RESUMEN

The article describes experiments in animals with laser revascularization of the myocardium and first operations of laser transmyocardial revascularization of the myocardium in man without a combination with operations on the coronary vessels. Operations were performed on 15 IHD patients with the III-IV functional class of cardiac insufficiency. The operations were favourable in all the patients. Immediate and long-term results suggest that this operation can be an alternative to direct interventions on coronary arteries and seems to be the only possibility in patients for whom operations on the heart vessels are not indicated or contraindicated.

RESUMEN

A method for obliteration of the gallbladder, based on its mucoclasis with phenol preparations, was developed in experiments on unbred dogs. The safety of using preparations based on phenol for chemical demucosation of the gallbladder is demonstrated. The suggested method may serve as a prototype for the development of similar interventions in clinical practice in acute cholecystitis and cholelithiasis after endoscopic cholecystostomy and cholecystolithoextraction. The method was used in clinical practice in 3 patients with acute calculous cholecystitis and severe concomitant pathology and a high operative-anesthesiological risk.

RESUMEN

Magnetolaserotherapy (impulse power 3.6-15.2 mW, 10-50 mT) in rabbits with acute pyelonephritis resulted in significant improvement against the responses in the control group. Histologically, this appeared as exudation phase reduction, more rapid proliferation, replacement of the inflammation focus for granulation tissue.

Pharmacochemical achievements have brought about considerable improvement in treating peptic ulcers. But possible side effects demand a very cautious approach to the administration of every new drug, and stimulate the search for nontraditional treatment. In 638 patients who were treated by semiconductor laser transcutaneously (external irradiation), an average term of ulcer healing was 11.5 days, that is, half the time required for controls. The healing process under laser therapy depends on the disease gravity as well as the diameter and depth of the ulcer. Morphologically, we noted a quick restoration of microvessels in the ulcer area. The most optimal parameters of laser irradiation are chosen with the help of biophotometry.

On the basis of experimental studies conducted on 535 albino Wistar rats and clinical observations over 125 patients, and from complex appraisal of morphological, biochemical, and tensiometric data the conclusion is drawn that optimal operative cutting of tissues is achieved by means of far infrared carbon dioxide laser beam with a wavelength of 10.6 microns. Comparison of these characteristics showed the strength of the postoperative scar to be much less in intraoperative tissue cutting with a steel scalpel or high-energy beam of an AIG laser with a wavelength of 1.06 microns. Low-intensity laser radiation increased the strength of the postoperative scar in the early periods in all groups. Tissue cutting with a high-energy carbon dioxide laser beam with a wavelength of 10.6 microns in combination with pre- and postoperative exposure of the zone of the incision to low-intensity 0.89 micron wavelength laser beam considerably increases the strength of the postoperative scar and is the method of choice in the treatment of aseptic wounds.

RESUMEN

In this study in 90 rats the measured rate of healing, pO2 in sites of postoperative scar and histological specimen of identical excisional wounds produced by CO2-laser, scalpel and combination are compared. The data of healing laser wounds via scalpel ones show slow laser wound healing. We explain, this by inhibition of the inflammation phase due to diminishing macrophages migration into the wound. pO2 data in postoperative scar after laser and scalpel wounds show that laser thermal alteration does not influence CO2 regimen in the scar.

RESUMEN

External semiconductor laser treatment was realized in 470 patients with peptic ulcer. The average time of ulcer healing was twice shorter than in control cases. Ulcer epithelialization depended on the severity of the disease, diameter and depth of the ulcer defect. Prophylactic treatment is recommended to reduce the number of recurrences.

RESUMEN

A comparative analysis of the efficiency of coagulation of the wound surfaces of the liver and spleen by plasma jet of argon, neon and helium was made. The coagulation rate was shown to increase with the elevation of temperature and power of the plasma jet. Results of the experimental studies have shown that using argon is most expedient for achieving reliable and rapid hemostasis.

RESUMEN

Local reaction of the microvascular bed and angiogenesis of closed and infected open wounds have been studied in 72 rats. Selective contrasting of the functioning microvessels was achieved by means of intravenous injection of peroxidases, as tracers. The dynamics of microcirculatory disorders and the periods of capillary circulation recovery in the wound within 1-20 days after the injury were described. The principal resemblance of the alterations of the microvascular bed in closed and infected open wounds was established.

RESUMEN

In experiments on rats, the state of microcirculation in purulent wounds that had been exposed to a defocussed beam from a CO2 laser is described and compared to that in similar but nonexposed wounds. The laser treatment was found to accelerate the normalization of blood supply to the wound walls; the stimulatory effect was maximal when the thermal damage to the tissues was minimal.

An analysis of the article "Clinico-morphological estimation of regenerative processes of the stomach wound made by laser scalpel" by prof. A. I. Nechai et al. published in the Journal "Vestnik khirurgii" No. 1, 1985 has shown that the authors committed a number of fundamental technical errors when performing experimental studies and operations on gastrointestinal organs in the clinic which were followed by unsatisfactory results and conclusions on inexpediency of using lasers for such operations. The 10-year experience of the All-Union Research Center of Laser Surgery of the USSR Ministry of Health as well as that of most researchers dealing with this problem proves the opposite--the use of laser technique was found to improve processes of healing the gastric and intestinal wounds and to reduce the amount of postoperative complications and lethality.

In this paper, results of the morphological studies of mammalian testicular tissue after exposure of one of the testes to powerful laser radiation are reported. The results obtained are compared with those reported in the literature on the biological effect of penetrating radiation. No negative influence of powerful laser radiation is registered at the organism level. The criteria for estimation are the tissue response, morphological picture of spermatogenesis, and reproductive functions of exposed animals and of their progeny.

The mechanism of myocardial revascularization was studied in 30 dogs with the aid of impulse carbonic acid laser after ligation of the anterior descending branch of the left coronary artery. In the control group, 4 dogs died of myocardial infarction. In 26 animals laser canals were formed. The walls of the canals comprised cardiomyocytes with the manifestations of a coagulation thermal necrosis. Morphological studies disclosed the aseptic productive nature of the inflammatory reaction and the absence of marked leukocytic infiltration of the tissues adjacent to the area of necrosis. The laser canals were lined with endothelium by the 10th-14th day after operation, they were not obliterated or sclerosed. Morphological, laboratory, and radionuclide studies attest to adequate perfusion of the myocardium with the aid of the laser canals preventing the development of myocardial ischemia and infarction.
