RESUMEN

OBJECTIVE: To assess the mental health of health workers during a pandemic. MATERIAL AND METHODS: Eight hundred and twelve medical workers took part in a cross-sectional Internet survey. The questionnaire included a socio-demographic block, a block of questionnaires for assessing the level of symptoms of anxiety and depression (PHQ-9 and GAD-7). The data were processed using statistical methods. RESULTS: The prevalence of anxiety and depression among healthcare providers during the pandemic was 48.77% and 57.63% respectively. Subjectively poor quality of sleep was noted by 37.4% of respondents. Cluster analysis distinguished 4 groups of respondents: group 1 was characterized by high scores on PHQ-9 and GAD-7 and a low subjective assessment of sleep quality; group 2 had low scores on PHQ-9 and GAD-7 and a high subjective assessment of sleep quality; respondents from clusters 3 and 4 had average total scores on PHQ-9 and GAD-7, however, group 4 was characterized by significantly lower values of subjective sleep quality assessment. CONCLUSION: The division into clusters makes it possible to understand which groups of medical workers require psychological (psychotherapeutic) support in the first place. Cluster 1 respondents with high levels of anxiety and depression, as well as poor sleep quality, need priority assistance.

RESUMEN

A method of identification and quantitative determination of baclofen in blood by HPLC with mass spectrometry detection has been developed. It is characterized by high sensitivity, specificity, linearity, accuracy, reproducibility, and a low detection for quantitative determination. The method has been used for diagnostics of acute baclofen poisoning in patients.

RESUMEN

Electronic textbooks and libraries abroad. - The current article presents a brief analysis of the modern state and perspectives of electronic textbooks and libraries abroad. The material is based on the open access foreign internet sources. Using particular examples it has been shown that despite a long history of electronic textbooks and libraries development, the latter remains at the very early stage. The bureaucratic attempts to speed up this process result in costly failures. It is necessary a painstaking and cautious work in the mode of scientific practical experiment to give electronic textbooks and libraries a chance to make worthy contribution into the progress of training, education and culture.

RESUMEN

The objective of the present study was to select and develop simpler methods for the quantitative determination of baclofen in blood with the use of HPLC and tandem MS (MS-MS) techniques and its qualitative determination in cadaveric organs by the GC/MS technique. These mathods were shown to be suitable for the purpose of forensic medical analysis, clinical, toxicological, and therapeutic monitoring. The special emphasis is laid on the methods used to investigate the biological materials obtained from the subjects who died from baclofen intoxication.

RESUMEN

The study included 44 patients who were divided into 3 groups according to the type of anesthesia. In group 1 patients (n = 15), initial anesthesia was accomplished by inhaled sevoflurane and intravenous fentanyl (2.5-3.5 microg/kg); basal anesthesia was performed with sevoflurane. In Group 2, midazolam, 0.1-0.15 mg/kg, fentanyl, 5.2 +/- 0.01 vg/kg, and ketamine, 0.85 +/- 0.13 mg/kg were given for induction. Basic anesthesia was carried out, by administering fentanyl in a dose of 4.71 +/- 0.4 microg/kg/hour, halothane, 0.5-1.5 ob %. In Group 3, midazolam, 1.2 +/- 0.01 mg/kg) and fentanyl, 7.8 +/- 0.6 microg/kg) were used to induce anesthesia. Basic anesthesia was effected with fentanyl, 5.31 +/- 0.5 microg/kg/hour, ketamine, and diprivan. Anesthetic management using halogen-containing inhalational anesthetics at coronary bypass surgery in patients at high anesthetic risk was ascertained to cause a significant reduction in the degree of manifestations of oxidative stress and facilitated a better intraoperative period. Sevorane was found to have the most significant effect on oxidative stress.

RESUMEN

Ventricular arrhythmias are recorded in most patients at coronary bypass surgery without extracorporeal circulation. The stages, such as the pulling of the sternal edges apart, the opening of the pericardium, the revision and dislocation of the heart, and revascularization of coronary arteries, are most dangerous due to the fact that they may lead to the development of arrhythmias. The major proarrhythmogenic factors at coronary bypass surgery without extracorporeal circulation are mechanical irritation of reflexogenic areas and myocardial ischemia, the mechanical factors playing the leading role in the development of ventricular arrhythmias.

RESUMEN

Fibrillation of the atriums is registered in 20-40% of cases after the surgery of direct revascularization of the myocardium and runs alongside with impairments that are life-menacing. Seventy-six (76) patients were examined after direct revascularization of the myocardium, including 20 subjects with paroxysms of atrium fibrillation. The arrhythmia-related risk was shown to be increasing in persons (older than 60) with an impaired left column of the left coronary artery, in unstable angina pectoris diagnosed before surgery, malfunction of the left ventricle and in case of a rapid warming-up of a patient after the artificial blood circulation with cold cardioplegia is over.

RESUMEN

The activity of sympathyco-adrenal, and of serotonin-hystain-cholinergic systems was studied in 34 patients operated for the ischemic heart disease without extracorporeal blood circulation. A moderate increase of parameters characterizing the condition of sympathyco-adrenal, and of serotonin-hystain-cholinergic systems was noted in patients in the perioperative period. The activity of the studied systems reduced due to a total anesthetization. At the same time, the activation of the factors of the neurohumoral system was limited by the adaptation norms with sufficient reserves being preserved.

RESUMEN

New method for optimized computing thermoelectric coolers is proposed for the case of variable temperatures within heat-transfer media. The operation of the device is analyzed when the temperature of the cooled medium is greater than the temperature of the heated one, i. e. under conditions of the negative temperature difference. The comparative analysis of the computed and experimental data in values of the cooling and electric power demonstrates fully satisfactory results.

RESUMEN

The increase of the absolute capacity of temperature control devices used in extracorporeal blood circulation apparatus has necessitated the intensification of heat exchange and, as a consequence, the work of heat exchangers under anisothermal conditions. This concerns mainly thermoelectric temperature control devices. Calculations in the process of their design and, to a greater extent, the optimization of their parameters under the conditions of anisothermality in the soldered joints become far more complicated. The numerical analysis of such thermoelectric device with anisothermal soldered joints has demonstrated the possibility of its analytical optimization by assuming it to be a thermoelectric device working under the average effective values of temperature and the coefficients of electrical characteristics. The results thus obtained allow to considerably decrease labor consumption in designing the above-mentioned devices.

The prospects for the use of thermoelectric systems to regulate the blood temperature in extracorporeal circulation apparatus are outlined. The temperature and electric conditions in the thermoelectric thermoregulating system were optimized, which enabled extremal values of their basic parameters to be obtained. A conclusion is drawn on the expediency of using these systems for the purpose in view.
