RESUMEN

INTRODUCTION: Synovial sarcoma is a malignant soft tissue neoplasm which occurs most of the time in teenagers and young adults. Facial, and especially parotid gland localization, is very uncommon. CASE REPORT: A 15-year-old male patient, with no prior history, was hospitalized for swelling in the left parotid area noted 5 months before. The mass was painful and there was no facial paralysis. A CT scan revealed a tumoral process of mixed density in the left parotid gland. The thorax and abdominal CT scan was normal. The patient was initially treated by surgery and adjuvant chemotherapy. He died, 8 months after this multimodal therapy. DISCUSSION: Five percent of salivary gland primitive tumors are of mesenchymatous origin, 0.3 to 1.5% of which are sarcomas. The diagnosis of parotid gland synovial sarcoma is confirmed by immune-histochemistry and cytogenetic tests. Surgery combined to radiotherapy seems to be the best treatment.

AIM: The aim of this study was to establish the status of the means of information and to search for the variability in risk estimation. MATERIALS AND METHODS: A questionnaire has been mailed to a hundred plastic surgeons randomly sorted among the habilitation list to be returned in an anonymous enclosed envelope. This random sample comprised 50 liberal practioners and 50 plastic surgeons working at hospital. The first part of this questionnaire dealed with basic data such as the type of activity (reconstructive or aesthetic), years of experience and means of information. The second part dealed in seven items with the most important preoperative information to be given to the patient and the most feared complications according to each surgeon before a reduction mammaplasty and abdominoplasty. RESULTS: A total of 50 (50%) of the 100 questionnaires were returned. Years of experience influenced significantly the importance given to information and the way of transmitting it. Important variability existed between the surgeons' answers. Moreover, current complications were in many cases never mentioned. Risk estimation depended on the experience of the surgeon and the information procedures were influenced by the surgeon's type of activity. CONCLUSION: Important variability in risk estimation exists among the plastic surgeons leading to an important variability in the kind and way of delivering information.

RESUMEN

The Seip Berardinelli or undiagnosed endocrine metabolic syndrome is a rare autosomal recessive pathology mainly described in families of Portuguese origins. This syndrome presents various metabolic disturbances responsible of various dysmorphies. We report the case of two brothers seen during their childhood for respiratory, speaking and feeding problems related to a class 3 of Angle and macroglossia. The object was to study the interaction between the size of the tongue, the prognathism and the disturbances presented in order to organize early surgery (partial glossectomy).
