional diagnosis. As a result the extent of measurement random errors of main respiration indexes, which guarantee the prescribed level of right diagnosis probability are established.

RESUMEN

The article presents the metrological, electrophysiological and economic aspects of the substantiation of the choice of the optimal discreteness of impulses and their frequency in diagnostic programmed cardiostimulation. It has been shown that in conducting the intracardiac electrophysiological examination the traditional choice of the discreteness of the interval delta T3 = 10 msec, and the discreteness of the frequency delta Fc = 10 impulses/min does not meet the metrological requirements. From the metrological viewpoint, the minimal values of the discreteness constitute delta T3 = 20 msec and delta Fc = 20 impulses/min, which are compatible with obtaining diagnostic information and thus justified from the clinical and expedient from the economic point of view.

RESUMEN

The authors have proposed a model of the blood pump from the skeletal automuscles of the body for carrying out aortal counterpulsation under experimental conditions. The pump is run by means of direct electric stimulation of the muscle. Have demonstrated that it is possible in principle to design a completely implanted small system of aortal counterpulsation.

RESUMEN

To successfully accomplish stimulation of a nerve a portable stimulator with bipolar electrodes has been designed. The level of the nerve trunk resistance was found to comprise 6 kOm. The threshold pulsed current amplitude at which the effect becomes apparent lies within the range of 1-2 mA, with the threshold of the potential pulses amplitude amounting to 4-12 V. The construction of the electrodes is described and the block diagram along with technical characteristics of the stimulator are presented.

RESUMEN

An independent power supply of electrocardiostimulators improves the electric safety of the patient and of the attending staff, does not produce any additional interferences with the functioning of instrumentation and makes it possible to design mobile portable devices. The storage battery used as a power source should have a sufficiently great capacity, provide for some back-up time ensuring normal operation of the apparatus, have small dimensions and mass. These requirements are met with most adequately by the air-zinc battery "Krona Vts". To estimate the continuous operation time for the apparatus experimental characteristics of the battery's pulse discharge are given.

To realize medical control over parameters of cardiostimulators a tester has been devised that makes it possible to exercise control over the principal parameters of implanted and external cardiostimulators, such as the amplitude, polarity and the pulse recurrence frequency, as well as the amplitude and frequency of switching on and off of the monitored stimulators, resistance of electrodes and the voltage of power supply batteries.
